Background: Throughout their careers, doctors and other healthcare professionals experience numerous transitions. When supporting transitions, opportunities for development and learning should be maximized, while stressors having negative impacts on well-being should be minimized. Building on our international data, this study aimed to develop a conceptual model of the trainee-trained transition (i.e. the significant transitions experienced by doctors as they complete postgraduate training moving from trainee/resident status to medical specialist roles). Methods: Employing Multiple and Multidimensional Transitions (MMT) theory and current conceptualizations of clinical context, this study undertook secondary analysis of 55 interviews with doctors from three countries (Netherlands, Cananda and the UK) undergoing trainee-trained transitions. Results: Through this analysis, the Transition-To-Trained-Doctor (T3D) conceptual model has been developed. This model takes into consideration the multiple contexts and multiple domains in which transitions take place. Discussion: This model is significant in that it has several uses and is applicable across countries: to remind doctors, managers and medical educators of the complexity of transitions; to frame and facilitate supportive conversations; and as a basis to teach about transitions. Recently able-bodied people have taken up wheelchair sports. This paper aims to explore why people are taking up a sport which may be considered to 'belong' to disabled people and explore the impact of reverse integration. A questionnaire covering demographic details, experiences of wheelchair sport and perceptions of both able-bodied and disabled wheelchair athletes was distributed by e-mail via elite wheelchair athletic associations in the UK, Canada, The Netherlands and the USA. Twenty participants were recruited (11 disabled athletes, four female, and nine able-bodied athletes, three female). Able-bodied people initially became involved in wheelchair sports in order to share an activity with their disabled friends or family. Continuing participation was reinforced by friendship, challenge, achievement, the opportunity for good competition, development of the sport and to change society's perceptions of disability. Planning and implementing educational programs in medicine optimally requires a background in educational theory and practice. An avenue of training open to such practitioners is a master's degree in medical education. A single 1998 report lists the programs known at that time and information about them remains scarce. The authors have re-examined all current programs offering master's degrees in medical or health sciences education in the English-speaking world, including the Netherlands. The authors contacted the programs identified in the 1998 report to establish how many were still in operation. The search was extended using Pubmed and other search engines. A further verification targeted a selected sample of 10 prominent US medical schools. Twenty-one currently operating programs were identified: six in the US, eight in the UK, three in Canada, three in Australia and one in Holland. Seven of nine original master's programs were still in existence. URLs, website and other logistical information about each program are tabled. A master's degree in medical or health sciences education is the most specific method for medical faculty to obtain a credentialed grounding in educational theory and practice. This article seeks to explain variations in public support for the deployment of military troops to Afghanistan by means of the strategic narratives employed by national governments. Focusing on the UK, Canada, the Netherlands and Denmark, we argue that strong narratives about the why-what-and-how of overseas military missions increase the likelihood of popular support, while weak story lines are likely to result in a souring public opinion environment. Contrary to most current studies of public opinion and the support for international missions, we thus emphasise the role played by political leaders in shaping public attitudes towards the projection of military power. Surely, politicians are affected by polls and the wishes of public opinion – but popular attitudes are far from immune from political elites’ attempts to rationalise the use of military force via strategic narratives.
