Background: Improving equity and early access to palliative care for underserved populations in Canada is a priority. Little is known regarding access to palliative and end-of-life care in the Black population. Methods: We undertook a scoping review using the framework by Arksey and O'Malley to identify knowledge, access gaps, and experiences of palliative and end-of-life care among Blacks living with life-limiting illnesses in Canada. Primary studies, discussion papers, books, and reports were considered eligible. We followed a comprehensive search strategy developed by an information scientist. Searches were performed in the following bibliographic databases: Medline, EMBASE, PsycINFO via OVID, CINAHL via EBSCOhost, Scopus and Cochrane Library via Wiley. The search strategy was derived from three main concepts: (1) Black people; (2) Canada and Canadian provinces; (3) Palliative, hospice, or end-of-life care. No publication date or language limits were applied. Titles and abstracts were screened for eligibility by one reviewer and full text by two independent reviewers. Results: The search yielded 233 articles. Nineteen articles were selected for full-text review, and 7 articles met the inclusion criteria. These studies were published between 2010 and 2021, and conducted in the provinces of Ontario and Nova Scotia only. Studies used both quantitative and qualitative methods and included cancer decedents, next of kin, family caregivers and religious leaders. Sample sizes in various studies ranged from 6 − 2,606 participants. Included studies reported a general lack of understanding about palliative and end-of-life care, positive and negative experiences, and limited access to palliative and end-of-life care for Blacks, across all care settings. Conclusion: Findings suggest limited knowledge of palliative care and inequities in access to palliative and end-of-life care for Blacks living with life-limiting illnesses in 2 Canadian provinces. There is an urgent need for research to inform tailored and culturally acceptable strategies to improve understanding and access to palliative care and end-of-life care among Blacks in Canada. The consequences of parallel paper and electronic medical records (EMR) and their impact on informational continuity are examined. An interdisciplinary team conducted a multi-site, ethnographic field study and retrospective documentation review from January 2010 to December 2010. Three case studies from the sample of older patients with hip fractures who were transitioning across care settings were selected for examination. Analysis of data from interviews with care providers in each setting, field observation notes, and reviews of medical records yielded two themes. First, the lack of interoperability between electronic information systems has complicated, not eased providers’ ability to communicate with others. Second, rather than transforming the system, digital records have sustained health care’s ‘culture of documentation’. While some information is more accessible and communications streamlined, parallel paper and electronic systems have added to front line providers’ burden, not lessened it. Implementation of truly interoperable electronic health information systems need to be expedited to improve care continuity for patients with complex health-care needs, such as older patients with hip fractures. Little research has been carried out on young people transitioning out of care in Canada. The objective of this paper was to describe and comment on the services provided to youth leaving care systems in Canada, with a focus on the four provinces with the largest populations (Ontario, Quebec, British Columbia, and Alberta). The Quebec government offers only one limited-access transition program, which has just been extended to age 25. Ontario, British Columbia, and Alberta offer several transition programs, which include financial, education, and life skills components. In British Columbia, these offer support up to age 29. The 6 remaining provinces and the 3 territories offer support to a maximum age that ranges from 21 to 26. Most offer a general financial allowance, and some offer additional supports that can include a housing allowance, tuition waivers, and job training. British Columbia and Ontario offer the most supports, including medical assistance, tuition waivers, and mental health supports. Research is needed to find out which supports are most beneficial, and under which circumstances. [ABSTRACT FROM AUTHOR]

Background: Homelessness is becoming an international public health issue in most developed countries, including Canada. Homelessness is regarded as both political and socioeconomic problems warranting broad and consistent result-oriented approaches.Methods: This paper represents the qualitative findings of a project that explored risk factors associated with family homelessness and strategies that could mitigate and prevent homelessness among families using a focused ethnographic study guided by the principles of participatory action research (PAR). The sample includes 36 family members residing at a family shelter who participated in focus groups over two years (between April 2016 and December 2017). Most of the participants were single-parent women.Results: The analysis yielded five major themes including, life challenges, lack of understanding of the system, existing power differentials, escaping from hardship, and a theme of proposed solutions for reducing family homelessness in the community.Conclusion: The findings illustrated the complex nature of family homelessness in Ontario; that the interaction of multiple systems can put families at risk of homelessness. Findings from this study underscore the need for urgent housing protocols aimed at educating homeless families on how to navigate and understand the system, enhance their conflict resolution skills, and develop strategies beyond relocation to help them to cope with difficulties with housing. Background: In Ontario, Canada, Indigenous communities experience some of the province's worst drinking water, with issues ranging from deteriorating water quality to regulatory problems and lack of support. When water is known, or suspected, to be unsafe for human consumption, communities are placed under a Drinking Water Advisory. Between 2004 and 2013, approximately 70% of all on-reserve communities in Ontario were under at least one Drinking Water Advisory. Despite the widespread impact of Drinking Water Advisories on health and wellbeing, little is known about First Nation individuals' perceptions and experiences living with a Drinking Water Advisory. This study presents information shared by members of a community who have lived with Boil Water Advisories on and off for many years, and a long-term Boil Water Advisory since 2017. The goal of this paper is to unpack and explore the Boil Water Advisories from the perspective of community members and provide considerations for current and future Boil Water Advisory management. Methods: Methodological choices were driven by the principles of community-based participatory research. Two data collection methodologies were employed: hard copy surveys and interviews. Results: Forty-four individuals (19.5%) completed a survey. Eight Elders and 16 key informants participated in 20 interviews. Respondents expressed varying degrees of uncertainty regarding protective actions to take while under a Boil Water Advisory. Further, 79% of men but only 46% of women indicated they always adhere to the Boil Water Advisory. Knowledge gaps that could lead to risky behaviours were also identified. Finally, Boil Water Advisories were demonstrated to have physical, financial, and time impacts on the majority of respondents. Conclusions: A direct outcome was the identification of a critical need to reinforce best practices for health protection through community education and outreach. More broadly, Chief and Council were able to use the findings to successfully advocate for improved drinking water for the community. Additionally, benefits of participatory research and community ownership include enhanced local research capacity, and increased awareness of, and desire for, research to inform decisions. Background: Over the last three decades, there has been worldwide recognition of peer support contributions to improve mental healthcare provision. However, in the current