Objectives: The main objective of this study is to examine the relationship between health and an early exit from paid employment in the Finnish, Polish, and Spanish populations. The authors have addressed the following 3 issues: who chooses not to work according to gender patterns, whether the health status is a determinant of being non-employed, and what diseases are associated with being non-employed.Material and Methods: The studied material consists of data from the Collaborative Research on Ageing in Europe (COURAGE in Europe) project. The analysis was based on a sample of 5868 individuals, including 1214 from Finland, 2152 from Poland and 2532 from Spain. In the paper, these data were complemented with the results of the PArticipation To Healthy Workplaces And inclusive Strategies in the Work Sector (PATHWAYS) project.Results: The results of the study revealed that self-rated health was a strong predictor of being non-employed for different reasons. Chronic diseases, such as arthritis, angina, diabetes, chronic obstructive pulmonary disease, depression and hypertension, were typically associated with an increased risk of being non-employed due to health problems and early retirement. Women more frequently reported arthritis and depression, while a higher proportion of men suffered from angina and diabetes.Conclusions: The results obtained by the authors highlight the importance of evaluating gender patterns in work cessation and, at the same time, the importance of gender-focused actions in terms of preventing withdrawal from the labor market. In principle, all chronic diseases have some impact on participation in the labor market. It is worth noting, however, that each of them affects workforce participation in a different manner, depending on the specific disease, country and gender. Objectives: Current demographic trends, such as society aging, the spreading of chronic diseases, as well as early retirement choices, lead to the dwindling labor market population. Taking into consideration the foregoing tendencies and their consequences, the issue of the shrinking labor force resources seems to be the most important challenge for the labor market. In response to it, this paper identifies individual and institutional causes of withdrawal from the labor market, and explores the views of Polish stakeholders on existing strategies for the occupational re-integration of persons with chronic diseases (PwCDs).Material and Methods: Polish results of 2 European projects: "Participation to Healthy Workplaces and Inclusive Strategies in the Work Sector" (the PATHWAYS project) and "The Collaborative Research on Aging in Europe" (COURAGE in Europe) complemented each other. The cross-sectional population data and data from stakeholders' interviews were used.Results: The authors identified several causes which could encourage people to withdraw from the labor market before attaining the official retirement age at 3 different levels: individual, occupational, and institutional. Although research has shown that the macro-level situation is crucial to staying occupationally active, in Poland stakeholders have reported the lack of effective coordination in the implementation of policies for the occupational re-integration of PwCDs.Conclusions: A decision to cease work prematurely reflects a combination of many different factors, such as individual determinates, work characteristics, personal convictions, and systemic solutions. The study demonstrated that, despite the awareness of the dwindling resources of labor force, the employment-related challenges faced by PwCDs as well as people who are close to the retirement age remain unaddressed. Objectives: Health Impact Assessments (HIA) approach can be executed by calculating the attributable burden of disease. The most common indicators used in the HIA methodology are: premature mortality, morbidity, life-expectancy, and Disability Adjusted Life-Year (DALY). The term Disability Adjusted Life-Years (DALYs) indicates months/years lost due to a premature death or disability. The aim of the study was to present health benefits, expressed in terms of lower total mortality and cardio-respiratory hospitalization rates, due to a decreased particulate matter (PM) concentration in ambient air, in Silesian voivodeship.Material and Methods: In this paper, results obtained from the APHEKOM (Improving Knowledge and Communication for Decision Making on Air Pollution and Health in Europe) project, which provided specialized HIA tools, useful for assessing health benefits resulting from reducing air pollution, were used. Both short-term and long-term exposure HIA tools were applied with regard to the appropriate data for Silesian voivodeship. Exposure data were obtained from the Regional Environmental Inspectorate in Katowice, while population and health data were obtained from the Central Statistical Office of Poland or from the Silesian Voivodeship Office, respectively.Results: Health benefits that are related to an improvement of ambient air quality in Silesia region are similar to previous estimates obtained for Kraków city. The reduction of short-term exposure to PM10 by 5 μg/m3 results in a lower number of yearly non-external deaths (2.6-2.75 per 100 000 inhabitants). This effect was also shown to be similar in the city of Zabrze, as well as in the whole Silesia region.Conclusions: The Health Impact Assessments tools developed by the World Health Organization (WHO) can help public health experts make decisions in order to improve the health of populations living in particular regions of Europe. It is estimated that every third person living in Europe suffers from allergic diseases. Allergies are a growing health problem in Poland where 40% of the population have allergy symptoms, including 12% afflicted with asthma. The actual cost of allergic diseases is difficult to estimate due to the lack or incompleteness of the relevant data. The aim of this review is to present estimates of the indirect costs of allergic diseases in Poland and globally, using asthma, allergic rhinitis and atopic dermatitis as examples. The analysis also includes the impact of allergic diseases on the costs to the social welfare system and employers. The literature review of the indirect costs of allergic diseases shows that the indirect costs of a disease, which substantially exceed the direct costs, increase with the disease activity and severity. Interestingly, some studies have found that the indirect costs of lost productivity due to hours missed from work to take care of a sick child could be threefold higher than those of absence due to a worker's own illness. Background: This paper is composed of two parts, the first one addresses the issue of legal grounds for teleconsultation and telemedicine in Poland, the other tries to answer the questions whether the provision of health services with use of telemedical tools does not breach medical professional secrecy or personal data protection., Material and Methods: The material comprised a whole set of Polish and European legal acts that could have any impact on the legal basis of telemedicine in Poland. The method applied was of interpretative nature, and the texts of above-mentioned acts were thoroughly analyzed, providing the first comprehensive study on this subject., Results: The medical professional secrecy is not violated if the range of transmitted data is limited to those, which are absolutely necessary in the diagnostic and therapeutic process. This rule involves the transmission of indispensable personal and medical data, however, sharing the patient's appearance should be preceded by his/her informed consent., Conclusions: The achievements of information technology greatly contribute to the quality of diagnostic and therapeutic processes. Due to the rapid development of modern means of telemedicine we face the need to establish legal regulations, specifying the principles of distant exchange of medical data.

