This Guide assesses the key clinimetric attributes in the assessment of Parkinson's Disease (PD), with the intention to offer rapid and pragmatic information on the most relevant scales used in PD. Parkinson's disease affects approximately 4 million people globally and is most commonly seen in people over the age of 50. The disease is a progressive disorder of the nervous system, and presents a number of movement and cognitive symptoms, thereby greatly affecting a patients quality of life. The use of scales for assessment in neurological disorders such as PD arises from the need to quantify disorders and states (such constructs as disability, symptoms, quality of life). Assessment scales are often categorised into two categories: generic (i.e. those scales usable in any health condition), and specific (i.e. scales developed for exclusive use in PD). They can have a variety of components: single-item and multi-item or composite scale; unidimensional and multidimensional; and as disease-centered and patient-centered measures. The creation and validation of scales is complex, with scales undergoing numerous studies to assess criteria such as acceptability, reliability, and responsiveness. In the process of validation of a scale the following attributes should be tested to ascertain whether a scale is an effective instrument of measurement.

In recent years, considerable advances have been made in our knowledge and understanding of Parkinson's disease (PD). In particular, there has been an explosion of information regarding genetic contributions to the etiology of PD and an increased awareness of the importance of the non-motor features of the disease. We had to know first that a hormone has adaptive value before we could ask whether this is due to a syntoxic or a catatoxic mechanism. Such observations, as the fact that an indomethacin-induced intestinal ulcer can be prevented by ethylestrenol, orthat cortisol aggravates certain infections, reveal nothing about how these hormones work; but only findings of this type can tell us where further research would be rewarding. Of course, scientists can rarely identify by direct observation the tbings that they are looking for; most of the time they have to be guided by indirect indices. The ebernist often first detects a compound, or even a particular functional group in its molecule, by inference from a color reaction, a revealing X-ray diffraction pattern or the formation of a characteristic precipitate. The physician must first suspect the presence of a microbe through certain clinical signs and symptoms before he can verify his diagnosis by looking for a particular organism. It is perhaps not too daring to hope that in our first efforts to clarify the role of hormones in resistance, simple, directly visible indicators might also serve us best.

Chronic deep brain stimulation (DBS) has been a rapidly evolving area of neurotherapeutics since its initial introduction for the treatment of Parkinson's disease and essential tremor in the 1990s. For these conditions, DBS is now considered accepted therapy for patients failing to adequately respond to m- ical treatment. Since the 1990s, new clinical indications, anatomic targets, and technologies have contributed to an expanding role for DBS in the treatment of other movement disorders such as dystonia and Tourette syndrome as well as for other neurologic disorders such as epilepsy and cluster headache. Early experience has also been reported for psychiatric syndromes, such as obs- sive–compulsive disorder and depression.
