Scientific-funding bodies are increasingly under pressure to use journal rankings to measure research quality. Hiring and promotion committees routinely hear an equivalent argument: ‘this is important work because it is to be published in prestigious journal X’. But how persuasive is such an argument? This paper examines data on citations to articles published 25 years ago. It finds that it is better to write the best article published in an issue of a medium-quality journal such as the OBES than all four of the worst four articles published in an issue of an elite journal like the AER. This article focuses on all the papers submitted at the Third International Fructan Symposium held at the Utah State University Campus in Logan, USA, from July 21 to July 24, 1996. As for the previous symposium, all aspects of the physiology, biochemistry, chemistry and molecular biology of fructans in plants and micro-organisms were covered in a series of reviews and research presentations. On this occasion, however, only research papers were considered for publication by "The New Phytologist," resulting in a volume, which gives a comprehensive and detailed account of current research in this area.

This paper provides new data of personal consumption expenditure at current and constant prices from 1900 to 1938–1939. The series is linked in with the official estimates, available from 1948–1949, to provide a broadly consistent series of estimates of consumers’ expenditure from Federation, based on new series of data. We comment on the differences in estimates often used to compare trends in real incomes, and attribute the differences as being mainly due to limitations of official data. We use the present results to describe the changes in the pattern of expenditure by main functional groupings, and compare the changes in real per capita expenditure in Australia with that in the UK and USA since the end of the nineteenth century. Objective: The aim of this study was to examine modifiable mediators for socioeconomic disparities in childhood obesity in the United States.Methods: This study used the data of 1,211 mother-child dyads from a US national birth cohort from pregnancy to 6 years post partum. Socioeconomic status was indicated by maternal education (college graduate vs. less) and family income (>185% vs. ≤185% of the poverty line). Single- and multiple-factor mediation analyses were conducted for socioeconomic disparities in childhood obesity at 6 years, adjusting for demographics.Results: The confounder-adjusted relative risk of childhood obesity was 1.79 for low maternal education and 1.42 for low family income. Low-maternal-education-related obesity was individually mediated by maternal preconception BMI (percentage of indirect effect, 8.8%), smoking during pregnancy (7.0%), infant weight gain (14.4%), child sleep duration (11.4%), and TV viewing during weekdays at 6 years (4.9%). Low-family-income-related obesity was mediated by maternal preconception BMI (18.5%), smoking during pregnancy (6.3%), child sleep duration (12.8%), and the home learning environment at 6 years (26.2%). In multiple-mediator analysis, significant mediators together mediated 54.0% of maternal-education-related or 39.4% of family-income-related disparities.Conclusions: Maternal preconception BMI, smoking during pregnancy, infant weight gain, child sleep, TV viewing, and the home learning environment substantially mediated socioeconomic disparities in childhood obesity in the United States. Introduction Collabo RATE is a brief patient survey focused on shared decision making. This paper aims to (i) provide insight on facilitators and challenges to implementing a real-time patient survey and (ii) evaluate Collabo RATE scores and response rates across multiple clinical settings with varied patient populations. Method All adult patients at three United States primary care practices were eligible to complete Collabo RATE post-visit. To inform key learnings, we aggregated all mentions of unanticipated decisions, problems and administration errors from field notes and email communications. Mixed-effects logistic regression evaluated the impact of site, clinician, patient age and patient gender on the Collabo RATE score. Results While Collabo RATE score increased only slightly with increasing patient age ( OR 1.018, 95% CI 1.014-1.021), female patient gender was associated with significantly higher Collabo RATE scores ( OR 1.224, 95% CI 1.073-1.397). Clinician also predicts Collabo RATE score (random effect variance 0.146). Site-specific factors such as clinical workflow and checkout procedures play a key role in successful in-clinic implementation and are significantly related to Collabo RATE scores, with Site 3 scoring significantly higher than Site 1 ( OR 1.759, 95% CI 1.216 to 2.545) or Site 2 (z=−2.71, 95% CI −1.114 to −0.178). Discussion This study demonstrates that Collabo RATE can be used in diverse primary care settings. A clinic's workflow plays a crucial role