literature, little attention has been paid to exploring perspectives of peer supporters on their involvement in mental health services provision. The purpose of the present study was to examine peer supporters' perspectives on the implementation of a transitional discharge model (TDM), an intervention for the community integration of people with mental illness. Methods: This paper represents ethnographic qualitative data collected as part of a study that used mixed methods to evaluate the implementation of TDM across nine hospitals from the Province of Ontario, in Canada. The study involved a sample of 66 peer supporters, who were recruited from participating Consumer/Survivor Initiative Organizations and Peer Support Programs. The study collected data using two sets of focus groups, which were held at 6 months and 1-year post implementation. Data analysis used an ethnography model of qualitative analysis. Results: Peer supporters expressed that their involvement in mental healthcare enhanced clients' autonomy and hope about their recovery, as well as established a safety net and reduced hospital readmissions. Peer supporters articulated that they assumed several roles to facilitate clients' transition from hospital to the community. These roles included: assisting clients in building their capacity and developing healthy routines; attending regular on-ward and community meetings; accompanying clients to their appointments; and working with clients to set goals for their recovery. The study showed hindrances to effective implementation of peer support programs, such as a lack of understanding and appreciation of peer supporter roles, lack of careful allocation of peer supporters to clients, and an absence of appropriate protocols for ensuring the safety and supervision of the peer supporters. Conclusions: Results of the TDM implementation demonstrated that involving peer supporters in mental healthcare delivery may benefit clients by enhancing autonomy and hope about their recovery, as well as establishing a safety net and reducing hospital readmissions. Background: Historically transgender adults have experienced barriers in accessing primary care services. In Ontario, Canada, health care for transgender adults is accessed through primary care; however, a limited number of practitioners provide care, and patients are often waiting and/or traveling great distances to receive care. The purpose of this protocol is to understand how primary care is implemented and delivered for transgender adults. The paper presents how the case study method can be applied to explore implementation of health services delivery for the transgender population in primary care. Methods: Case study methodology will be used to explore this phenomenon in different primary care contexts. Normalization Process Theory is used as a guide. Three cases known to provide transgender primary care and represent different Ontario primary care models have been identified. Comparing transgender care implementation and delivery across different models is vital to understanding how care provision to this population can be supported. Qualitative interviews will be conducted. Participants will also complete the NoMAD (NOrmalization MeAsure Development) survey, a tool measuring implementation processes. The tool will be modified to explore the implementation of primary care services for transgender individuals. Documentary evidence will be collected. Cross-case synthesis will be completed to compare the cases. Discussion: Findings will provide an Ontario perspective on the implementation and delivery of primary care for transgender adults in different primary care models. Results may be applicable to other primary care settings in Canada and other nations with similar systems. Barriers and facilitators in delivery and implementation will be identified. Providing an understanding and increasing awareness of the implementation and delivery of primary care may help to reduce the invisibility and disparities transgender individuals experience when accessing primary care services. Understanding delivery of care could allow care providers to implement primary care services for transgender individuals, improving access to health care for this vulnerable population. This paper examines the role of in-migrants in the functional transformation of amenity-rich places. Its overarching goal is to determine if and how internal migrants arriving from larger settlement areas (i.e., counterurbanites) are contributing to the creation of functionally diverse (hybrid), or functionally limited (pure) commercial landscapes in places endowed with heritage assets. We focus on Paris, an historic town situated at the confluence of the Grand and Nith Rivers in southern Ontario. Three objectives guide the research: (1) to determine which commercial landscape form (hybrid or pure) and landscape generating process (creative enhancement or destruction) best characterizes the downtown; (2) to establish counterurbanite presence in this commercial core; and (3) to assess their role in the downtown's recent evolution. Evidence gleaned from secondary sources, and local fieldwork reveals that creative enhancement (functional addition), rather than creative destruction (functional displacement) is the dominant process. This has given rise to a hybrid landscape, where quotidian businesses (i.e., those selling ordinary goods and services) accompany five non-quotidian (i.e., distinctive) product types (i.e., authentic heritage, infused heritage, heritage-enhanced, faux-authentic heritage, and non-heritage boutique), each relying in different ways on the town's heritage assets. Information gathered from 35 proprietors finds that counterurbanites, and other inmigrants, dominate the downtown core, with only six non-migrant proprietors represented. In-comers, including counterurbanites, sell both quotidian and nonquotidian products, with non-migrant vendors offering only ordinary goods and services. These findings reveal that by selling both product types, counterurbanites are contributing to the creation of a hybrid landscape in the city's countryside. Their attraction is thus an important ingredient for facilitating diversity and economic viability in small settlement areas. [ABSTRACT FROM AUTHOR]

The City of Edmonton’s Drainage Operations branch oversees the inspection, maintenance, and repair of the city’s drainage network. This network covers an area of 700.6 km2 to serve the city’s growing population of nearly 1 million people. The activities performed by this division consume a large amount of funding and are, therefore, receptive to improvements in productivity. The study presented in this paper was conducted to develop improved work methods and engineered productivity standards for the various drainage operations. The study focused on six crews, which accounted for over 25% of the total drainage operations budget. The six crews were divided based on their respective duties: (1) cleaning mains by low pressure flushing (LPF); (2) cleaning mains by high pressure flushing; (3) scheduled mechanical cleaning of catch basins (CBC); (4) inspecting mains by televising; (5) commercial establishment investigation; and (6) service-line rodding and televising. The study utilized the concept of work simplification and focused on two crew activities—LPF and CBC work tasks—to improve crew work methods, to develop an established work standard, and to verify the proposed improvements based on the simulation model’s output. These activities are described in greater detail in two case studies. The work measurement concept was implemented to develop engineered productivity standards for the remaining crews in order to improve their productivity as well. This paper describes the application of an industrial engineering philosophy of work measurement—lean production theory—and the technique of simulation analysis to capture current work methods, generate and test alternative methods, and develop new productivity standards for drainage maintenance operations crews. Floating Catchment Area (FCA) methods are a popular tool to investigate accessibility to public facilities, in particular health care services. FCA approaches are attractive because, unlike other accessibility measures, they take into account the potential for congestion of facilities. This is done by 1) considering the population within the