Tungsten functional paper (TFP) is a paper-based radiation-shielding material, which is lead-free and easy to cut. We developed a radiation protection undergarment using TFP for prostate cancer patients treated with permanent 125I seed implantation (PSI). The aim of this study was to evaluate the shielding ability of the undergarment with respect to household contacts and members of the public. Between October 2016 and April 2017, a total of 10 prostate cancer patients treated with PSI were enrolled in this prospective study. The external radiation exposure from each patient 1 day after PSI was measured with and without the undergarment. Measurements were performed using a survey meter at 100 cm from the surface of the patient's body. The exposure rates were measured from five directions: anterior, anteriorly oblique, lateral, posteriorly oblique, and posterior. The measured radiation exposure rates without the undergarment, expressed as mean ± standard deviation, from the anterior, anteriorly oblique, lateral, posteriorly oblique, and posterior directions were 1.28 ± 0.43 μSv/h, 0.70 ± 0.34 μSv/h, 0.21 ± 0.062 μSv/h, 0.65 ± 0.33 μSv/h and 1.24 ± 0.41 μSv/h, respectively. The undergarment was found to have (mean ± standard deviation) shielding abilities of 88.7 ± 5.8%, 44.0 ± 42.1%, 50.6 ± 15.9%, 72.9 ± 27.0% and 90.4 ± 10.7% from the anterior, anteriorly oblique, lateral, posteriorly oblique, and posterior directions, respectively. In conclusion, this shielding undergarment is a useful device that has the potential to reduce radiation exposure for the general public and the patient's family.

PurposeTo determine the accuracy of estimated tumour location and required plaque position using fundus diagrams, and to evaluate their applicability in daily clinical practice.Patients and methodsBetween September 2013 and March 2016, all patients treated with ruthenium plaque brachytherapy for choroidal melanoma at the Department of Ophthalmology in Poznań underwent pretreatment planning with the use of printed or electronic fundus diagrams www.oculonco.comThe estimated distances were then verified intraoperatively.ResultsThere were 40 eyes of 40 patients: 15 men, 25 women, with a median age of 61 years (range: 21-88). The median longitudinal basal diameter (LBD) of the treated melanomas was 10.4 mm (range: 6.5-14.9) and the median thickness was 4.1 mm (range: 1.9-6). The final postition of the anterior tumour margin was within 1 mm of the estimated location in 39 cases (97 %), and within 2 mm in 1 case (3 %). Median follow-up was 15 months (range: 3-36). By the close of the study, there was one local tumour recurrence (3%) and one patient died of unrelated cause 1 year after treatment. No patient developed metastases by study close.ConclusionsThe plaque planning system using the paper fundus diagrams proved to be accurate, easily applicable and should aid plaque placement where computer modelling is not possible.

Background: The steep radiation dose gradients in cervical cancer brachytherapy (BT) necessitate a thorough understanding of the behavior of afterloader source cables or needles in the curved channels of (patient-tailored) applicators., Purpose: The purpose of this study is to develop and validate computer models to simulate: (1) BT source positions, and (2) insertion forces of needles in curved applicator channels. The methodology presented can be used to improve the knowledge of instrument behavior in current applicators and aid the development of novel (3D-printed) BT applicators., Methods: For the computer models, BT instruments were discretized in finite elements. Simulations were performed in SPACAR by formulating nodal contact force and motion input models and specifying the instruments' kinematic and dynamic properties. To evaluate the source cable model, simulated source paths in ring applicators were compared with manufacturer-measured source paths. The impact of discrepancies on the dosimetry was estimated for standard plans. To validate needle models, simulated needle insertion forces in curved channels with varying curvature, torsion, and clearance, were compared with force measurements in dedicated 3D-printed templates., Results: Comparison of simulated with manufacturer-measured source positions showed 0.5-1.2 mm median and <2.0 mm maximum differences, in all but one applicator geometry. The resulting maximum relative dose differences at the lateral surface and at 5 mm depth were 5.5% and 4.7%, respectively. Simulated insertion forces for BT needles in curved channels accurately resembled the forces experimentally obtained by including experimental uncertainties in the simulation., Conclusion: The models developed can accurately predict source positions and insertion forces in BT applicators. Insights from these models can aid novel applicator design with improved motion and force transmission of BT instruments, and contribute to the estimation of overall treatment