Every so often, in Chile there is a discussion about the role of physicians in the care of people on hunger strike (HS). In this document, we review the ethical aspects of health care for persons in HS, aiming to provide guidelines to medical doctors who are required to attend them. First, we make an important distinction between HS and suicide, since the former is used as a protest and denunciation tool, while suicide seeks deliberately to end a life. Then we describe the three roles that the health professional can fulfill: as a treating doctor, as an expert or as an official of a prison. The respect for the autonomy and dignity of the person in HS must prevail whatever the role of the physician. Therefore, we maintain that under no circumstances, people who have autonomously decided to be in HS should be fed by force. Due to the complexity of the issue, we make special considerations about the management of minors and the non-competent persons in HS. In conclusion, we adhere to the principles that inspire the Declaration of Malta, which indicate that it would be preferable to "allow a person on hunger strike to die in dignity, rather than subjecting them to repeated interventions against their will".

Background: Many Chilean biomedical investigators publish the results of their research in international journals, not easily accessible to fellow physicians of the country., Aim: To assess the amount and quality of papers published by Chilean authors in international journals., Data Source and Methods: Using MEDLINE databases from January to August, 1997, all papers in which Chile appeared in the field "AD" (Address of author) were selected. Papers with a clear relationship to human medicine were identified., Results: During the study period, 390 papers with authors residing in Chile appeared in MEDLINE. Of these, 188 (48%) were published in Chile and the rest was published abroad. Thirty manuscripts had a greater relationship to clinical medicine and the authors of nine of them were invited to submit a translation of their abstract into Spanish to have them reproduced in the present article., Conclusions: A significant proportion of biomedical research papers originated in Chile is published abroad with the consequent lack of diffusion among non-specialized readers from their own country.

This paper studies a shelter network for Jewish scientists displaced by nazism from the archive of Alexander Lipschütz, a physiologist who lived in Chile since 1926. From the context of the anti-Semitic persecution and the way in which it affected German science and their universities, we have analyzed letters sent to and from Lipschütz between 1935 and 1936, with special attention to people who contacted him to flee Germany and considered Latin America as a possibility to live. We suggest this was a network of personal agencies, charged with subjectivities and intimacy, which had to take into account local anti-Semitism and academic xenophobia.

Objective: To study the prevalence of adverse drug events (ADE) in hospitalized patients in Chile. As part of our research, we also assessed the validity of the method used to identify the occurrence of an ADE based on the discharge diagnoses of the patient., Design: The study included 1,7 million patients hospitalized during 2019-2020. We analyzed the following variables for each patient: ICD-10 discharge diagnoses, sociodemographic information, and clinical outcome indicators, i.e., diagnosis-related group (DRG) weight, length of stay, and mortality. To validate the method for the identification of ADEs, first, we generated a random representative sample of patients, stratified by sex and medical specialty, hospitalized in a Chilean public hospital in 2019, and then we compared the outcome of the method with the opinion of a group of clinical experts that reviewed each patient's discharge summary retrospectively., Results: The prevalence of ADEs in hospitalized patients in Chile during 2019 and 2020 was 2,7% and 3,1%, respectively. The precision of the method used to identify ADEs was 83,3% or higher (CI 90%)., Conclusions: This paper uses nationwide data to describe the prevalence of ADEs and their correlation with different factors associated with the patient, the patient's disease, and the health service. These studies are essential to designing public health policies that effectively address healthcare quality and patient safety.

As a part of the EIS-COVID project on the access and use of information during the COVID-19 pandemic in Chile, the objective of this paper was to ascertain how people's informational environment was constructed during the first stage of the pandemic. It discusses the results of a qualitative study of people belonging to risk groups for COVID-19: people over 18 and under 65 with chronic diseases (hypertension and diabetes) and people 65 and over. Ninety semi-structured interviews were conducted in the Metropolitan and Valparaíso regions between September 2020 and January 2021. The results reveal the problematic nature of the information overload encountered by these groups and the strategies they used to navigate it: a) information avoidance; b) content corroboration and active search for reliable sources; and c) differentiated media use.

