The HIV epidemic in Bolivia has received little attention on a global scale in light of the country's low HIV prevalence rate. However, by profiling the largest city in this land-locked Latin American country, Carina Heckert shows how global health-funded HIV care programs at times clash with local realities, which can have catastrophic effects for people living with HIV who must rely on global health resources to survive. These ethnographic insights, as a result, can be applied to AIDS programs across the globe. In Fault Lines of Care, Heckert provides a detailed examination of the effects of global health and governmental policy decisions on the everyday lives of people living with HIV in Santa Cruz. She focuses on the gendered dynamics that play a role in the development and implementation of HIV care programs and shows how decisions made from above impact what happens on the ground.

As global health institutions and aid donors expanded HIV treatment throughout Africa, they rapidly'scaled up'programs, projects, and organizations meant to address HIV and AIDS. The 1992 creation of the National HIV/AIDS Program was a fundamental step for Argentina to reach the second lowest burden of HIV/AIDS in South America. From 2000 to 2010, Argentina further reduced the already low HIV/AIDS incidence of 15.9 per 100,000 by 25 percent and reduced the burden by 21 percent. This study analyzes the national and inter-provincial burden of disease, the demographics of new HIV cases, the demand and supply-sides of service delivery, and conducts a cost-benefit analysis of the National HIV/AIDS Program over the last decade. Though the National HIV/AIDS Program was an instrumental step towards these achievements, this book also examines other key programmatic innovations that have been essential to the country's success in the fight against HIV/AIDS, including the introduction of universal free antiretroviral treatment; a comprehensive legal framework for sexual and reproductive rights; the introduction of incentives and results-based financing in the HIV/AIDS program; electronic monitoring of supplies and medicines; and implementation of an electronic clinical governance system for improving the quality of care and patient follow-up, among others. Despite high costs of the Program, this study found the Argentine National HIV/AIDS Program is cost-beneficial. From 2000 to 2010, 4,379 potential lives were saved. Nonetheless, the fight against this epidemic poses continuous challenges, including a stubbornly high number of new infections among young men who have sex with men, inequalities in HIV/AIDS rates between provinces, insufficient coverage of HIV diagnostic testing, insufficient HIV testing of tuberculosis patients, low expenditure on HIV prevention, high comparative cost of antiretroviral treatment, and questions regarding the long-term financial sustainability of the AIDS program, considering the increasing number of patients in treatment.'Thirty Years of the HIV/AIDS Epidemic in Argentina: An Assessment of the National Health Response'delves into the combination of factors that make Argentina a success story in combating HIV/AIDS.

A comprehensive study of global AIDS activism over the past twenty-five years.Few diseases have provoked as many wild moralistic leaps or stringent attempts to measure, classify, and define risk and treatment standards as AIDS. In Politics in the Corridor of Dying, Jennifer Chan documents the emergence of a diverse range of community-based, nongovernmental, and civil society groups engaged in patient-focused AIDS advocacy worldwide. She also critically evaluates the evolving role of these groups in challenging authoritative global health governance schemes put in place by what she describes as overcontrolling or sanctimonious governments, scientists, religious figures, journalists, educators, and corporations.Drawing on more than 100 interviews conducted across eighteen countries, the book covers a broad spectrum of contemporary sociopolitical issues in AIDS activism, including the criminalization of HIV transmission, the fight against'big pharma,'and the politics of the President's Emergency Plan for AIDS Relief. In the years since the end of apartheid, South Africans have enjoyed a progressive constitution, considerable access to social services for the poor and sick, and a booming economy that has made their nation into one of the wealthiest on the continent. At the same time, South Africa experiences extremely unequal income distribution, and its citizens suffer the highest prevalence of HIV in the world. As Archbishop Desmond Tutu has noted, “AIDS is South Africa's new apartheid.” In Ancestors and Antiretrovirals, Claire Laurier Decoteau backs up Tutu's assertion with powerful arguments about how this came to pass. Decoteau traces the historical shifts in health policy after apartheid and describes their effects, detailing, in particular, the changing relationship between biomedical and indigenous health care, both at the national and the local level. Decoteau tells this story from the perspective of those living with and dying from AIDS in Johannesburg's squatter camps. At the same time, she exposes the complex and often contradictory ways that the South African government has failed to balance the demands of neoliberal capital with the considerable health needs of its population.

