After a compound and its various effects - or, as the case may be, any particular one of these - have been discovered there is still a long way ahead until it is available for use in daily practice as a finished product. Before reaching the doctor, the substance is examined by chemists, pharmacologists and pharmacists - just to mention a few of the most important stages in a whole research sequence. Before the drug is finally made available on the market, the findings resulting from animal experiments must be confirmed in clinical studies. The expectations involved in its development must be met, and sufficient evidence has to be established as to the drug's effects and side effects, indications and contraindications, and the questions of safe use and appropriate dosage. Only then may the drug be registered and introduced to the market. In this respect Metipranolol eye drops, which have recently been launched on the market, formed no exception and had likewise to go through all these various stages. A series of investigators at many different centers participated in this procedure, and at the invitation of the manufacturers, Messrs. Dr. Gerhard Mann, they assembled for discussions at a symposium held in Berlin in January 1983, and reported on the substance and their experiences with it.

This volume of Advances in Myocardiology is derived from a part of the proceedings of the 10th Congress of the International Society for Heart Research, which was held in Moscow on September 23-29, 1980. This book contains selected papers which have been arranged in two sections, Cardiac Hypertrophy, Adaptation, and Pathophysiology and Cardiac Hypoxia, Is­ chemia, and Infarction. The first section, on the pathophysiology of heart hypertrophy and failure, contains 24 chapters that focus on the derangement of biochemical, physiological, and immunological processes during the de­ velopment of heart disease due to a wide variety of pathogenic factors. Some of the recent developments in understanding the myocardial synthetic ma­ chinery in heart hypertrophy are also described in these papers, and we believe that the contents of this section will stimulate further research in the area of heart disease. The second section, which mainly deals with myocardial ischemia, contains 35 chapters providing the necessary back­ ground for the diagnosis of ischemic heart disease and some possible ther­ apeutic approaches. It hardly needs to be emphasized that ischemic heart disease is a major cause of death in highly industrialized countries, but unfortunately the exact mechanism by which ischemic insult leads to the development of heart cell damage is far from understood. We are hopeful that the articles in this section will provide valuable information in this field and thus will help in improving the treatment of ischemic heart disease.

Many of the most effective treatments for disease have been discovered empir­ ically. Nowadays, however, we think that understanding the biology of a disease will lead us to design better treatments, and to improve the application of treatments we already have. To accomplish this, vast sums are expended on cancer research. Even so, to the casual observer of clinical oncology the proliferation of studies and trials of ever-different combinations of therapies looks like empiricism, at the best. With the emergence of the biological sciences in the seventeenth and eighteenth centuries, some explanations of congenital malformations with a basis in scientific fact were formulated. It was not, however, until the thalidomide tragedy in 1961 that the necessity for further study of the effects of environmental factors on congenital malformations was recognized. By the early 1970s concentrated efforts had begun to be directed toward evaluating the effects on the unborn child of mood-altering drugs taken during pregnancy. Over the past decade, research and treatment programs have led to the recognition of a full range of psychoactive drugs that affect the developing fetus either somatically or behaviorally. Programs for the treatment of the pregnant drug addict have been developed which concentrate not only on the pharmacologic therapy of the woman but also on the social and psychologic characteristics that place her and her unborn child at risk. This book is an attempt to bring together clinicians and researchers who have been active in developing programs for the recognition and management of the chemically dependent pregnant woman and her newborn. The contributions of the multiple disciplines which the authors represent emphasize the need for a multifactorial approach to the problems of drug use and abuse during pregnancy.