A growing interest in the development and implementation of preventive programs at workplace has recently been observed. According to the modern model of occupational health protection, recognition, monitoring and elimination of any potential risks at work are obligatory. This means that occupational health prevention also addresses the issue of psychosocial risks at work. There is a need of common understanding what the psychosocial risks are and how they affect human functioning to prevent them adequately and effectively. The aim of the paper is to present the present knowledge of psychosocial factors at work in a systematic way.

Exposure to electromagnetic fields (EMF) occurs when man is exposed to the effect of electric, magnetic and electromagnetic fields and contact currents different from those resulting from physiological processes in the organism or other natural phenomena. In Poland, the system of protection against EMF has been functioning for over 35 years. In 2001, when the Minister of Labor and Social Policy issued the regulation introducing the maximum admissible intensities (MAI) for electromagnetic fields and radiation within the range of 0 Hz-300 GHz, the system was directed mainly towards evaluation of exposure to EMF occurring in the occupational environment. The system is linked via MAI values with human protection in the natural environment. In this paper, the background, principles and the range of the national system of protection against EMF and its monitoring are presented. The project of implementation of EU directives, following Poland's accession to the European Union is also discussed.

Directive 2000/54/EC deals thoroughly with issues referring to the protection of workers from risks related to occupational exposure to biological agents in the European Union. The ordinance of the Minister of Health on harmful biological agents at work and the protection of exposed workers implements its content into Polish legislation. This paper reviews essential records of the ordinance: definition of biological agents; classification of biological agents into four risk groups according to their level of risk of infection; employers' obligations concerning the protection of workers exposed to biological agents. The results of Phare project supporting implementation of the directive in Poland regarding distribution of information on occupational exposure to biological agents among State Sanitary Inspection and occupational medicine service staff and employers are also presented.