in implementation. Cost-effectiveness is an important goal for emergency care delivery. The many diagnostic, treatment, and disposition decisions made in the emergency department ( ED) have a significant impact upon healthcare resource utilization. Cost-effectiveness analysis ( CEA) is an analytic tool to optimize these resource allocation decisions through the systematic comparison of costs and effects of alternative healthcare decisions. Yet few emergency medicine leaders and policymakers have any formal training in CEA methodology. This paper provides an introduction to the interpretation and use of CEA with a focus on application to emergency medicine problems and settings. It applies a previously published CEA to the hypothetical case of a patient presenting to the ED with chest pain who requires risk stratification. This paper uses a widely cited checklist to appraise the CEA. This checklist serves as a vehicle for presenting basic CEA terminology and concepts. General topics of focus include measurement of costs and outcomes, incremental analysis, and sensitivity analysis. Integrated throughout the paper are recommendations for good CEA practice with emphasis on the guidelines published by the U.S. Panel on Cost-Effectiveness in Health and Medicine. Unique challenges for emergency medicine CEAs discussed include the projection of long-term outcomes from emergent interventions, costing ED services, and applying study results to diverse patient populations across various ED settings. The discussion also includes an overview of the limitations inherent in applying CEA results to clinical practice to include the lack of incorporation of noncost considerations in CEA (e.g., ethics). After reading this article, emergency medicine leaders and researchers will have an enhanced understanding of the basics of CEA critical appraisal and application. Background: Children with medical complexity (CMC) often face significant barriers to accessing care, obtaining appropriate insurance coverage for medical devices, technology, supplies, home nursing and social services. These challenges, when viewed through the lens of social determinants of health, highlight concerns about healthcare inequity. These inequities can impact CMC by limiting access to follow‐up appointments, leading to disproportionate use of emergency department services, restricting support services, reducing the quality of medical products and increasing the likelihood of adverse events. Addressing these concerns requires comprehensive policy changes at both state and federal levels. Achieving successful collaborations between states and federal agencies is particularly challenging and may take months or even years to accomplish. Objectives: Through an exploratory qualitative approach, this study facilitates a nuanced inquiry into the experiences and systemic challenges encountered by medical professionals and primary caregivers managing CMC who require a paediatric tracheostomy. Methods: Qualitative interviews were conducted with 17 health professionals and primary caregivers residing in the United States. A thematic analysis was used to analyse the transcribed interview data. Results: Using exploratory thematic analysis, we identified challenges and opportunities for improvement regarding (a) access to health insurance, (b) procurement of essential medical supplies, (c) logistical constraints and (d) identifying interim solutions. Conclusion: Building on our findings, we discuss how socioecological factors impact health and quality of life of CMC and families. Additionally, we address the growing gap in quality of care through a comprehensive approach that considers patient needs, regulatory frameworks and affordability. This paper presents longitudinal data representing federal funding for health services research and discusses the observed trends in the larger context of overall funding for research and development in the United States. Objectives: To explore (1) differences in validity and feasibility ratings for geriatric surgical standards across a diverse stakeholder group (surgeons vs. nonsurgeons, health care providers vs. nonproviders, including patient-family, advocacy, and regulatory agencies); (2) whether three multidisciplinary discussion subgroups would reach similar conclusions.Data Source/study Setting: Primary data (ratings) were reported from 58 stakeholder organizations.Study Design: An adaptation of the RAND-UCLA Appropriateness Methodology (RAM) process was conducted in May 2016.Data Collection/extraction Methods: Stakeholders self-administered ratings on paper, returned via mail (Round 1) and in-person (Round 2).Principal Findings: In Round 1, surgeons rated standards more critically (91.2 percent valid; 64.9 percent feasible) than nonsurgeons (100 percent valid; 87.0 percent feasible) but increased ratings in Round 2 (98.7 percent valid; 90.6 percent feasible), aligning with nonsurgeons (99.7 percent valid; 96.1 percent feasible). Three parallel