Sudden death in athletes is a global problem. Although it is a relatively rare phenomenon (1/100,000 persons), when it does occur, it is often as an incomprehensible event. In fact, it strikes subjects who presumably should be much healthier than the general population. In the previous 20 years, many authors have studied this problem in an attempt to understand the causes and prevent these events, and it has been determined that, in the vast majority of cases, athletes who die suddenly have an underlying heart disease (arrhythmogenic cardiomyopathy, hypertrophic cardiomyopathy, coronary anomalies, channelopathies, etc.).In most cases these diseases do not produce major symptoms and do not preclude sports activity even at the highest levels, although they do increase the incidence of sudden death. How to discover these diseases in asymptomatic athletes is a hotly debated issue. In particular, there is controversy as to whether all athletes should undergo detailed medical screening, including electrocardiogram, or whether the costs of this screening are too high in relation to the event incidence. Although winning at a sport is important, this is only so if it is achieved in natural physical conditions and with the correct training. Whether that be in the field or Emergency Department, general ward, operating suite or intensive care unit, all patients carry a common theme of potential life-taking processes which must to identified and treated in a timely fashion or severe morbidity and death rapidly follow. Since the cardiovascular system subserves the body in maintain­ ing metabolic stability through global and regional blood flow at an adequate pressure to insure appropriate autoregulation of blood flow distribution, it would be difficult to describe the mechanisms of cardiovascular instability their diagnosis and treatment without placing them within the context of overall metabolism and tissue vi­ ability. Accordingly, this book has been grouped into four arbitrary subsets. First, we address issues of basic cardiovascular physiology. Classic developments of ventricular pump function and arterial re­ sistance are balanced with newer applications of ventriculo-arterial coupling, right ventricular function, and tissue oxygen delivery.

Sudden death is probably the greatest challenge facing modem cardiology today. This is mainly due to the impact of its brusque appearance and the socioeconomic implications. The incidende is presently decreasing somewhat, mainly due to the decline in new cases of ischemic heart disease on one hand, and to better preven­ tion in risk patients on the other. Nevertheless, the figures are still high and represent over 300.000 patients per year in the United States alone. This book is an updating of the problem of sudden death from a multifactorial standpoint. It includes not only electrophysiologic data but also covers aspects ranging from epidemiology to prevention. Risk markers and triggering mechanicsms of sudden death are reviewed, with special emphasis on the role of electrical instability, ischemia and depressed ventricular function. The book includes the contributions from many experts, often pioneers in their respective fields. It is our hope that the book serves as an Updating for those cardiologist who are not specialized in these subjects, but may also be of interest to the expert. The field of cardiac arrhythmias has been evolving so fast during the last years that scientific meetings are frequently necessary to present technological advances, to communicate results of relevant and innovative researches, to assess the impact of recently developed diagnostic and therapeutical tools, to discuss controversial aspects, and to reach a consensus on the most appropriate evaluation and management of specific problems. This is the main reason why in 1988 we started to organize a biannual International Workshop on Cardiac Arrhythmias. Since then many editions of the workshop have taken place and over the years the fame and popularity of the event have increased continuously. People age 65 and older are the fastest growing segment of the U.S. population. In the 2010 census 16% of the population, 50 million people, were age 65 and older. That number is projected to increase to 66 million by the year 2050. Life expectancy has also increased, with recent CDC reports indicating life expectancy at 77.9 years. Age-adjusted death rates have decreased significantly with the largest changes occurring in older patients.
