The third Symposium of the Foundation for Life Sciences was held in February 1983 at the Newport Inn Conference Centre in Sydney. It was direced towards an understanding of the molecular neuropathology of muscle and nerve under a wide variety of conditions that may be induced by external agents or genetic lesions. The first session on experimental neurology explored the processes involved in maintenance of nerve and muscle function. This included many papers on myelination, studies on immune reactions affecting nerves, on synapses, and on neuronal development. This section was expanded to explore the control of muscle function in nerves, including a discussion on cross reinnervation. Toxic models of disease in the nervous system were then discussed, including pathological states induced by physical agents such as kainic acid, diphtheria toxin, and IDPN. A new dimension was added to the Symposium when for the first time psychologists participated and contributed to the session on external stressors and their effects on behavior. Heavy metals, herbicides, repetitive work, anxiety, and their effects on behavior and health were all represented. The discussion in this session attracted much interest from the participants, particularly the basic scientists.

It is an honor to be asked to write a foreword for this timely book, which assembles contributions from authorities working in the area of pathological nervous system cell death, and has been expertly edited by Vassilis Koliatsos and Rajiv Ratan. That the inap­ propriate demise of brain or spinal cord cells is at the root of many neurological diseases has been appreciated since the early days of microscopic neuropathology, but it has only been in the last decade or so that pervasive therapeutic nihilism has lifted. In the journey of medical progress, we have reached the shores of a marvelous new land. Three major scientific thrusts in particular have converged to produce the press of ideas covered here. First, burgeoning information about the fundamental nature of central nervous system cell--(;ell signaling, both the fast signaling mediated by conventional neu­ rotransmitters and the usually slower signaling mediated by neuromodulators and growth factors. A central theme emerging in recent years has been the duality of these signaling mechanisms, which serve the nervous system in health, but which can become the very mediators of neuronal or glial cell degeneration in disease settings. Glutamate-mediated neurotransmission and excitotoxicity have been the defining and best-studied examples, but many other examples have also emerged. Second, delineation of the molecular under­ pinnings of programmed cell death, and an appreciation of their awesome power.

The symposium from which this book originates represents a sig­ nificant watershed in the field of intraoperative neural monitoring, since the participants concluded that electrophysiologic monitoring techniques should be considered a'standard of care'for surgical pro­ cedures that place the central nervous system (CNS) at risk for injury. Specifically, it was agreed that the somatosensory-evoked potential (SEP) is a remarkably reliable and sensitive indicator of several aspects of CNS function, and should be routinely employed as an intraopera­ tive monitor during many neurosurgical and orthopedic procedures. The significance of this conclusion cannot be overstated, for at the time of this writing, intraoperative monitoring methods based on evoked-potential analyses are still considered experimental and are not in routine use. The reasons for this are not clear, given the accu­ mulation of literature and expertise on this subject over the past five years. Granted, the cost of electrophysiological monitoring equip­ ment is high, but only initially. The benefits of injury prevention far outweigh these costs, from both medical and economic viewpoints. It is our sincere hope and goal that the medical community be made aware of the value of intraoperative neural monitoring.

Much of our knowledge of the cerebral circulation has been derived from studies of species other than human. There is increas­ ing recognition of species differences and concern that studies in animals may be misleading if unquestioningly applied to the human. A dramatic example of this occurred in the early his­ tory of the study of the circulation of the brain. Galen of Pergamo (131-201 AD) described a rete mirabile or'marvelous network'of blood vessels at the base of the human skull that he must have derived from observations of certain animals. This vascular structure was supplied by the carotid arteries which, after penetrating the cranium,'are divided into a large number of very small and thin branches in the region between the cranium and the dura matter. In fact, these numerous arteries rejoin and unite like the roots of a tree trunk....'The authority of Galen's writings dominated scientific thought for about 1500 years.