Revista Médica de Chile was founded in 1872 by Sociedad Médica de Santiago (currently Chilean Society of Internal Medicine) and it is one out of twenty five medical journals founded during the XIX Century that still remain active. In monthly issues it presents clinical and biomedical research articles, as well as research and position papers dealing with medical education, public health or medical ethics. Special attention receive the impact on health care given by the emergence of new private medical schools in Chile, the relevance of methodologies to assess clinical competences in graduates, the certification of specialty programs and specialists, the appropiate use of simulators in clinical training, telemedicine, organ transplants, and current issues in medical ethics, such as abortions and euthanasia. Institutional and international collaboration in medical research is clearly reflected in recent years. Articles are subjected to external peer review and ICMJE Recommendations are carefully considered throughout the review process. Most articles are published in Spanish but English abstracts are mandatory and a growing number of articles are currently published in English. An Open Access electronic version is published by SciELO. Revista Médica de Chile is indexed in the most important international data bases and it is a member of the ICMJE ruling committee. The long life and prestige of this journal can be considered a testimony of the cultural heritage and progress in Chilean medicine.

Background: Despite recent initiatives and efforts, gender inequality still exists in medicine and academia. There is a higher proportion of male authors in international scientific publications., Aim: To compare the proportion of female and male authors in the scientific publications of the main medical journals in Chile., Material and Methods: We reviewed 1,643 Scientific articles published between 2015 and 2020 in two medical journals from Chile. Three authors analyzed the title, abstract, and authors of all published articles, recording the sex of the first author, co-authors, and corresponding author., Results: The reviewed articles had a mean of 5.3 authors and there was a significant difference between men and women (a mean of 2.8 men and 2.4 women; p < 0.001). Forty-six percent (n = 761) of the articles had a female first author. In a higher proportion of papers, men completed both positions (1st and corresponding author) simultaneously., Conclusions: There are fewer female authors in scientific publications. Chile is one of the countries with a high rate of gender gap in the world. The underrepresentation of women in academia is an example of this.

The Chilean House of Representatives is discussing the bill on "Dignified death and palliative care", which regulates the so-called medical assistance in dying or euthanasia. This paper will critically analyze the different aspects of the bill, particularly concerning four main dimensions, namely, the patient's condition at the time of requesting euthanasia; the doctor-patient relationship and respect for the autonomy of both parties; the idea of medicine involved in it; and the possible therapeutic alternatives to euthanasia. This critical analysis raises the need to discuss in more depth a project that has to do with such important decisions about our lives and that implies a radical change in the way of thinking and practicing medicine.

Approximately 5% of COVID-19 patients will have a severe disease requiring invasive or non-invasive mechanical ventilation. In this conditions, sedatives and analgesics are fundamental to promote tolerance, comfort and synchrony with the mechanical ventilator. High and unusual requirements for sedation, analgesics and neuromuscular blockers have been reported in these patients, contributing to prolonged exposure, a high rate of delirium and prolongation of mechanical ventilation. These factors, added to the progressive shortage of these drugs, a high demand for care and less capacity for personalized attention, have created an adverse scenario for their proper and rational use. This paper proposes different pharmacotherapeutic optimization strategies for a rational management of sedation, analgesia and neuromuscular block in critically ill patients with COVID-19, with the therapeutic alternatives available in Chile.

