Le groupe francophone des myélodysplasies signe ici la 4e édition de cette monographie. Au cours des dernières années, les mécanismes d'initiation et d'évolution ont été mieux compris dans ces pathologies, grâce à l'avènement des nouvelles méthodes de biologie moléculaire, notamment le séquençage à haut débit du génome. Outre la nécessaire mise à jour des connaissances sur le diagnostic et les traitements des syndromes myélodysplasiques (SMD), les spécialistes français de ces maladies proposent leurs recommandations pour la pratique clinique. Depuis l'édition de 2010, si le pronostic de ces affections touchant le sujet âgé reste basé sur le score IPSS et sa révision récente (IPSS révisé), d'autres facteurs pronostiques émergent, principalement la présence de mutations géniques. Les auteurs développent à l'attention des hématologistes, internistes et autres hospitaliers non hématologistes leurs propositions de consensus pour une attitude thérapeutique dans les SMD. Dans les SMD de faible grade, la correction des cytopénies reste la base du traitement, et de nouvelles thérapeutiques apparaissent dans ce domaine. Pour les SMD de haut grade, si l'allogreffe de cellules souches hématopoïétiques reste le seul traitement curatif, chez les patients non allogreffables les agents hypométhylants sont devenus le traitement de référence. Les liens avec les affections dysimmunitaires et plus globalement la contribution du système immunitaire dans le développement des SMD sont décrits, pouvant déboucher sur de nouvelles voies thérapeutiques.
