The past several decades have seen remarkable improvements in several major public health issues affecting young people: smoking rates are down, traffic crash fatalities have declined, and other unintentional injuries have declined in number. Yet, similar successes have not been replicated in mental health. Why are we, as a society, failing to make needed investments in children's mental health? In this book, the history of the Brazilian Psychiatric Reform is told by one of its main protagonists. In the early 1980s, there were about 80 thousand people admitted to psychiatric hospitals in Brazil, with average lengths of hospital stay of approximately 25 years. The psychiatric reform process that took place in the country was responsible for closing more than 60 thousand beds in mental asylums, most of them characterized by conditions of violence and abandonment.The Brazilian Psychiatric Reform was inspired by the psychosocial care model introduced by psychiatrist Franco Basaglia in Italy and was marked by the broad participation of social movements, such as the anti-asylum movement and other human rights movements. This process gave rise to a model of mental health care based on open-door territorial mental health services, guided by the principle of treatment in liberty, in addition to other strategies of deinstitutionalization. More than a proposal to restructureor modernize the mental health care model, the objective of the Brazilian Psychiatric Reform was the construction of a new social place for the diverse and singular subjective experience of madness. By intending to produce new imaginaries, new social representations and new meanings for these experiences, the Brazilian Psychiatric Reform led to one of the larger experiences of deinstitutionalization in the world and to the large scale implementation of a new model of mental health care in which the old asylum-centric paradigm was replaced by a new democratic psychosocial care model.

This book is a primer on Stepped Care 2.0. It is the first book in a series of three. This primer addresses the increased demand for mental health care by supporting stakeholders (help-seekers, providers, and policy-makers) to collaborate in enhancing care outcomes through work that is both more meaningful and sustainable. Our current mental health system is organized to offer highly intensive psychiatric and psychological care. While undoubtedly effective, demand far exceeds the supply for such specialized programming. Many people seeking to improve their mental health do not need psychiatric medication or sophisticated psychotherapy. A typical help seeker needs basic support. For knee pain, a nurse or physician might first recommend icing and resting the knee, working to achieve a healthy weight, and introducing low impact exercise before considering specialist care. Unfortunately, there is no parallel continuum of care for mental health and wellness. Asa result, a person seeking the most basic support must line up and wait for the specialist along with those who may have very severe and/or complex needs. Why are there no lower intensity options? One reason is fear and stigma. A thorough assessment by a specialist is considered best practice. After all, what if we miss signs of suicide or potential harm to others? A reasonable question on the surface; however, the premise is flawed. First, the risk of suicide, or threat to others, for those already seeking care, is low. Second, our technical capacity to predict on these threats is virtually nil. Finally, assessment in our current culture of fear tends to focus more on the identification of deficits (as opposed to functional capacities), leading to over-prescription of expensive remedies and lost opportunities for autonomy and self-management. Despite little evidence linking assessment to treatment outcomes, and no evidence supporting our capacity to detect risk for harm, we persist with lengthy intake assessments and automatic specialist referrals that delay care. Before providers and policy makers can feel comfortable letting go of risk assessment, however, they need to understand the forces underlying the risk paradigm that dominates our society and restricts creative solutions for supporting those in need.