Introduction: Autism Spectrum Disorders (ASD) are neurodevelopmental disorders of increasing prevalence. People with ASD have multiple health, education, and community needs, yet there is little information about their situation in Chile., Objective: To learn about the demographic and clinical characteristics, caregiver's first concerns, and age of diagnosis of ASD individuals., Patients and Method: Participants were parents/caregivers of ASD persons, who answered the Caregiver Needs Survey, developed by Autism Speaks specifically for this purpose, and translated into Spanish. The survey is comprised of 4 sections: demographic information, characteristics of the ASD individual, past and present use of Health and Education Services, and parents/caregivers' perceptions of satisfaction, impact, stigma, and quality of life. Data from the first two sections are reported in this paper., Results: The survey was answered by 291 caregivers (86% mothers) of 291 mostly male ASDs (89%), aged between 1-40 years (X: 10.4 SD: 6.1). The average age of parents' first concerns was 29.2m (SD: 23.8) where the main ones were: interaction difficulties (79.4%), unusual response to sensory stimuli (69.8%), behavioral problems (65.3%), unusual gestures/movements (64.3%), and lack of eye contact (63.6%). The ave rage age of diagnosis was 58m (SD: 36.5), with an average delay of diagnosis of 29m. The diagnosis was most frequently made by pediatric neurologists (44.7%), child psychiatrists (19.2%), and pedia tricians (5.5%). The most frequent comorbidities were language impairment, cognitive deficit, and behavioral problems., Conclusions: The late age of diagnosis of ASD and the large gap between the age of first concerns and diagnosis, represent a critical loss of treatment opportunities and jeopardize the chances of a better long-term outcome.

The current COVID-19 pandemic has the potential to overwhelm the capacity of hospitals and Intensive Care Units in Chile and Latin America. Thus local authorities have an ethical obligation to be prepared by implementing pertinent measures to prevent a situation of rationing of scarce healthcare resources, and by defining ethically acceptable and socially legitimate criteria for the allocation of these resources. This paper responds to recent ethical guidelines issued by a Chilean academic institution and discusses the main moral principles for the ethical foundations of criteria for rationing during the present crisis. It argues that under exceptional circumstances such as the current pandemic, the traditional patient-centered morality of medicine needs to be balanced with ethical principles formulated from a public health perspective, including the principles of social utility, social justice and equity, among others. The paper concludes with some recommendations regarding how to reach an agreement about rationing criteria and about their implementation in clinical practice.

In Chile, there are four bills to legalize euthanasia, an act which public surveys report as supported by most of the population. At the legislative, healthcare and social level, there is an active debate about euthanasia, the rights of terminally ill patients and the context of Palliative Care (PC) in the country. Chilean literature on euthanasia focuses mainly on the ethical analysis of the act itself but does not address the moral legitimacy of the legalization of this practice. This distinction is relevant since the probity of a particular action does not determine the moral legitimacy of its implementation at a public policy level. One aspect of this dimension is the potential negative impact of the legalization of physician-assisted death (PAD) on the development of PC services. This issue is particularly relevant in Chile, where PC provision is currently suboptimal and mostly restricted to cancer patients. This paper analyses available evidence on the potential impairment of PC development after PAD legalization. Although the analysis of evidence has some limitations, this concern is not supported by the available evidence. However, any project about PAD legalization must contemplate a factual commitment with the development of minimum PC provision, according to international recommendations.

This short paper focuses on the role of mathematical models to analyze the impact of pandemics on health resources and the different trade-offs that may be included in them. There is a large body of literature suggesting that mathematical modeling may be helpful to estimate how much additional equipment and infrastructure are necessary to mitigate an increase in demand for health services during a large-scale outbreak of an infectious disease. I comment on the crucial role of these models with a special focus on their strengths and limitations.

The code of ethics of the Medical College of Chile was modified in December 2019. The amendment was mainly to article 8, which refers to the doctor's duty to care for the pregnant woman and the child she is carrying. The change maintains this duty, but allows doctors to perform abortions, introducing three considerations that act as new principles or values for the medical profession: the plurality of values existing in society, the autonomy of women and what is established by law. This paper is a reflection on codes of ethics, their relationship with values, and with legislation. Also it shows the consequences that this modification represents for the principles governing medical activity, the status of medical specialties, the commitment of doctors to their patients and the validity of the code of ethics.

