Background: Self‐harm is a major public health concern. Lifetime prevalence is high, and rates of self‐harm are rising; however, available interventions do not benefit everyone, and engagement with therapy can be low. Qualitative accounts allow for a greater understanding of what is helpful to individuals. This study aimed to synthesize the experiences of interventions for self‐harm, from participants who have participated in these themselves. Methods: Participants had self‐harmed at least once and undergone an individual psychotherapeutic intervention for self‐harm. Papers not written or translated to the English language were excluded. Four databases (Medline, CINAHL, Web of Science and PsycINFO) were systematically searched and each paper was assessed using the CASP quality appraisal tool. A meta‐ethnographic approach to the synthesis was taken. Results: Ten studies encompassing 104 participants were included. Four overarching themes were developed and the importance of recognizing the person beyond the self‐harm emerged through a line of argument synthesis. Building a trusted, therapeutic relationship founded on patience and without judgement was essential for the perceived success of therapy, which was unique to each person and often went beyond reducing self‐harming behaviours. Limitations: Papers included in the study showed a lack of diversity with regard to ethnicity and gender. Conclusions: The findings illustrate the importance of the therapeutic alliance when working with self‐harm. Clinical implications of this paper include the importance of utilizing key therapeutic competencies which should be considered fundamental to change within psychotherapeutic interventions for self‐harm, with the uniqueness of each patient recognized throughout. Objectives: This paper aimed to explore the experiences of depressed adolescents who completed but did not 'respond' to standard psychotherapy, based on a lack of improvement in pre‐post symptoms scores. Design: This was a qualitative study employing interpretative phenomenological analysis (IPA). Method: Seventy‐seven adolescents with moderate to severe depression were interviewed as part of a qualitative arm of a randomised controlled trial. Five adolescents' post‐treatment interviews were purposively sampled, based on lack of improvement on pre‐post symptom scores, and adolescents still scoring above the clinical threshold for depression. The interviews were analysed using IPA. Results: Adolescents made sense of their depression as part of their identity and held negative expectations of therapy. Some aspects of therapy brought up intolerable feelings that contributed to disengagement in the therapeutic process and culminated in disappointing and hopeless endings. On the other hand, where a stronger therapeutic relationship was developed, some participants experienced certain improvements. Conclusions: Findings highlight how actively exploring the adolescent's therapy expectations, developing a strong early therapeutic relationship and being mindful of the potential impact of endings are important in therapeutic work with adolescents with depression, especially where they may have a strong sense of hopelessness and self‐criticism. Moreover, the finding that adolescents experienced improvements in other domains despite a lack of symptom reduction highlights the need to review how treatment outcomes are currently defined. Background Within contemporary medical practice, Parkinson's disease ( PD) is treated using a biomedical, neurological approach, which although bringing numerous benefits can struggle to engage with how people with PD experience the disease. A bio-psycho-social approach has not yet been established in PD; however, bio-psycho-social approaches adopted within dementia care practice could bring significant benefit to PD care. Methods This paper summarises existing bio-psycho-social models of dementia care and explores how these models could also usefully be applied to care for PD. Specifically, this paper adapts the bio-psycho-social model for dementia developed by Spector and Orrell (), to suggest a bio-psycho-social model, which could be used to inform routine care in PD. Results Drawing on the biopsychosocial model of Dementia put forward by Spector and Orrell (), this paper explores the application of a bio-psycho-social model of PD. This model conceptualises PD as a trajectory, in which several interrelated fixed and tractable factors influence both PD's symptomology and the various biological and psychosocial challenges individuals will face as their disease progresses. Using an individual case study, this paper then illustrates how such a model can assist clinicians in identifying suitable interventions for people living with PD. Conclusion This model concludes by discussing how a bio-psycho-social model could be used as a tool in PD's routine care. The model also encourages the development of a theoretical and practical framework for the future development of the role of the PD specialist nurse within routine practice. Implications for practice A biopsychosocial approach to Parkinson's Disease provides an opportunity to move towards a holistic model of care practice which addresses a wider range of factors affecting people living with PD. Abstract: This study discusses the current status of evidence‐based psychological therapies for generalised anxiety disorder (GAD). It points to the dominance of cognitive‐behavioural therapies (CBTs) and compares this dominance with the position of humanistic‐experiential therapies (HEPs) in this area. The paper hypothesises several reasons for this situation, including historical developments as well as HEPs’ ambivalence