The major objective of setting values of chemical concentrations in the work environment is to reduce the risk of occupation-related exposure to levels regarded as safe to humans. In the standard setting process, a critical assessment of the available literature data on toxicity of a given chemical and related health effects in the population exposed, depending on concentrations and exposure duration, is essential. Such an assessment performed by a group of experts is a difficult and responsible task since the available data are frequently incomplete and extrapolation of the results of animal experiments aimed at setting limits of concentrations safe to humans raises multiple doubts. The aim of this paper was to compare the procedures of setting permissible values for chemical factors in Poland and throughout the world and to highlight differences in their interpretation. In addition, a brief history of setting admissible limits, as well as definitions of hygiene standards used in Poland, the USA, Germany, the UK and in the former Soviet Union together with the ways of their formulation are presented. The country-to-country variations of attitudes towards setting admissible levels of exposure to carcinogenic agents are worthy of special mention. Maximum admissible concentrations of chemicals in the work environment suggested by experts and adopted by the state administration should not be regarded as constant and completely safe to the human health, thus the existing systems should take account of the need to verify these values with an inflow of updated information on chemical toxicity.

Exposure of miners to natural radiation in which radon-222 plays the major role has been studied in Poland since the end of the nineteen sixties. The work environment measurements and personal monitoring methods have been developed for monitoring the exposure. A quite wide range of doses resulting from big differences in radon concentrations in mines is characteristic of miners' exposure. It was estimated that about 16% of miners received doses above 5 mSv per year. This group of miners should be provided with individual dosymentry. In this paper methodological and legal aspects of the situation in mines are analysed. A large number of Polish recommendations and legal regulations must be changed in order to harmonise them with the directives of the European Union.

Since the second World War, excess mortality of males has been steadily growing in Poland. The aim of this paper was to analyze the basic relationships between excess male mortality and some social and economic factors, with special reference to both age and place of residence. Data published in Demographic Yearbooks and included in reports produced by the Government Population Council were used in the analysis. The excess male mortality is expressed in terms of male/female mortality ratio, and also in terms of the difference between the average female and male life expectancy. In the early 1990s the general male mortality rate in Poland was by 23% higher than the general female mortality rate, whereas in males at younger working age (20-44 years) mortality was three times higher, and in the older age (45-64 years) groups 2.7 times higher than the female mortality. Compared with the majority of European countries, Poland is characterised by high rates of excess male mortality, which points to a deteriorated health status of the population. At present, excess mortality of the working age males is much higher than in the 1960s and 1970s. Our analysis of the 1960-1994 trends revealed that the highest excess male mortality occurs in the 20-24 age group. Although recently a falling trend has been observed in the infant, juvenile and post working age groups, a continuous increase is noted in the working age population of Poland. Causes of death were also included in our analysis. Among circulatory diseases, the highest excess mortality was due to acute myocardial infarction (the risk of death from this disease was 8 times higher for males than for females). Accidents, injuries and poisoning constitute another leading group of causes responsible for excess mortality (6/1 male/female death risk ratio). The excess male mortality rates are higher in the rural than in the urban areas. The excess male mortality was also reflected in the indices of average life expectancy. In 1995, the average life expectancy was 67.6 years for males and 76.4 years for females. Thus, in Poland males live 8.6 years shorter than females on average. Increased excess mortality among the working age males, a considerable difference between male and female average life expectancy, disturbed demographic male/female balance, these are at least some of the reasons why further in depth studies of excess male mortality in Poland should continue.

The paper presents a brief survey of literature on the occurrence of byssinosis in the Polish flax industry. Moreover, the most essential conclusions from studies on the prevalence of the disease in the plants processing flax in other countries were presented. The need to undertake a similar analysis in flax industry in Poland was emphasized and proposals of problems to be included were presented.