subgroups rated validity at 96.8 percent (group 1), 100 percent (group 2), and 97.4 percent (group 3). Feasibility ratings were 76.9 percent (group 1), 96.1 percent (group 2), and 92.2 percent (group 3).Conclusions: There are differences in validity and feasibility ratings by health professions, with surgeons rating standards more critically than nonsurgeons. However, three separate discussion subgroups rated a high proportion (96-100 percent) of standards as valid, indicating the RAM can be successfully applied to a large stakeholder group. A firm's decision to manufacture abroad depends on location, governance, and strategic factors. Governance factors are firm-specific. In spite of this, most empirical studies of foreign direct investment (FDI) have been conducted at the industry level (making it impossible to look at firm-specific determinants), and only a handful have considered governance, location, and strategic factors simultaneously. This paper is the first large sample study of the determinants of foreign direct investment at the product and firm-level. It examines the impact of location and governance factors, and of four types of strategic interactions, on a Japanese firm's propensity to manufacture in the U.S. The results support the view that foreign direct investment is explained by location, governance, and strategic variables. Economies of scale and trade barriers encourage Japanese FDI in the U.S. The larger a Japanese firm's R&D expenditures, the greater the probability it will manufacture in the U.S., but this is not the case for advertising expenditures. Some strategic factors are also important: Japanese firms with medium domestic market shares have the highest propensity to invest in the U.S. There is evidence of follow-the-leader behavior between firms of rival enterprise groups, but none of 'exchange-of-threat' between American and Japanese firms. The US government has been increasingly supporting postdoctoral training in biomedical sciences to develop the domestic research workforce. However, current trends suggest that mostly international researchers benefit from the funding, many of whom might leave the USA after training. In this paper, we describe a model used to analyse the flow of national versus international researchers into and out of postdoctoral training. We calibrate our model in the case of the USA and successfully replicate the data. We use the model to conduct simulation-based analyses of effects of different policies on the diversity of postdoctoral researchers. Our model shows that capping the duration of postdoctoral careers, a policy proposed previously, favours international postdoctoral researchers. The analysis suggests that the leverage point to help the growth of domestic research workforce is in the pregraduate education area, and many policies implemented at the postgraduate level have minimal or unintended effects on diversity. SIG/USE celebrated members' work and achievements in research methods at the 2012 ASIS&T Annual Meeting through a keynote speech, brief talks and awards. In her opening talk, professor Lisa Given challenged attendees to expand research methods to engage participants more fully, include qualitative findings and explore information behavior in nontraditional media. The topics of two-minute lightning talks ranged from research techniques for exploring young people's information behavior to imaging brain activity related to relevance decisions. Others addressed incorporating mobile technologies in research, collaborative search, direct interaction with participants in contrast with background server log checks and working with data collections from social Q&A sites. Following the talks, small group discussions further explored topics such as cognitive approaches, content analysis and text analytics and usability. The symposium wound to a close with the 2011 Elfreda A. Chatman Research Award presentation on information strategies of the homeless and a preview of the 2012 proposed study on refugees' information seeking. Additional awards were presented for best paper and poster to support travel and further presentations. The data citation panel of the March 2012 Research Data Access and Preservation (RDAP) Summit targeted an issue that has eluded agreement. The practice of data citation has ranged from general acknowledgements to citing papers describing data to specifically citing the original data. A task group on data citation standards and practices, under the aegis of the International Council for Science, organized a workshop and report and will deliver the results of a survey on current practices. The group's forthcoming white paper will examine emerging proposals for standardization and best practices, tools and infrastructure to support data citation, challenges and opportunities.While panelists and attendees acknowledged the difficulty of standardization at an international level, the emergence of DataCite and ORCID for citing data and