This paper explains how Iran developed its nuclear programme to the point where it threatens to achieve a weapons capability within a short time frame, and analyses Western policy responses aimed at forestalling that capability. Key questions are addressed: will the world have to accept an Iranian uranium-enrichment programme, and does having a weapons capability mean having the Bomb? For nearly two decades, Western strategy on the Iran nuclear issue emphasised denial of supply. Since 2002, there has also been a demand-side dimension to the strategy, aimed at changing Iran's cost–benefit calculations through inducements and pressure. But the failure of these policies to prevent Iran from coming close to achieving a nuclear-weapons capability has promoted suggestions for fallback strategies that would grant legitimacy to uranium enrichment in Iran in exchange for intrusive inspections and constraints on the programme. The paper assesses these ‘second-best'options in terms of their feasibility and their impact on the proliferation risks of diversion of nuclear material and knowledge, clandestine development and NPT break-out, and the risk of stimulating a proliferation cascade in the Middle East and beyond. It concludes that the risks are still best minimised by reinforcing the binary choice presented to Iran of cooperation or isolation, and strengthening denial of supply.

This book summarizes the results of two symposia held on hydroxy-pyridones as antifungal agents organized on the occasion of the International Summit on Cutaneous Antifungal Therapy 1996 in Vancouver and the 19th World Congress of Dermatology 1997 in Sydney. Some of the papers deal with the basic mechanisms of action and how these translate into product features, e.g. activity spectrum. Other papers describe the unique properties of hydroxy-pyridones, such as high penetration into and through keratinic tissues or their activity as dual inhibitor on 5-lipoxygenase and cyclooxygenase which leads to a remarkable antiinflammatory effect. Last but not least various accounts are given which show how this unique product profile aids the treatment of various fungal diseases, especially onychomycosis.

The international symposium'New Trends in Allergy', held in Munich from July 13 to 15, 1990, brought together for the third time since 1980 some of the most experienced researchers working in the field of allergy. This volume comprises the papers presented at this meeting. All over the world, and not merely in the industrialized countries, allergy is becoming a cause of evermore serious diseases. In recent years, research in the field of allergy has provided numerous impor­ tant and fascinating results extending our knowledge considerably. Despite the new insights into basic mechanisms of allergic reactions, improved diagnostic methods, and new therapeutic approaches, how­ ever, many questions remain to be answered, including: Are allergies really increasing in frequency? If so, what are the reasons? Especially, does environmental pollution playa role? Which factors influence IgE synthesis? Can the IgE immune response be switched off? Does the nervous system interact with allergic reactions? If so, what are the mechanisms? Are new approaches in allergy prophylaxis and allergy therapy effi­ cient? What measures have proven useful and deserve to be employed in daily practice? In this volume, these questions and other current topics are dealt with. As each issue is covered by authors competent in the respective fields, the result is an extensive and critical review of the state of the art. Going through these papers, one comes to the conviction that allergy research is a multifacetted, explosively expanding, most stimulating field of work.

This book presents a collection of academic papers based on the concept of apoptosis (natural cell decay). In the beginning, the ensuing research revolved around the skin peeling effect caused by sunburn, searching for possible alternatives to mitigate this phenomenon that, with increased frequency, can become a known catalyst for skin cancers on susceptible areas of the body. However, the scope of this project gradually expanded to explore a possible correlation between the natural cycle of apoptosis and the development of multiple cancer entities. In addition to the discussion of UVR exposure as a common risk factor, the book ponders the relevance of apoptosis in relation to a multitude of well-known pathogens such as heavy metal poisoning, continuous exposure to electromagnetic fields, potential hazards of new methods of information management and communication, overuse of chemical pesticides, the biological and botanical effects of radioactive contamination, among others. In the early 20th century, tanned skin was associated with good health. However, people began to protect themselves against potential overexposure to avoid sunburns. Around 1945, the first sunscreen products became available. The concept of expressing acidity as the negative logarithm of the hydrogen ion concentration was defined and termed pH in the beginning of the 20th century. The general usefulness of the pH concept for life science was recognized and later gained importance to analytical research. Reports on results of pH measurements from living skin established the term acid mantle – the skin's own protective shield that maintains a naturally acid pH. It is invisible to the eye but crucial to the overall wellbeing of skin. Chronic alkalization can throw this acid mantle out of balance, leading to inflammation, dermatitis, and atopic skin diseases. It is therefore no surprise, that skin pH shifts have been observed in various skin pathologies. It is also obvious that the pH in topically applied preparations may play an important role. Optimal pH and buffer capacity within topical preparations not only support stability of active ingredients and auxiliary materials, but may also increase absorption of the non-ionized species of an acidic or a basic active ingredient. They may even open up opportunities to modify and “correct” skin pH and hence accelerate barrier recovery and maintain or enhance barrier integrity. Further efforts are needed to standardize and improve pH measurements in biological media or pharmaceutical/cosmetic vehicles to increase and ensure quality, comparability, and relevance of research data. In this volume, we present a unique collection of papers that address past, present and future issues of the pH of healthy and diseased skin. It is hoped that this collection will foster future efforts in clinical and experimental skin research.