around embracing mainstream mental health classification systems, and corresponding types of research. The paper then highlights some recent developments in HEPs for anxiety disorders (particularly emotion‐focused therapy; EFT); developments that are breaking new conceptual grounds while also generating new outcome research. The reader of this commentary on Haley's paper by Stein will note that Stein refers to Haley as ‘the author of the critiqued paper’ or ‘ACP,’ rather than by name. This unusual style was occasioned by the Editor's decision that Stein should comment on the paper without any knowledge of its authorship, in order that the focus of Stein's comments be on the substance of that paper and that his comments not be influenced by who had written it. It is unclear what the best psychological treatment is for depression and anxiety in people with co-morbid personality disorder. Trials of different psychological treatment options for this patient group have been conducted, but this evidence has not previously been systematically reviewed or critically appraised. We set out to conduct a scoping review in order to describe which psychological therapies appear most effective in treating depression and/or anxiety in patients with co-morbid personality disorder. PsycINFO, Cochrane library trials, Medline and Embase databases were searched for studies involving randomized, controlled, experimental, parallel-arm comparisons, examining any well-defined, psychotherapeutic intervention for adults, in an outpatient setting, with a clearly defined diagnosis of depression and/or anxiety, and co-morbid personality disorder. A total of 1662 papers were identified. Fifteen met criteria for inclusion and were reviewed. There was weak evidence to support the use of cognitive behavioural therapy as a psychological treatment for depression in patients with co-morbid personality disorder. However, the literature is characterized by considerable methodological heterogeneity, and further research is needed before there is sufficient evidence to indicate which psychological treatment would be most effective in treating anxiety and/or depression in this patient group. Over the last several decades single session psychotherapy has become a mainstream psychotherapeutic approach. A steadily increasing demand for psychotherapeutic services and a continuous reduction in mental health care resources worldwide are both largely responsible for this development. This paper describes single session psychotherapy and provides a brief review of the relevant literature. Single session psychotherapy delivered in walk-in settings is also discussed. Therapists who are unfamiliar with single session psychotherapy will find that this paper provides a concise introduction to the essential processes and outcomes of this psychotherapeutic approach. Purpose This is the third paper in a series that reviews strategies for optimizing the validity and utility of randomized clinical trials (RCTs) in addictions treatment research. Whereas the two previous papers focused on design and implementation, here we address issues pertaining to data processing and statistical analysis. Scope Recommendations for enhancing data quality and utility are offered in sections on data coding and entry; and data format, structure and management. We discuss the need for preliminary data analyses that examine statistical power; patterns of attrition; between-group equivalence; and treatment integrity and discriminability. We discuss tests of treatment efficacy, as well as ancillary analyses aimed at explicating treatment processes. Conclusions Safeguards are necessary to protect data quality, and advance planning is needed to ensure that data formats are compatible with statistical objectives. In addition to treatment efficacy, statistical analyses should evaluate study internal and external validity, and investigate the change mechanisms that underlie treatment effects. The goal of this paper is to show (i) how the moment-to-moment tracking and processing of emotion to completion —in an emotionally engaged patient–therapist dyad where the individual feels safe and known—constitutes a powerful mechanism of therapeutic transformation, and (ii) how positive emotions are sensitive affective markers of that transformational process. Evidence from transformational studies is used to elaborate the vital role of positive emotions in the process of change in general, and, more specifically, in the course of therapeutic work with painful and overwhelming emotional experience. It is proposed that these emergent positive emotions are affective markers which signal the operation of healing transformational processes in psychotherapy. Several types of positive emotion that arise spontaneously during moment-to-moment experiential therapeutic work will be described and their role as markers of different transformational processes will be elucidated. Topic: The Brief Services psychotherapy paradigm has evolved to produce cost‐effective and clinically significant outcomes in children's mental health. Though its clinical evaluation is still in its infancy, it has been utilized to a much lesser degree with autism spectrum disorders, which typically require a longer‐term approach to psychotherapy treatment. Purpose: The current paper introduces a novel, four‐session intervention that aims to decrease anxiety experienced by clients presenting with high‐functioning autism. Externalizing Metaphors Therapy is based upon the externalization of problems, transformation of metaphoric imagery, shifting of maladaptive emotional schemas, and the generalization of problems. A case study is utilized to visually aid in the understanding