Summary: After more than 120 hand‐upper extremity and 37 face transplant procedures performed worldwide, vascularized composite allotransplantation (VCA) now falls under the scope of organ transplant legislation in Europe and the United States. While in the USA, VCA has been considered as standard care since 2014, VCA in Europe is still performed through clinical research trials, except in United Kingdom. However, after two decades of favourable experience with upper extremity transplantation (UET), professionals in Europe are proposing hand allotransplantation as "controlled standard" care, as opposed to face transplantation (FT), which is still a challenging activity. The European Committee on Organ Transplantation (CD‐P‐TO) has elaborated a position paper to provide recommendations concerning regulatory aspects for UET and FT. It is aimed at Health Authorities in charge of the oversight – and coordination – of organ donation and transplantation, and at professional groups to help them manage such complex and costly programs dedicated to properly selected patients. Objectives: (i) To determine the current state of online grocery shopping, including individuals' motivations for shopping for groceries online and types of foods purchased; and (ii) to identify the potential promise and pitfalls that online grocery shopping may offer in relation to food and beverage purchases. Design: PubMed, ABI/INFORM and Google Scholar were searched to identify published research. Setting: To be included, studies must have been published between 2007 and 2017 in English, based in the USA or Europe (including the UK), and focused on: (i) motivations for online grocery shopping; (ii) the cognitive/psychosocial domain; and (iii) the community or neighbourhood food environment domain. Subjects: Our search yielded twenty-four relevant papers. Results: Findings indicate that online grocery shopping can be a double-edged sword. While it has the potential to increase healthy choices via reduced unhealthy impulse purchases, nutrition labelling strategies, and as a method to overcome food access limitations among individuals with limited access to a brick-and-mortar store, it also has the potential to increase unhealthy choices due to reasons such as consumers' hesitance to purchase fresh produce online. Accessible summary In the Western world, many people who have mental health problems experience poor physical health. Research suggests that those with mental illness often have difficulties in accessing health care services, reporting that when they do, their physical health needs are often overlooked., Nurses are expected to deliver safe and effective care that meets the needs of service users. In order to help mental health nurses address physical health care needs, changes to nurse education, supported by government policy, have been made in the United Kingdom. Despite these changes, mental health nurses still feel less confident and prepared to address physical health needs of service users. However, this same problem is also reported by nurses throughout Europe and in other Western countries, who have undergone different training and education programmes., This paper discusses important factors affecting the mental health nurses' ability to deliver physical care and how they impact on confidence and competence., The paper concludes by highlighting the role of the mental health nurse being key in addressing the physical health needs of those in their care, and how this will only be realized when education and practice provide sufficient opportunity for them to develop and maintain their knowledge and skills., Abstract In the Western world, there is increasing concern regarding the ability of health care professionals to meet the physical health care needs of those diagnosed with mental illness. This discussion paper seeks to explore some of the issues facing mental health nurses in delivering care to those who have both mental and physical illness. Criticism of the National Health Service for failing to meet the physical health needs of people with serious mental illness has generated a number of political strategies aimed at addressing inequalities between mental and physical health care, a consequence being a change in pre-registration nurse education. It is envisaged that such changes will enable all nurses to deliver safe, effective care to service users, with adult and mental health nurses being more adept at meeting both the mental and physical health needs of those in their care. The impact of three key areas identified within the literature will be considered: the impact of co-existing physical and mental illness; the role of mental health nurses and perceptions of service users re physical health care; and education, training and learning from others in the Western world. The vulnerability of deafblind people is considered axiomatic; they are seen not only as a vulnerable group but also as one of the most vulnerable. This paper aims to synthesise existing knowledge to determine what is known about such vulnerability. A comprehensive literature search was undertaken between April 2013 and May 2014. The review method was informed by systematic review principles. An approach based on a 'hierarchy of evidence' would have reduced the amount of literature reviewed significantly, to the point where synthesis would not be possible. Included material was appraised and an interpretative rather than aggregative approach to synthesis adopted. Drawing on principles of critical interpretive synthesis, rather than