Approximately 7.4 million people in the United States live with an intellectual or developmental disability (IDD), defined by the Centers for Disease Control and Prevention as'a group of conditions due to an impairment in physical, learning, language, or behavior areas. These conditions begin during the developmental period, may impact day-to-day functioning, and usually last throughout a person's lifetime.'Individuals with IDD and their caretakers face exceptional barriers to staying healthy and accessing appropriate health services. Among these barriers are difficulty finding care providers that are adequately trained in meeting their specialized needs, unwieldy payment structures, and a lack of coordination between the various systems of care with which patients with IDD may interact (e.g., education, social work, various segments of the health care system). The National Academies of Sciences, Engineering, and Medicine hosted a workshop to discuss promising innovations in (1) workforce development, (2) financing and payment, and (3) care coordination; and to share visions for improved systems of care. Participants noted that while many existing approaches could serve as models for improving care, large changes will need to be made in these 3 facets of the care system in order to make them accessible to all IDD patients. This publication summarizes the presentations and discussions of the workshop.

Embodied Politics illuminates the influential force of public health promotion in indigenous migrant communities by examining the Indigenous Health Project (IHP), a culturally and linguistically competent initiative that uses health workshops, health messages, and social programs to mitigate the structural vulnerability of Oaxacan migrants in California. Embodied Politics reconstructs how this initiative came to exist and describes how it operates. At the same time, it points out the conflicts, resistances, and counter-acts that emerge through the IHP's attempts to guide the health behaviors and practices of Triqui and Mixteco migrants. Arguing for a structurally competent approach to migrant health, Embodied Politics shows how efforts to promote indigenous health may actually reinforce the same social and political economic forces, namely structural racism and neoliberalism, that are undermining the health of indigenous Oaxacans in Mexico and the United States.

Oral health care and medical health care both seek to maintain and enhance human health and well-being. Yet, dentistry and primary care in the United States are largely separated and isolated from each other. Each has its own siloed systems for education, service delivery, financing, and policy oversight. The result has been duplication of effort, a cultural gap between the two professions, and lost opportunities for productive collaboration and better health. On December 6, 2018, in Washington, DC, the National Academies of Sciences, Engineering, and Medicine held a workshop titled Integrating Oral and General Health Through Health Literacy Practices. The Institute of Medicine (IOM) Committee on Quality Measures for the Healthy People Leading Health Indicators was charged by the Office of the Assistant Secretary for Health to identify measures of quality for the 12 Leading Health Indicator (LHI) topics and 26 Leading Health Indicators in Healthy People 2020 (HP2020), the current version of the Department of Health and Human Services (HHS) 10-year agenda for improving the nation's health. The scope of work for this project is to use the nine aims for improvement of quality in public health (population-centered, equitable, proactive, health promoting, risk reducing, vigilant, transparent, effective, and efficient) as a framework to identify quality measures for the Healthy People Leading Health Indicators (LHIs). The committee reviewed existing literature on the 12 LHI topics and the 26 Leading Health Indicators. Quality measures for the LHIs that are aligned with the nine aims for improvement of quality in public health will be identified. When appropriate, alignments with the six Priority Areas for Improvement of Quality in Public Health will be noted in the Committee's report. Toward Quality Measures for Population Health and the Leading Health Indicators also address data reporting and analytical capacities that must be available to capture the measures and for demonstrating the value of the measures to improving population health. Toward Quality Measures for Population Health and the Leading Health Indicators provides recommendations for how the measures can be used across sectors of the public health and health care systems. The six priority areas (also known as drivers) are population health metrics and information technology; evidence-based practices, research, and evaluation; systems thinking; sustainability and stewardship; policy; and workforce and education.

Today, local, state, and federal funding agencies have recognized the importance of the role the community plays in assuring a healthy population. For all of our advances in health care and achievements in public health that have contributed to an increased life expectancy, there still remain persistent problems in one's community that are public health in origin. These persistent public health problems are well-studied from an academic standpoint, yet they evade a solution that is practical, economical, and socially just. These are complex, community-based, public health problems that disproportionally impact minority and immigrant populations. This book explores this topic in greater detail. Specifically, what have we, as public health academicians and practitioners learned when partnering with minority and immigrant community members to help them address persistent public health issues that affect them and their families on a daily basis? These are lessons that need to be shared so disadvantaged populations can build their capacity to address and solve persistent public health problems in their communities.

Although drinking, smoking and obesity have attracted social and moral condemnation to varying degrees for more than two hundred years, over the past few decades they have come under intense attack from the field of public health as an'unholy trinity'of lifestyle behaviours with apparently devastating medical, social and economic consequences. Indeed, we appear to be in the midst of an important historical moment in which policies and practices that would have been unthinkable a decade ago (e.g., outdoor smoking bans, incarcerating pregnant women for drinking alcohol, and prohibiting restaurants from serving food to fat people), have become acceptable responses to the'risks'that alcohol, tobacco and obesity are perceived to pose. Community development, planning and partnerships have become important terms in health promotion but, up until now, debate around these concepts have been discussed more in planning science than in public health literature. Much of the debate about health policy in the United States has focused on the availability of health insurance coverage and the relatively large number of individuals who are uninsured. While tackling the problem of the uninsured might improve access to and utilization of health care, it would likely have little effect on the health of the population, as there is only a weak connection between health insurance coverage and health. Expanding health insurance coverage alone is unlikely to significantly improve the health of the population or narrow health disparities within the population, given that many of the major causes of poor health such as smoking, obesity, and physical inactivity are largely unaffected by health insurance. The narrow focus on the uninsured in the health policy debate comes at the expense of other policies that could improve health faster and more significantly for every dollar spent. It is well known that the United States spends approximately twice as much per capita on health care as most other developed nations, but that there is little difference in population health between the United States and other developed nations. This suggests that we are on the'flat part of the curve'of health care spending with respect to health and as a result need to pursue other approaches for improving population health. In light of the imbalance in the health policy debate in the US, in November 2007, the Institute of Government and Public Affairs and the College of Medicine of the University of Illinois sponsored a conference entitled, Beyond Health Insurance: Public Policy to Improve Health. The purpose of the conference was to make available to the public new research on policies that can significantly improve the health of the US population. The conference focused on four areas: reducing racial and ethnic health disparities, preventing disease and promoting health, developing and regulating pharmaceuticals, and improving consumer information.

Improving health in populations in which health is poor is a complex process. This book argues that the traditional government approach of exhorting individuals to live healthier lifestyles is not enough - action to promote public health needs to take place not just through public agencies, but also by engaging community assets and resources in their broadest sense. The book reports lessons from the experience of planning, establishing and delivering such action by the five-year Sustainable Health Action Research Programme (SHARP) in Wales. It critically examines the experience of SHARP in relation to current literature on policy; community health and health inequalities; and action research. The authors make clear how this regional development has produced opportunities for developing general concepts and theory about community-based policy developments that are relevant across national boundaries and show that complex and sustained community action, and effective local partnership, are fundamental components of the mix of factors required to address health inequalities successfully.
