As we move into the 21st century it is becoming increasingly difficult to offer appropriate introductory clinical experiences for medical students. Many schools offer clinical experiences in the first year of medical school, when the learner has little background in the traditions and origins of the doctor-patient interaction. Die großen Fortschritte der Kinderheilkunde in den letzten Jahrzehnten sind nur zum kleinen Teil Allgemeingut der Ärzte geworden, weil die Ausbildung in diesem Fache vielfach noch unzulänglich ist und ihr klinisches Denken auf der Universität nicht genügend ausgebildet werden konnte. Die ins Leben hinaustretenden jungen Ärzte, die nicht eine besondere Fachausbildung erworben haben, machen regelmäßig die Erfahrung, daß ihre auf der Universität er­ worbenen Kenntnisse den Anforderungen der Praxis nicht entsprechen. So erklärt sich das Bedürfnis vieler Ärzte, die in der Studienzeit unbefriedigend gebliebene Ausbildung im Selbstunterrichte nachzuholen, wozu in erster Linie die Erwerbung einer sicheren Diagnostik nötig ist. Der Wunsch der Herausgeber der Enzyklopädie der klinischen Medizin, eine Diagnostik der Kinderkrankheiten in den Rahmen ihrer Bücher aufzunehmen, erschien danach wohl berechtigt. Ich entsprach ihrer Aufforderung gerne, da ich persönlich von dem Nutzen einer solchen, nach gewissen Gesichtspunkten gerichteten Darstellung als Ergänzung der Lehrbücher, überzeugt bin. Aus einer 15jährigen Hauspraxis, aus langjährigem Unterricht an den großen Kinder­ kliniken in Heidelberg und Zürich und aus meiner Konsiliartätigkeit glaube ich mit den Schwierigkeiten wohl vertraut zu sein, die dem Studierenden und dem praktischen Arzte hier Schritt auf Tritt begegnen.
