This book reviews the brilliant progress made in the past three decades in clinical outcomes for osteosarcoma patients treated with a multidisciplinary approach, including limb-salvage surgery combined with neoadjuvant multidrug chemotherapy and aggressive management of pulmonary metastasis. Osteosarcoma was a miserable disease for adolescents and young adults until the early 1970s, with a survival rate that was less than 10–15% even after amputation for affected limbs because of the progression of pulmonary metastasis. With the development of neoadjuvant chemotherapy for osteosarcoma, including high-dose methotrexate, doxorubicin, cisplatin, and ifosfamide during the late 1970s and the 1980s, however, the prognosis has dramatically improved. Limb-salvage surgery for patients with extremity osteosarcoma is now a gold-standard surgical procedure for more than 90% of patients with localized disease. Additionally, aggressive pulmonary metastasectomy for patients with lung metastasis fromosteosarcoma has contributed to improvement of their survival. More recently, carbon-ion radiotherapy has also been introduced for patients with unresectable osteosarcoma of the trunk, as in the spine and pelvis. In this volume the author provides valuable descriptions of an important new treatment modality for a multidisciplinary approach for osteosarcoma patients.

The age-standardized mortality rate for hemorrhagic stroke has decreased worldwide over the past two decades, but incidence, number of deaths, and lost disability-adjusted life-years (DALY) continue to increase. The European Collaboration on Boron Neutron Capture Therapy (BNCT), conceived in 1987 and successful in 1989 in gaining financial support as a Concerted Action through the Medical and Health Research Programme of the Commission ofthe European Communities (CEC) in Brussels, considered it an opportune moment to hold its annual Plenary Meeting on 18-20 Septem­ ber 1991 as an International Workshop entitled'Towards Clinical Trials of Glioma with BNCT'. The background to this consideration was influenced by the world-wide resurgence ofinterest in NCT over the last 2 decades and by the exemplifica­ tions at the Fourth International Symposium on Neutron Capture Therapy for Cancer held in Sydney in December 1990, where it was strongly indicated that within the next 2 years clinical trials would be started both in Europe and the United States. In particular at the High Flux Reactor of the Joint Research Centre of the CEC at Petten in The Netherlands, an epithermal neutron beam designed and installed in the summer of 1990 recently became operable at full reactor power. An extensive series ofexperiments, including the nuclear and radiobiological characterisation of the beam and a healthy tissue tolerance study on canines has started and has the aim to define the preconditions for clinical trials onpatients with Grade III/IV glioma. However, as with any other new therapy modality, it must be demon­ strated that BNCT is safe for the patient and has a reasonable chance of being an effective therapy.

Finora si è spesso trattato l'anziano con la cultura dell'adulto, molto diversa da quella geriatrica, con il risultato di considerare malato anche chi ha solo il torto di dimostrare i propri anni. In sostanza, mentre le età della crescita e della piena efficienza sono ben conosciute nei loro aspetti normali e patologici, non altrettanto avviene per l'età della decadenza, per la quale ci si avvale impropriamente di un altro metro di valutazione. Il risultato inevitabile di tutto questo è che gli interventi di supporto per la terza età possono essere scorretti, così come l'impiego delle risorse umane e finanziare ad essa destinate.
