In recent years there has been considerable interest in the diagnosis and understanding of ventricular repolarisation, particularly the QT interval prolongation and abnormal T and T/U wave morphology associated with torsades de pointes. Advances in ion channel cloning have greatly improved our understanding of the role of ionic channels in mediating cardiac repolarisation. Unfortunately, it is increasingly recognised that a number of drugs, both those associated with altering repolarisation, and others for non-cardiac conditions can increase the propensity for polymorphic ventricular tachycardia, syncope and even ventricular fibrillation and sudden death. In this volume, arrhythmia specialists from St. George's Hospital Medical School, London discuss the mechanisms behind QT prolongation and torsades de pointes. It is the practical experience gained in the operating room and on the wards dealing with complications and deviations from the typical or average scenario that matures and fully develops a surgeon. The typical textbook demonstrates the “right” or standard way to do things and the implicit assumption is that if these guidelines are followed then the patient and the surgeon's life will be complication free. Good results come from experience and experience is gained by making mistakes. Cardiac resynchronization therapy is a new therapy for advanced heart failure patients. This therapy has been shown to improve quality of life, exercise capacity, NYHA classification, and even reverse the detrimental'reverse remodeling'that occurs in advanced heart failure - in patients already receiving optimal pharmacological therapy. The typical patient has NYHA III/IV HF symptoms, a QRS > 120 ms, LVEDD > 55 mm, without any indication required for pacing. The therapy is available in an ICD device, too. CRT has been shown to be effective in patients with both ischemic and non-ischemic cardiomyopathy. The implant procedure is part of a learning curve. Many physicians who already perform electrophysiology procedures, or PPM, or ICD implants are the ideal implanter - but are surprised at the challenges this implant presents. This book will help'flatten'the learning curve for new physician implanting, and will provide a'guide'for those who have already started implanting. The success rate is about 90% - so after 10 years of application, we have found that there are still patients who do not get access to this great therapy because of the complexity of the patient's anatomy, or physician skills. This book bears the fruits of a collaboration between the “pupil” (RDC) and the “mentor” (PL). During an extended sabbatical of the pupil in Boston in 1994, we probed together the concept that endothelial dysfunction served as a common denominator of vascular disease, with the balance between inflammation and its inhibition as a fulcrum of the regulation of the behavior of endothelial cells. As practicing cardiologists in our clinical lives, we sought to link to endothelial function the mechanisms of action of risk factors and of pharmacologic agents used to treat and prevent vascular disease. The pupil therefore authored a few reviews on the mechanism of action of risk factors and included them in a small book, published in Italian, for which the mentor wrote a preface. The book was greeted with favor from the Italian cardiological community, and provided the nidus for the present, more ambitious endeavor, which includes updated reviews on the pathogenesis of vascular disease and on the most novel aspects of vascular biology. This enterprise was enabled by the contributions of many of our former or present collaborators and colleagues, without whose enthusiasm and engagement this work could never have seen light. We largely underestimated the devotion necessary on our own side at the beginning, but it ultimately yielded a product that we feel achieves our original goals.