of this new treatment intervention. Over the past 30 years a systemic approach has been applied to complex cases involving intellectual disability and behaviours of concern. This paper describes a model that draws on systemic family therapy and network therapy as well as the use of a reflective team. A single session intervention was provided for families of people with intellectual disability. A systemic consultation was used as a pre‐session meeting for professionals working with the family. The systemic consultation and single session therapy allowed the family and the professional network to gain insight. Involving the professionals in the therapy session also enhanced the alliance between professionals and family. This approach assists everyone involved in the case to find a way to progress. Accessible Summary: Lots of people with intellectual disabilities in the UK are given medication to manage their behaviours and the NHS wants this to stop. In Cornwall, we wanted to see how often we give medication to people instead of other health care such as communication or occupational assessments etc. We found that around half the people we work with because of their behaviours get medication to manage their feelings and many do not get other assessments. We want to change this in Cornwall and stop over medicating people by learning from this study. Abstract: Background: The use of psychotropic medication to manage challenging behaviours of people with intellectual disabilities is a contentious issue which NHS England has now focused on. This paper looks to evaluate this within the multidisciplinary context. Method: Records of clients (n = 106) open to a Community Intellectual Disabilities team for care relating to challenging behaviours were examined. Those recommended psychotropic medications in the absence of a mental health diagnosis were scrutinised for supporting MDT assessments. Results: Over half the clients with challenging behaviours were being prescribed psychotropic medications and many of those had very few MDT assessments to inform care. Conclusions: This study demonstrates the extent psychotropics are used in the absence of MDT informed support for people with intellectual disabilities and behaviours that challenge. A vision to simply withdraw such medication might be simplistic. Local MDT plans are in development to systematically reduce reliance on this type of restrictive practice. [ABSTRACT FROM AUTHOR]

Abstract: This paper addresses how musical metaphors can be useful in understanding the substructure of psychotherapy by considering how our patients resonate inside us, what that resonance does to and for us, and how these acoustic properties seem to be foundational in the construction of the therapist as an instrument of healing. From this perspective, psychotherapy involves “living music” with another person, as the process of psychotherapy always involves the passing of sound back and forth across an interpersonal divide. This requires both psychotherapist and patient to step into the roles of performer and audience member. Additionally, psychotherapists can be meaningfully thought of not only as an “instrument” of healing, but also an “instrumentalist” who produces unique “therapeutic music.” Moreover, the patient and therapist cocreate the concert space in which treatment is performed. Abstract: The physical spaces in which we conduct psychotherapy are an important yet underacknowledged aspect of psychotherapeutic work. Although a few contemporary publications have provided suggestions for the creation of welcoming, comfortable, and efficient practice spaces, considerations of what it means to transform a room into a therapeutic space have remained largely absent from recent literature. This paper reviews the existing literature on this subject and describes the ways in which the therapist's office is intimately tied to several processes of the therapy itself, by way of creating an adaptive environment that is able to meet the needs of both therapist and client over the course of their work together. abstract This paper discusses the use of Cognitive Analytic Therapy (CAT) with carers of people with dementia and raises the wider issue of service delivery and provision of psychological therapy services to carers. Carers experience loss and emotional pain (; ; ) while providing full-time care to the person with dementia who has complex and changing needs. This frequently results in major depression and anxiety, increasing the possibility of referral to mental health services. Treating carers' distress is vital not only for their mental health but to enable them to continue in their caring role, without which the current care system would collapse. This need is being addressed in Newham by the tertiary psychology service, Psychotherapy for Older Adults (PTOA), set up in 2006-07 to accept referrals of people over 65 with mental health problems. This paper outlines how the PTOA is well placed to identify and treat carers. We propose that CAT is an effective and flexible therapy in this setting, and illustrate this with a case study. abstract In this paper I explore programmes of work undertaken to address institutional racism within two institutions providing psychoanalytic psychotherapy training and services. The paper suggests that training institutions of this kind may be paradoxically positioned. The same features that we might expect to facilitate reflection and change in favour of increased tolerance and diversity may act as points of organizational resistance to change. The paper draws on a range of theoretical resources that may be helpful in understanding the acute anxieties that are often mobilized