precision. Background: Seed implant brachytherapy (SIBT) is an effective treatment modality for head and neck (H&N) cancers; however, current clinical planning requires manual setting of needle paths and utilizes inaccurate dose calculation algorithms., Purpose: This study aims to develop an accurate and efficient deep convolutional neural network dose engine (DCNN-DE) and an automatic SIBT planning method for H&N SIBT., Methods: A cohort of 25 H&N patients who received SIBT was utilized to develop and validate the methods. The DCNN-DE was developed based on 3D-unet model. It takes single seed dose distribution from a modified TG-43 method, the CT image and a novel inter-seed shadow map (ISSM) as inputs, and predicts the dose map of accuracy close to the one from Monte Carlo simulations (MCS). The ISSM was proposed to better handle inter-seed attenuation. The accuracy and efficacy of the DCNN-DE were validated by comparing with other methods taking MCS dose as reference. For SIBT planning, a novel strategy inspired by clinical practice was proposed to automatically generate parallel or non-parallel potential needle paths that avoid puncturing bone and critical organs. A heuristic-based optimization method was developed to optimize the seed positions to meet clinical prescription requirements. The proposed planning method was validated by re-planning the 25 cases and comparing with clinical plans., Results: The absolute percentage error in the TG-43 calculation for CTV V100 and D90 was reduced from 5.4% and 13.2% to 0.4% and 1.1% with DCNN-DE, an accuracy improvement of 93% and 92%, respectively. The proposed planning method could automatically obtain a plan in 2.5 ± 1.5 min. The generated plans were judged clinically acceptable with dose distribution comparable with those of the clinical plans., Conclusions: The proposed method can generate clinically acceptable plans quickly with high accuracy in dose evaluation, and thus has a high potential for clinical use in SIBT., (© 2023 American Association of Physicists in Medicine.)

Brachytherapy began at the turn of the 20th century, contemporary with external-beam radiotherapy. Physicists and physicians together developed the field. There has not been a period since the beginning that has not witnessed innovations and progress in brachytherapy. At the time of this article, the pace of change in the field has never been more rapid, particularly in image-guided brachytherapy and the development of unconventional sources and treatment techniques.

Background: After brachytherapy, fewer prostate biopsy cores at diagnosis can underestimate the pathological characteristics of prostate cancer (PCa) with lower concordance, resulting in improper treatment, particularly in patients with low-risk nonpalpable cT1c PCa. The aim of this study was to assess the relationship between the number of biopsy cores at diagnosis and long-term clinical outcomes after brachytherapy for cT1c PCa., Methods: We reviewed 516 patients with localized cT1c PCa with Gleason scores of 3 + 3 = 6 or 3 + 4 = 7 who underwent brachytherapy as monotherapy without hormonal therapy between January 2005 and September 2014 at our institution. Clinical staging was based on the American Joint Committee on Cancer manual for staging. Thus, the cT1c category is based solely on digital rectal examination. The primary outcome was biochemical recurrence (BCR). Based on the optimized cutoff value for biopsy core number obtained from receiver operating characteristic analysis, patients were divided into the biopsy cores ≤8 (N = 123) and ≥9 (N = 393) groups. The BCR-free survival rate was compared between the groups. Prognostic factors for BCR were evaluated, including age, initial prostate-specific antigen (PSA) level, Gleason score, positive core rate, PSA density, prostate magnetic resonance imaging findings, and biopsy core number., Results: The median patient age was 66.0 years (interquartile range [IQR]: 61.0-71.0 years), and the median follow-up time was 11.1 years (IQR: 9.5-13.3 years). The median number of core biopsies was 12 (IQR: 9-12). The area under the curve was 0.637 (95% confidence interval [CI]: 0.53-0.75), and the optimal biopsy core cutoff value for BCR prediction was 8.5 (sensitivity = 43.5%, specificity = 77.1%). Although fewer patients had Gleason scores of 3 + 4 = 7 (19/123 [15%] vs. 125/393 [32%], p < 0.02) in the biopsy cores ≤8 group, the 10-year BCR-free survival rate was significantly lower in the biopsy cores ≤8 group than in the biopsy cores ≥9 group (93.8% vs. 96.3%, p < 0.05). Multivariate analysis revealed that a lower biopsy core number (hazard ratio: 0.828, 95% CI: 0.71-0.97, p < 0.03) and a Gleason score of 3 + 4 = 7 (hazard ratio: 3.26, 95% CI: 1.37-7.73, p < 0.01) significantly predicted BCR., Conclusions: A low number of prostate core biopsies results in worse BCR-free survival after brachytherapy as monotherapy in patients with cT1c PCa., (© 2024 Wiley Periodicals LLC.)