catchment area of a facility to calculate a variable that measures level of service, and then 2) aggregating the level of service by population centers subject to catchment area constraints. In this paper we discuss an effect of FCA approaches, an artifact that we term demand and level of service inflation. These artifacts are present in previous implementations of FCA methods. We argue that inflation makes interpretation of estimates of accessibility difficult, which has possible deleterious consequences for decision making. Next, we propose a simple and intuitive approach to proportionally allocate demandand and level of service in FCA calculations. The approach is based on a standardization of the impedance matrix, similar to approaches popular in the spatial statistics and econometrics literature. The result is a more intiuitive measure of accessibility that 1) provides a local version of the provider-to-population ratio; and 2) preserves the level of demand and the level of supply in a system. We illustrate the relevant issues with some examples, and then empirically by means of a case study of accessibility to family physicians in the Hamilton Census Metropolitan Area (CMA), in Ontario, Canada. Results indicate that demand and supply inflation/deflation affect the interpretation of accessibility analysis using existing FCA methods, and that the proposed adjustment can lead to more intuitive results. Background: This study reports on physicians’ experiences with chronic pain management. For over a decade prescription opioids have been a primary treatment for chronic pain in North America. However, the current opioid epidemic has complicated long-standing practices for chronic pain management which historically involved prescribing pain medication. Caring for patients with chronic pain occurs within a context in which a growing proportion of patients suffer from chronic rather than acute conditions alongside rising social inequities. Methods: Our team undertook an ethnographic approach known as institutional ethnography in the province of Ontario, Canada in order to explore the social organization of chronic pain management from the standpoint of primary care physicians. This paper reports on a subset of this study data, specifically interviews with 19 primary care clinicians and 8 nurses supplemented by 40 hours of observations. The clinicians in our sample were largely primary care physicians and nurses working in urban, rural and Northern settings. Findings: In their reflections on providing care for patients with chronic pain, many providers describe being most challenged by the work involved in helping patients who also struggled with poverty, mental health and addiction. These frustrations were often complicated by concerns that they could lose their license for inappropriate prescribing, thus shifting their work from providing treatment and care to policing their patients for malingering and opioid abuse. Interpretation: Our findings show that care providers find the treatment of patients with chronic pain–especially those patients also experiencing poverty–to be challenging at best, and at worst frustrating and overwhelming. In many instances, their narratives suggested experiences of depersonalization, loss of job satisfaction and emotional exhaustion in relation to providing care for these patients, key dimensions of burnout. In essence, the work that they performed in relation to their patients’ social rather than medical needs seems to contribute to these experiences. Their experiences were further exacerbated by the fact that restricting and reducing opioid dosing in patients with chronic pain has become a major focus of care provision. [ABSTRACT FROM AUTHOR]

While a growing body of research is examining the impacts of prolonged occupational sitting on cardiovascular and other health risk factors, relatively little work has examined the effects of occupational standing. The objectives of this paper were to examine the relationship between occupations that require predominantly sitting and those that require predominantly standing and incident heart disease. A prospective cohort study combining responses to a population health survey with administrative health-care records, linked at the individual level, was conducted in Ontario, Canada. The sample included 7,320 employed labor-market participants (50% male) working 15 hours a week or more and free of heart disease at baseline. Incident heart disease was assessed using administrative records over an approximately 12-year follow-up period (2003-2015). Models adjusted for a wide range of potential confounding factors. Occupations involving predominantly standing were associated with an approximately 2-fold risk of heart disease compared with occupations involving predominantly sitting. This association was robust to adjustment for other health, sociodemographic, and work variables. Cardiovascular risk associated with occupations that involve combinations of sitting, standing, and walking differed for men and women, with these occupations associated with lower cardiovascular risk estimates among men but elevated risk estimates among women. AIM-This paper assesses alcohol policies and interventions in Finland and the Canadian province of Ontario, using the policy options and interventions recommended in WHO's Global strategy to reduce the harmful use of alcohol (2010). DATA & METHODS - The information and data are based on archival sources, surveys, legislative and government documents, and published papers. The paper assesses both jurisdictions on 10 areas in the WHO document and their sub-topics: 1. leadership. 2. health services response. 3. community action. 4 drinking and driving policies and countermeasures. 5. availability of alcohol. 6. marketing of alcoholic beverages. 7. pricing policies. 8. reducing the negative consequences of drinking and alcohol intoxication. 9. reducing the public health impact of illicit alcohol and informally produced alcohol, and 10. monitoring and surveillance. RESULTS - Ontario had several recent noteworthy developments in line with WHO recommendations: health services response, controls of drinking and driving, pricing policies, reducing the negative consequences of drinking and intoxication, and monitoring and surveillance. Finland has emphasised pricing policies in recent years, and there have also been significant developments in community action, controls of drinking and driving, alcohol advertising, and monitoring and surveillance. In this paper we contend neo-liberal education policy which supports the creation of schooling choices in public education systems is reshaping, conflating and branding ethnicity. We make these points in reference to school choice in Toronto, Canada, and the establishment of an Africentric ethno-centric school. We argue that one of the registers within which education and ethnicity in Toronto operates relates to the conflation of commodification, ethnicity and geography, and that this conflation indicates one of the limits of school choice as a possible way to redress Black student disadvantage. We suggest education policy, which enables the establishment of ethno-centric schools, enters the realm of other debates about race, equity and difference that include the practices of marketing and branding. This paper employs a descriptive case study method to analyze and critically review the emergence of the provincial poverty reduction strategy in Ontario, Canada which was implemented in 2008 and renewed in 2014. The purpose of this study is two-fold: first, it defines the principles of neoliberalism and explores the historical growth of neoliberal thought in Canada, and specifically within Ontario, beginning in the 1980s to the present-day. Drawing on a combination of primary, secondary and grey literature, this paper discusses the ways in which neoliberal ideologies and rhetoric became deeply rooted in political thought and discourse within the province. Employing a critical theory framework, the paper highlights the contrasting ways in which neoliberal values were adopted by the different political parties in power and the detrimental impact this espousal had on individuals living in poverty within Ontario. Second, the paper illustrates the powerful ways in which anti-poverty grassroots movements and social advocacy groups assembled to push for the creation of a