authors, respectively, are positive signs. Social scientists have expressed strong views on corporate influences over science, but most attention has been devoted to broad, Black/White arguments, rather than to actual mechanisms of influence. This paper summarizes an experience where involvement in a lawsuit led to the discovery of an unexpected mechanism: A large corporation facing a multibillion-dollar court judgment quietly provided generous funding to well-known scientists (including at least one Nobel prize winner) who would submit articles to “open,” peer-reviewed journals, so that their “unbiased science” could be cited in an appeal to the Supreme Court. On balance, the corporation’s most effective techniques of influence may have been provided not by overt pressure, but by encouraging scientists to continue thinking of themselves as independent and impartial. This paper introduces a type of situs analysis into sociological studies of agricultural labor markets. Situs, as it relates to a nonhierarchical differentiation of occupations in the agricultural sector, is operationalized as two or more internally consistent, mutually exclusive, though not necessarily functionally equivalent, occupational families. A three-category situs classification is proposed: (1) production agriculture; (2) agribusiness; and (3) agricultural education/research. Each occupational family (situs) is composed of a series of hierarchically ranked status positions. Taken as a whole, the proposed classification scheme can be viewed as three distinct, but parallel agricultural status ladders. The conceptual and empirical utility of situs analysis is demonstrated using data from the United States Census. Seventy agricultural occupations are grouped according to situs affiliation. Each situs is then examined with respect to modal social and economic characteristics. A simple earnings determination model is also reported for each group of agricultural occupations. Results show that situses differ in their social, economic and demographic make-up and that cultural studies of agricultural labor markets and suggestions for future research projects are offered. The Census of Agriculture is an important source of data for persons interested in studying the structure of agriculture. The census has been used to study agriculture and quality of life in Kansas recently and aggregated state level data was used to describe the relationship between farm size, agriculture and energy consumption. As sociological interest in agriculture, grows, it is likely that the Census of Agriculture will become the source of many more studies. Unfortunately, the units of analysis reported in the Census of Agriculture like-aggregate national, state and county statistics-create problems for those who use them for sociological purposes. This short article will illustrate some of the problems inherent in the use of census materials by reexamining some of the data used on recent papers on energy and agriculture. A serious difficulty posed by the use of census material for sociological research stems from the fact that census data are aggregated on the bases of political units like states and counties.

A simple chance-constrained model of life insurance company portfolio choice is presented in this paper and several hypotheses derived from the model are examined using a cross-sectional, time series panel of 15 annual observations for 92 large U.S. life insurance companies. In addition to the usual yield variables, non-yield variables such as the relative amount of surplus and the cost of reserve liabilities are significant determinants of the composition of life insurance company portfolios. This paper provides a comprehensive examination of the disciplinary role of the corporate takeover market using a sample of U.S. target firms over the period 1979 to 1998. The time period spanned allows a broader study not only of the disciplinary role of the takeover market in general, but also of the interaction between the takeover market and alternative governance mechanisms during the 1980s and 1990s. Overall, our evidence is consistent with the view of the corporate takeover market as a "court of last resort," that is, it is an external source of discipline that intercedes when internal control mechanisms are relatively weak or ineffective. The current ethical structure for collaborative international health research stems largely from developed countries' standards of proper ethical practices. The result is that ethical committees in developing countries are required to adhere to standards that might impose practices that conflict with local culture and unintended interpretations of ethics, treatments, and research. This paper presents a case example of a joint international research project that successfully established inclusive ethical review processes as well as other groundwork and components necessary for the conduct of human behavior research and research capacity building in the host country.