being a determiner of whether material should be included or excluded, a critique of the literature is offered within the synthesis. Twenty-eight references were identified for inclusion, originating from the UK, USA, Australia, Continental Europe and the Nordic Countries. No empirical studies specifically examining the experience of vulnerability of deafblind people were found. However, deafblind people describe feelings of vulnerability in studies exploring their experiences more generally, and in personal accounts of living with the impairment. Literature produced by practitioners and specialist organisations also explores the topic. Deafblind people are identified as a population 'at risk' of various adverse outcomes, particularly when compared to the non-deafblind majority, and deafblind people describe being and feeling vulnerable in various situations. The literature largely relates to negative outcomes and includes significantly less exploration of positive risk taking, coping capacity and resilience. Deafblind people do not appear to describe themselves as being vulnerable as a permanent state, suggesting a need for greater exploration of the experience among all sections of this heterogeneous population, with consideration of the concepts of resilience and coping capacity. This paper is a report of a meta-ethnography of qualitative studies of older peoples' views on risk of falling and need for intervention. Background. Falls and falls-related injuries in older people are worldwide problems. A conceptual understanding of older people's views about falls risk and need for intervention is useful for understanding factors likely to impact on acceptance of risk and recommended interventions. Data Sources. Seven electronic databases were searched 1999-2009. Reference lists of included articles were screened for eligible papers. Review Methods. Assessment of quality was carried out. Themes and concepts were extracted using a meta-ethnographic approach to compare similarities and differences across the retrieved studies. A line of argument was developed to produce an explanatory framework of the extracted themes and concepts. Results. Eleven relevant qualitative research articles of reasonable quality were identified. Six key concepts were identified: beyond personal control; rationalizing; salience; life-change and identity; taking control and self-management. A line of argument synthesis describes how older people approach self-appraisal of falls risk and intervention need, and how they cope and adapt to falls risk and intervention need. Conclusion. In response to having an elevated risk status and perceived associations with frailty and impact on an independent life-style, some prefer to adapt to this reality by taking control and implementing self-management strategies. Healthcare professionals should take into account beliefs about risk and negotiate choices for intervention, recognizing that some individuals prefer to drive the decision-making process to preserve identity as a competent and independent person. Abstract: Evidence Based Policy has been articulated and practiced in Europe, particularly under the ‘New Labour’ policies of the former Labour government in the United Kingdom. In the United States, the impact of research on policy has been inconsistent due to differing relationships between researchers and policy makers. This paper gives an overview of evidence based policy and presents critiques based on its reliance on positivist methods and technical approach to policy making. Using these critiques as a framework, the paper discusses the case of Housing First, a policy adopted by the Bush Administration in order to address the problem of chronic homelessness. The case is an example of research driven policy making but also resulted in a progressive policy being promoted by a conservative administration. In discussing the case, the paper elaborates on the relationship between evidence and policy, arguing that evidence based policy fails to integrate evidence and values into policy deliberations. This paper questions why the 'West' and in particular European states came to achieve a global dominance in naval warfare, a process which began in the fifteenth century and continues today as a result of the massive investment made by the United States in a fleet that is truly global in scale. What Black demonstrates is that this dominance was not inevitable or technologically predetermined, but was due to specific social, cultural, political, economic and strategic factors which sometimes caused non European naval powers to limit the development of a more aggressive deep ocean capability or to ignore it entirely. In contrast, in the case of major European powers like Great Britain and France, a combination of fortunate circumstances and intense military competition, which manifested itself as 'arms racing', resulted in an aggressive policy that exploited technological and organisational developments in the naval domain to the fullest extent possible. As a result, by the late eighteenth century there was no countervailing maritime power capable of limiting European maritime expansion. The paper explains how the Europeans, and in particular the British, consolidated and reinforced their naval supremacy in the nineteenth century and questions why this naval revolution has endured in the shadow of the more commonly known revolution in land warfare which took place at about the same time. This paper provides an update of a previous review ' Red Meat in the Diet' published in