Background: Palliative care is insufficiently integrated in the continuum of care for older people. It is unclear to what extent healthcare policy for older people includes elements of palliative care and thus supports its integration. Aim: (1) To develop a reference framework for identifying palliative care contents in policy documents; (2) to determine inclusion of palliative care in public policy documents on healthcare for older people in 13 rapidly ageing countries. Design: Directed documentary analysis of public policy documents (legislation, policies/strategies, guidelines, white papers) on healthcare for older people. Using existing literature, we developed a reference framework and data extraction form assessing 10 criteria of palliative care inclusion. Country experts identified documents and extracted data. Setting: Austria, Belgium, Canada, Czech Republic, England, Japan, Mexico, Netherlands, New Zealand, Singapore, Slovenia, South Korea, Spain. Results: Of 139 identified documents, 50 met inclusion criteria. The most frequently addressed palliative care elements were coordination and continuity of care (12 countries), communication and care planning, care for family, and ethical and legal aspects (11 countries). Documents in 10 countries explicitly mentioned palliative care, nine addressed symptom management, eight mentioned end-of-life care, and five referred to existing palliative care strategies (out of nine that had them). Conclusions: Health care policies for older people need revising to include reference to end-of-life care and dying and ensure linkage to existing national or regional palliative care strategies. The strong policy focus on care coordination and continuity in policies for older people is an opportunity window for palliative care advocacy. [ABSTRACT FROM AUTHOR]

The aim of the paper is to analyse a specific case of transformation of communicative memories into cultural memories under the influence of state institutions in Teschen Silesia [Cieszyn Silesia; Těšín Silesia]. In 1920, the multi-ethnic territory of the former Duchy of Teschen was divided between Poland and Czechoslovakia. The author focuses on the state of local social memories after one hundred years since the division and raises the question, what kind of social memories are supported by the Polish and Czech local administrations and what kind of social memories are ignored or suppressed by them. Both sides of the Polish-Czech state border have at least one common feature today, namely the colonization of the past by the national states present here, in that the history of the Polish part of Teschen Silesia is commonly seen here as the history of Poland, and the history of the Czech part of this area as the history of the Czech Republic. It is an unconscious - and thus unreflective - conceptual structure that affects local national identities. The author calls this cognitive structure "demarcation logic". [ABSTRACT FROM AUTHOR]

