Four years ago the National Institutes of Child Health and Human Development (NICHD) brought together a group of scientists to Belmont, Maryland to examine the status of human milk banking. During those deliberations, the idea was generated to organize a series of research conferences concerning human lactation and the composition and biological effects of human milk. The first one, organized by Robert G. Jensen from the University of Connecticut and Margaret C. Neville from the University of Colorado, dealt with methodologic issues. An additional meeting to explore the effects of maternal and environmental factors upon human lactation and the composition of human milk was organized by Margit Hamosh from Georgetown University and me, and was held in January, 1986 in Oaxaca, Mexico. Those meetings provided the foundation for the design of the present conference,'The Effects of Human Milk Upon the Recipient Infant'. In addition to a grant from the NICHD, the conference was generously supported by Milupa AG from the Federal Republic of Germany; Wyeth Limited and Mead Johnson of Canada; and Ross Laboratories, Heinz USA, the Mead Johnson Nutritional Group, Wyeth International Limited, Gerber Products Company, the La Leche League International, Glaxo Incorporated and Sandoz Pharmaceutical Corporation from the United States.

This book provides a comprehensive review of our knowledge of vitamin B-6 requirements and metabolism throughout pregnancy and lactation. It concentrates on two primary points: adequacy of available methodologies and a presentation of the extant knowledge on vitamin B-6 metabolism and requirements in pregnancy, infancy, lactation, and in women of childbearing age. Many of the chapters offer new perspectives on how decisions have been and are being made with regard to the establishment of dietary recommendations about vitamin B-6 and the adequacy of research to support these decisions.

The National Academies of Sciences, Engineering, and Medicine last reviewed the state of the science on nutrition during pregnancy and lactation 30 years ago. The resulting consensus study reports from the Institute of Medicine—Nutrition During Pregnancy (IOM, 1990) and Nutrition During Lactation (IOM, 1991)—summarized the scientific evidence and provided nutrient recommendations. In the decades since the release of these two reports, the body of evidence on the relationships between nutrition during pregnancy and lactation and maternal and infant health and chronic disease has continued to grow and evolve. At the same time, the demographics of the population have shifted, giving rise to new considerations. To explore the evidence that has emerged, the National Academies conducted a 2-day workshop in January 2020. This publication summarizes the presentations and discussions from the workshop.

Almost every aspect of energy and nutrient metabolism is altered by hormonal and other physiological changes during pregnancy and lactation. While it is evident that hormonal adjustments affect nutrient requirements, these are rarely considered when nutrient recommen­ dations are made for pregnant or lactating women, and often neglected during evaluation of nutritional status. In addition, changes in nutrient metabolism during the stages of pregnancy and oflactation are usually considered separately, while in reality events during pregnancy can have a major influence on nutritional status and nutrient requirements during lactation. The purpose of this volume is to describe changes in the metabolism of important nutrients during pregnancy and lactation, including the physiological basis for these changes and their implications for nutrient requirements and assessment. Authors have considered such issues as inter-relationships between endocrine changes and nutrient metabolism at the tissue, cellular and molecular level; alterations in nutrient binding proteins; the efficiency of nutrient absorp­ tion and retention; and the impact on maternal as well as fetal nutritional status. Another unique aspect of this book is the focus on pregnancy and lactation as a continuum.

On the basis of a comprehensive literature review and analysis, Nutrition During Lactation points out specific directions for needed research in understanding the relationship between the nutrition of healthy mothers and the outcomes of lactation. Of widest interest are the committee's clear-cut recommendations for mothers and health care providers. The volume presents data on who among U.S. mothers is breastfeeding, a critical evaluation of methods for assessing the nutritional status of lactating women, and an analysis of how to relate the mother's nutrition to the volume and composition of the milk. Available data on the links between a mother's nutrition and the nutrition and growth of her infant and current information on the risk of transmission through breastfeeding of allergic diseases, environmental toxins, and certain viruses (including the HIV virus) are included. Nutrition During Lactation also studies the effects of maternal cigarette smoking, drug use, and alcohol consumption.

