Shortly after Langenbuch performed the first cholecystectomy in 1877 in Berlin, the first bile duct injury associated to cholecystectomy occurred and was published. For decades frequency maintained low, until the presentation of laparoscopic cholecystectomy, when the number of cases complicated with bile duct injury doubled. A coinfecção por hepatites virais permanece como causa importante de complicações e mortalidade em indivíduos infectados pelo HIV. O objetivo deste trabalho foi avaliar a prevalência e fatores associados à coinfecção pelo vírus da hepatite C (HCV) e presença de fibrose hepática por métodos não invasivos. Foi realizada uma análise transversal retrospectiva avaliando 4443 pacientes infectados pelo HIV acompanhados na coorte do INI/FIOCRUZ entre março de 2012 e novembro de 2016. A prevalência da coinfecção pelo HCV encontrada nesta população foi de 4%. O compartilhamento de agulhas e maior tempo de infecção pelo HIV foram significativamente associados com a infecção pelo HCV. A EHT mediana foi 8,0 (6,6-11,9) kPa e 63% (n=67) apresentavam fibrose significativa. Ausência de resposta virológica sustentada prévia e maior tempo de infecção pelo HCV foram associados com fibrose significativa em modelo ajustado para idade e sexo. A prevalência da fibrose foi menor quando definida por biomarcadores (20%) do que aquela definida pela EHT. Pudemos concluir que 4% dos indivíduos infectados pelo HIV apresentam coinfecção pelo HCV e que 67% dos pacientes coinfectados pelo HIV-HCV apresentam fibrose hepática significativa pela EHT. É necessário melhores estratégias de saúde pública no sentido de garantir maior acesso ao tratamento do HCV para redução da transmissão e da incidência de complicações hepáticas em pacientes infectados pelo HIV.

The cirrhotic liver is significantly more vulnerable to injury. This vulnerability is particularly evident after the advent of portal hypertension. In the normal liver, up to 75 percent can be resected without decompensation. By contrast, in the cirrhotic liver with portal hypertension, liver decompensation can occur even after less than 50 percent of the liver is resected. This book, “Portal Hypertension: New Insights” begins with an attempt to provide insights into this unanswered puzzle. The normal liver is soft and elastic. By contrast, the cirrhotic liver is inelastic and rigid. Flow in rigid tubes is governed by the Hagen-Poiseuille law. The law dictates that pressure gradient increases exponentially by the fourth power of the radius of the rigid sinusoid. This makes the cirrhotic liver virtually incapable of accommodating any increase in flow. The increase in pressure gradient is equivalent to the fourth power of the vessel radius. The first chapter explores the major new insight that decompensation in the cirrhotic liver is related to the loss of elasticity and the development of vessel wall rigidity, which thereafter subjects flow in the hepatic sinusoids to the Hagen-Poiseuille law. The rest of the book addresses portal hypertension in its clinical context. It invites the reader to survey multiple avenues associated with portal hypertension, including idiopathic non-cirrhotic portal hypertension, INCPH and non-alcoholic fatty liver disease, NAFLD. Endoscopic access to the small bowel has advanced significantly since the introduction of video capsule endoscopy and deep enteroscopy in early 2000. Other major advances have occurred in imaging modalities involving computed tomography and magnetic resonance studies. Due to these advances, the recent 2015 ACG guideline changed the terminology from “obscure” to “small bowel” bleeding because the majority of cases now can be found to have a small bowel source. The improvements in technology have advanced our ability to visualized vascular findings, inflammatory lesions, and small bowel neoplasms. Articles in this issue are devoted to these improvements in technology.

Alcohol abuse remains a significant problem world-wide. The most commonly affected organ remains the liver with a risk of alcoholic liver disease (ALD) which can range from asymptomatic to alcoholic hepatitis to alcoholic cirrhosis. In 2010, alcohol-attributable cirrhosis was responsible for 0.9% of all global deaths. Given the sheer magnitude of alcohol abuse and alcoholic liver disease, an update on this topic is pertinent and relevant. This issue will focus on a variety of topics including alcoholism, the pathogenesis of ALD, the spectrum of ALD, the pathology of ALD, and the long-term management of patients with ALD. Upper gastrointestinal bleeding is the leading emergency leading to hospitalization and urgent endoscopy. The field of gastrointestinal bleeding is rapidly evolving. The epidemiology is changing with more complex older patients on anticoagulant and antithrombotic agents presenting with upper gastrointestinal bleeding. The initial management has rapidly evolved with new transfusion thresholds, the use of risk stratification scores and no more nasogastric tubes. There is new data and recommendations on optimal timing of endoscopy.