The aim of this study is to understand siblinghood in old age through the perspective of Czech older adults. Forty-one in-depth interviews were analyzed, and four themes characterizing siblinghood among older persons were identified in the data: (1) Siblinghood as a unique bond, understood through genetic relatedness, the sense of belongingness, shared memories, and awareness of being the last members of the family of origin. (2) Being consanguineous and, as a result, irrevocably bonded by mutual commitments. (3) Being emotionally supportive and ready for help if needed and as resources allow. (4) The rules of doing siblinghood: the genealogical rules vs. the rule of family of procreation priority; the rule of genetic relatedness vs. the selectivity rule, or rather the preferences of particular siblings; the rules of "good siblinghood" (being interested in themselves, mutual respect, no envy). Introduction and aims: Czech music therapists lacked the skills and knowledge to adapt their practice to virtual music therapy (VMT) at the beginning of the COVID-19 pandemic, when physical contact was restricted. The aims of this project were to implement the best evidence-based recommendations concerning VMT in the practice of Czech music therapists and increase the number of therapists using VMT. Methods: The project used the JBI Evidence Implementation Framework to promote change in the practice of participants. A baseline audit of 26 practitioners was conducted using questionnaires and interviews. The audit criteria consisted of 8 best practice recommendations, followed by the implementation of targeted strategies and a follow-up audit. Results: Of the 26 music therapists, 18 began practicing VMT following the rollout of the implementation strategies. Strategies included educational workshops, instructional materials, technological support, individual counseling, and interactive training sessions. There was an increase in compliance with all audit criteria. The most critical barriers were lack of knowledge about VMT and lack of technical skills; lack of resources; technological challenges; low confidence of music therapists to provide VMT; and reduced need for VMT after the lockdown restrictions were relaxed. Conclusions: The implementation project helped to overcome temporary pandemic restrictions, increase the quality of VMT practice, develop strategies for VMT training, and identify reasons for using VMT during the pandemic and beyond. Background: Studies performed in Central European countries showed a high prevalence of missed nursing care in various clinical settings before the COVID‐19 pandemic. Aims: The aim of the study was to investigate which domains of the work environment were significant predictors of missed nursing care activities in Czech hospitals during the COVID‐19 pandemic. Methods: A cross‐sectional study was used. The RANCARE guideline and STROBE checklist were followed for reporting in the study. The sample consisted of 371 nurses from four acute care hospitals. The MISSCARE Survey and the Practice Environment Scale of the Nursing Work Index questionnaires were used to collect data. The data were analyzed using multiple linear and logistic regression analyses. Results: Nurses reporting unfavorable environments consistently describe a higher frequency of episodes of missed care. Prevalence estimates of missed care in Czech acute care hospitals during the COVID‐19 pandemic was predicted from the overtime work, the nurses' perception of the "Nursing foundations for the quality of care," and their satisfaction with their current position. Conclusions: Missed nursing care could be mitigated by improving the nurses' work environment. Domains of the nurse work environment are known as structural modifiable factors and their refinement could be a cornerstone for interventions to reduce the prevalence of missed nursing care. Implications for nursing policy: Monitoring the conditions and aspects of the nurse work environment in hospitals and considering nurses' concerns about the work environment on an ongoing basis are important strategies for nurse supervision as well as for policymakers. While the focus on emotions has been associated with the rise of psychosocial welfare and has promised a gateway to accommodate individually diversified needs of citizens in policies, the article shows that the role of emotions needs to be better understood. Highlighting emotions can serve both to empower and to patronize those who experience them. Referring to emotions can thus strengthen hierarchies and downplay individual requests to initiate a change. The analysis of professional discourses on birth care in Czechia shows the value of contextualising emotions. While midwifery discourses apply the emotional context of birth to support women in their specific birth choices, medical discourses use the emotional context to patronize them and to limit their requirements. As a result, policy demands are seen as illegitimate when coming from midwives, who want to see women's choices more respected in care. We analyse this dynamic through intimacy. As a conceptual framework used in sociology of care, 'intimacy' ties individual emotional experiences to collective discourses on care, the body and related feelings. Viewing professional discourses on birth care through intimacy reveals the role of emotions in the collective recognition of the personal struggle for the right to give birth in conditions that respect bodily and emotional integrity, which informs how we think of the role of emotions in policies in general. Background Cardiometabolic diseases (CMDs) are the number one cause of death. Selective prevention of CMDs by general practitioners (GPs) could help reduce the burden of CMDs. This measure would entail the identification of individuals at high risk of CMDs—but currently asymptomatic—followed by interventions to reduce their risk. No data were available on the attitude and the extent to which European GPs have incorporated selective CMD prevention into daily practice. Methods A survey among 575 GPs from the Czech Republic, Denmark, Greece, the Netherlands and Sweden was conducted between September 2016 and January 2017, within the framework of the SPIMEU-project. Results On average, 71% of GPs invited their patients to attend for CMD risk assessment. Some used an active approach (47%) while others used an opportunistic approach (53%), but these values differed between countries. Most GPs considered selective CMD prevention as useful (82%) and saw it as part of their normal duties (84%). GPs who did find selective prevention useful were more likely to actively invite individuals compared with their counterparts who did not find prevention useful. Most GPs had a disease management programme for individuals with risk factor(s) for cardiovascular disease (71%) or diabetes (86%). Conclusions Although most GPs considered selective CMD prevention as useful, it was not universally implemented. The biggest challenge was the process of inviting individuals for risk assessment. Using longitudinal qualitative research, the study focuses on the transition to fatherhood and the life changes which fatherhood effects in men's everyday lives. Comparative thematic analysis was used in the study of data provided by two waves of research focusing on 16 dual-earner Czech parental couples. The first wave of interviews took place during pregnancy and the second on the child reaching 18 months. The expectant fathers evinced vague plans concerning approaching fatherhood; some stressed involvement in childcare while others emphasized the provider role. The concept of a "hazy" transition to fatherhood evolved based on the narratives of first-time fathers and is used to describe the indistinct character of the transition to fatherhood. Identity theory is used as the explanatory framework. I suggest that the hazy transition to fatherhood involves the low social appraisal of fatherhood, the breadwinner role, and the central worker identity. [ABSTRACT FROM AUTHOR]

The purpose of this study is to examine how older people make sense of the changing urban environment – that is, how they experience, perceive and interpret their everyday interaction with its materiality, as well as their social ties, networks and relations. The results, based on seven focus groups and 37 individual in-depth interviews with older residents of the three most populous Czech cities, show how older people maintain the continuity of their activities, autonomy and independence within the limits of their personal resources in an active relationship with a changing urban environment and within the post-socialist context. The research supports the results of former studies that emphasise the ability of older adults to negotiate their position and actively cope with change while they age in place.