(166)Ho can be considered as a potential radionuclide for intravascular brachytherapy (IVBT) using liquid-filled balloons owing to its suitable nuclear decay characteristics. The possibility of producing (166)Ho with adequate specific activity using moderate flux reactors and natural holmium target makes it an attractive alternative of (188)Re for developing IVBT agents. Keeping in mind the high thermodynamic stability of lanthanide complexes with polyazamacrocycles, (166)Ho complex of 1,4,8,11-tetraazacyclotetradecane-1,4,8,11-tetraacetic acid (TETA) was prepared and studied for its suitability as a possible agent for IVBT. (166)Ho was produced with adequate specific activity and high radionuclidic purity by irradiating natural Ho(2)O(3) powder. TETA was synthesized by a single step procedure using cyclam as the starting material. (166)Ho-TETA complex was prepared with excellent radiochemical purity and the complex was found to retain its stability for 7 days at room temperature. Biodistribution studies carried out in Wistar rats showed major renal clearance of the injected activity with almost no retention in any of the vital organ/tissue.

Purpose: To assess the accuracy of the movement of a brachytherapy source using a high-speed camera for evaluating source position, dwell time, and transit dose., Methods: A high-speed camera was used to record the source position of an Ir-192 source relative to a ruler within a custom positioning jig in a remote afterloading system. The analyzed frames can be used to assess dwell positions and times. Treatment plans had multiple dwell times equal to 0.1, 0.5, 1.0, and 2.0 s in 2.5- and 5-mm step sizes. Images were acquired at a rate of 146 frames/s. Acquired images were processed to automatically track the actual source using the correlation between a template image and each frame. The brachytherapy dose calculation formalism (AAPM TG43-U1) was applied to each frame to evaluate the transit dose contribution to the total dose., Results: The differences in measured source positions from the nominal for dwell times equal to 0.1, 0.5, 1.0, and 2.0 s in treatment plans were approximately ≤1 mm. The corresponding differences in measured dwell times from the nominal values at 5 mm steps were -15, -9, -5, and 5 ms, respectively. The source velocities at 5 mm steps were approximately 393 mm/s. The dose differences at 5 mm from the source movement with and without the transit dose for these dwell times were 38%, 7%, 3%, and 2%, respectively., Conclusions: Recording a brachytherapy source using a high-speed camera allowed the evaluation of positional and dwell time accuracies as well as dosimetry assessments, such as the transit dose, based on the application of AAPM TG-43U1., (© 2022 American Association of Physicists in Medicine.)

188ReO(4)(-), (188)Re-MAG(3), and (188)Re-DTPA are currently under investigation as radiation sources in liquid-filled balloons for prevention of restenosis following coronary angioplasty. Because (99m)Tc-labeled ethylene dicysteine (EC) is a well-established agent for renal tubular function imaging, the use of [(188)Re] rhenium-labeled EC as a potential agent for prevention of restenosis after angioplasty is worth evaluation. In this article, the preparation and pharmacological behavior of [(188/186)Re]Re complex of EC are reported. The yield of the Re complex was optimized by varying the parameters of complexation. The complex prepared under the optimized conditions was found to be stable over a period of 7 days when stored at pH 2 and at 4 degrees C. The pharmacological behavior of [(188/186)Re]Re-EC confirms its similarity to (188)Re-MAG(3) and its superiority over (188)ReO(4)(-) for use in endovascular brachytherapy.