Background: Sedentary lifestyle and physical inactivity in Chile, has become a major problem and a public health priority in the last decade, as it increases the risk of non-communicable diseases., Aim: To identify which factors are associated with sedentary lifestyle and physical inactivity in Chile., Material and Methods: Qualitative systematic review of papers published between 2009-2019 in BIREME, SCIELO, WoS, Scopus and Medline databases, retrieving 29,752 and leaving finally five articles for review., Results: The main risk factors associated with sedentary lifestyle (according to the Chilean National Health Survey) were: living in an urban area, high income levels, being smoker or ex-smoker, being physically inactive, having a DVD, computer or notebook at home and a motorized vehicle. Physical inactivity was associated with an age ≥ 65 years, hypertension and sedentary lifestyle. There are some common factors such as female sex, obesity, type 2 diabetes mellitus and metabolic syndrome., Conclusions: here are independent factors associated with both sedentary lifestyle and physical inactivity in Chile. Some factors are common for the two unhealthy behaviors such as female gender, obesity, type 2 diabetes mellitus and metabolic syndrome.

This paper summarizes the main features of review articles in medical topics. Articles can be classified as narrative reviews, systematic reviews or meta-analysis. Narrative reviews are appropriate to update etiology, pathophysiology or clinical aspects of diseases, and advances in basic and preclinical sciences. In systematic reviews the authors define its purpose, limit its scope, describe the literature search, define the inclusion and exclusion criteria adopted to select primary studies, and the criteria applied to assess the quality of their results and conclusions. Meta-analysis are quantitative, statistically analysed systematic reviews that consider mainly primary studies conducted prospectively with simultaneous randomized controls, pooling the data obtained from each of these primary studies in order to get a single estimate of effect. Systematic analysis and meta-analysis are important to evaluate new diagnostic and therapeutic tools, and they are most relevant to evidence-based medicine, mainly for the design of clinical guidelines and the adoption of new health care policies. Review articles published in Revista Médica de Chile were compared in one or two-year periods separated by ten years in between: in the "2001 period" 26 reviews were all narrative; in the "2010 period" 30 reviews were narrative and another 4 were systematic reviews; in the "2019 period" 14 reviews were narrative and another 7 were systematic reviews. No meta-analysis had been published in these periods, in this journal. Meta-analysis including primary studies performed in Chile by Chilean investigators have been published in English language in other medical journals. The educational and professional role of review articles is recognised, with a word of caution on a strict adherence to ethical rules adopted by scientific and clinical publications, mainly with respect to authorship and potential conflicts of interest.

Hypertension (HTN) in children and adolescents is an important pathology, of, guarded prognosis, associated with modifiable and non-modifiable factors. The estimated prevalence is around 3.5% which increases progressively with age. The ideal method for its diagnosis is the measurement of blood pressure (BP) with auscultatory instruments. According to the American Academy of Pedia trics (AAP), BP should be measured in children older than three years of age once a year, and in children younger than three years of age if they present risk factors. Once the HTN is confirmed, the evaluation should be directed towards the detection of a causative disease and/or the search for risk factors associated with a primary HTN. The objective of treating primary and secondary HTN in pediatrics is to achieve a BP level that decreases the risk of target organ damage. Therapeutic op tions include treatment according to specific etiology, non-pharmacological and pharmacological one. This paper presents the position of the Chilean Society of Pediatrics Nephrology Branch with the aim of guiding pediatricians and pediatric nephrologists in the correct management of HTN in childhood. In this second part, recommendations on antihypertensive treatment are presented with an emphasis on lifestyle changes.