provincial poverty reduction strategy. The analysis ends with a critique of the neoliberal influences on the strategy's recommendations and the future outlook of the poverty reduction strategy based on the current political climate within the province. Since the mid-1990s, the reuse of brownfield properties for urban intensification has emerged as a core strategy in government efforts to remediate pollution and support renewal, regeneration, and retrofitting. While upper levels of government in Canada engaged in early efforts to devise policies, programs, and funding strategies to support redevelopment, the job of overseeing it has fallen mainly to local governments. This paper investigates the role of municipalities in Ontario, Canada's most populous and industrialized province, in managing and facilitating brownfields redevelopment. Survey data from 43 municipalities, coupled with information gathered from six site visitations and provincial information, reveal that despite common goals, tools, and approaches put forward by Ontario municipalities in their Community Improvement Plans, the management of planning, funding, and redevelopment issues continues to be a challenge for many, resulting in some very proficient municipalities and numerous hopeful ones with limited capacity to address this demanding issue. The COVID-19 pandemic has negatively impacted the mental health of individuals globally. However, less is known about the characteristics that contributed to some people having mental health problems during the pandemic, while others did not. Mental health problems can be understood on a continuum, ranging from acute (e.g., depression following a stressful event) to severe (e.g., chronic conditions that disrupt everyday functioning). Therefore, the purpose of this article was to generate profiles of adults who were more or less at risk for the development of mental health problems, in general, during the first 16-months of the COVID-19 pandemic in Ontario, Canada. Data were collected via online surveys at two time points: April-July 2020 and July-August 2021; 2,188 adults (Mage = 43.15 years; SD = 8.82) participated. Surveys included a demographic questionnaire and four previously validated tools to measure participants' mental health, subjective wellbeing, physical activity and sedentary behaviour, and sleep. A decision tree was generated at each time point for those with mental health problems, and those with no mental health problems. Results showed that subjective wellbeing was the biggest contributor to mental health status. Characteristics associated with no mental health problems among adults included having good wellbeing, being a good sleeper (quantity, quality, and patterns of sleep), and being over the age of 42. Characteristics associated with mental health problems included having poor wellbeing and being a poor sleeper. Findings revealed that specific characteristics interacted to contribute to adults' mental health status during the first 16 months of the COVID-19 pandemic. Given that wellbeing was the biggest contributor to mental health, researchers should focus on targeting adults' wellbeing to improve their mental health during future health crises. Background. During the COVID-19 pandemic, public health approaches and diseasetransmission varied widely across Canadian regions. This may have led to different trajectories for moderate-to-vigorous intensity physical activity (MVPA) and screen time during this period. The purpose of this investigation was to describe age- and gender-specific regional trends in MVPA and screen time for Canadian youth (ages 12-17 years) and adults (ages 18+) from 2018 to 2021. Methods. Data was collected using the Canadian Community Health Survey, which includes representative data for 5 distinct regions: Atlantic Canada, Québec, Ontario, the Prairie Provinces, and British Columbia (BC). Participants aged 12+ in each region self-reported their total daily screen time, as well as MVPA in 5 domains: overall, recreational, school, occupational/household and active transportation. Results were compared for 2018 (pre-pandemic), January-March of 2020, September-December of 2020, and 2021 using repeated measures t -tests. Results. Among youth, all regions except for Atlantic Canada and BC experienced significant reductions in the proportion of youth meeting MVPA recommendations in the fall of 2020 (all p < 0.001), although these had returned to baseline for all regions except Ontario by 2021. Trends varied across regions among adults aged 18-64 years. In Québec, there was 7-percentage point reduction in the proportion of males meeting the MVPA recommendations in the fall of 2020 compared to 2018, while there was a 4-percentage point increase among females in 2021 (all p<0:05). In Ontario and the Prairie provinces, males saw a 4-percentage point decrease in activity recommendation adherence in 2021, when compared to 2018 (p<005). There were no other significant differences for any region when comparing the fall of 2020 or 2021 with 2018 (all p>0:05). Among adults aged 65+ years, significant increases in MVPA were observed in Atlantic Canada and the Prairies in the fall of 2020, and in Atlantic Canada, Québec and the Prairies in 2021 (all p < 0:05). With limited exceptions, self-reported screen time increased significantly across regions and age groups for both males and females (all p < 0.05). Conclusions. MVPA levels of Canadians during the COVID-19 pandemic varied both by region and age group. Self-reported MVPA of Canadian youth dropped in most regions in the fall of 2020, before returning to pre-pandemic levels in 2021. Activity levels of Canadians aged 18-64 years were relatively stable during the pandemic and increased for Canadians aged 65+ in most regions. Differences in trajectories across genders observed at the national level were often less apparent in individual regions. Recreational screen use increased across all regions, ages and genders with very few exceptions. These results highlight the differences and similarities in activity and screen time trajectories across the Canadian population and suggest the need for additional research to identify best practices for promoting healthy movement behaviours during future pandemics. For more than fifty years, Jamaican farm workers have been seasonally employed in Canada under the Seasonal Agricultural Worker Program (SAWP). In Canada, these workers live and work in conditions that make them vulnerable to various health issues, including poor mental health. This ethnographic study investigated Jamaican SAWP workers' mental health experiences in Southern Ontario. Several common factors that engender psychological distress among Jamaican workers, ranging from mild to extreme suffering, were uncovered and organised into five themes: (1) family, (2) work environments and SAWP relations, (3) living conditions and isolation, (4) racism and social exclusion, and (5) illness and injury. I found that Jamaican workers predominantly use the term 'stress' to articulate distress, and they associate experiences of suffering with historic plantation slavery. Analysis of workers' stress discourses revealed their experiences of psychological distress are structured by the conditions of the SAWP and their social marginalisation in Ontario. This article presents and discusses these findings in the context of SAWP power dynamics and concludes with policy recommendations aimed at improving the mental health of all SAWP workers. Even with the benefits of sustainability and climate change reporting, there is limited information on how municipalities are reporting on performance for external stakeholders in comparison with private sector organizations. The purpose of this research was to gain an understanding of the current state of sustainability and climate change reporting at the local level and to investigate the extent to which municipalities across Ontario, Canada, report. We used content analysis to identify the presence or non-presence of information on the websites of 38 municipalities and analyzed the results using descriptive statistics. Our analysis showed that the sample municipalities were not widely reporting on sustainability or climate change performance. Also, we identified a gap between the number of plans and reports produced by sample municipalities, with the latter being less common, indicating