“Toward Precision Assessment and Psychotherapy: Understanding Individual Differences through Neurobiology, Genetics, and Epigenetics” provides a transformative approach to the understanding of mental health in the 21st century. It does so by encouraging the replacement of traditional subjective techniques with more precise and objective measures that are designed to discover individual differences in our patients. Just as in personalized medicine, where diagnostic and treatment techniques are becoming more targeted toward the specific conditions of a particular patient, so to in the mental health field we need to develop targeted and focused methods through the use of clinical science concepts and through the adoption of an agnostic, unassuming posture where the presentation of each patient is viewed as unique. For example, treatments may draw from an integrative approach that incorporates genomic and epigenomic analyses of the patient to inform a more targeted medication regimen or psychotherapy approach (some psychiatrists are beginning to use genomic analysis to aid in the choice of a specific antidepressant, for example, that would most likely be effective for a certain patient). Diagnostically, paper and pencil questionnaires leading to current diagnostic labels, long the standard, must be replaced, for the most part, by objective measures of stress that prioritize “reports from the body”. Additionally, research methods of extremely complex psychological disorders can be enhanced by breaking them down into smaller subtypes (or endophenotypes) versus large phenomenologically-based symptom clusters (i.e. DSM diagnoses). Individual differences, a consistent focus throughout the book, may be the product of genetic variants and/or epigenetic mechanisms. The latter mediate gene X environment interaction that is now the primary paradigmatic lens through which we investigate and seek to explain resultant behavioral profiles. A major focus of the book is on the effects of early adversity, particularly trauma, and how they contribute to a “re-programming” of the brain through epigenetic alterations of gene expression. This results in increased vulnerability to possible psychological disorders, such as various anxiety conditions, depression, and character disorders which may manifest later in life. By identifying the epigenetic effects caused by early stressors, that is, how gene expression is altered, we can eventually make significant advances in primary prevention. This book represents an attempt to move us into a transitional domain and beyond, where causes and the treatment of psychological disorders are re-conceptualized through our developing insights from neurobiology, genetics, and epigenetics.

Disorders of the peripheral nervous system (PNS) are the cause of prominent neurological symptoms including weakness, sensory loss, pain and autonomic dysfunction associated with deficits, morbidity and mortality. These disorders may be primary hereditary or cryptogenic neurologic disorders confined to the PNS or part of the pathology of both the central nervous system and the PNS. Most PNS disorders are secondary to other system disorders and may be responsive to treatment of the primary disease. The regulation of the organism has traditionally been ascribed to two distinct systems-the nervous and the endocrine. Though coordination between the two systems has been acknowledged, researchers and authors have tended to deal with them as comprising separate categories of cells involved in different activities. With this approach, a given regulatory mechanism would be evaluated as to whether it should be accounted for by nervous or endocrine functions. The past 15 years, however, have witnessed numerous important discoveries and conceptual developments concerning the morphological, physiological, and bio­ chemical relations between the nervous and endocrine systems. Advances in im­ munocytochemical studies have revealed that there are a wide variety of messenger substances that function in both regulatory systems. As a result, researchers have been stimulated to investigate neuronlike properties of endocrine cells and, con­ versely, endocrine or secretory features of neurons. It has thus become obvious that the rigidities in the classic criteria of neurotransmitters and hormones may rather impede further advances in these research fields. The activities of neurons are no longer evaluated simply in terms of EPSP, IPSP, and the release of classic trans­ mitters such as acetylcholine, noradrenaline, and GABA. Hormonal actions are no longer analyzed solely with regard to concentrations of classic aminic and peptidic hormones in the systemic blood circulation. The concept of the paraneuron, which we proposed in 1975, has become one of the theoretical bases for the development of this trend of study.

In 1985, volume 74 of the Springer-Verlag Handbook of Experimental Phar­ macology, under the editorship of H. -H. Frey and D. Janz, appeared. In this volume the then available data on the topic of antiepileptic drugs were col­ lated and analysed. Over the intervening years knowledge in this area has grown progressively. More new antiepileptic drugs than the total number of agents that were in common use 15 years ago have in the interval either come on to the market or are about to do so. As well, further agents are at a fairly advanced stage of development, whilst the already established drugs have by and large held their places in clinical practice. Knowledge of epileptogenesis has advanced considerably. The mechanisms of action of antiepileptic drugs at the molecular level and in various animal models of epileptic seizures and of the epileptic state are much better understood than they were previously. As well, more information is available concerning the natural history of human epilepsy, and this knowledge is important in making optimal use of the various agents that are now available. Therefore, it has seemed appropriate at this stage in the evolution of knowledge to produce a second volume dealing with Antiepileptic Drugs in the Handbook of Experimental Pharmacology series.

ELLIOTT M. BLASS Fifteen years have passed since the first volume on developmental psychobiology (Blass, 1986) appeared in this series and 13 since the publication of the second volume (Blass, 1988). These volumes documented the status of the broad domain of scientific inquiry called developmental psychobiology and were also written with an eye to the future. The future has been revolutionary in at least three ways. First, there was the demise of a descriptive ethology as we had known it, to be replaced first by sociobiology and later by its more sophisticated versions based on quantitative predictions of social interactions that reflected relatedness and inclu­ sive fitness. Second, there was the emergence of cognitive science, including cogni­ tive development, as an enormously strong and interactive multidisciplinary effort. Making the'functional'brain more accessible made this revolution all the more relevant to our discipline. In the laboratory, immunocytochemical detection of immediate / early genes, such as los, now allows us to trace neuronal circuits activated during complex behaviors. The'functional'brain of primates, especially humans, was also made very accessible through neuroimaging with which we can look at and into brains as they solve and attempt to solve particular tasks. Those of us who were trained in neurology as graduate students two or three decades ago recognize only the people in white coats and patients in beds or on gurneys when we visit neurologi­ cal units today. The rest is essentially new.

This volume of Cerebral Cortex is dedicated to Sir John Eccles, who was an active member of the advisory board for the series until his death in May 1997. His input as to what topics should be covered in future volumes of this series will be sorely missed. The present volume is concerned with neurodegenerative disorders and age­ related changes in the structure and function of the cerebral cortex, a topic that has attracted increasing interest as longevity and the number of aged individuals in the population increase. Although much of the research on the neurodegenerative effects of aging has been centered on Alzheimer's disease, most of the aging popu­ lation will not be afflicted by this disease. They will, however, be affected by the consequences of normal aging, so the first few chapters of this volume are con­ cerned with that topic. Chapter 1, by Marilyn S. Albert and Mark B. In 1957 Otto SCHAUMANN, one of the pionieers in pharmacological research on morphine and the first to prepare synthetic opiates, presented a mono­ graph entitled'Morphin und morphiniihnliche Verbindungen'as Volume 12 of the Handbook of Experimental Pharmacology.
