Background: Palliative care is insufficiently integrated in the continuum of care for older people. It is unclear to what extent healthcare policy for older people includes elements of palliative care and thus supports its integration. Aim: (1) To develop a reference framework for identifying palliative care contents in policy documents; (2) to determine inclusion of palliative care in public policy documents on healthcare for older people in 13 rapidly ageing countries. Design: Directed documentary analysis of public policy documents (legislation, policies/strategies, guidelines, white papers) on healthcare for older people. Using existing literature, we developed a reference framework and data extraction form assessing 10 criteria of palliative care inclusion. Country experts identified documents and extracted data. Setting: Austria, Belgium, Canada, Czech Republic, England, Japan, Mexico, Netherlands, New Zealand, Singapore, Slovenia, South Korea, Spain. Results: Of 139 identified documents, 50 met inclusion criteria. The most frequently addressed palliative care elements were coordination and continuity of care (12 countries), communication and care planning, care for family, and ethical and legal aspects (11 countries). Documents in 10 countries explicitly mentioned palliative care, nine addressed symptom management, eight mentioned end-of-life care, and five referred to existing palliative care strategies (out of nine that had them). Conclusions: Health care policies for older people need revising to include reference to end-of-life care and dying and ensure linkage to existing national or regional palliative care strategies. The strong policy focus on care coordination and continuity in policies for older people is an opportunity window for palliative care advocacy. [ABSTRACT FROM AUTHOR]

In the context of the COVID-19, it is very significant to explore the dynamic fluctuation of China's export growth to the international market and examine the causes of this fluctuation in a multi-dimensional way. The dynamic fluctuation level is measured in the short and medium-long cycle respectively based on monthly data. Factor decomposition of the dynamic fluctuation is made from the growth effect, the structure effect and the competitiveness effect by the improved CMS model, which results show that the growth effect has decreased first and then increased; The structure effects include market structure effects and commodity structure effects, and the former effect basically fluctuates around the X-axis while there is a negative commodity structure effect; There are different specific competitiveness effects on 21 categories of goods. China should rely on the RCEP agreement to strengthen and stabilize trade cooperation other countries, and strengthen its core position in the industry chain and value chain in the Asia Pacific region. The current paper creatively introduce the trade expansion margin including the ternary margin and product structure into the CMS model, explaining the internal relationship between the trade expansion margin analysis at the product- market level and the CMS analysis at the overall level; meanwhile, this paper analyses the dynamic fluctuation structure of China's export growth to Japan and South Korea through a CMS factor decomposition from the perspective of technological structure and factor ladder, and studies the source and dynamic distribution evolution of this growth pattern. This paper analyzes the integration processes of international marriage migrants in Japan and South Korea and identifies the steps and conditions required for integration. It contributes to the wider discussion of obstacles that migrants face during the integration process. Despite Japan and South Korea's reluctance toward the influx of large numbers of international migrants, their stance is more positive toward (female) marriage migrants. The qualitative data used for the analysis were collected by authors during the period between 2007 and 2022 on women from former Soviet Union countries, married to South Korean (54) or Japanese (50) men. The analysis of women's experiences identifies four major steps leading to receiving society integration: Pre-migration, arrival, early years and long-term settlement. The findings show that early language acquisition and receiving family support resulted in relatively smooth integration. However, human capital appears to be both a driver and an obstacle to integration. Furthermore, mismatched gender role expectations, limited social networks and employment opportunities increased difficulties in the integration process. Additionally, stronger ties with the receiving family and weaker ties with the home countries influenced women's decision to settle in the country. The largest differences between Japan and South Korea were the higher employment opportunities for marriage migrants in South Korea, especially for those with professional skills and Korean language knowledge. This article examines the policies of development cooperation promoted by Japan and Korea in Central Asia, focussing on the roles of discourses treating Japan and Korea as potential non-Western models for the region's development and modernisation in their relations with Central Asian states. We trace the evolution of relevant developmental approaches made by Japan and Korea, analysing discourses and practices of sharing experiences, as well as the key drivers behind those shifts, and we seek to locate these changes in the larger context of regional political economy and international relations. Furthermore, the article seeks to address the ideational and normative aspects of Japanese and Korean development cooperation in Central Asia. Due to the problematic degradation properties of plastics, the decomposition of plastic results in the formation of numerous microplastics (MPs), less than 5 mm in diameter. These MPs enter the soil and the ocean, eventually passing through the air, water, or food chain back to the human body and harming human health. In the last 80 years, male semen analysis parameters have shown a significant decline for unknown reasons, speculated to be caused by pollutants. No studies examined the relationship between human MP exposure and male infertility. In this article, we reviewed the relevant animal experimental research literature in recent years and calculated that the minimum human equivalent dose of MPs leading to abnormal male semen quality is 0.016 mg/kg/d. The literature comparison found that MP exposure in Japan and South Korea was close to this value. These results suggest that MPs can affect male semen quality and that MPs may significantly impact male fertility. [ABSTRACT FROM AUTHOR]