a need for an improved evaluation of plan implementation. Further, we found that a provincial regulation that required municipalities to make their energy conservation and demand management plans public did not guarantee publication of the plan on a municipality's website. This study contributes to the growing field of sustainability and climate change planning and reporting by local governments and offers empirical evidence specific to Ontario, Canada. Geochemical tracers have the potential to provide valuable insights for constructing conceptual models of groundwater flow, especially in complex geological contexts. Nevertheless, the reliability of tracer interpretation hinges on its integration into a robust geological framework. In our research, we concentrated on delineating the groundwater flow dynamics in the Innisfil Creek watershed, located in Ontario, Canada. We amalgamated extensive hydrogeological data derived from a comprehensive 3D geological model with the analysis of 61 groundwater samples, encompassing major ions, stable water isotopes, tritium, and radiocarbon. By seamlessly incorporating regional physiographic characteristics, flow pathways, and confinement attributes, we bolstered the efficiency of these tracers, resulting in several notable findings. Firstly, we identified prominent recharge and discharge zones within the watershed. Secondly, we observed the coexistence of relatively shallow and fast-flowing paths with deeper, slower-flowing channels, responsible for transporting groundwater from ancient glacial events. Thirdly, we determined that cation exchange stands as the predominant mechanism governing the geochemical evolution of contemporary water as it migrates toward confined aquifers situated at the base of the Quaternary sequence. Fourthly, we provided evidence of the mixing of modern, low-mineralized water originating from unconfined aquifer units with deep, highly mineralized water within soil–bedrock interface aquifers. These findings not only contribute significantly to the development a conceptual flow model for the sustainable management of groundwater in the Innisfil watershed, but also offer practical insights that hold relevance for analogous geological complexities encountered in other regions. Amid Earth's ongoing sixth mass extinction event, numerous measures have been proposed to recover the populations of species at risk of extinction. However, the methods and objectives of different species' recovery plans sometimes conflict with each other, causing a conundrum we refer to as recovery–action antagonism. Recovery–action antagonism reduces the cost‐effectiveness of conservation programs and can increase the extinction risk of nontarget species. We describe a method to identify interactions between recovery actions, including antagonisms proposed for different at‐risk species in a given location. The method includes a process to evaluate potential drivers of recovery‐action antagonism and other interaction types using principal coordinates analysis and distance‐based redundancy analysis. We illustrate various applications of the method through case studies performed in Pelee Island and Rouge National Urban Park, two biodiverse areas in Ontario, Canada. Potential antagonism was identified between 1.5% (Pelee) and 5% (Rouge) of the evaluated recovery actions. Although the rate of antagonism was low in our case studies, the method allows the identification of a variety of interactions, which can help to prioritize similar and complementary actions that will benefit a large number of species while minimizing actions that may have competing outcomes. Reconciling diverse forest values within policy and decision-making processes is an ongoing challenge in forestry. The use of environmental impact assessment (EIA) provides potential for improving forest management and making it more responsive to diverse interests. This paper examines EIA in Canadian and Finnish forest planning. In Finland there has been a reluctance to see EIA as a tool for forest planning while in Canada some provinces have long applied EIA to forest management. Ontario, Canada, provides one example of applying EIA to forest planning at a range of scales in order to advance integrated planning and help conflict management. The paper provides a brief analysis of the Finnish forest planning system, an illustration of the Ontario EIA forest management experience, and then considers the application of EIA to Finnish forest management. The paper concludes that EIA may be workable for Finnish state forests and would likely enhance planning and management, but given the existing institutional frameworks EIA would be difficult to apply to private forests. Canada’s book trade is fascinating not only because it really consists of two book trades—the Anglophone and the Francophone—but also because it is a site of intense and sustained policy interventions designed to foster greater domestic control. Examining the country’s book policies, Statistics Canada data, trade association membership rosters and trade directories, this paper reveals that Canada’s contemporary book trade is characterized by three features: circa 40 years of government interventions in response to foreign dominance; the central role of foreign firms despite these interventions; and the dominance of Toronto and Montreal as domestic sites of book production. With the implementation of new policies supporting renewable natural gas production from organic wastes, Canada began replacing traditional disposal methods with highly integrated biogas production strategies. Herein, data from published papers, Canadian Biogas Association, Canada's national statistical agency, and energy companies' websites were gathered to gain insight into the current status of anaerobic digestion plants in recovering energy and resource from organic wastes. The availability of materials prepared for recycling by companies and local waste management organizations and existing infrastructures for municipal solid waste management were examined. Governmental incentives and discouragements in Canada and world anaerobic digestion leaders regarding organic fraction municipal solid waste management were comprehensively reviewed to identify the opportunities for developing large-scale anaerobic digestion in Canada. A range of anaerobic digestion facilities, including water resource recovery facilities, standalone digesters, and on-farm digesters throughout Ontario, were compared in terms of digestion type, digester volume, feedstock (s), and electricity capacity to better understand the current role of biogas plants in this province. Finally, technology perspectives, solutions, and roadmaps were discussed to shape the future in terms of organic fraction municipal solid waste management. The findings suggested that the biogas industry growth in Canada relies on provincial energy and waste management policies, advanced technologies for diverting organic waste from landfills, improving biogas yield using existing pretreatment methods, and educating farmers regarding digester operations. In this paper we examine how the researcher's body can be used as a tool for data collection in the process of ethnographic fieldwork. We focus in particular on the tensions inherent in undertaking embodied ethnographic research in the sexualized setting of a queer women's bathhouse event in Toronto, Canada. Our discussion addresses three moments within the research process: preparing our bodies to attend the bathhouse; positioning our bodies within the spaces of the bathhouse; and interacting with our bodies during the event. From May to mid-August 2021, the Ontario, Canada provincial public health agency, Public Health Ontario, in collaboration with local public health authorities and federal food safety partners, investigated a spatiotemporal cluster of 38 patients with Salmonella Typhimurium infections across multiple public health districts in Ontario. Five (13%) patients were hospitalized; no deaths were reported. The outbreak was linked to consumption of ready-toeat seasoned tofu from one manufacturer that was distributed to multiple Ontario restaurants. Isolates from the seasoned tofu were within one or fewer allele differences to the outbreak strain by whole genome sequencing. Evidence from food safety investigations conducted by local public health authorities and the Canadian Food Inspection Agency (CFIA), revealed that unsanitary conditions could have led to cross-contamination of the tofu, and insufficient heating of the tofu at the production level likely resulted in failure to eliminate the pathogen. The CFIA issued a food recall for the tofu at hotel, restaurant, and institution levels. Tofu was identified as a novel outbreak-associated food vehicle for S. Typhimurium in this outbreak. Interventions that target the production level and all parts of the supply chain and include additional safeguarding steps that minimize microbial growth are important. [ABSTRACT FROM AUTHOR]