Background: Transrectal prostate brachytherapy (BT) involves the permanent implantation of radioactive seeds through a needle, which must be inserted along a straight path to satisfy dose distribution requirements. However, this process is complicated by bevelled needle tips that can cause deflection during penetration. In clinical practice, physicians typically rotate the bevelled tip intermittently or apply manual correction forces near the insertion point, to reduce needle deflection. While assisted rotational insertion robots have made substantial progress in the past 20 years, tissue sticking can be caused by rotation of the bevelled tip and there are currently few studies on the use of corrective forces. As such, an auxiliary needle insertion guide for transrectal prostate BT, based on corrective forces, is investigated in this study for the first time., Methods: The proposed BT guide is designed to reduce needle deflection and was experimentally verified by in vitro experiments. An energy-based deflection model was developed to predict needle motion as corrective forces were applied during insertion. An experimental platform was constructed to perform corrective force-assisted punctures, using the magnitude of the corrective force (A), the position of corrective force application (B) and the puncture speed (C) as test factors, with needle deflection as a test indicator. Design-Expert 8.0.5b software was used for simulation, and a high-definition camera acquired pictures of the needle tip as it pierced the tissue. A regression equation was also established for the test factors and test indicators, using Design-Expert software. Optimal parameter combinations (A, B and C) were determined through optimization., Results: Calculated needle deflection values were then compared with the measured position of the needle insertion point, producing an average error of 0.39 ± 0.28 mm. Deflection was as low as 0.8 mm using optimal puncture parameters CONCLUSIONS: A needle-tissue interaction model, considering tissue nonlinearity, which experimental results demonstrated to be highly accurate. Purpose: Conventional 99m Tc-macroaggregated albumin ( 99m Tc-MAA) planar scintigraphy overestimates lung shunt fraction (LSF) compared to SPECT/CT. However, the respiratory motion artifact due to the temporal mismatch between static SPECT and helical CT (HCT) may compromise the SPECT quantitation accuracy by incorrect attenuation correction (AC) and volume-of-interest (VOI) segmentation. This study aims to evaluate AC and VOI segmentation effects systematically and to propose a CT map for LSF and tumor-to-normal liver ratio (TNR) estimation in static 99m Tc-MAA SPECT/CT., Methods: The 4D XCAT phantom was used to simulate a phantom population of 120 phantoms, modeling 10 different anatomical variations, nine TNRs (2-13.2), nine tumor sizes (2-6.7 cm diameter), eight tumor locations, three axial motion amplitudes of 1, 1.5, and 2 (cm), and four LSFs of 5%, 10%, 15%, and 20%. An analytical projector for low-energy high-resolution parallel-hole collimator was used to simulate 60 noisy projections over 360°, modeling attenuation and geometric collimator-detector response (GCDR). AC and VOI mismatch effects were investigated independently and together, using cine average CT (CACT), HCT at end-inspiration (HCT-IN), mid-respiration (HCT-MID), and end-expiration (HCT-EX) respectively as attenuation and segmentation maps. SPECT images without motion, AC, and VOI errors were also generated as reference. LSF and TNR errors were measured as compared to the ground truth., Results: HCT-MID has slightly better performance for AC effect compared with other CT maps in LSF and TNR estimation, while HCT-EX and HCT-MID perform better for VOI effect. For a respiratory motion amplitude of 1.5 cm and a LSF of 5%, the LSF errors are 19.56 ± 4.58%, -6.79 ± 1.74%, 77.29 ± 14.74%, and 111.25 ± 18.29% corresponding to HCT-MID, HCT-EX, HCT-IN, and CACT in static SPECT. The TNR errors are -12.38 ± 6.42%, -20.55 ± 11.25%, -20.89 ± 9.98%, and -22.89 ± 14.38% respectively. HCT-MID has the best performance for LSF estimation for LSF > 10% and TNR estimation, followed by HCT-EX, HCT-IN, and CACT., Conclusions: The HCT-MID is recommended for AC and segmentation to alleviate respiratory artifacts and improve quantitation accuracy in 90 Y radioembolization treatment planning. Background: Image-guided brachytherapy (BT) robots can be used to assist urologists during seed implantation, thereby improving therapeutic effects. However, safety issues must be considered in the design of such robots, including their structure, mechanical movements, function, materials and actuators. Previous reviews focused on image-guided prostate BT robot technology (e.g., imaging and robot navigation technology and robot system introduction); however, this review is the first time that safety issues have been investigated as part of a study on low-dose-rate (LDR) prostate BT robots., Methods: Multiple electronic databases were searched for LDR prostate BT robot articles published during the last 24 years (1996-2020), with a particular focus on two aspects of robots: safety in design and use., Results: We retrieved a total of 26 LDR