Thirty years later the measurements of the peptides, especially NT-proBNP, has not met all of the expectations of a simple and useful tool in cardiology; the mechanical stress on the heart does not explain all of the findings made in clinical studies. The association between AIDS and cancer was recognized from the beginning of the AIDS epidemic, when the appearance of Kaposi sarcoma in a cluster of young men was one of the first signs of this new disease. It was soon recognized that AIDS was caused by infection with a novel virus (HIV) and that AIDS patients are prone to develop a number of “AIDS-defining” cancers: Kaposi sarcoma, lymphoma, and cervical cancer. The development of effective combination anti-HIV therapy starting around 1996 converted AIDS from a death sentence to a manageable disease and led to dramatic shifts in the epidemic. As this therapy was able to improve immune function in patients, the incidence of most “AIDS-defining” cancers decreased. There is a misconception, however, that AIDS has gone away. In fact, as AIDS patients are living longer, the number of AIDS patients has more than doubled in the United States since 1996, and the AIDS population overall has increased in age. Also, as AIDS patients are less likely to die of other complications, cancer is coming to the forefront as one of the most common causes of death in regions where AIDS drugs are widely available. Moreover, the three “AIDS-defining” cancers are now taking a back seat to a number of other HIV-associated cancers, such as Hodgkin lymphoma, lung cancer, and anal cancer. In the developing world, AIDS-associated cancers are a major public health problem, and in some regions of sub-Saharan Africa, Kaposi sarcoma is the most common tumor in men.In recent years, there has been a vast increase in our understanding of HIV-associated cancers. The Epstein-Barr Virus (EBV), also known as Human Herpes Virus 4, has been identified and described as the etiological agent of infectious mononucleosis. It is the first human virus found to be implicated in oncogenesis. EBV is associated with a number of biologically diverse cancers, mainly lymphoproliferations of B-cell origin. With the development of basic research and clinical medicine, new ideas related to the pathogenesis and clinical aspects of EBV-associated diseases have arisen recently. A significant progress in the number of hematopoietic stem cell and solid organ transplantations performed worldwide, involving high-risk patients, has contributed to an increase in the development of EBV-associated malignant post-transplant lymphoproliferative disorders. In the stem cell transplant setting, a post-transplant lymphoproliferative disorder has, in most cases, the nature of a secondary cancer. Apart from post-transplant lymphoproliferative disorders, other types of lymphoproliferative disorders are recognized and presented in this book. The main aim of this book is to provide readers with an overview of the impact of EBV on parameters that contribute to the development of malignancy in immunocompromised patients, particularly in recipients of a solid organ or hematopoietic stem cell transplant. The recent developments in modern vaccinology are mainly based on: (i) cloning of microbial genes into recombinant vectors containing genetic information for expression of desired neutralizing immunogens; (ii) alternatives of attenuated vectors with deleted genes permitting the insertion of several foreign genes expressing antigens exposed to the host immune system during the abortive replication of such vectors; (iii) combined vaccines with the aim to protect against many diseases with a limited number of administrations; (iv) evidence demonstrating the ability of animals to respond serologically to DNA injections considered as a potential method of vaccination; (v) the possibility to manipulate the immune system with new and improved immunomodulators enhancing the immune response; and (vi) new microcarrier systems for particular immunogens or immunomodulators delivery, either in a single dose or sustained release, and presentation to the immune system for a relevant response. The Reverse Transcriptase (RT) of Human Immunodeficiency Virus Type 1 (HIV-1) arguably ranks amongst one of the most extensively studied retroviral enzymes. Heterologous expression and purification of HIV-1 RT in the early eighties, approval of the first nucleoside analogue RT inhibitor (NRTI) in 1987, discovery of resistance to RT inhibitors, approval of the first non-nucleoside analogue RT inhibitor (NNRTI) in 1996 and the various crystal structures of RT with and without bound substrate(s) and/or inhibitors represent only a few of the important milestones that describe the a bench-to-bedside success in the continuing effort to combat HIV-1 infection and its consequences. Nucleoside and nonnucleoside RT inhibitors remain important components in frequently used drug regimens to treat the infection. RT inhibitors also play important roles in recently validated strategies to prevent transmission of the virus. The relevance of HIV-1 RT as a drug target has simultaneously triggered interest in basic research studies aimed at providing a more detailed understanding of interactions between proteins, nucleic acids, and small molecule ligands in general terms. In light of the ever-growing knowledge on structure and function of HIV-1 RT, this enzyme serves as a valuable “model system” in efforts to develop novel experimental tools and to explain biochemical processes. This monograph is designed to provide an overview of important aspects in past and current HIV-1 RT research, with focus on mechanistic aspects and translation of knowledge into drug discovery and development. Beginning his work on the monograph to be published in English, this author tried to present more or less general notions of the possibilities of mathematics in the new and rapidly developing science of infectious immunology, describing the processes of an organism's defence against antigen invasions. The results presented in this monograph are based on the construc­ tion and application of closed models of immune response to infections which makes it possible to approach problems of optimizing the treat­ ment of chronic and hypertoxic forms of diseases. The author, being a mathematician, had creative long-Iasting con­ tacts with immunologists, geneticist, biologists, and clinicians. As far back as 1976 it resulted in the organization of a special seminar in the Computing Center of Siberian Branch of the USSR Academy of Sci­ ences on mathematical models in immunology. The seminar attracted the attention of a wide circle of leading specialists in various fields of science. All these made it possible to approach, from a more or less united stand point, the construction of models of immune response, the mathematical description of the models, and interpretation of results.