Health care raises structural issues in a democratic society, such as the role assigned to the central government in the management of health risk and the redistributive consequences generated by the implementation of social insurance. These are often cause of strong political controversy. This paper examines the United States of America health reform, popularly known as "ObamaCare". Its three main elements, namely individual mandate, creation of new health insurance exchanges, and the expansion of Medicaid, generated a redistribution of health risks in the insurance market of that country after almost a century of frustrated legislative efforts to guarantee minimum universal coverage. The article proposes that a change of this magnitude in the United States will produce effects in a forthcoming parliamentary discussion on the health reform in Chile, which still maintains a highly deregulated private health system.

Under Chilean public law, liability of a public hospital appears when the institution fails to accomplish its mandatory duties or incurs in negligence. As in private law liability, this system requires to determine the duties of hospitals and, in case of a medical accident, professional standards or duties. This paper explores the two main categories of medical malpractice, namely erroneous diagnosis and treatment failures based on public law theory and judiciary methods.

This paper describes and analyzes the psychiatric reform process in three Latin American countries (Brazil, Chile and Colombia) based on the Caracas Declaration of 1990. It compares the psychiatric reform processes in these three countries and highlights the role of national mental health laws in these processes. Our goal when investigating the experiences in other countries of Latin America is to draw conclusions for the Argentine psychiatric reform and for the future of such reforms in the region.

The Chilean physician Hans Betzhold published the book Eugenesia (Eugenics) in 1939, which was a work that received multiple awards and ran to a second edition in 1942. Both editions and the participation of Betzhold at the Second Peruvian Conference on Eugenics in 1943 attest to the fact that he was an important actor in the field of Chilean eugenics. This paper analyzes his transition from the publication of Eugenesia, in which he proposes a National Eugenics Department combining existing projects and laws to make the eugenic ideal a reality until its intervention, in the year 1943, when his optimism yields to disillusion regarding the task of creating a "Chilean superman."

The aim of this paper was to analyze selected variables that could be involved in vaccination coverage of children in different communal territories of Chile, including the vaccines bacillus Calmette-Guérin (BCG) at birth, pentavalent at 6 months and measles-mumps-rubella (MMR) at 12 months and 6 years, in the period 2008-2011. The methodology includes three phases. The first corresponds to the operationalization of twenty socio-territorial variables that account the general life contexts of the target population. The second phase involves the definition of a scale of values through a panel of experts to weigh the importance of each of the variables. The third phase integrates the information into a spatial decision support system for the identification of territorial patterns, through multi-criteria evaluation and multivariate cluster analysis. The results indicate that it is feasible to group territories into four types. In general terms, it is possible to observe that as the life context improves, higher levels of coverage can be found for each vaccine.

Background: During the years 2008 and 2009, 1,191 biomedical articles authored by Chilean investigators working in Chile were indexed in PubMed., Aims: To evaluate the potential visibility of those articles, according to scientometric indexes of the journals where they were published., Methods: Those journals where the articles had been published were identified and each journal’s Impact Factor (JIF), 5-year JIF, SCImago Journal Rank (SJR), SCImago Quartiles (Q) for 2010 and the Source Normalized Impact per Paper (SNIP) for 2008-2009 were identified., Results: Three hundred and twelve articles (26,2%) were dedicated to experimental studies in animals, tissues or cells and they were classified as “Biomedicine”, while 879 (73,8%) were classified as “Clinical Medicine”; in both areas the main type of articles were original reports (90% and 73.6%, respectively). Revista Médica de Chile and Revista Chilena de Infectología concentrated the greater number of publications. Articles classified in Biomedicine were published more frequently in English and in journals with higher scientometric indexes than those classified in Clinical Medicine., Conclusions: Biomedical articles dealing with clinical topics, particularly case reports, were published mostly in national journals or in foreign journals with low scientometric indexes. It can be partly attributable to the authors’ interest in reaching local readers. The evaluation of research productivity should combine several scientometric indexes, selected according to the field of research, the institution's and investigators’ interests, with a qualitative and multifactorial assessment.