prostate BT robots. BT robots were divided into ultrasound, computed tomography, magnetic resonance imaging and fusion-guided systems. The conditions associated with each system were then analysed to develop a set of requirements for the safety of prostate BT robots. Recommendations are also provided for future BT robot development., Conclusions: The transrectal approach for prostate seed implantation is safer than the traditional transperineal approach. Background: This study measured and calculated dose distributions around a unique gold plaque for whole-eye radiotherapy (to treat retinoblastoma). The applicator consists of a pericorneal ring attached to the four extraocular muscles and four legs, each loaded with I-125 seeds. They are inserted beneath the conjunctiva in-between each pair of muscles and attached anteriorly to the ring. The applicator was designed in such a way that the dose is directed toward the middle of the eye while sparing surrounding tissues., Purpose: (I) To compare the measured and calculated data obtained by thermoluminescent dosimeters (TLDs) in a solid-water phantom, a Gafchromic film in a solid-water phantom, the treatment planning systems, and Monte Carlo simulations; (II) to use Monte Carlo simulations for the determination of the dose to the organs at risk by taking the gold shielding and the anisotropy into account., Methods: The dose around the applicator was measured using TLDs and Gafchromic EBT2 film in eye-shaped solid-water phantoms. Dose calculations were performed with the TheraPlan Plus and BrachyVision planning system and Monte Carlo simulations with egs&#95;brachy code. A computer-aided design drawing of the applicator was created and used to create the input file for the Monte Carlo simulations., Results: Monte Carlo calculated dose to the optic nerve is 64.8% of the central dose in the eye, whereas the planned dose is 93.7%. The Monte Carlo lens dose varies from 72.0% to 86.1%, whereas the planned dose varies from 73.0% to 84.3%. Monte Carlo-calculated dose to the bony orbit is 11.3%, whereas the planned dose is as high as 54.7% compared to the dose in the center region of the eye., Conclusions: The measured and Monte Carlo-simulated dose distributions matched well, whereas planned dose distributions showed discrepancies in some areas of the eye and outside of the eye due to their ignorance of the shielding effects of the plaque., (© 2022 American Association of Physicists in Medicine.)

While most Radiation Oncology clinics have adopted electronic charting in one form or another, no consensus document exists that provides guidelines for safe and effective use of the Radiation Oncology electronic medical records (RO-EMR). Task Group 262 was formed to provide these guidelines as well as to provide recommendations to vendors for improving electronic charting functionality in future. Guidelines are provided in the following areas: Implementation and training for the RO-EMR, acceptance testing and quality assurance (QA) of the RO-EMR, use of the RO-EMR as an information repository, use of the RO-EMR as a workflow manager, electronic charting for brachytherapy and nonstandard treatments, and information technology (IT) considerations associated with the RO-EMR. Purpose: Brachytherapy (BT) deals with high gradient internal dose irradiation made up of a complex system where the source is placed nearby the tumor to destroy cancerous cells. A primary concern of clinical safety in BT is quality assurance to ensure the best matches between the delivered and prescribed doses targeting small volume tumors and sparing surrounding healthy tissues. Hence, the purpose of this study is to evaluate the performance of a point size inorganic scintillator detector (ISD) in terms of high dose rate brachytherapy (HDR-BT) treatment., Methods: A prototype of the dose verification system has been developed based on scintillating dosimetry to measure a high dose rate while using an 192 Ir BT source. The associated dose rate is measured in photons/s employing a highly sensitive photon counter (design data: 20 photons/s). Dose measurement was performed as a function of source-to-detector distance according to TG43U1 recommendations. Overall measurements were carried out inside water phantoms keeping the ISD along the BT needle; a minimum of 0.1 cm distance was maintained between each measurement point. The planned dwell times were measured accurately from the difference of two adjacent times of transit. The ISD system performances were also evaluated in terms of dose linearity, energy dependency, scintillation stability, signal-to-noise ratio (SNR), and signal-to-background ratio (SBR). Finally, a comparison was presented between the ISD measurements and results obtained from TG43 reference dataset., Results: The detection efficiency of the ISD was verified by measuring the planned dwell times at different dwell positions. Measurements demonstrated that the ISD has a perfectly linear behavior with dose rate (R 2  = 1) and shows high SNR (>35) and SBR (>36) values even at the lowest dose rate investigated at around 10 cm from the source. Standard deviation (1σ) remains within 0.03% of signal magnitude, and less than 0.01% STEM signal was