Objective: There is minimal research on the role racism plays in recovery-oriented practice for Black youth. Clinicians' perspectives on building interagency partnerships have been understudied. We sought to characterize and contextualize interagency relationships in the mental health sector in Ontario and explore their impact on mental health provision. Research Design and Methods: A mixed methods approach was used to explore clinicians' perspectives on the collaborative nature of partnerships in the sector. An online survey was disseminated to clinicians in Toronto, Ontario between March 2020 and April 2020 followed by 7 focus groups which were conducted in Toronto between May 2020 and December 2020 with Black youth, caregivers, community, and clinicians. Quantitative data were analyzed using SPSS and visualized using social network analysis, while Nvivo 12 was used to perform a thematic analysis. Results: Interagency collaboration between mainstream agencies was common. Partnerships between Black and mainstream agencies were infrequent. Most mainstream agencies expressed disinterest in future partnerships with Black agencies. Feelings of unpreparedness among clinicians to provide culturally safe responsive care to Black youth became evident. Conclusions: Project partnerships between mainstream agencies are common. There is a paucity of Black agencies building organizational partnerships with mainstream agencies. There is a limited understanding of the systemic impacts of racism as a barrier to building interagency partnerships and its implications for recovery-oriented practice. The Canadian recovery guidelines do not identify racism as a social determinant of health or discuss potential implications for recovery-oriented practice. Background: In 2018, Canada legalized recreational cannabis use with the purpose of protecting youth and restricting access. However, concerns have been raised that this objective has not been met as rates of cannabis use among youth aged 16–24 have not declined. Youth cannabis use is associated with various adverse effects including psychosis, anxiety, depression, suicidality, respiratory distress, cannabinoid hyperemesis syndrome, and intoxications. Service providers play a crucial role in addressing youth cannabis use. This study aimed to understand Ontario service providers' perceptions, practices, and recommendations on youth cannabis use. Methods: This mixed method study included a survey and two focus groups. The survey was distributed to mental health service providers serving youth aged 16–24 across Ontario who were given the option to participate in a focus group. The survey included closed and open-ended questions regarding perceptions, practices, and recommendations, while the focus groups explored these categories in greater depth. Descriptive statistics were used to analyze close-ended questions and interpretative content analysis was applied for open-ended questions. Focus group data were analyzed using thematic analysis. Results: The survey was completed by 160 service providers and 12 participated in two focus groups. Regarding perceptions, 60% of survey participants agreed with legalization, 26% had a strong understanding of medical versus recreational cannabis, 84% believed that cannabis has physical and mental health risks, and 49% perceived stigmatization. Less than half of the survey participants reported screening or assessing cannabis use, 16% stated they are highly familiar with treating cannabis use, and 67% reported that they rarely work with families. Subthemes identified in the focus groups under perceptions included normalization and stigmatization, harms for youth, and stigma, racism, and discrimination. Subthemes under practice included cannabis not being the primary focus, challenges with screening, assessment, and intervention, and referral to specialized services. Both the survey and focus group participants recommended increasing public education, enhancing service provider training, improving regulation and policies, reducing stigma and minimization, improving service access, and providing more culturally responsive services. Conclusion: Youth cannabis use in Canada remains a significant public health concern, necessitating a more comprehensive plan to protect Ontario youth and reduce associated harms. Introduction: The accurate identification and appropriate investigation of child maltreatment is a key priority for promoting the optimal health and development of children. Healthcare providers are often well-positioned professionals to report suspected child abuse and neglect, and, therefore, interact regularly with child welfare workers. Little research has examined the relationship between these two groups of professionals. Methods: We interviewed healthcare providers and child welfare workers in order to examine the referral and child welfare investigation processes to understand strengths and identify areas of improvement for future collaboration. Thirteen child welfare workers from child welfare agencies and eight healthcare providers from a pediatric tertiary care hospital in Ontario, Canada were interviewed to meet the study's objectives. Results: Healthcare providers spoke about positive experiences making reports, factors impacting reporting decisions, areas for improvement (e.g., difficulties communicating, lack of collaboration, and disruption of therapeutic alliance), training, and professional roles. For interviews with child welfare workers, identified themes included healthcare professionals' perceived expertise and understanding the role of child welfare. Both groups brought up the need for increased collaboration as well as systemic barriers and legacies of harm. Discussion: Our core finding was a reported lack of communication between the groups of professionals. Other identified barriers in collaboration included a lack of understanding of each other's roles, hesitation for healthcare providers making reports, as well as legacies of harm and systemic inequities in both institutions. Future research should build on this examination by including the voices of healthcare providers and child welfare workers to identify sustainable solutions for increased collaboration. (1) Background: Canada had a unique approach to COVID-19 vaccine policy making. The objective of this study was to understand the evolution of COVID-19 vaccination policies in Ontario, Canada, using the policy triangle framework. (2) Methods: We searched government websites and social media to identify COVID-19 vaccination policies in Ontario, Canada, which were posted between 1 October 2020, and 1 December 2021. We used the policy triangle framework to explore the policy actors, content, processes, and context. (3) Results: We reviewed 117 Canadian COVID-19 vaccine policy documents. Our review found that federal actors provided guidance, provincial actors made actionable policy, and community actors adapted policy to local contexts. The policy processes aimed to approve and distribute vaccines while continuously updating policies. The policy content focused on group prioritization and vaccine scarcity issues such as the delayed second dose and the mixed vaccine schedules. Finally, the policies were made in the context of changing vaccine science, global and national vaccine scarcity, and a growing awareness of the inequitable impacts of pandemics on specific communities. (4) Conclusions: We found that the triad of vaccine scarcity, evolving efficacy and safety data, and social inequities all contributed to the creation of vaccine policies that were difficult to efficiently communicate to the public. Introduction: The Canadian government has committed to a national action plan (NAP) to address violence against women (VAW). However, a formalized