ABSTRACT Both human research and animal research operate within established standards and procedures. Although the human research environment has been criticized for its sometimes inefficient and imperfect process, reported abuses of human subjects in research served as the impetus for the establishment of the Nuremberg Code, Declaration of Helsinki, and the National Commission for the Protection of Human Subjects of Biomedical and Behavioral Research and the resulting Belmont Report. No similar, comprehensive and principled effort has addressed the use of animals in research. Although published policies regarding animal research provide relevant regulatory guidance, these policies have not emerged from the process of specifying consistent and reasoned ethical principles. The lack of a fundamental effort to explore the ethical issues and principles regarding the use of animals in research has led to unclear and disparate policies. Recent studies have increased our understanding of animal cognition and emotion, suggesting that animals' potential for experiencing a wide variety of harms, such as pain and fear, is greater than has been previously appreciated. Furthermore, relationships between methods of captivity and certain laboratory procedures and the resulting adverse physical, social and psychological effects have been established. In light of this information, current protections may need to be reconsidered and modified. This paper explores the historical convergence and divergence in the creation of human and animal research guidelines, as well as opportunities to align ethical frameworks with new scientific discoveries. Objectives: To determine dementia prevalence and costs attributable to dementia using Veterans Health Administration (VHA) data with and without Medicare data.Data Sources: VHA inpatient, outpatient, purchased care and other data and Medicare enrollment, claims, and assessments in fiscal year (FY) 2013.Study Design: Analyses were conducted with VHA data alone and with combined VHA and Medicare data. Dementia was identified from a VHA sanctioned list of ICD-9 diagnoses. Attributable cost of dementia was estimated using recycled predictions.Data Collection: Veterans age 65 and older who used VHA and were enrolled in Traditional Medicare in FY 2013 (1.9 million).Principal Findings: VHA records indicated the prevalence of dementia in FY 2013 was 4.8 percent while combined VHA and Medicare data indicated the prevalence was 7.4 percent. Attributable cost of dementia to VHA was, on average, $10,950 per veteran per year (pvpy) using VHA alone and $6,662 pvpy using combined VHA and Medicare data. Combined VHA and Medicare attributable cost of dementia was $11,285 pvpy. Utilization attributed to dementia using VHA data alone was lower for long-term institutionalization and higher for supportive care services than indicated in combined VHA and Medicare data.Conclusions: Better planning for clinical and cost-efficient care requires VHA and Medicare to share data for veterans with dementia and likely more generally. The 2009 Academic Emergency Medicine consensus conference focused on “Public Health in the ED: Surveillance, Screening and Intervention.” One conference breakout session discussed the significant research value of health-related data sets. This article represents the proceedings from that session, primarily focusing on emergency department (ED)-related data sets and includes examples of the use of a data set based on ED visits for research purposes. It discusses types of ED-related data sets available, highlights barriers to research use of ED-related data sets, and notes limitations of these data sets. The paper highlights future directions and challenges to using these important sources of data for research, including identification of five main needs related to enhancing the use of ED-related data sets. These are 1) electronic linkage of initial and follow-up ED visits and linkage of information about ED visits to other outcomes, including costs of care, while maintaining deidentification of the data; 2) timely data access with minimal barriers; 3) complete data collection for clinically relevant and/or historical data elements, such as the external cause-of-injury code; 4) easy access to data that can be parsed into smaller jurisdictions (such as states) for policy and/or research purposes, while maintaining confidentiality; and 5) linkages between health survey data and health claims data. ED-related data sets contain much data collected directly from health care facilities, individual patient records, and multiple other sources that have significant potential impact for studying and improving the health of individuals and the population. This paper constructs new rankings of economics journals, economics departments, and economists that employ a measure of teaching-focused research productivity, an area of growing importance in recent years. The ranking methodologies presented here use information from articles that were published from 1991 through the early part of 2005 within the Journal of Economic Literature's "economic education" classifications (A200-A290). The Journal of Economic Literature tops the list of journals, followed by the Review of Economics and Statistics and the American Economic Review. Among the top institutions are Vanderbilt University, Indiana University, and the University of Wisconsin. Others that rank high here, such as Oberlin College and Denison University, do not often fare as well using methodologies that evaluate more traditional types of economics research. This paper explores the public health system's differential construction of Mexican and Cuban immigrants' "deservingness" of citizenship benefits and its preparation of them for different roles in U.S. society. Civic institutions such as the public health care system are charged with inculcating normative behavior in immigrants and instilling in them different conceptions about their rights and responsibilities. Faced with limited resources under the implementation of Medicaid managed care, hospital administrators created new categories of "deserving" and "undeserving" immigrants based on neoliberal standards of individual responsibility and self-discipline. As a result, hospital policies construct different types of "cultural citizenship" for Cuban and Mexican immigrants, preparing the former to be active citizens and discouraging the latter from pressing demands on American civil institutions. I show that this negative construction of Mexican immigrants' moral worth leads to unmet health needs and poor health outcomes. [ABSTRACT FROM AUTHOR]

This paper empirically evaluates the impact of the US Advanced Technology Program sponsored consortia on the ex-post research productivity of participating firms. We find that there is a positive association between the intensity of participation in research consortia and the overall research productivity of participants. We also find a positive impact of consortia on the research productivity of participants in the technological areas targeted by the consortia. This positive impact is higher when the average technological proximity of participants is high. The articles might be considered an extra dividend from dollars recently invested in rural research. The focus of this particular national research effort was so calculatedly rural and so unusually rural that the research program was given the name Project Rural. From the point of view of ethnographers living their way into these communities, that assignment required them to give inordinate attention to school problems, school programs and school people. Educator designs for supporting cataclysmic and holistic change in the funding agency were largely translated into a new source for funding ongoing operations and the opportunity to employ new support personnel or purchase new curriculum package.