Basic research on the pharmacology of itch has exploded in the wake of two very influential papers that were published in Nature (2007) and Science (2009). Long overlooked as a milder form of pain, itching has rapidly gained a new appreciation in both research and clinical communities because of its complexity and its negative effects on the quality of life of the distressed patients. Like pain, not all itches are the same. Unlike pain, there are no standard drugs equivalent to aspirin and morphine. Epidemiological studies emphasize the high incidence and economic costs of itch (pruritus). It is the most prevalent symptom of a wide variety of allergic and inflammatory skin conditions (e.g., psoriasis, atopic dermatitis), is associated with several systemic diseases (e.g., chronic kidney and liver disease), and occurs in patients undergoing hemodialysis, spinal administration of opioids, and in those suffering from AIDS. The reader will learn about the multiple pathways for itch and their interactions with pain. The relationship between these closely related, yet distinct sensory phenomena, will be emphasized. Both itch and pain use several common molecules to send signals to the brain. Thus, drugs that have been, and are being, developed as analgesics may also attenuate intractable itch. This has been an exciting and very necessary turn of events since traditional H-1 receptor antagonists are ineffective in blocking the pruritus associated with kidney failure and cholestasis. The clinical chapters will provide insights into contemporary treatment regimens for pruritus in different human scenarios.

There are many human cancers which actively synthesize specific characteristic proteins such as melanomas, thyroid cancer and squamous cell carcinoma. Many cancer researchers have of course tried to utilize this specific activity as a key for the selective treatment of cancers. In the past for example, the molecular hybrid compound of DOPA, a substrate of melanin, and nitrogen mustard N-oxide hydrochloride, a ctyotoxic anti-tumor drug, was synthesized as Melphalan and used to treat malignant melanoma. A major problem arose though in that it was soon found to be highly suppressive toward bone marrow and quite toxic while not being remarkably effective. Thus, malignant melanoma could not be cured by it. Such failure led us to develop a novel bimodal therapeutic system which includes the use of non-toxic potentially cytocidal chemicals which selectively accumulate within the cancer cells and which are converted by a controllable modality into an actively cytocidal element in situ. We can now non-surgically cure malignant melanoma and glioblastoma with our selective cancer treatment, neutron capture therapy (NCT); as can be found in this volume. Included are 124 papers on the latest breaking developments discussed at the Sixth International Symposium on NCT for Cancer held in Kobe during the late autumn of 1994.

Where do you begin to look for a recent, authoritative article on the diagnosis or management of a particular malignancy? The few general oncology textbooks are generally out of date. Single papers in specialized journals are informative but seldom comprehensive; these are more often preliminary reports on a very limited number of patients. Certain general journals frequently publish good in-depth reviews of cancer topics, and published symposium lectures are often the best overviews available. Un­ fortunately, these reviews and supplements appear sporadically, and the reader can never be sure when a topic of special interest will be covered. Cancer Treatment and Research is a series of authoritative volumes that aim to meet this need. It is an attempt to establish a critical mass of oncology literature covering virtually all oncology topics, revised frequently to keep the coverage up to date, easily available on a single library shelf or by a single personal subscription. We have approached the problem in the following fashion: first, by dividing the oncology literature into specific subdivisions such as lung can­ cer, genitourinary cancer, pediatric oncology, etc.; second, by asking emi­ nent authorities in each of these areas to edit a volume on the specific topic on an annual or biannual basis. Each topic and tumor type is covered in a volume appearing frequently and predictably, discussing current diagnosis, staging, markers, all forms of treatment modalities, basic biology, and more.

Radiation therapy of cutaneous cancers and other dermatologic disorders is not covered adequately in many current textbooks of dermatology and radiation oncology. This book is intended to fill that gap. Both text and illustrations are oriented toward the practical aspects of radiation therapy. The beginner will find a concise introduction to physical and biological principles, selec­ tion of radiation factors, dose definitions, indications for treatment, and radiation seque­ lae. There is no doubt among experts that the prevalence of allergic diseases has increased in many industrialized countries in recent years. The rea­ sons for this increase are unknown; only suppositions exist. Many people focus on environmental influences. However, the assumption that air pollution alone is responsible for this increase seems to be too simple: many other influences, including the genetic predisposition of individual patients, allergen exposure, and possibly socioeconomic factors, also have to be taken into consideration. Although our understanding of the complex mechanisms of allergic diseases has considerably improved thanks to the progress made in ex­ perimental immunology and allergology, we still have a long way to go before this scientific knowledge is translated into new therapeutic mo­ dalities. For this reason, the scientific community welcomed the gathering of scientists from very different disciplines and different parts of the world at an international symposium,'New Trends in Allergy IV'together with'Environmental Allergy and Allergotoxicology III'in Hamburg in 1995. This volume contains the invited papers, covering a wide range from basic science to practical clinical diagnosis and therapy. A further unique feature of this event was the concomitant first official workshop of the Environmental Pollution and Allergy Committee of the International Association of Allergy and Clinical Immunology (IAACI), at which the state of scientific knowledge in this field was defined and formulated.

