Um nur einiges herauszugreifen: Für die Genese des juvenilen Diabetes wird die Infektion mit dem Coxsackie­ Virus nicht mehr in Betracht gezogen. Dagegen hat sich unbezweifelbar zeigen lassen, daß die Erzeugung eines Diabetes im Tierversuch durch Virusinfektion an immunopathologische Vorgänge und genetische Faktoren geknüpft ist. Die Genese des menschlichen, akut entstehenden Insulinmangel-Diabetes auf immu­ nologischer plus hereditärer Basis wird weiterhin in der Diskussion bleiben. Pathogenetische Erkenntnisse sind zu erhoffen. In Zusammenhang damit steht die genetische Verbindung der Insulin-Antikörperproduktion bei insulinbedürf­ tigen Diabetikern mit Histokompatibilitäts-(HL-A)-Antigenen, die das Diabetes­ risiko erhöhen, andererseits aber Schutzfunktionen für die Diabetesentstehung signalisieren können. Ein weiteres Problem, dem sich die wissenschaftliche Arbeit in naher Zukunft mit Sicherheit widmen wird, ist die Frage nach der Bedeutung des Somatostatin und der D-Zelle im Inselsystem, in der dieses produziert wird. Hier wird sich die Frage der Sicherung der von OReI und UNGER entwickelten Hypothese der Insel als eines multihormonellen Mikroorganismus stellen, in dem nebeneinander Insulin, Glukagon und Somatostatin gebildet werden und gegenseitige regulato­ rische Funktionen ausüben.

Survival of extremely premature neonates has improved significantly following the advances in neonatal intensive care. Extrauterine growth restriction is a serious issue in this population. Nutritional exposures during critical period of life influence the individual's risk of disease throughout life. Nutritional deficit and poor growth are associated with long term neurodevelopmental impairment, short stature and metabolic disorders in extremely preterm neonates. Optimising nutrition in the early postnatal life of the preterm neonate is therefore a priority. However this is easier said than done considering the frequency of feed intolerance, fear of necrotising enterocolitis, and the hesitancy in adopting an aggressive approach to parenteral nutrition in this population. Some of the finest researchers in the field have come together to provide the clinical perspective on the A to Z of nutrition in the preterm neonate in simple and clear fashion in this book.

Die chronische Pyelonephritis entsteht definitionsgemäß als Folge einer bakteriellen Infektion. Sie wird wegen der Art der morphologischen Organschäden zur Gruppe der interstitiellen Nierenentzündungen gezählt. Es handelt sich offenbar um eine verhältnismäßig häufige Krankheit, da sie bei 40 ~ der Kinder und bei etwa 20 ~ der Er­ wachsenen mit dialysepflichtiger Niereninsuffizienz als Grundleiden angenommen wird (112). Obwohl sich viele Un­ tersuchungen mit dieser Erkrankung befaßt haben, sind wesentliche Fragen der Ätiologie, Pathogenese und Klinik noch ungeklärt geblieben. So ist zum Beispiel noch völlig offen, ob es beim Menschen ebenso wie im Tierexperiment eine chronische bakterielle Nephritis auch ohne die Einwirkung'prädisponierender'Faktoren gibt (12,14,23,24,35,36,38,67,117,118,186,192, 194,293,298). Auch der Einfluß einer bakteriellen Infektion auf die Form der morphologischen Veränderungen in der Niere und den Verlauf ~es Leidens ist umstritten (6,10,11,16,17, 26,37,75,77,84,90,95,103,121,131,157,179,199,200,202,203, 211,229,239,265,271,282,296,313,328,329,332,338,342,349, 350,352). Nach wie vor ist ebenfalls ungeklärt, welche Me­ chanismen dafür verantwortlich sind, daß auch in bakterio­ logisch sterilen Nieren die Entzündung nicht ausheilt und in der chronischen Phase der Pyelonephritis zu einem allmählich fortschreitenden Verlust an Nierenparenchym führt (58,59,84,95,131,182,228). Weiterhin herrscht Un- klarheit über die Bedeutung verschiedener Nierenfunktions­ proben für Diagnose und Verlaufskontrolle der chronischen Pyelonephritis. Ein weiteres klinisch wichtiges Problem stellt die Beziehung zwischen chronischer Pyelonephritis und Hochdruckkrankheit dar. Bei etwa 20% aller Hochdruck­ kranken finden sich Hinweise auf eineNierenerkrankung. Die Hälfte dieser Nierenkranken soll an einer bakteriellen Nephritis leiden. Somit besteht die Möglichkeit einer kau­ salen Beziehung zwischen chronischer Pyelonephritis und arterieller Hypertonie (2,37,48,83,105,129,148,219,263, 309,317,339,356,357).