the Nutrition Bulletin in 2005. An update on red meat consumption levels in the UK and other countries is provided, and a summary of the nutritional content of red meat in the diet is given. Current evidence on dietary and lifestyle factors associated with red meat consumption and the effects of red meat intake on health and chronic disease outcomes are discussed. Understanding emerging trends in public perceptions of nanomaterials is critically important for those who regulate risks. A number of surveys have explored public perceptions of their risks and benefits. In this paper we meta-analyse these surveys to assess the extent to which the following four hypotheses derived from previous studies of new technologies might be said to be valid for nanotechnologies: risk aversion will prevail over benefit appreciation; an increase in knowledge will not result in reduced aversion to risks; judgements will be malleable and subject to persuasion given risk-centric information; and contextual, psychometric and attitudinal predictors of perceived risk from prior studies can help anticipate future perceptions of nanotechnologies. We find that half the public has at least some familiarity with nanotechnology, and those who perceive greater benefits outnumber those who perceive greater risks by 3 to 1. However, a large minority of those surveyed (44%) is unsure, suggesting that risk judgements are highly malleable. Nanotechnology risk perceptions also appear to contradict some long-standing findings. In particular, unfamiliarity with nanotechnology is, contrary to expectations, not strongly associated with risk aversion and reduced ‘knowledge deficits’ are correlated with positive perceptions in this early and controversy-free period. Psychometric variables, trust and affect continue to drive risk perceptions in this new context, although the influence of both trust and affect is mediated, even reversed, by demographic and cultural variables. Given the potential malleability of perceptions, novel methods for understanding future public responses to nanotechnologies will need to be developed. [ABSTRACT FROM AUTHOR]

Objective: To systematically review and critically appraise the economic evaluations of one to one interventions to reduce sexually transmitted infections (STIs) and teenage conceptions. Design: Systematic review. Data Sources: Search of four electronic bibliographic databases from 1990 to January 2006. Search keywords included teenage, pregnancy, adolescent, unplanned, unwanted, cost benefit, cost utility, economic evaluation, cost effectiveness and all terms for STIs, including specific diseases. Review Methods: We included studies that evaluated a broad range of one to one interventions to reduce STIs. Outcomes included major outcomes averted, life years and quality adjusted life years (QALY). All studies were assessed against quality criteria. Results: Of 3,190 identified papers, 55 were included. The majority of studies found one to one interventions to be either cost saving or cost effective, although one highlighted the need to target the population to receive post-exposure prophylaxis to reduce transmission of HIV. Most studies used a static approach that ignores the potential re-infection of treated patients. Conclusion: One to one interventions have been shown to be cost saving or cost effective but there are some limitations in applying this evidence to the UK policy context. More UK research using dynamic modelling approaches and QALYs would provide improved evidence, enabling more robust policy recommendations to be made about which one to one interventions are cost effective in reducing STIs in the UK setting. The practice of deliberately concealing garments within the structure of buildings is described. These finds provide a means of exploring how space was conceived and experienced in the past, and how these deliberately hidden garments mediated, and continue to mediate, the relationship between people and the spaces they occupied, and may continue to occupy. The Deliberately Concealed Garments Project was set up in 1998 to locate, document and analyze garments found hidden within buildings. Concealments have preserved many textiles in the UK, mainland Europe, Australia and North America. The significance of these caches rests not only in the finds themselves, as rare items of dress, but also because of what they reveal about perceptions of built space. The concealments are believed to serve a protective function, not against the weather or immodesty, but against incoming malevolent forces. As apotropaic (evil-averting) agents they protect from within rather than as outer coverings or internal divisions. This article focuses on the U.S.-European relations from the twentieth to the twenty-first century in context of conflict in Iraq. The events that led to the recent U.S.-Great Britain campaign in Iraq created severe strains in the U.S.'s relations with some of its European allies. These strains, in turn, have impacted on both NATO and the EU. Even before the military campaign turned into occupation, a start was made on repairing the damage to transatlantic relations. The purpose of this paper from an American's perspective is not so much to predict the outcome of diplomatic initiatives, as it is to examine the main trends and factors that caused the strains: the "Messianic" aspects of U.S. foreign policy, the changes in U.S.-European power relations since World War II, post-cold war events and experiences, and the key diplomatic events that preceded the Iraqi Campaign.