The number of people living with HIV/AIDS (PLWHA) in the United States is growing each year largely due both to advances in treatment that allow HIV-infected individuals to live longer and healthier lives and due to a steady number of new HIV infections each year. The U.S. Centers for Disease Control and Prevention (CDC) estimates that there were 1.2 million people living with HIV infection in the United States at the end of 2008, the most recent year for which national prevalence data are available. Each year, approximately 16,000 individuals die from AIDS despite overall improvements in survival, and 50,000 individuals become newly infected with HIV. In 2011, the CDC estimated that about three in four people living with diagnosed HIV infection are linked to care within 3 to 4 months of diagnosis and that only half are retained in ongoing care. In the context of the continuing challenges posed by HIV, the White House Office of National AIDS Policy (ONAP) released a National HIV/AIDS Strategy (NHAS) for the United States in July 2010. The primary goals of the NHAS are to: reduce HIV incidence; increase access to care and optimize health outcomes; and reduce HIV-related health disparities. Monitoring HIV Care in the United States addresses existing gaps in the collection, analysis, and integration of data on the care and treatment experiences of PLWHA. This report identifies critical data and indicators related to continuous HIV care and access to supportive services, assesses the impact of the NHAS and the ACA on improvements in HIV care, and identifies public and private data systems that capture the data needed to estimate these indicators. In addition, this report addresses a series of specific questions related to the collection, analysis, and dissemination of such data. Monitoring HIV Care in the United States is the first of two reports to be prepared by this study. In a forthcoming report, also requested by ONAP, the committee will address the broad question of how to obtain national estimates that characterize the health care of people living with HIV in the United States. The second report will include discussion of challenges and best practices from previous large scale and nationally representative studies of PLWHA as well as other populations.

This report explores the emerging epidemics of HIV among men who have sex with men (MSM) in low and middle income countries (LMIC). A case study approach is used to explore these epidemics in detail in Peru, Ukraine, Kenya, and Thailand. Systematic reviews of epidemiology and prevention science were used to understand these epidemics, and to assess the evidence for interventions in prevention, access to care and treatment, and human rights protections. The Goals model was then used to assess the potential impact at country level of responding to epidemics among MSM. Benefits were seen for overall HIV epidemic control in all four countries with greater investment in MSM prevention, treatment, and care. Costing and cost-effectiveness work found these interventions to be cost effective. Human rights and policy analyses were consistent with the epidemiology and costing—responding to HIV among MSM is both a public health priority and a human rights imperative.

HIV/AIDS is holding firm as one of the worst diseases in history and the leading cause of death in sub-Saharan Africa. This third part of the AWLAE series on HIV/AIDS and agriculture in sub-Saharan Africa focuses on the epidemic as a challenge to human development in general and rural development in particular. In the face of the impact of the epidemic as described in parts one and two of the series, the agricultural sector can play an important role in mitigating some of its effects. Strategies for agricultural intervention are of particular importance in sub-Saharan Africa, given the fact that most of the countries hardest hit by the epidemic are heavily reliant on agriculture. Different agricultural sector based mitigation strategies are discussed. It is further argued, however, that such interventions need to be complemented by interventions from the health and other sectors, in particular by treatment regimes including access to anti-retroviral drugs. The text is followed by an annotated bibliography.

Accurate information is a vital requirement to enable the most effective international response to AIDS. These guidelines provide National AIDS Councils (or equivalents) with technical guidance on how to measure the core indicators for the implementation of the Declaration of Commitment on HIV/AIDS, adopted by Member States of the United Nations during the General Assembly Special session on HIV/AIDS in June 2001. Included are the detailed specifications of the core indicators, the information required and the basis of their construction, and their interpretation.

In recent years, Europe and Central Asia has experienced the world's fastest growing HIV/AIDS epidemic. Yet, in the Western Balkan countries the HIV prevalence rate is under 0.1 percent, which ranks among the lowest. This may be due to a low level of infection among the population--or partly due to inadequate surveillance systems.