plan for implementation has not been published. Building on existing recommendations and consultations, we conducted the first formal and peer-reviewed qualitative analysis of the perspectives of leaders, service providers and survivors on what should be considered in Canada’s NAP on VAW. Methods: We applied thematic analysis to qualitative data from 18 staff working on VAW services (11 direct support, 7 in leadership roles) and 10 VAW survivor participants of a community-based study on VAW programming during the COVID-19 pandemic in the Greater Toronto Area (Ontario, Canada). Results: We generated 12 recommendations for Canada’s NAP on VAW, which we organized into four thematic areas: (1) invest into VAW services and crisis supports (e.g. strengthen referral mechanisms to VAW programming); (2) enhance structural supports (e.g. invest in the full housing continuum for VAW survivors); (3) develop coordinated systems (e.g. strengthen collaboration between health and VAW systems); and (4) implement and evaluate primary prevention strategies (e.g. conduct a gender-based and intersectional analysis of existing social and public policies). Conclusion: In this study, we developed, prioritized and nuanced recommendations for Canada’s proposed NAP on VAW based on a rigorous analysis of the perspectives of VAW survivors and staff in Canada’s largest city during the COVID-19 pandemic. An effective NAP will require investment in direct support organizations; equitable housing and other structural supports; strategic coordination of health, justice and social care systems; and primary prevention strategies, including gender transformative policy reform. Background: Autosomal dominant (AD) spinocerebellar ataxias (SCAs) encompass a large group of rare disorders, which occurs in individuals of different ethnic backgrounds. To date, demographics, and clinical descriptions of AD SCA in Canada are lacking. Methods: A retrospective chart review of patients with a genetically confirmed diagnosis of AD SCAs was performed at five tertiary centers across Canada in the provinces of Quebec, Alberta, and Ontario. Demographic, genetic, and clinical information were collected and analyzed. Results: A total of 203 patients with AD SCA were identified. Weighted estimated prevalence of AD SCA in three large Canadian provinces was calculated (2.25 cases per 100.000) which is in keeping with the figures documented worldwide. We found that the distribution of the most common SCA differed when comparing provinces. The most prevalent SCA diagnosis in Ontario was SCA3 (49%), while the most prevalent SCA diagnosis in Alberta and Quebec was SCA2 in 26% and 47%, respectively. SCA6 was the third most prevalent SCA subtype in Quebec (14%), which was not seen as commonly in other provinces. SCA1 was uncommonly seen in both Alberta and Quebec, despite being common in Ontario. Conclusions: In this largest Canadian study, we describe the prevalence, distribution, and clinical characteristics of AD SCA. We found that the distribution of the most common SCA differed in the three provinces studied. This finding reflects the heterogenous nature of the Canadian population. [ABSTRACT FROM AUTHOR]

Simple Summary: The atmosphere is an important transport pathway of microplastics to remote and urban environments. We assessed the efficacy of moss bags as an active biomonitoring technique for atmospheric microplastic deposition. Microplastics were observed in all moss bags deployed along an urban intensity gradient; moss bags exposed in the most densely populated and trafficked areas accumulated a higher number of microplastics compared with those exposed in low-density areas. Microplastics (plastic particles <5 mm) were first identified in the environment during the 1970s and have since become ubiquitous across every environmental compartment. However, few studies have focused on atmospheric microplastics, and even fewer have used biological monitoring to assess their atmospheric deposition. Here, we assess the efficacy of moss bags as an active biomonitoring technique for atmospheric microplastic deposition. Moss (Pleurozium schreberi) bags were exposed in duplicate at nine deployment sites across a gradient of urban intensity in southern Ontario, Canada. A total of 186 microplastics (mp) were detected in the moss bags, resulting in a mean accumulation of 7.9 mp g−1 dry weight moss across all sites during the exposure period (45 days). The median microplastic length was 0.56 mm (range 0.03–4.51 mm), and the dominant microplastic type was fibres (47%), followed by fragments (39%). Microplastic accumulation significantly increased with urban intensity, ranging from 3.7 mp g−1 in low-density suburban areas to 10.7 mp g−1 in densely populated and trafficked urban areas. In contrast, microfibres by proportion dominated in suburban (62%) compared with urban areas (33%). Microplastic deposition was estimated to range from 21 to 60 mp m−2 day−1 across the nine deployment sites. The results suggest that moss bags may be a suitable technique for the active biomonitoring of atmospheric microplastic deposition in urban environments. [ABSTRACT FROM AUTHOR]

This study's goal was to determine the prevalence, temporal trends, seasonal patterns, and temporal clustering of Salmonella enterica isolated from environmental samples from Ontario's poultry breeding flocks between 2009 and 2018. Clusters of common serovars and those of human health concern were identified using a scan statistic. The period prevalence of S. enterica was 25.3% in broiler breeders, 6.4% in layer breeders, and 28.6% in turkey breeders. An overall decreasing trend in S. enterica prevalence was identified in broiler breeders (from 27.8% in 2009 to 22.1% in 2018) and layer breeders (from 15.4% to 4.9%), while an increasing trend was identified in turkey breeders (from 12.0% to 24.5%). The most common serovars varied by commodity. Among broiler breeders, S. enterica serovars Kentucky (42.4% of 682 submissions), Heidelberg (19.2%), and Typhimurium (5.4%) were the most common. Salmonella enterica serovars Thompson (20.0% of 195 submissions) and Infantis (16.4%) were most common among layer breeders, and S. enterica serovars Schwarzengrund (23.6% of 1368 submissions), Senftenberg (12.9%), and Heidelberg and Uganda (9.6% each) were most common among turkey breeders. Salmonella enterica ser. Enteritidis prevalence was highest in submissions from broiler breeders (3.7% of 682 broiler breeder submissions). Temporal clusters of S. enterica serovars were identified for all poultry commodities. Seasonal effects varied by commodity, with most peaks occurring in the fall. Our study provides information on the prevalence and temporality of S. enterica serovars within Ontario's poultry breeder flocks that might guide prevention and control programs at the breeder level. The Green Energy and Green Economy Act was quickly passed in 2009. Due to the breadth of the Act, it should have received a rigorous legislative review-and-consultation process, but did not due to green-labeling. Ontario did not meet their ethical fiduciary responsibility to consult with Indigenous peoples. With the COVID-19 Economic Recovery Act, 2020, there were no public hearings even though changes to the Environmental Assessment Act would allow for the exemption or streamlining of projects from the process. If a project was exempted, there would be no environmental assessment, and no legal fiduciary responsibility to consult with Indigenous peoples; the legal duty to consult would not be triggered even though Indigenous peoples would potentially be impacted. Background: The human immunodeficiency virus (HIV) epidemic has disproportionately affected African, Caribbean and Black (ACB) communities in Canada. We investigated the prevalence and factors associated with HIV infection among ACB people in Ontario. Methods: A cross-sectional survey of first- and second-generation ACB people aged 15-64 years in Toronto and Ottawa (Ontario, Canada). We collected sociodemographic information, self-reported HIV status and offered dried blood spot (DBS) testing to determine the prevalence of HIV infection. Factors associated with HIV infection were investigated using regression models. Results: A total of 1,380 people were interviewed and 834 (60.4%) tested for HIV. The HIV prevalence was 7.5% overall (95% confidence interval [CI] 7.1-8.0) and 6.6% (95% CI 