Although issues of inclusion, diversity and achievement have become a powerful agenda for change in teacher education policy in England, bilingual pupils are still conceptualised in policy terms as a problem. This absence of linguistic integration in English teacher education is in tension with the increasing number of school pupils speaking a diversity of languages at home, and contrasts with policy towards bilingual pupils in other parts of the United Kingdom, in Europe and in the United States. The standards for qualified teacher status only require trainee competence to be demonstrated in understanding pupil language backgrounds and in providing support for those learning 'English as an Additional Language' (EAL). The authors' research investigated the way teacher education policy in England ignores the positive attributes of bilingual learners and the resultant lack of debate about the preparedness of trainee teachers to work effectively with bilingual pupils. This article presents key findings from their work with secondary schools in the south of England. They interviewed trainees, teachers and Local Education Authority (LEA) officers, administered a questionnaire to trainees and analysed policy documents. The findings indicate teacher education in England pays bilingualism lip service at best, persisting with a policy discourse emphasising the problem of EAL. The research raises important questions concerning teacher education policy in relation to bilingualism, and highlights the significance of school contexts in relation to effective teacher preparation. The article concludes by arguing for policies to improve teacher confidence and competence in England's increasingly linguistically diverse classrooms. [ABSTRACT FROM AUTHOR]

A response to a study on whether terrorism is a force for urban dispersal or whether terrorism drives people together is presented. It cites the costly travel effect and the city walls effect which counter the tall targets effect. The author says that terrorism is in decline in Europe and North America, but is on the rise in the Middle East. The incidence of terrorism in large cities in the U.S. and Great Britain which is indicative of the tall targets effect is mentioned. The article elaborates on the correlation between terrorism and urban land use.

This article continues from an earlier paper that examined Australian laws regulating the use of preimplantation genetic diagnosis (PGD) for the purpose of what the author has tentatively termed 'negative enhancement' to develop that analysis to include an examination of similar laws in the UK and Europe. The term 'negative enhancement' is used to describe those instances where individuals seek to use PGD to achieve outcomes that, commonly, are socially not preferred. In a recent survey by the Genetics and Public Policy Centre, it was found that 3% of IVF-PGD clinics in the United States reported having provided PGD to couples who seek to select an embryo for the presence of a particular disease or disability, such as deafness, in order that the child share the characteristic with the parents. The idea of 'negative enhancement' is, therefore, both a paradox and a useful means to describe the hidden assumptions behind claims that enhancement technologies can only lead us in one direction -- towards a race of blond, blue-eyed, able-bodied, intellectually magnificent and athletically superior beings. In Australia there does appear to be a legal (if not ethical) consensus that PGD should only be used to select against serious disability. In the UK under the Human Fertilization and Embryology Act (1990) to test using PGD, there must be a significant risk of a serious physical/mental disability/illness/medical condition implying that choosing in favour of a disability would not be acceptable. The proposed amendments to the Human Fertilisation and EmbryologyAct currently before the House of Lords contain an explicit prohibition on the use of PGD to select in favour of a disability. In Europe, the Charter of Fundamental Rights of the European Union, 2000 O.J. (C 364) 1 (December 7, 2000) explicitly prohibits eugenic practices in Article 3(2) but there is uncertainty with respect to how that will be interpreted in relation to disability. Specific regulation of PGD exists in a number of European countries but prohibitions and limits vary across the states.