Milk is the one food that sustains life and promotes growth in all newborn mammals, including the human infant. By its very nature, milk is nutritious. Despite this, it has received surprisingly little attention from those interested in the cultural impact of food. In this fascinating volume, Stuart Patton convincingly argues that milk has become of such importance and has so many health and cultural implications that everyone should have a basic understanding of it. This book provides this much-needed introduction. Patton's approach to his subject is comprehensive. Breast-Feeding: Early Influences on Later Health is a new book which draws together areas of research in early lifel programming of adult health, with a unique focus on the post-natal period in terms of early life programming particularly the extent to which differences in infant feeding practices can lay an indelible imprint on metabolism and behaviour, and hence affect later function and risk of disease. This is an area where there is much less information currently available than there is for fetal programming, and the book raises many new questions and highlights numerous areas where further research is needed. Considerable advances have been made in science in order to understand the varied mixture of bioactive components in human milk. The 94th Nestlé Nutrition Institute Workshop was designed to provide a comprehensive overview of the latest findings in human milk research and its potential to modulate mucosal immunity, the microbiome, and its impact on the neonate. Breast feeding has a great impact on the growth of infants both physically and psychologically. Human breast milk is beneficial to infant health because it contains the necessary macro- and micro-nutrients for tissue accretion, repair and behavioural developments. The production of milk is a complex biological process and its composition and volume is dependent upon a variety of factors such as the health and dietary status of the mother. Moreover, it is influenced by the different stages and duration of breast feeding. Environmental factors, both global and local, may also alter lactation, milk composition and nutritional value. Growth and nutrition during the fetal period and the first 24 months after birth are important determinants of development in early childhood. Optimal nutrition and health care of both the mother and infant during these first 1000 days of an infant's life are closely linked to growth, learning potential and neurodevelopment, in turn affecting long-term outcomes. Children with low birth weight do not only include premature babies, but also those with intrauterine growth restrictions who consequently have a very high risk of developing metabolic syndrome in the future. Epidemiology, epigenetic programming, the correct nutrition strategy and monitoring of outcomes are thus looked at carefully in this book. Iodine is an important trace element and an essential substrate for the synthesis of thyroid hormones. Iodine deficiency can lead to goiter, hypterthyroidism, mental retardation and impaired growth. In this new book, the authors present topical research in the study of the characteristics, sources and health implications of iodine. Research on omega-3 fatty acids has come a long way since its beginnings in the middle 70's: Starting with studies on the role of omega-3 fatty acids in the secondary prevention of cardiovascular disease, interest soon turned to the mechanisms of and the need to balance the omega-6 to the omega-3 ratio for homeostasis and normal development. Today, it is widely accepted that docosahexaenoic acid (DHA) and arachidonic acid are essential for brain development during pregnancy, lactation and throughout the life cycle. It is also no longer controversial that DHA can affect brain function, mental health and behavior, and studies on supplemental DHA in age-related macular degeneration have revealed significant interactions between DHA and genetic variants. Finally, it addressed the question of whether a distinct diet during pregnancy could reduce food allergy in the progeny. This book is a valuable and complete source of knowledge for all professionals involved in pediatrics, nutrition policy, women's health, immunology, epidemiology and public health.

In Part I of Nutrition During Pregnancy, the authors call for revisions in recommended weight gains for pregnant women. They explore relationships between weight gain during pregnancy and a variety of factors (e.g., the mother's weight for height before pregnancy) and places this in the context of the health of the infant and the mother. The National Academies Food Forum hosted a public workshop in August 2023 that explored the state of the science on the linkages between dietary patterns and diet-related chronic disease across the lifespan. Seafood--including marine and freshwater fish, mollusks, and crustaceans--is a healthy food choice, but it can also contain contaminants. It is currently unclear how much seafood children or pregnant and lactating women are consuming, and what impact seafood consumption is having on children's growth and development. The U.S. Department of Health and Human Services, the U.S. Department of Agriculture, the U.S. Environmental Protection Agency, and the National Oceanic and Atmospheric Administration tasked the National Academies of Sciences, Engineering, and Medicine with convening an expert committee to examine associations between seafood intake for children, adolescents, and pregnant and lactating women and child growth and development. The committee also evaluated when to conduct risk-benefit analyses (RBAs), while considering contextual factors such as equity, diversity, inclusion, and access to health care, and explored how these factors might impact RBAs.