This paper presents the results of a recently conducted survey of 1500 professional economists living in the United States and Canada The study was undertaken in an attempt to ascertain professional economists' views of the worth and usefulness of their own scientific research. The response rate of the survey was 49 percent (734 respondents) yielding statistical results which suggest that economists do believe their research has some social value. Economists in this survey believe, however, that the spillover benefits from that research are not substantial and are probably limited to certain groups. [ABSTRACT FROM AUTHOR]

Group inequalities in the United States are most often attributed to the characteristics of the individuals who belong to these groups; thinking about structural causes of group inequalities is rare. This paper reviews cognitive, cultural, and systemic reasons for this bias. The efficacy of education as a way to increase structural thinking was investigated in two studies of college students' causal thinking about group inequalities. Both studies involved a course on intergroup relations that covered structural sources of racial or ethnic inequalities. Results supported hypotheses that the course would increase structural thinking about racial or ethnic inequality, and that structural thinking would generalize to inequalities nor explicitly covered in the course. Both course content and active learning pedagogy were related to structural thinking about inequalities. Active learning was also related to applying structural thinking to targets of change. This paper explores the thesis that in a rural low-income area with a high rate of out-migration, the social class position of a family influences the pattern of out-migration, the structure of the migration process, and the economic life chances of individual migrants and families in the areas of destination. Data are from a restudy, after twenty years, of persons who had been living in three isolated mountain neighborhoods in eastern Kentucky. It was found that social class differences in the form or strategy of out-migration contributed to the maintenance and stability of tile social class hierarchy within this migration system. A migrant's social class origin influenced not only when he left the mountains, where he moved, and with whom, but also his subsequent level of living in the urban area. [ABSTRACT FROM AUTHOR]

The present paper is a report of the help patterns among older people and their adult children in three countries—Denmark, Britain, and the United States. The data come from interviews conducted in mid-1962 with nationwide probability samples of approximately 2,500 persons aged 65 and over in each of the three countries. Family help patterns in old age in Denmark, Britain, and the United States differ by the social class position of the older person. The class position of old people effects primarily the size of their immediate families, their family structure, and their living arrangements. While the social class of the old person effects the magnitude and direction of parent-child help, persons in every social class report that they help their children and that their children help them. Objective: Despite the high prevalence of obesity and associated health risks in the United States adult population, few primary care providers (PCPs) have time and training to provide weight-management counseling to their patients. This study aims to compare the effect of referral to a comprehensive automated digital weight-loss program, with or without provider email feedback, with usual care on weight loss in patients with overweight or obesity.Methods: A total of 550 adults (mean [SD], 51.4 [11.2] years, BMI = 35.1 [5.5] kg/m2 , 72.0% female) were enrolled through their PCPs (n = 31). Providers were randomly assigned to refer their patients to a 12-month internet weight-loss intervention only (IWL), the intervention plus semiautomated feedback from the provider (IWL + PCP), or to usual care (EUC). Weight was measured at baseline and at 3, 6, and 12 months.Results: Weight changes (mean [SE]) at 12 months were -0.92 (0.46), -3.68 (0.46), and -3.58 (0.48) kg in the EUC, IWL, and IWL + PCP groups, respectively. Outcomes were significantly