In the last few years, human metabolic disorders have become some of the most prevalent and prejudicial diseases worldwide. Hepatic steatosis, among many health problems, has several risk factors related to the disease prognosis such as obesity, nutritional problems, diabetes, hypertension, hyperuricemia and even sleep apnea. Alcohol abuse also can lead to a severe liver-steatosis state. The progression of hepatic steatosis can lead to complications such as cryptogenic cirrhosis and hepatocellular carcinoma. Drug therapy has instead been focused on the mitigation of risk factors, however, new potential therapeutic perspectives are being discussed. This book presents the origins, diagnosis and epidemiology of hepatic steatosis with new perceptions. The authors also aim to show the molecular basis and signaling involved in liver disorders, discussing the inflammatory process, lipid metabolism and angiogenesis mechanisms. The role of nutrition in the treatment and development of steatosis was also presented and new advances were highlighted. Recent studies revealed the need for the use of bariatric surgery, functional foods and the modulation of endogenous systems, such as the sirtuins and renin-angiotensin systems, to treat liver disease. Obesity is now considered a global epidemic. Central obesity is the state of excess adiposity in the abdominal region. It is associated with increased, cardiovascular or cancer mortality. The International Diabetes Federation (IDF) has recommended the assessment of central obesity mandatory for diagnosis of metabolic syndrome. Hitherto, there is a huge gap in our understanding of the role of central obesity as a health risk across different populations around the world. There also exist a vast population variation in the relationship between generalised obesity as measured by body mass index and /or percent body fat and the central adiposity measures. For instance, Asian-Indians have a typical phenotype of higher body fat at lower BMI levels compared to their European counterparts. Development and use of region specific standards of measurement of central obesity is in need until a universal agreement is achieved. With this background, the present volume has been organized with some very informative articles from highly reputed scholars, researching on obesity in general and on central obesity, in particular. We have here articles based on empirical data as well some review articles critically examining some theoretical, epidemiological and clinical aspects pertaining to the theme of this volume. Ce premier'Rapport de recommandations sur la prise en charge des personnes infectées par les virus de l'hépatite B ou de l'hépatite C'est le fruit d'une démarche collective impulsée par la Ministre de la Santé et réalisée sous l'égide de l'Agence nationale de recherches sur le sida et les hépatites virales (ANRS) et de l'Association française pour l'étude du foie (AFEF). Ce rapport a été coordonné par le Pr Daniel Dhumeaux (hépatologue au CHU Henri Mondor de Créteil et président du Programme national de lutte contre les hépatites B et C 2009-2012). Pour produire ses recommandations, il a mobilisé pendant plus d'une année quelque 200 cliniciens, chercheurs, patients et associations de patients. L'élaboration du rapport s'est organisée autour de 22 groupes thématiques d'experts et d'un comité indépendant des experts, fait de personnalités scientifiques et du monde associatif, dont la mission a été d'assurer la validation et la synthèse des textes. Par les mesures qu'il met en avant, ce rapport se propose, selon trois axes : – de relancer la prévention des hépatites B et C, jugée en retard sur plusieurs points (vaccination contre l'hépatite B notamment), – d'articuler les étapes de la prise en charge des patients atteints d'hépatites B et C selon des parcours de soins et des stratégies thérapeutiques spécifiques qui leur sont propres, – de soutenir, à tous les niveaux, les valeurs d'équité dans les soins par des mesures concrètes de lutte contre les inégalités sociales. En émettant ces recommandations, le collectif responsable du rapport a eu clairement conscience de la contrepartie économique des traitements nouveaux, en particulier de l'hépatite C, qui implique des choix et des indications raisonnables. Avec des thérapeutiques dont l'efficacité est désormais proche de 100%, l'éradication de cette infection peut devenir un objectif réaliste. En l'absence de vaccin, elle ne passera que par l'identification et la prise en charge de tous les patients atteints et des coûts de médicaments réduits. Ce premier rapport, destiné aux pouvoirs publics, à tous ceux qui luttent contre ces maladies (professionnels de santé, associations de patients, industriels du médicament) et aux médias, apporte un état des lieux épidémiologique, sanitaire et social de l'épidémie dans notre pays et émet des recommandations fortes pour être plus efficace à tous les échelons de la riposte. Il est souhaité qu'il donne lieu à une véritable mobilisation collective afin que ces hépatites soient maîtrisées à tous les niveaux de notre société, y compris dans les groupes les plus vulnérables et les régions les plus touchées.

Today, the cellular and molecular mechanisms of adipogenesis are well known.