6.1-7.1) in the adult population (15-49 years). Higher age (adjusted odds ratio [aOR] 2.8; 95% CI 2.77--2.82), birth outside of Canada (aOR 4.7; 95% CI 1.50-14.71), French language (aOR 9.83; 95% CI 5.19-18.61), unemployment (aOR 1.85; 95% CI 1.62-2.11), part-time employment (aOR 4.64; 95% CI 4.32-4.99), substance use during sex (aOR 1.66; 95% CI 1.47-1.88) and homosexual (aOR 19.68; 95% CI 7.64-50.71) and bisexual orientation (aOR 2.82; 95% CI 1.19-6.65) were associated with a positive HIV test. Those with a high school (aOR 0.01; 95% CI 0.01-0.02), college (aOR 0.00; 95% CI 0.00-0.01) or university education (aOR 0.00; 95% CI 0.00-0.01), more adequate housing (aOR 0.85; 95% CI 0.82-0.88), a higher social capital score (aOR 0.61; 95% CI 0.49--0.74) and a history of sexually transmitted infections (aOR 0.40; 95% CI 0.18-0.91) were less likely to have a positive HIV test. Conservation budgets are limited, requiring strategic prioritization among actions to efficiently protect species. Systematic prioritization approaches typically determine locations for conservation that most effectively balance species protection with cost. Proxies for cost are frequently used in prioritizing land for protection. Here, we combine financial cost estimates for private land acquisition and species habitat models into a spatial prioritization to explore cost‐effective habitat protection, using a case study of species at risk in Ontario, Canada. Our findings suggest a key trade‐off, whereby protecting the areas with the greatest concentration of species at risk may not be the best strategy for protecting these species. Instead, protecting species at risk may be most cost effective in areas where species‐at‐risk richness is still relatively high, but land costs are relatively low, such as in central Ontario. However, the budget required to adequately protect species at risk through land purchase would be much larger than is currently available for conservation efforts, even if public lands are preferentially protected. Therefore, to effectively protect all species at risk in Ontario, we recommend the use of alternative conservation measures, such as easements and incentives for restoration on private land, to supplement already protected areas. Introduction: Unhealthy food and beverage marketing in various media and settings contributes to children's poor dietary intake. In 2019, the Canadian federal government recommended the introduction of new restrictions on food marketing to children. This scoping review aimed to provide an up-to-date assessment of the frequency of food marketing to children and youth in Canada as well as children's exposure to this marketing in various media and settings in order to determine where gaps exist in the research. Methods: For this scoping review, detailed search strategies were used to identify relevant peer-reviewed and grey literature published between October 2016 and November 2021. Two reviewers screened all results. Results: A total of 32 relevant and unique articles were identified; 28 were peer reviewed and 4 were from the grey literature. The majority of the studies (n = 26) examined the frequency of food marketing while 6 examined actual exposure to food marketing. Most research focussed on children from Ontario and Quebec and television and digital media. There was little research exploring food marketing to children by age, geographical location, sex/gender, race/ethnicity and/or socioeconomic status. Conclusion: Our synthesis suggests that unhealthy food marketing to children and adolescents is extensive and that current self-regulatory policies are insufficient at reducing the presence of such marketing. Research assessing the frequency of food marketing and preschooler, child and adolescent exposure to this marketing is needed across a variety of media and settings to inform future government policies. Background: Nine migrant agricultural workers died in Ontario, Canada, between January 2020 and June 2021. Methods: To better understand the factors that contributed to the deaths of these migrant agricultural workers, we used a modified qualitative descriptive approach. A research team of clinical and academic experts reviewed coroner files of the nine deceased workers and undertook an accompanying media scan. A minimum of two reviewers read each file using a standardized data extraction tool. Results: We identified four domains of risk, each of which encompassed various factors that likely exacerbated the risk of poor health outcomes: (1) recruitment and travel risks; (2) missed steps and substandard conditions of healthcare monitoring, quarantine, and isolation; (3) barriers to accessing healthcare; and (4) missing information and broader issues of concern. Conclusion: Migrant agricultural workers have been disproportionately harmed by the COVID-19 pandemic. Introduction: South Asian communities comprise one of the fastest-growing populations in Canada, but there is a paucity of research on if and how immigrant men use sexual health services. Objectives: Our study aimed to understand factors influencing sexual health service use by South Asian immigrant men living in Ontario, Canada. Methods: An interpretive description (ID) research methodology was used, followed by Braun and Clarke's thematic guidelines to collect and analyze data. Altogether, 18 semi-structured interviews were conducted between May and July 2021 with South Asian immigrant men between 20 and 45 years of age in Ontario. Results: We found that culture shaped the overall perception and use of sexual health services, while the perceived severity of sexual illness also affected the utilization of preventative care. Moreover, there are taboos about sexual health, and while sex positivity and sex negativity exist, sexual health information is shared with trusted sources. We also found South Asian men shared sexual health more openly by following a lifestyle that normalizes sexual health discussions rather than South Asian cultural norms. Conclusions: This study provides a comprehensive understanding of the individual, structural and cultural determinants of health influencing sexual health service use to strengthen existing health communication strategies to improve service utilization for diverse communities in Canada. Background: As of 2019, men who have sex with men (MSM) in Canada are ineligible to donate blood if they have had oral or anal sex with another man in the last 3 months. Deferral policies targeting MSM are largely interpreted as unjust by gay, bisexual, and other men who have sex with men (GBMSM) - shaping their desire to donate blood and engage with blood operators. This mixed methods study explores interest in blood donation among GBMSM as well as willingness (and eligibility) to donate under four different deferral policies.Methods: We surveyed 447 GBMSM who were recruited from the Ontario-wide #iCruise study. Participants were asked whether they were interested in blood donation and if they were willing to donate under each of our four deferral policies. We also completed interviews with 31 of these GBMSM. Participants were asked to describe their feelings about blood donation, their views on our different deferral policies, the impact of a policy change, as well as other means of redress.Results: Most participants (69%) indicated that they were interested in donating blood. Despite this, an interpretation of the MSM deferral policy as discriminatory was common among all participants. Our mixed methods findings indicate that, among those who were interested in blood donation, the adoption of one of the alternative policies presented in this study (specifically Policy 2 or Policy 3) would significantly increase the number of participants willing to donate and be viewed as "a step in the right direction." However, many participants who were not interested in blood donation argued that a gender-neutral deferral policy would need to be implemented for them to donate. Participants recommended that blood operators consider efforts to repair relations with GBMSM beyond policy change, including pop-up clinics in predominantly queer areas and diversity sensitivity training for staff.Conclusion: We argue that the most impactful policy shift would be the implementation of an individual risk-based deferral policy that is applied to all donors regardless of sexual orientation or gender identity.