It is a great pleasure for me to see this work in print. As outlined by Dr. Traupe, at least 20 different types of ichthyosis have been identified to date. This book will effectively help to establish a correct diagnosis, as a basis for genetic counseling. Dr. Traupe proposes a new classification of the ichthyoses, based on reason­ able clinical criteria. He lets the reader share in his enormous knowledge and safely guides him through the difficult field of nomenclature. He describes the particular nature of these scaling disorders in a manner as simple as possible. During the last few years, research on ichthyosis has become a fascinating scientific challenge due to the introduction of novel investigative approaches. The main advantage of this book lies in the fact that the author successfully combines recent achievements made in lipid analysis, enzymology, electron microscopy, and molecular genetics. Heiko Traupe began his career in dermatology 11 years ago, as a resident in the Department of Dermatology in Munster. As we were both interested in the genetic aspects of skin diseases, this was the starting point of a most fruitful collaboration, which is still continuing. During our common work in Munster, Dr. Traupe was able, thanks to his unceasing clinical curiosity, to describe many novel aspects within the field of ichthyosis, and I took pride in coauthoring papers on new items such as cryptorchidism as a fea­ ture associated with X-linked recessive ichthyosis, autosomal dominant lamellar ichthyosis, and alopecia ichthyotica.

This book is based upon a series of papers originally presented at a NATO Advanced Studies Workshop of the same title held at the study Center HI Cappuccini,'a converted monastery in San Miniato, a small village located between Pisa and Florence, Italy, in October 1991. Authors were asked to submit their completed chapters by the following February; these were then scanned onto computer disks, edited and returned to the authors for final revision, and updating, with a final deadline of February 1993. The authors were encouraged to make whatever modifications they wanted, especially regarding updating their chapters, with an eye to making the final product both comprehensive and current. In this we succeeded beyond our expectations, with most chapters extensively altered and many completely re-written and significantly expanded. Thus, although the original meeting was held in 1991, the chapters in this volume may be regarded as current from at least February, 1993, with some of the final updated revisions received as late as July, 1993. This book, as agreed in our original contract, has been delivered to the publisher'camera-ready.'This means that all of the scanning, editing, proofing and typesetting were done here, by the office of the Department of Dermatology at the New Jersey Medical School. We essentially produced the book, which the publisher, for the most part, then photocopied. This has been an enormous burden, borne mainly by my colleague in this division, Robert A.

Books mark the progress of Man since they were invented. Through them we are able to gain insight into the minds of our predecessors better than through any other medium. They describe how the delicate interplay between practice and ideal, which is better known as evolution, has brought forward the societies in which we now live. A book marks the synthesis of knowledge in a different way from individual papers. A certain maturity and volume of understanding and knowledge is necessary before the material is suitable for a book. The timing of the cognitive and analytical synt- sis represented by a book is therefore crucial; too soon and it is lost in speculation, too late and it is old news. Non-melanoma skin cancer is common; it causes morbidity, it causes a burden on society, and treatment has been traditionally almost exclusively surgical. Decades of medical science have however now brought forward a number of techniques which may help both the diagnosis and treatment of skin cancer without physically removing it, either alone or in combination in treatment programs tailored to the individual patients.

The antiphospholipid syndrome (Hughes syndrome) is now over a quarter of a century old. Although most of the major clinical features were described in the original papers between 1983 and 1985, the ensuing 25 years have seen a filling out of the clinical detail and recognition that the syndrome is of major medical importance. More significantly, much has been learnt of the prevalence and epidemiology of the syndrome as well as the biology of the thrombotic process so central to the condition. Sadly, all that lags behind is treatment, the choice of drugs still very limited. The syndrome now embraces all disciplines. It is now regarded as the commonest treatable cause of recurrent pregnancy loss, and has changed the face of obstetrics. It has become a major chapter in the field of neurology, being an important cause of stroke, migraine, seizures, and memory loss. In cardiology, it is at last becoming recognized as an important cause of young heart attacks, as well as providing leads in the study of accelerated atheroma. And so the list goes on – idiopathic bone fracture, abdo- nal angina, renal vascular hypertension, leg ulcers… This book, produced by clinical leaders in the field, brings together the many strands of another “great mimic”.

In recent years the development of new technologies has permitted the production of'functional'or'smart'textiles. These fabrics are capable of sensing changes in environmental conditions or body functions and are adequately responding to them. They are able to absorb substances from the skin or to release therapeutic or cosmetic compounds. For instance, they can be used in underwear with an integrated cardio-online system or as textiles with carrier molecules. The focal point of interest in biofunctional textiles lies currently on the use of textiles supporting therapy and prevention in dermatology. This volume collects information about new trends in the interaction between textiles and the skin, particularly the development of antimicrobial finished textiles. It presents a selection of papers which will contribute to further consolidate the dialogue between dermatologists, allergologists, biomaterial scientists and textile engineers.

This second edition of the popular Classics in Clinical Dermatology with Biographical Sketches, first published in 1953, has been expanded considerably. The proceedings of the Symposium on Acne which took place during the 20th World Congress of Dermatology in July 2002 in Paris review the current and future aspects of research, clinical entities and treatment of acne.