On February 26-27, 2014, the National Academies of Sciences, Engineering, and Medicine's Forum on Global Violence Prevention convened a workshop titled Mental Health and Violence: Opportunities for Prevention and Early Intervention. The workshop brought together advocates and experts in public health and mental health, anthropology, biomedical science, criminal justice, global health and development, and neuroscience to examine experience, evidence, and practice at the intersection of mental health and violence. Participants explored how violence impacts mental health and how mental health influences violence and discussed approaches to improve research and practice in both domains. Although the general public in the United States assumes children to be generally healthy and thriving, a substantial and growing number of children have at least one chronic health condition. Many of these conditions are associated with disabilities and interfere regularly with children's usual activities, such as play or leisure activities, attending school, and engaging in family or community activities. In their most severe forms, such disorders are serious lifelong threats to children's social, emotional well-being and quality of life, and anticipated adult outcomes such as for employment or independent living. However, pinpointing the prevalence of disability among children in the U.S. is difficult, as conceptual frameworks and definitions of disability vary among federal programs that provide services to this population and national surveys, the two primary sources for prevalence data. Opportunities for Improving Programs and Services for Children with Disabilities provides a comprehensive analysis of health outcomes for school-aged children with disabilities. Mental, neurological, and substance use (MNS) disorders have a substantial impact on global health and well-being. Disorders such as depression, alcohol abuse, and schizophrenia constitute about 13 percent of the total burden of disease. Worldwide, MNS disorders are the leading cause of disability, and the 10th leading cause of death. Despite this high burden, there is a significant shortage of resources available to prevent, diagnose, and treat MNS disorders. Approximately four out of five people with serious MNS disorders living in low- and middle-income countries do not receive needed health services. This treatment gap is particularly high in Sub-Saharan Africa (SSA). Challenges to MNS care in SSA countries include a lack of trained mental health professionals, few mental health facilities, and low prioritization for MNS disorders in budget allocations. African countries, on average, have one psychiatrist for every 2 million people, whereas European countries have one psychiatrist per 12,000 people. Expanding on previous efforts to address the development and improvement of sustainable mental health systems in SSA, the Institute of Medicine convened this 2015 workshop series, bringing together key stakeholders to examine country-specific opportunities to improve the health care infrastructure in order to better prevent, diagnose, and treat MNS disorders. Global health is an area for study, research and practice that places a priority on improving health and achieving equity in health for all people worldwide. Global mental health is the application of these principles to the domain of mental ill-health. The most striking inequity is that concerning the disparities in provision of care and respect for human rights of people living with mental health problems (MHP) between rich and poor countries. Low and middle income countries (LMIC) are home to over 80% of the global population, but command less than 20% of the share of the mental health resources. The consequent'treatment gap'is in itself a contravention of basic human rights. Even where treatment is provided, far too often this falls far below minimum acceptable standards. Failure to provide basic necessities such as adequate nourishment, clothing, shelter, comfort and privacy, and the administration of unauthorized and unmonitored detention, shackling and chaining are all well documented abuses, described recently as a'failure of humanity'. Latinos are the fastest growing and largest minority group in the United States. In 2008, this group numbered over 47 million; by 2050, the population is expected to triple, reaching 133 million. Traditionally, Latinos have immigrated to large urban centers (e.g., New York, Los Angeles) that over long periods of time developed a complex infrastructure to receive new immigrants. Increasingly, new Spanish-speaking immigrants are moving into areas of the country previously unfamiliar to them. Although urban co-ethnic communities continue to be the destination of many newcomers, immigrants from Mexico, Central America, and South America in pursuit of low-skilled labor opportunities are settling in small towns and rural areas of the South and Midwest. This new demographic trend has resulted in the creation of the term'new growth communities,'which refers to small rural areas that are now home to a small but rapidly growing Hispanic population. Unfortunately, these communities, which are now present in many states across the country (e.g., Illinois, North Carolina), lack the infrastructure necessary to meet the needs of Latino immigrants (e.g., access to health care, immigration assistance, and breaking down language barriers). The lack of an infrastructure and the lack of an established ethnic community to facilitate the assimilation of new immigrants present an ongoing challenge, especially in the area of Latino mental health. The volume focuses on dealing with systemic issues and on providing innovative ideas for development of infrastructure of services. The 2005 edition is an update of the first Mental Health Atlas 2001, and contains the most complete global data available on mental health resources. It shows no substantial change in global mental health resources since 2001, while there continue to be marked and growing differences in availability between high- and low-income countries. For example, while the survey of 192 countries does show a slight increase in the total number of psychiatrists from 3.96 to 4.15 per 100 000 people worldwide, distribution across regions ranges from 9.8 in Europe to just 0.04 in Africa. One fifth of the more than 100 countries supplying figures to the survey spend less than 1% of their health budget on mental health. All of us, vulnerable human beings that we are, need to work together to ensure that the environment we live in is conducive to peace of mind, and free of the horrors that jeopardize mental well-being. This publication grew out of the commitment of the United Nations and others to this cause, and is the product of a series of lively meetings that the authors held at UN Headquarters with civil society groups, government officials, and United Nations staff—a dialogue that coincided with the World Health Organization's focus on mental health in 2001. I hope it will be a valuable resource for practitioners, policymakers, and United Nations field and headquarters staff wherever trauma strikes, and wherever peace of mind is threatened.

In the past dozen years, the field of international mental health has been transformed from a sub-discipline that attracted only a handful of anthropologists, psychiatrists, and psychologists, into a field that has now become a major concern in international public health. The World Mental Health Casebook will serve as a resource for those engaged in the widening efforts to address the burden of mental disorders in low-income countries. The case studies contained here describe: the'Shanghai Model'of psychiatric rehabilitation for persons with schizophrenia, an extraordinary mental health program for poor patients in Lima, Peru who are undergoing treatment for multi-drug resistant tuberculosis, how Guatemalan refugee women in Mexico were trained to become mental health promoters, the development of mental health services in primary care settings in Nepal, the attempts to improve the lives of women in India through literacy programs, the radical reform of psychiatric services in Trieste, Italy, and research from Fiji and Hong Kong that shows how traditional practices can lower the prevalence of post-partum depression. Each case history, through concrete and pragmatic examples, offers lessons that can inform the functioning of services elsewhere. In general, the Casebook, will also encourage others to undertake careful documentation and evaluation of programs so that the mistakes of the past can be avoided, and the successes replicated.

Massive traumatic stress resulting from armed conflict and terrorism is a prime concern of governments, non-government organizations and the United Nations. Yet there is little systematic knowledge of how to address psychological problems of these proportions. These situations are further complicated by the lack of culturally appropriate models for mental health care in many low-income countries. This volume describes a variety of public mental health and psychosocial programs in conflict and post-conflict situations in Africa and Asia. Each chapter details the psychosocial and mental health aspects of specific conflicts and examines them within their sociopolitical and historical contexts. Trauma, War, and Violence: Public Mental Health in Socio-Cultural Context will be of great interest to psychologists, social workers, anthropologists, historians, human rights experts, and psychiatrists working or interested in the field of psychotrauma.

The U.S. Department of Health and Human Services is carrying out an ambitious new program to assure that funds for public health programs are spent as effectively as possible. Under the new program, every state will develop a set of performance objectives to measure its progress in terms of outcomes, processes, and capacity. In the first phase of the program, states are to propose such measures to be achieved over three to five years. This book examines the technical issues involved in the development of performance measures in 10 areas: mental health, substance abuse, HIV infection, sexually transmitted diseases, tuberculosis, immunization, chronic diseases, disability, rape, and emergency medical services. From more than 3,200 candidate measures proposed by researchers, policymakers, and public health professionals, the panel proposes more than 50 potential outcome measures. The book details the advantages and limitations of potential measures as well as the data sources that can support them. This volume will be an invaluable resource to everyone involved in public health.
