It is well known that certain diseases and patient conditions are associated with increased perioperative risk. GERD is the most common gastrointestinal disease in the world affecting over 10-20% of the adult population. GERD is often perceived as a Western disease and very little literature available about this disease in the Asian population. With growing obesity and westernization in Asia, the prevalence of GERD is rapidly increasing. With growing morbidity and health care cost there is a significant need for understanding the disease from the Asian perspective. This book would fill the gap in the knowledge on GERD among Asians and will highlight the difference in terms of epidemiology, diagnosis and management of GERD from the Western perspective. Conformal radiation therapy represents a new challenge for radiation oncologists. It offers the prospect of either increasing the radiation dose to target tissues while delivering a similar dose to organs at risk, or reducing the dose to organs at risk while maintaining the dose to target tissues. First, lymph node areas at risk are established using the available data from pathological examination of surgical specimens and/or pattern of locoregional relapse. Then, based on a three-dimensional description of the anatomical regions where the areas at risk are located, guidelines for the delineation of the clinical target volumes are proposed. The Fifth World Congress of the International Society for Diseases of the Esophagus was held in the historic city of Kyoto, Japan, from August 5 through 8, 1992. Approximately 40 countries throughout the world participated and roughly 500 presentations were made. Excellent authors were selected and they were requested to send in their manuscripts for publication of this book. It is our ardent hope that this book will prove to be beneficial to the doctor interested in the esophagus and that it will provide the reader with first-hand information from leading scientists and clinicians in this field. The incidence of esophageal diseases vary greatly from country to country and in recent years, worldwide interest in these diseases has resulted in various international studies. The International Society for Diseases of the Esophagus was inaugurated by Professor Komei Nakayama in 1979 and since that time it has actively contributed to the exchange of information regarding these diseases and has made endeavors in bringing about advancement in the struggle against diseases of the esophagus in every way possible.

The radiation therapist's primary concern is the treatment of patients with malignant dis­ ease. However, there are definite indications for radiation treatment for benign diseases that do not respond to conventional methods of treatment. It may be the treatment of choice in the unusual instance of a life-threatening benign disease that cannot be surgi­ cally or medically managed. The present volume by Order and Donaldson represents a major statement on the uti­ lization of radiation techniques in the management of benign disease. The initial report of the Committee on Radiation Treatment of Benign Disease from the Bureau of Radiological Health recommended that consideration be given to the quality of radiation, the total dose, overall time, underlying organs at risk and shielding factors before the institution of radiation therapy. Infants and children should be treated with ionizing radiation only in very exceptional cases and after careful evaluation of the potential risk compared with the expected benefit. Direct irradiation of the skin areas overlying organs that are particularly prone to late effects such as the thyroid, eye, go­ nads, bone marrow, and breast should be avoided. Meticulous radiation protection tech­ niques should be used in all instances and the depth of penetration of the x-ray beam should be chosen in accordance with depth of the pathologic process'.

The third triennial congress of the International Society for Diseases of the Esophagus (lSDE) was held in Munich in September 1986 under the presidency of J. Rudiger Siewert. The very well organized and epoch-making meeting was part of the International Esophageal Week and was held in conjunction with the International Conference of Esophageal Diseases, the European Study Group of Diseases of the Esophagus (GEEMO), and International Organization for Statistical Studies of Esophageal Diseases (OESO). I am very pleased that it has been possible to publish the proceedings of the congress so rapidly. The problems associated with diseases of the esophagus are multifaceted, varying considerably according to locality; their study thus neces­ sitates a large international forum, which is one of the founding aims of the ISDE. We hope that this volume, by virtue of the in-depth and up-dated reports it contains, can playa role in advancing the work being done in this field. Owing to its central position in the retroperitoneal and pelvic cavity, the ureter is rapidly affected by a variety of pathological processes. Furthermore, any disease involving the ureter can also have an effect on the kidneys. For several decades, the diagnosis of diseases affecting the ureter was based on intravenous excretory urography and techniques of direct opacification. However, the diagnostic strategy has been extensively modified by the advent of helical computed tomography and magnetic resonance imaging. Endoscopy has revolutionized clinical gastroenterology. In 1961 Basil Hirschowitz published the first flexible endoscopic examination of the stomach and duodenal bulb. We moved from flexible fiberendoscopes to current video-endoscopic equip­ ment. Current video-endoscopes incorporate a black and white or color'chip'at the tip of the instrument which transforms the visual image into electronic signals. The size of the pincet is constantly getting smaller, heading for 5 /Lm, further increasing the resolution. The signals are reassembled into high-quality color images in a video monitor. Endoscopes are now used to examine the entire gastrointestinal tract from esophagus to rectum, including the biliary and pancreatic ductal system. Targeted endoscopic biopsy offers rapid and precise diagnosis. Endoscopic ultrasonography is of unsurpassed accuracy in staging gastrointestinal tumors, in assessing pancreatic and biliary disease, and disorders of the rectum and anal canal. Moreover, targeted cytological sampling is possible of abnormalities of the intestinal wall or peri­ intestinal lymph node. Yet despite these glamorous achievements changes are to be expected in the overall emphasis of diagnostic endoscopy. Magnetic resonance cholangiopancreatography may very well compete for a substantial fraction of diagnostic ERCP. Virtual colonoscopy or computed tomographic colography may well compete with (and take over?) screening/surveillance colonoscopy.

The significant advances in medical, surgical and social care during the last two decades in the western world have resulted in a considerable prolongation of life expectancy. However, on the other hand, aged populations present a high incidence of neoplasias ideally requiring surgical or multidisciplinary therapeutic strategies. The decision making for providing the appropriate surgical care in these patients is a difficult and multivariant task.