when institutional racism is named and identified as something the organization has decided to tackle. Since its inception, psychoanalysis deeply engaged itself with questions of autonomy and influence, seeking to minimize the analyst's impact in order to allow for self-direction and uninterrupted growth. The relational turn in psychoanalytic thinking challenged the hermetic fantasy of the therapist's2 positioning as a blank slate and, having recognized the inevitability of influence, has sought to involve and incorporate the therapist more fully within the therapeutic dyad. However, some prejudiced practices are still at place. Since Freud's abandonment of hypnosis and touch, the therapeutic use of both trance and touch have been largely alienated from the psychoanalytical milieu. As a consequence, research and clinical applications of both disciplines developed disconnectedly, and became fragmented. This paper suggests that Freud's initial reasons for abandoning the practice of hypnosis and the use of touch were politically and socially embedded. The paper traces the original split between psychoanalysis, hypnosis and touch to a strategic juxtaposition of establishing psychoanalysis as science in-par with physics. It suggests that both trance and touch represented highly relational, unmediated challenges to the therapeutic dyad, which Freud was unable to incorporate into his practice at the time. This dissociated split is presented through examining Charcot's performance-hypnosis with Blanche Mary Wittman. The paper then sets to briefly discuss the nature of relational body-psychotherapy and relational hypnosis, demonstrating their relevance to modern relational psychoanalytic thinking. The alienation between these three disciplines results in loss of valuable fertilized dialogue which could enrich and inform practitioners from all three disciplines, and facilitate the amalgamation of a cohesive relational framework. Depression is a persistent and impairing illness for adolescents. Many adolescents have limited access to care and/or do not receive adequate treatment for their depression. Researchers have developed a number of empirically supported interventions for adolescent depression; the challenge is to bring these treatments into community settings and assess their effectiveness under real world conditions. This paper provides a critical examination of research conducted on the use of Interpersonal Psychotherapy for depressed adolescents (IPT-A). The paper presents evidence for the efficacy and effectiveness of IPT-A. Implementation and dissemination efforts are discussed in regard to lessons learned and directions for future research. This paper shows the convergence of voices in psychotherapy in the context of the assimilation model. Convergence is the link between patients' voices within the community of voices. The main aim of the paper was to explore (a) how convergence (and divergence) is shown during sessions and the usefulness of convergence for the process of assimilation; (b) if a well-structured patient is able to track the sociohistorical antecedents of his/her main voices; and (c) if, at the end of the therapy, the self becomes richer and with more resources. For this aim to be realized, a case study of a patient, María, treated with linguistic therapy of evaluation for 14 sessions, was analysed by using the Assimilation of Problematic Experiences Scale (APES). Three main problematic experiences or non-dominant voices were identified with the APES: inability to do things, dizziness and tiredness. María's main dominant voices were to cure, solve and overcome problems, to be always doing things and to cope. Results showed a convergence but no divergence of voices as early as session 3. Results also showed how continuity-benevolence assumptions were broken and that, at the end of therapy, the patient's self became richer due to assimilation through the dialogue between non-dominant and dominant voices. This paper discusses the politics and possibilities of linking the personal and political with therapeutic and social transformation through a teaching method provided in the art therapy training at Goldsmiths – the art therapy large group (ATLG). Three key ideas of May '68 are related to the ATLG and their relevance to other psychotherapies and psychotherapy trainings is considered. These ideas are: the importance of the ‘capitalist’ university as an essential terrain in the struggle for social change; the Atelier Populaire's use of art in an anti-capitalist critique of the commodification of art and artist in society, and the anti-imperialist character of the May events. These ideas are related to the theoretical base of the ATLG in the large verbal group literature, Performance Art and to the wide international membership of the ATLG, creating a forum for engaging with global issues. To illustrate these points, we give an example of the interface of the political and the impact of a real event – the university lecturers' strike in 2006 – and the learning that took place in relation to this through the ATLG. We conclude that through a critical engagement with the university within the global terrain of contemporary neoliberalism, the ATLG provides a territory that can integrate the political and therapeutic in arts / psychotherapy trainings; provide a critique and alternative to the commodification of art and artist and engage with issues of difference in the globalized market place. The ATLG prepares the artist / student / therapist / worker to critically engage in the personal and social transformation of the politics of art and psychotherapy provision in the public, private and voluntary sectors. Totton's (2006) edited book The Politics of Psychotherapy: New Perspectives provides a selection of arguments for the link between the domain of politics and psychotherapy that ground the journal Psychotherapy and Politics International. This paper provides a response that articulates the relationship between those two domains. The world of psychotherapeutic politics meshes all too well with my own personal predisposition to split and squabble, only to be able to define a position by way of what it is against. The author comments on the paper by Abraham Brafman about the psychotherapy sessions given to a patient. He centers on the idea of what the patient needs which he believes has no objective means to evaluate. He stresses that a person's self-diagnosis and the therapist's diagnosis are not static factors. He also reflects on the importance of the starting frequency on therapy sessions wherein mistakes can be made depending on the patients' conditions.

Aims Over the last three decades, the randomized controlled trial or ‘psychotherapy technology’ approach has been the dominant model of inquiry in research on addiction treatment. This period has yielded impressive discoveries, but recent failures to confirm core research hypotheses such as occurred in Project MATCH and the Cocaine Collaborative Treatment Study have raised questions about future research directions. The paper identifies several testable assumptions of the psychotherapy technology model. Methods A review is conducted on four substance use disorder behavioral interventions—motivational interviewing, cognitive–behavioral treatment, behavioral couples treatment and 12-Step-oriented treatment—to determine whether these assumptions are supported by research findings. Results Overall, the review suggests weak support for the technology model of psychotherapy research. Lack of support is interpreted as indicating flaws in several model assumptions about how to conceptualize and measure patient responsivity and the interaction of non-specific and specific therapeutic factors. The paper offers alternative strategies for addressing these issues drawn from the general psychotherapy process literature and provides illustrative examples of how these could be used to spur innovation in addiction treatment research. Conclusions The addiction treatment research field is coming up against the limitations of the psychotherapy technology model as the dominant paradigm guiding treatment research. The concluding paper in this series provides brief reflections from a range of key individuals involved in the development and interpretation of CORE benchmarks for NHS primary care psychological therapy and counselling services. Michael Barkham firstly reflects on some of the research challenges facing the use of benchmarks derived from practice-based evidence, before acknowledging the potential for such data to help bridge the research-practice gap as we learn the best use of it. Geoff Mothersole considers similar challenges in using real world data, and explores some of the challenges that managers face in using benchmarks for improved service management practice. Barry McInnes increases the magnification in his reflective focus and looks at the juxtaposition of national guideline recommendations from evidence-based practice in contrast to those introduced by the growing practice evidence-base suggesting differential therapist effectiveness. The first of two final strategic reflective perspectives includes Richard Evans's summary of some of the hidden lessons from the benchmarking endeavour to-date, before my own personal concluding summary looking at lessons learned for the on-going strategic development of enhanced support for routine quality evaluation. This paper aims to shed light on the ways in which ‘neutrality’ is both produced and resisted by socially competent actors in family therapy sessions. It draws upon recent and previous papers in this journal ( Stancombe and White, 1997; Stratton, 2003a, 2003b ), which highlight the importance of blame in therapeutic encounters. When families come to therapy, individual members frequently deliver competing accounts about the family troubles and who is to blame for them. This produces particular challenges for the therapist. We examine the practices of therapists in managing accountability in the session and in their own discussions. Family therapists operate with a professional ethic of neutrality, or multi-partiality. This paper is concerned with the linguistic strategies used by therapists to deal with overtly blaming accounts, how these strategies are responded to by family members in talk-in-interaction and how therapists go about crafting accountability-neutral versions. We show that the social and moral context of family work makes the therapist's job of communicating multi-partiality precarious. In producing accountability-neutral versions of families' troubles, therapists are forced to make practical-moral evaluations of competing versions of events. In this paper I examine two important issues for family therapy research. I attend to the concept of complaints, highlighting the conversational structure they take in a family therapy setting. I do this by outlining how clients construct their complaints and examining how the family therapist receives them. Data is taken from a corpus containing four different families and two different therapists, and is subjected to discursive analysis in order to provide a rich analysis of not just what is happening in the talk but how it is happening. I conclude that complaints are not received as useful to the therapeutic process, despite their constructed importance to the client, and therefore there are wider