Background: Some factors associated with spiritual well-being in dying patients have previously been reported. However, there has been no cross-cultural study comparing factors related to spiritual well-being. The current investigation may shed light on this under-investigated area through a comparison of diverse factors. Aim: We aimed to (1) examine factors associated with spiritual well-being in the last days and (2) compare those factors across three East Asian countries. Design: This is an international multicenter prospective cohort study. Setting/participants: Newly admitted inpatients with far advanced cancer in palliative care units in Japan, Korea and Taiwan were enrolled. Each patient was classified into one of two groups based on spiritual well-being score in the last days of life. Univariate and multivariate analyses were performed to identify the factors related to better spiritual well-being score in each country. Results: A total of 1761 patients treated at 37 palliative care units from January 2017 to September 2018 were analyzed. Seven variables were significant in Japan, three in Korea, and five in Taiwan. "Good death scale [acceptance]," "fatigue" and "expressed wish for hastened death" were unique in Japan. "Visit from a pastoral care worker within 48 h of death" was unique in Korea. "Patient's preferences for place of death," "dyspnea" and "continuous deep sedation" were unique in Taiwan. Conclusions: This study found novel factors related to spiritual well-being in the last days of life, several of which differed according to country. Objective: To estimate proportions of married couples aged 30 to 59 years who face simultaneous demands from elderly parents and children in China, Japan, Korea, and Taiwan, and the effects on couples’ life and marital satisfaction. Method: Using data from the 2006 East Asian Social Survey, proportions of sandwiched couples were estimated using three definitions: co-residence, co-residence or nearby residence, and co-residence or frequent provision of financial/physical assistance. Logistic regression models were used to assess the effects on life and marital satisfaction. Results: Three-generational co-residence is more common in Taiwan and Japan (24.3% and 21.4%), whereas co-residence or provision for both generations is higher in Taiwan and Korea (53.7% and 44.5%). The strain of being sandwiched is associated with lower life and marital satisfaction only among women. Discussion: Intergenerational transfers of care play a major role in elderly support in East Asia but take a toll on the well-being of married women. [ABSTRACT FROM AUTHOR]

This article examines street improvement projects in Tokyo and Seoul as case studies for tracing the active participation of Japanese urban planners in the global flow of urban planning concepts and technologies from the West to Japan, and then from Japan to Asia. Striving to recreate Tokyo as the “greatest capital in East Asia,” Japanese planners in the mid-Meiji Period envisioned improvement projects that, while adapted to meet local needs, exhibited a striking similarity with the primary concerns of contemporary “modern” Western planning ideas. Moreover, Japanese planners acted as transmitters of Western planning as these imported ideas and practices adapted in Tokyo in the 1880s were then exported to Korea along with Japanese imperialism in the early 1900s as planners sought to refashion Seoul as a suitable colonial capital to assert Japanese cultural hegemony. Street improvement projects in both cities provide tangible examples highlighting this technical innovation and conceptual diffusion overseas. [ABSTRACT FROM AUTHOR]