different in EUC versus IWL and EUC versus IWL + PCP (p < 0.001), but not in IWL versus IWL + PCP.Conclusions: Referral by PCPs to an automated weight-loss program holds promise for patients with obesity. Background: Uropathogen resistance, fluoroquinolone‐resistance (FQR), and extended spectrum beta‐lactamase (ESBL), has been observed to be emerging worldwide with prevalences above recommended thresholds for routine empirical treatment. The primary aim of our study was to determine the prevalence of FQR from a geographically diverse sample of United States emergency departments (EDs). Methods: We conducted a multi‐center, observational cohort study using a network of 15 geographically diverse US EDs. All patients ≥18 years of age with the primary or secondary diagnosis of urinary tract infection (UTI) in the ED identified using International Classification of Diseases (ICD‐10) diagnosis code of cystitis, pyelonephritis, or UTI from 2018 to 2020 were included. We calculated descriptive statistics for uropathogens and susceptibilities. Logistic regression analysis was used to identify antimicrobial resistance risk factors associated with FQR Escherichia coli. Results: Among 3779 patients who met inclusion criteria, median age was 62.9 years (interquartile range [IQR]: 41–77.6) and 76.3% were female. The most common diagnoses were complicated (41.2%) and uncomplicated cystitis (40.3%). E. coli was the most common pathogen (63.2%), followed by Klebsiella pneumoniae (13.2%) and Enterococcus species (5.8%). Across all sites, overall E. coli FQ‐resistance prevalence was 22.1%, ranging from 10.5 to 29.7% by site. The prevalence of ESBL‐producing uropathogen was 7.4%, ranging from 3.6% to 11.6% by site. Previous IV or oral antimicrobial use in the past 90‐days and history of a multi‐drug resistant pathogen were associated with FQ‐resistant E. coli (odds ratio [OR] 2.68, 95% confidence interval [CI]: 2.04–3.51, and OR 6.93, 95% CI: 4.95–9.70, respectively). Of the patients who had FQ‐resistant E. coli or an ESBL‐producing uropathogen isolated, 116 (37.1%) and 61 (36.7%) did not have any documented risk factors for resistance. Conclusion: FQ‐resistant E. coli is widely prevalent across US sites highlighting the need for ongoing monitoring of antimicrobial resistance and, at some locations, modification of empirical treatments. Objective: A first-in-human responsive deep brain stimulation (rDBS) trial (NCT03868670) for obesity is under way, which is based on promising preclinical evidence. Given the upfront costs of rDBS, it is prudent to examine the success threshold for cost-effectiveness compared with laparoscopic Roux-en-Y gastric bypass (LRYGB).Methods: Efficacy and safety data on LRYGB and safety data on rDBS were collected for established indications through a literature search. The success threshold was defined as minimum BMI reduction. Treatment costs were calculated via Medicare national reimbursement data.Results: LRYGB had a mean BMI reduction of 13.75 kg/m2 . Based on adverse events, LRYGB was a less-preferred health state (overall adverse event utility of 0.96 [0.02]) than rDBS (0.98 [0.01]), but LRYGB ($14,366 [$6,410]) had a significantly lower treatment cost than rDBS ($29,951 [$4,490]; p < 0.0001). Therefore, for rDBS to be cost-effective compared with LRYGB, the multiple models yielded a success threshold range of 13.7 to 15.2 kg/m2 .Conclusions: This study established a preliminary efficacy success threshold for rDBS to be cost-effective for severe obesity, and results from randomized controlled trials are needed. This analysis allows for interpretation of the economic impact of advancing rDBS for obesity in light of ongoing trial results and suggests an attainable threshold is needed for cost-effectiveness. We desired to carefully evaluate a novel autologous heterogeneous skin construct in a prospective randomised clinical trial comparing this to a standard‐of‐care treatment in diabetic foot ulcers (DFUs). This study reports the interim analysis after the first half of the subjects have been analysed. Fifty patients (25 per group) with Wagner 1 ulcers were enrolled at 13 wound centres in the United States. Twenty‐three subjects underwent the autologous heterogeneous skin construct harvest