implications for therapy and professionals more generally, particularly implications for multi-agency communication. This paper describes the background to the development and delivery of a self-help package for anxiety disorders. Evidence of effectiveness is summarized. The paper outlines the intervention and describes the experience of two mental health nurses, who set out to assess its acceptability, evaluate its outcomes in routine clinical practice and assess the feasibility of its delivery by mental health nurses. Acceptability of the intervention was high, judged by retention and attendance rates. The pilot study produced promising clinical outcomes, especially for people with depression secondary to anxiety. Clinical measures showed significant improvements from pre-course to 6-month follow-up in anxiety, psychological well-being and depression. The outcomes suggest that appropriately trained mental health nurses could deliver the intervention as a routine treatment. Bilingualism has risen dramatically due to increased migration patterns all over the world. This paper offers an overview of the current literature dealing specifically with bi- and multilingualism, and identifies a gap in systemic and family therapy literature about the subject of bilingualism. In the light of this, the author proposes a working definition of bilingual families. Some case examples illustrate different aspects of working systemically with a bilingual family and highlight potential challenges for a therapist, such as the assumption of more knowledge and expertise. The tools suggested relate to a rethinking of some modernist ideas regarding hypothesizing, and both the use of a social constructionist perspective and of self of the (bilingual) family therapist. A case is made for the exploration of meanings of difference including the lingua-cultural one. Clients' occupational narratives reveal the overall meaning of life events, signifying their place in a plot that integrates past, present and future. Occupational narratives have also been demonstrated to predict how clients respond to therapy. A closer examination of the details of occupational narratives and how they unfold in the therapeutic context has the potential to give new insights into the experience of therapy by clients. This paper presents narratives of two clients in a vocational program for persons living with AIDS. Comparing their occupational narratives enables us to shed light on the differences between narratives that account for different client outcomes and the extent to which narratives can be impacted upon through occupational therapy intervention. The context for this paper is the ongoing systemic/psychoanalytic debate. It offers an alternative perspective to the recent contribution by Brodie and Wright (2002), in which they are concerned to underscore difference between the two therapeutic approaches. Here it is argued that the relationship is a great deal more complex than Brodie and Wright's analysis might suggest. Attention is focused on significant areas of commonality, in particular the impact of social constructionist thinking across the two therapies as well as current developments in technique. A case is made for the consolidation of generic space where there is opportunity for cross-fertilization and for integrative and combined systemic/psychoanalytic approaches to be nurtured and developed where appropriate. Case vignettes are used to highlight different aspects of the generic component in child and family therapy and to underline the need for bridge-building between these key therapeutic approaches in the field. This paper considers the female anorexic's use of space by reviewing her spatial practice as demonstrated in her clinical presentation upon starting therapy. Two categories of concept about space underpin the paper: the notion that space is produced as conceived in a Marxist perspective by Lefebvre, and psychoanalytic concepts of mental space derived from the work of Bick (1968), Meltzer (1975) and Rey (1994). Beginning psychotherapy with an anorexic patient shows how particular spatial phantasies (phantasy is here construed as an unconscious configuration of the mind which affects how we think, feel and act (see Isaacs, 1943) as opposed to a conscious fantasy such as a daydream) and practises structure the intersubjective relationship between therapist and patient and between patient and aspects of her own experience. In this paper anorexia is considered as a defence against consuming desire for the object (the word ‘object’ is used to represent the earliest relationship the infant has to someone else whom he or she has yet to recognize as a whole autonomous person in their own right. Recent research indicates that childhood abuse experiences characterize a large subset of psychiatric inpatients. This paper presents a time-limited pilot group developed for adult male abuse survivors in an inpatient setting using: (1) techniques adapted from the existing literature on treatment of abuse survivors; and (2) approaches deriving from the interface of theory and current manifestations of distress. Operating largely outside of awareness, the procedural mode of processing has been neglected in psychotherapy and psychoanalysis, and yet it is critical to our understanding of therapeutic action. This group has made an outstanding contribution to a procedural theory of therapeutic action, integrating some of the most important current concepts in infant research, cognitive psychology, systems theories, and adult psychoanalysis. The procedural mode of “implicit relational knowing” is placed within the context of a moment-by-moment, mutual regulation model of