Postpartum depression (PPD) is a common condition worldwide and most of the available information is about treatment rather than prevention. This paper is an update on prevention and treatment of PPD. A simple review of the literature and a critical review of papers' methodology and conclusions was carried out. There is a consensus of the preeminence of psychosocial factors in the genesis of PPD. Considering the complications when the condition is not treated, it is of the utmost importance to implement early detection and management strategies. The use of psychosocial preventive interventions is an alternative that has support in the literature and should be seriously considered.

When there were giants on the earth, two of them, Rudolf Kraus and Giovanni Noe, met at the ancient Bacteriological Institute of Chile, and founded together a journal representing the principles of the institution. A nostalgic glance over the papers published therein for these so named giants lets us know a little about the study of infectious diseases in the thirties, when tuberculosis and malaria were two of the biggest problems for the world's public health... then and now.

Depression is a global public health priority. Intervention strategies at different levels of care must be implemented. Research is needed in the areas of depression prevention, treatment and rehabilitation. This paper reports a literature review, focused in Chile, on the magnitude, impact, recommended management strategies and new research on depression.

This paper reports the reflections of a group of members of the University of Chile Faculty of Medicine, about the changes in teaching methods that medical schools should incorporate. In a complex scenario, not only new and better knowledge should be transmitted to students but also values, principles, critical reasoning and leadership, among others. In the first part, a proposal to understand this educational development in the context of complex universities, incorporating pedagogical skills and reviewing institutional leadership, is carried out. In the second part, the training of teaching physicians, as part of the changes, is extensively discussed. Physicians hired as academics in the University should have the opportunity to work mainly as teachers and be relieved of research obligations. For them, teaching should become a legitimate area of academic development.

Private health insurance should be able to provide coverage to people considered as high risk, such as women and the elderly. The only way to do that is to organize implicit or explicit cross-subsidies from low to high-risk individuals. This paper examines how European private health insurance companies introduced regulatory measures that could be introduced in Chile such as open enrollment, community-rated premiums, lifetime coverage, a package of minimum benefits and a risk equalization scheme.

Health research is considered an essential element for the improvement of population health and it has been recommended that a share of the national health budget should be allocated to develop this field. Chile has undertaken efforts in the last decades in order to improve the governmental structure created to promote the development of health research, which has increased human resources and funding opportunities. On the other hand, the sustained economic growth of Chile in the last decades suggests that the health expenditure will maintain its increasing trend in the following years. This additional funding could be used to improve coverage of current activities performed in the health system, but also to address the incorporation of new strategies. More recently, health technology assessment (HTA) has been proposed as a process to support decisions about allocation of resources based on scientific evidence. This paper examines the relationship between the development of health research and the HTA process. First, it presents a brief diagnosis of the situation of health research in Chile. Second, it reviews the conceptual basis and the methods that account for the relationship between a HTA process and the development of health research. In particular, it emphasizes the relevance of identifying information gaps where funding additional research can be considered a good use of public resources. Finally, it discusses the challenges and possible courses of action that Chile could take in order to guarantee the continuous improvement of an articulated structure for health research and HTA.

The Chilean health system has not been completely oblivious to health technology assessment (HTA). In fact, significant advances in the areas of health prioritization using criteria of disease burden, effectiveness and cost-effectiveness among others, can be acknowledged. The introduction of the reform of Explicit Health Guarantees (GES) has been an important milestone in this arena, allowing the consideration of other dimensions such as social preferences in health. However, the application of HTA encompasses the entire health system and in that sense the institutionalization of a process properly defined and extensively validated in our country, is imminent. This paper discusses the foundations on which progress must be made in institutionalizing HTA, starting from the architecture of our health care system and in light of the economic and social reality. We review some background information first, and then discuss some important considerations in our context, including information on the institutional and legal framework. It concludes with the authors' view on some key elements to consider in HTA in Chile, which does not necessarily represent the vision of the Ministry of Health.