To provide East Asian evidence to the relationship between the use of complementary and alternative medicine (CAM) and the use of biomedicine, this article examines the institutionalization of traditional East Asian medicine (EM) in China, Korea, and Japan and how it affects the relationship between EM use and biomedicine use. It uses the 2010 East Asian Social Survey. Logistic regressions specify the statistical association between EM use and biomedical physician visits. These models show that the high institutional acceptance of EM promotes the concurrent use of EM and biomedicine. In addition, since these countries feature different ways of institutionalizing EM (unification in China, equalization in Korea, and subjugation in Japan), the concurrent use is more obvious under the Chinese and the Korean system than the Japanese system. It concludes that the CAM use can be complementary to biomedicine, depending on how CAM and biomedicine are institutionalized in medical systems. [ABSTRACT FROM AUTHOR]

Japan and South Korea have always taken what may be called a social investment approach to their social and economic development policies. They were able to achieve a high level of economic growth, in part, because of their targeted social spending that supported and protected the productive sectors of the society. Since the 1990s, however, there has been a marked shift in the targets of social investment, from predominantly skilled, male, industrial core workers to more peripheral, marginalized, and vulnerable population groups, such as women, children, and the elderly. Moreover, this new policy focus is now increasingly put forward from the perspective of inclusive welfare and the discourse of social inclusion, thus breaking from the earlier productivist thinking. Indeed, recent social investment policy debates in the two countries are often framed in terms of intergenerational equity, social and economic sustainability, and economic democracy. What are these ‘new social investments’, and why the shifts? This article looks at the new social investment policies in Japan and South Korea to understand factors behind the changes, and assess how ‘new’ are these new social investments. Environmental aid has become a major component of development aid. We analyzed the contingent characters of environmental aid of Japan and South Korea using the definition of Williams, which regards aid policy as donor driven and autobiographical of the donor agencies and societies from which they sprang. Both Japan and South Korea consider environmental aid as an important tool of their diplomacy. A combination of a moral obligation and domestic, international, political, and economic interests underpin both countries’ environmental aid policy. Seen from the stated policies and practices, both countries use accounts of their past as once-developing countries trying to catch up in their aid narrative. In this manner, the environmental aid of Japan and South Korea is autobiographic, reflecting their interpretation of their own development history and position in global politics. Multiculturalism in the Northeastern Asian states of Japan, South Korea, China and Taiwan as well as the members of ASEAN are interesting case studies in how states manage migration policy and create migrant policy. Examining their migration practices, we can understand East Asian perspectives on multiculturalism employing traditional migration frameworks revolving around control and management of entry of migrants, state-building and ethnocentric rationale for/or against pro-migrant policies. This review article will delineate how different states in the region construct migrant policies in line with their state’s interests, which include domestic, regional and international interests by employing the below nine articles. The baseline conditions in domestic politics and international security of Japan and South Korea compel these countries to deal with historical issues in provocative terms, with each side alienating the other. The current status of the Dokdo controversy between Japan and South Korea has remained largely unchanged since 1965, when the two countries signed the diplomatic normalization treaty: Japan continues its protest that South Korea has unlawfully occupied the islands, and South Korea responds that there is no dispute to be settled with Japan regarding the sovereign status of the islands. The path dependency from the past and domestic political institutions continue to deprive top decision makers in Japan and South Korea of political autonomy to move beyond the 1965 formula in addressing the issue of Dokdo in productive and forward-looking ways. The purpose of this study was to clarify the differences in relationships between grandmothers and grandchildren in Japan and South Korea. Ninety-seven and 194 pairs of grandmothers and grand- children from Tokyo and Seoul, respectively, completed measures to assess their evaluations of four types of grandmother roles ("transmission", "socialization agent", "secure base", and "parent surrogate") and their degree of norms for assisting grandmothers. Only the role of socialization agent was associated with country difference, but not with any demographic factors; it was more highly evaluated by living in Seoul. The norm for assisting grandmothers was higher for both grandmothers and grandchildren in Seoul than in Tokyo. In Seoul, grandmothers' expectations of assistance positively correlated with grandchildren's sense of obligation; however, this was not the case in Tokyo. In addition, in Seoul, the grandmother role of a socialization agent was positively associated with the norm of assistance for both grandmothers and grandchildren. Based on these results, we argue that intergenerational solidarity is stronger in South Korea than in Japan, and that grandmothers in South Korea play influential roles in enhancing the norm of assistance.