co-constructed exchanges. Nonlinear dynamic systems theory has greatly influenced their theory of change. The centerpiece is Louis Sander's concept of the “moment of meeting.” Although the concept of intersubjectivity is central in all the papers, the multiple uses of the term suggests that a careful re-examination of the definitions of intersubjectivity is sorely needed. In this paper, general guidelines and specific techniques that the authors have found helpful in the treatment of posttraumatic stress reactions during the infancy and toddler period are described. [n treatment planning it is important to recognize that traumatic events affect infants and their families in a number of ways. Moreover, the rapid developmental changes occurring in the first 3 years strongly influence how a trauma is experienced. Taking these factors into account, the authors identify a number of important therapeutic principles, derived from work with older children and adults, that can be applied to the treatment of infants and toddlers. Specific therapeutic techniques are described in relation to the child's age and developmental level. Suggestions for working with parents are provided, as well as a brief discussion of the role of medication. Seldom is the work of philosophers invoked by health professionals when examining aspects of care from a philosophical perspective. Instead, students of health care, especially nurses, have been introduced to ‘philosophies’ which are often superficially examined and poorly understood. This practice fails to develop in students an appreciation of the work of philosophers or to acquire the art of critical thinking. The introduction of models and theories of nursing in the past three decades has alerted nurses to the importance of possessing critical skills in order to identify sound theory and implement good practice. This paper goes beyond mere philosophising and examines aspects of mental health care from the perspectives of one of nineteenth century Europe’s most notable philosophers, Friedrich Nietzsche. It argues that understanding his work can enhance one’s ability to reflect on nursing practice, as well as bringing a new dimension to how we analyse ‘mental health’ problems. His work provides many insights into how we can improve our understanding of the effect of mental illness and mental health care on the individual, and how we conceptualise the process of care. This paper provides an overview of his life’s work, his impact on the history of ideas and develops some of the more provocative implications of his work for mental health care. Background: Adventure therapy is a short-term experiential psychotherapeutic approach that utilises adventurous activities and being in natural environments in order to facilitate therapeutic change in clients. The present paper examines how students with self-reported anxiety and depression experienced participating in an innovative intervention that combined individual therapy with an adventurous outdoor experience. Clients participating in the Adventure Therapy Project received between 10-15 one-to-one counselling sessions prior to the outdoor experience, as well as ongoing counselling following it. Method: Interpretative Phenomenological Analysis was used in order to record and analyse the experiences of three male and three female students who presented at a major UK university student counselling service with self-reported depression and anxiety. Findings: Twelve themes emerged which were categorised into four major domains: enhancing intrapersonal relationships; improving interpersonal relationships; providing an experiential outdoor venue for achieving therapeutic change; offering a secure, personal place for achieving inner healing. The intervention was perceived to improve the ways in which interviewees related towards themselves and other people. Interviewees perceived the counselling sessions as offering a safe therapeutic space within which they could unveil their anxieties and achieve inner healing. Conclusions: Outdoor experiences offer an experiential venue for achieving personal change and, it appears enhance the therapeutic process. abstract Historically there has been a struggle to understand the role of the body in a psychotherapeutic context and, although the status of bodily communications is an area of increasing interest and study, it remains one of the least understood aspects of therapy practice. Yet even when therapist and client ignore the body in their interaction, they are still confronted with the body as a metaphor for the whole self; as Freud stated: 'The ego is first and foremost a bodily ego' (, p. 26) so therapy with no body is impossible. Drawing on ideas from practice, theory and research this paper focuses specifically on borborygmi (tummy rumbling), a bodily phenomenon often considered inconsequential and intrusive in the therapeutic encounter. Data from a grounded theory study is presented offering a framework which reflects the processes involved in acknowledging and bringing embodied experiences into an open therapeutic dialogue. The following is the first part of a two-part paper that discusses the challenges faced by psychotherapists working in Venezuela during years of political and social unrest as a way of examining psychotherapy's dilemmas when dealing with political issues. This first part will discuss limitations in the ability of traditional psychotherapeutic technical recommendations to address clinical material stemming from highly polarized political scenarios. Historical examples of how these limitations have led to abuse will be shown. The specific difficulties of traditional notions of neutrality will be questioned.