Equity has been recognized as one of the driving principles of many health systems in the world. In Latin America, Chile has led the explicit inclusion of equity in their health policies, which is reflected in its recent health reform. On the other hand, Chile faces the challenge of defining and implementing a policy for health technology assessment (HTA), which should be consistent with the equity principles that underlie the Chilean national health system. This paper reviews the equity concept emphasizing the relevance of socioeconomic inequalities. Furthermore, it examines how international HTA agencies have explicitly included this element in the evaluation and decision processes. It presents the English case, which highlights the elements of procedural justice rather than adopting a normative position regarding equity. Finally, it examines the methods that have been developed aiming to make explicit consideration of equity in HTA. It concludes that the methodological development to incorporate equity elements with empirical basis is recent and limited. The consideration of equity is, in most of the cases, left to the instances of deliberative participation.

The increasing concerns of nations to improve efficiency, access and quality of health care, have encouraged a more appropriate use of health interventions. Thus, the interest of clinicians and decision-makers has shifted towards evidence-based medicine, comparative effectiveness research and health technology assessment (HTA). Although these concepts should not be understood as synonyms, they converge on common characteristics: a systematic approach to evidence, the focus on relevant outcomes for the patient, and the notion that policy-making for a group of patients will affect others. As a consequence, concerns not only involve efficiency and effectiveness, but also transparency, clinical practice and opportunity costs. This paper introduces the concept and processes of HTA in the first and second sections. Section three reviews the evolution of HTA in developed and Latin-American countries, analyzing the aspects that influence the structure and scope of HTA. The last section concludes with reflections on the challenges to implement HTA in Chile.

Objective: . This paper aims at assessing the contribution of Chile's Health Care Integrating Councils (CIRA, Spanish acronym) to strengthening governance in health., Materials and Methods: A literature review on the official documents related to the process of creation and development of CIRA was carried out; an ad hoc questionnaire was applied to all 29 health services of the country; finally, 35 semi-structure in-depth interviews were carried on a sample of six CIRA., Results: The CIRAs have become a tool for functional integration and a valuable space for dialogue, cooperation and learning for all of the actors of the Chilean public health network., Conclusions: In this study, we conclude that there is room for improvements of CIRA's role regarding governance of the health care network as long as CIRA is authorized to deal with strategic topics, such as investment in infrastructure, technology and human resources, and budgeting.

Providing information to patients and relatives (IPAR) is a clinical, ethical and legal need. IPAR is inherent to the medical team work as diagnostic and therapeutic procedures are. In some cases, it is a key step for patient's recovery. From an ethical point of view, the patient is not only the subject of the medical work but also its main owner and who must, previously informed, decide about options which would directly affect him. After the promulgation of a special law in Chile, this prerogative became a legal right. This paper proposes an empirically developed model or protocol that should be used to inform hospitalized patients and their relatives about their disease, in a pertinent way. Considering that IPAR is a skill that should be learned and practiced, the written protocol is provided to neurology residents as supporting material. Although IPAR protocols are completely justified, they have not been evaluated in terms of efficiency of communication, user satisfaction, patient and relatives reassurance and clinical impact. Therefore, they require a prospective validation.

Health care at population level is a complex problem. Having this in mind, the purpose of this paper is to focus on the goods that are ethically relevant in the process of caring for health at this level. We briefly analyze some of the Chilean health statistics that, although they show important improvements along the years, demonstrate that certain conditions are to be deemed as inadequate by both healthcare providers and patients. Ethics is a central component to determine how to structure and organize health care systems and how they should operate. We emphasize human dignity as an ethical corner stone of the health care system, along with other important values such as justice and humanization, under the scope of the ends of medicine, and other components such as technical competence of providers and the financing of the whole process. We conclude that as far as a health care system is organized in a way that medical practice is well ordered, primarily and fundamentally according the ends of medicine and the good of persons, such a health care system is ethically adequate.