and application procedure once; two subjects required two applications due to loss of the first application. The primary endpoint was the proportion of wounds closed at 12 weeks. There were significantly more wounds closed in the treatment group (18/25; 72%) vs controls (8/25; 32%) at 12 weeks. The treatment group achieved significantly greater percent area reduction compared to the control group at every prespecified timepoint of 4, 6, 8, and 12 weeks. Thirty‐eight adverse events occurred in 11 subjects (44%) in the treatment group vs 48 in 14 controls (56%), 6 of which required study removal. In the treatment group, there were no serious adverse events related to the index ulcer. Two adverse events (index ulcer cellulitis and bleeding) were possibly related to the autologous heterogeneous skin construct. Data from this planned interim analysis support that application of autologous heterogeneous skin construct may be potentially effective therapy for DFUs and provide supportive data to complete the planned study. A series of interviews with four ASIS&T members, all outstanding scholars in metricrelated research, provides insights into their interests, motivations and views on research in the area. Christine L. Borgman recalls being intrigued as a student when hearing about the history of bibliometrics, and she sees expanding metrics research methods as one of the most significant recent advances in the field. Blaise Cronin speaks of reading, meeting and corresponding with leaders in the field from early in his own career. He is impressed by the rise of journals other than JASIST covering informetrics, reflecting expanding interest in the field. Katherine W. McCain was drawn to the field when she was recognized for a citation analysis and collection assessment carried out while she served as a biology librarian. Among the challenges McCain identifies are more meaningful analysis of huge datasets, research funding and gaining understanding through the different perspectives of boundary spanners. Howard White recalls how a friendly conversation led to special access to a proprietary data gathering tool, resulting in the first author co-citation map and his co-authoring a highly acclaimed article. He notes significant advances in the mathematical foundations of bibliometrics and a rise in importance of bibliometrics for assessing national research programs. All four researchers are pleased to see the rise of altmetrics and data visualization capabilities, though they share concerns about the validity and reliability of metrics-based research and trustworthiness of platforms and data sets. Standard triplicate blood pressure (BP) measurements pose time barriers to hypertension screening, especially in resource-limited settings. We assessed the implications of simplified approaches using fewer measurements with adults (≥18 years old) not using anti-hypertensive medications from the US National Health and Nutrition Examination Survey 1999-2016 (n = 30 614), and two datasets from May Measurement Month 2017-2018 (n = 14 795 for Nepal and n = 6 771 for India). We evaluated the proportion of misclassification of hypertension when employing the following simplified approaches: using only 1st BP, only 2nd BP, 2nd if 1st BP in a given range (otherwise using 1st), and average of 1st and 2nd BP. Hypertension was defined as average of 2nd and 3rd systolic BP ≥140 and/or diastolic BP ≥90 mm Hg. Using only the 1st BP, the proportion of missed hypertension ranged from 8.2%-12.1% and overidentified hypertension from 4.3%-9.1%. Using only 2nd BP reduced the misclassification considerably (corresponding estimates, 4.9%-6.4% for missed hypertension and 2.0%-4.4% for overidentified hypertension) but needed 2nd BP in all participants. Using 2nd BP if 1st BP ≥130/80 demonstrated similar estimates of missed hypertension (3.8%-8.1%) and overidentified hypertension (2.0%-3.9%), but only required a 2nd BP in 33.8%-59.8% of participants. In conclusion, a simplified approach utilizing 1st BP supplemented by 2nd BP in some individuals has low misclassification rates and requires approximately half of the total number of measurements compared to the standard approach, and thus can facilitate screening in resource-constrained settings.