monitored at 0.1 cm source-to-detector distance. Stability of 0.54% is achieved, and afterglow stays less than 1% of the total signal in all the irradiations. Excellent symmetrical behavior of the dose rate regarding source position was observed at different radiation planes. Finally, a comparison with TG-43 reference dataset shows that corrected measurements agreed with simulation data within 1.2% and 1.3%, and valid for the source-to-detector distance greater than 0.25 cm., Conclusion: The proposed ISD in this study anticipated that the system could be promoted to validate with further clinical investigations. Background: Brachytherapy of thin skin tumors using beta particles can protect underlying sensitive structures such as the bone because of the rapid dose falloff of this type of radiation in tissue. The current work describes a skin brachytherapy applicator, based on beta radiation, that can provide the needed cell-killing radiation dose matched to the shape of individual skin tumors., Materials and Methods: The applicator and its template were fabricated using 3D printing technology. Any clinically approved beta-emitting isotope in the form of a radioactive gel could theoretically be used in this applicator. Monte Carlo simulations were employed to study the capability of the applicator in conforming dose distribution based on the shape of the tumor. Dose profile in the shallow depth, transverse dose profiles at different depths, and the percent depth dose from this applicator were calculated. The radioisotope of choice for our calculations was Yttrium-90 (Y-90)., Results: Using the proposed applicator, it is possible to create a desired dose profile matching the tumor surface shape., Conclusion: The short-range of the beta radiation, together with the dose conforming capability of the applicator, may lead to minimal interactions with the healthy tissue around the skin lesion., (© 2019 The Authors. Purpose: The purpose of this study is to improve upon a recently introduced bi-objective treatment planning method for prostate high-dose-rate (HDR) brachytherapy (BT), both in terms of resulting plan quality and runtime requirements, to the extent that its execution time is clinically acceptable., Methods: Bi-objective treatment planning is done using a state-of-the-art multiobjective evolutionary algorithm, which produces a large number of potential treatment plans with different trade-offs between coverage of the target volumes and sparing organs at risk. A graphics processing unit (GPU) is used for large-scale parallelization of dose calculations and the calculation of the dose-volume (DV) indices of potential treatment plans. Moreover, the objectives of the previously used bi-objective optimization model are modified to produce better results., Results: We applied the GPU-accelerated bi-objective treatment planning method to a set of 18 patients, resulting in a set containing a few hundred potential treatment plans with different trade-offs for each of these patients. Due to accelerations introduced in this article, results previously achieved after 1 hour are now achieved within 30 seconds of optimization. We found plans satisfying the clinical protocol for 15 of 18 patients, whereas this was the case for only 4 of 18 clinical plans. Higher quality treatment plans are obtained when the accuracy of DV index calculation is increased using more dose calculation points, requiring still no more than 3 minutes of optimization for 100 000 points., Conclusions: Large sets of high-quality treatment plans that trade-off coverage and sparing are now achievable within 30 seconds, due to the GPU-acceleration of a previously introduced bi-objective treatment planning method for prostate HDR brachytherapy. Higher quality plans can be achieved when optimizing for 3 minutes, which we still consider to be clinically acceptable. Background: Interstitial low dose rate brachyther-apy is established organ spar-ing treatment of T1-&#8202;T2 penile carcinoma. Experience with high-dose rate brachyther-apy is limited in this indication., Materials and Methods: Twenty-six patients with early penile carcinoma were treated by high-dose rate brachyther-apy at dose 18 × 3 Gy per fraction twice daily between 2002-&#8202;2018 at the Department of Oncology and Radiother-apy, University Hospital in Hradec Kralove. Breast interstitial brachyther-apy template was used for fixation and precise geometry reconstruction of stainless hollow needles., Results: Median follow up was 85 months (range 7-&#8202;200 months). Acute reaction usually consisted of grade 2 mucositis that dissolved dur-ing 8 weeks after the treatment. Local recurrence occurred in 6 patients, 5 of them were successfully treated with partial amputation. One patient had a nodal recurrence successfully salvaged by lymphadenectomy. One patient developed necrosis of the glans requir-ing partial amputation. Currently, there are 24 patients alive without signs of dis-ease. One patient died of cardiac comorbidity, one died of duplicate lung cancer. Nineteen patients have a preserved penis (73%), 18 of them sexually active before treatment report satisfactory intercourse., Conclusion: Hyperfractionated interstitial high-dose rate brachyther-apy with 18 × 3 Gy per fraction twice daily is a promis-ing method in selected patients with penile carcinoma and deserves further evaluation in a larger prospective study. Simulators have revolutionized medical education and training across various disciplines, offering unique advantages in skill acquisition and performance improvement. In the context of interventional radiation therapy (IRT), simulators have emerged as valuable tools for training healthcare professionals in these complex procedures. This narrative review summarized the available evidence on the use of simulators in IRT training, highlighting their impact on proficiency, engagement, and self-confidence as well as their benefits for medical physicists and radiation therapists. A systematic search was conducted in PubMed, resulting in inclusion of 10 papers published since 2009, with 5 of them published since 2020. Publications originated from centers in USA, Ireland, Switzerland, Canada, and Japan, covering a range of IRT settings, including general, prostate, and cervical IRT. The review demonstrated that simulators provide a controlled and realistic environment for skill acquisition, allowing healthcare professionals to practice procedures, optimize image quality, and enhance technical proficiency. The use of simulators addressed the barriers associated with limited caseload and procedural complexity, ultimately contributing to improved education and IRT training. While cost considerations may exist, simulators offer long-term costeffective solutions, balancing the potential benefits in improving educational outcomes and patient care. Overall, simulators play a crucial role in IRT training, enhancing the skills and competence of healthcare providers and improving access to quality IRT care worldwide. Future research should focus on evaluating the long-term impact of simulation-based training on clinical outcomes and patient satisfaction, exploring different simulation models and training approaches, and addressing region-specific barriers to optimize the utilization of IRT. Radical treatment of localised prostate cancer is recognised to be an unnecessary intervention or overtreatment in many men. Consequently, there has been a rapid uptake in the use of focal ablative therapies. However, there are several biological and practical concerns about such approaches as they have yet to be proved as robust treatment options. In particular, the multifocal nature of prostate cancer argues against unifocal treatment, while limitations in imaging can preclude the accurate identification of the number, location, and extent of prostate cancer foci. To date, a number of ablative options have reported results on mainly low-risk disease. Most series are relatively immature, with a lack of consistent follow-up, and the morbidity of retreatment is often not considered. The authors consider focal therapy to be an investigational modality, and encourage prospective recording of outcomes and recruitment of suitable patients., Patient Summary: Focal therapy of prostate cancer is the targeted destruction of cancer within a specific part of the prostate gland, sparing the rest of the prostate and nearby tissue. This procedure could potentially reduce side effects when compared with established standard treatments, such as surgery or radiotherapy, which treat the entire prostate. Studies show that for most men with low-risk cancer, active surveillance is the preferred treatment option. However, the available data regarding all forms of focal therapy are still poor and inconclusive. Objective: In squamous cell carcinoma of the penis (PeCa), treatment options for primary tumors vary by disease stage and may include surgery, radiation, topical chemotherapy, or laser excision. This review aims to highlight the current evidence on the value of radiotherapy as an organ-preserving strategy in primary PeCa., Material and Methods: Manuscripts on primary PeCa treatment with external beam radiotherapy (EBRT) and brachytherapy were evaluated via Scopus, PubMed/MEDLINE, and Web of Science TM (2013-2023) to assess their efficacy and safety. Animal studies, studies with <5 patients, and case reports were excluded., Results: Radiotherapy offers the potential for organ preservation with tumor control rates comparable to radical surgery, while disease-specific survival rates up to 70 % were experienced with EBRT. Brachytherapy (BT) is the preferred method of irradiation for glans-limited tumors, whereas a higher relapse risk is expected for tumors >4 cm. BT shows 73 % amputation-free survival at 8-10 years and 81 % progression-free survival at 5-10 years. Compared with BT, total amputation significantly improves 5-year disease-free survival rate. BT offers a superior 5-year local control and penile preservation rates compared to EBRT. Common acute toxicities of brachytherapy include radiodermatitis, sterile urethritis, and urethral adhesions. The primary late adverse events of BT are soft tissue necrosis (0-31 %) and meatal stenosis (0-43 %)., Conclusion: BT is a favorable radiation modality, offering an efficient and conservative approach. HDR BT is favored for its enhanced dose distribution and radiation protection.
