Background/Aims: Extensive research focuses on the advantages and disadvantages of various positions throughout active second stage labour, but there is limited focus on midwives' practice. This study aimed to explore midwives' practice of maternal positions throughout the active phase of second stage labour. Methods: An integrative review of three databases was carried out for research papers published between 2015 and 2022. Thematic analysis was used for data analysis and synthesis. Results: Five eligible research papers were included. There were four key findings: maternal positions practised by midwives, midwives as the prime decision maker, midwives' personal convenience and comfort, and barriers to practising maternal positions. Conclusions: Midwives were seen as prime decision makers, who hindered informed decision making on maternal positions during active second stage labour. Inadequate knowledge, skills and confidence were barriers to practice and attributed to midwives adopting positions that were convenient and comfortable for themselves. Strategies are needed to enhance practice through review and evaluation of current clinical settings, regular updates to evidence-based practice and refreshing essential midwifery skills. This paper uses the concept of epistemic injustice to shed light on the discriminatory treatment of experts in and by development aid. While the literature on epistemic justice is largely based on philosophical reasoning, I provide an empirical case study which substantiates theoretical claims with findings from social science research. Drawing on expert interviews conducted in South Africa and Tanzania, I reveal how epistemic injustice is experienced, practiced and institutionalised in a field which claims to work towards global justice. Focusing on aid-related advisory processes, the paper highlights how epistemic authority therein is tied to identity-based prejudice. The systematic credibility deficit policy experts from aid-receiving countries suffer is closely interrelated with the credibility excess so-called 'international' experts profit from. Their privilege is backed by an imaginary that maintains the idea of Northern epistemic superiority and sustained by prevailing employment and procurement practices of donor organisations. The paper suggests that the concurrence of testimonial and hermeneutical injustice prevents experts from the Global South from taking the lead in interpreting their own societies' realities. This, I argue, is not only detrimental to the countries whose knowers are marginalised but also a root cause of persisting global inequality. [ABSTRACT FROM AUTHOR]

The study aims to understand the foremost challenges in the transition to online teaching and learning during the COVID-19 pandemic. The study adopts the PRISMA approach to screening the selection of journal articles and review papers according to the research aims and the inclusion criteria. The journal articles and review papers were extracted and stored in Microsoft Excel and Google Scholar, Academic. Microsoft, Semantic Scholar, Elsevier, and Emerald Insight databases searched relevant documents using formulated keywords. A statistical technique was applied using the M.S. Excel analysis tool (PivotTable and an independent t-Test) to analyze data and determine the differences between teachers and students. The review revealed the evidence that the majority of the studies were primarily focused on the individual developing countries and results from other developing countries were not considered. In addition, the foremost challenges in the transition to online teaching and learning during the COVID-19 pandemic were inadequate skills and training, inadequate Internet/Infrastructure, lack of supporting resources and lack of online student engagement and feedback. Finally, the independent t-test reveals there is no statistically significant difference in challenges in the transition to online teaching and learning during the COVID-19 pandemic. Both teachers and students encounter similar challenges. The systematic review raised concerns that higher learning needs to effectively implement long term strategies and support teachers and students in getting into online teaching and learning.

Background: Sub-Saharan Africa (SSA) countries are facing an epidemiological shift from infectious disease to chronic diseases, such as cardiovascular diseases (CVDs). CVDs incidence in SSA are frequently attributed to the prevalence of hypertension, diabetes, and overweight/obesity. Nevertheless, some researchers contend that CVDs are not a priority public health problem in SSA. Method: This paper systematically reviews the evidence on CVDs and their relation with hypertension, diabetes mellitus and obesity/overweight in Ghana, Nigeria, South Africa, Sudan and Tanzania. The publication's content was analyzed qualitatively using the directed content analysis method and the results were presented in a tabular format. Result: The paper illustrates the rising prevalence of CVDs as well as the three related risk conditions in the selected SSA countries. Conclusion: The review indicates a poor health system response to the increasing risk of CVDs in SSA. Purpose: This paper aims to explore the effects of the customer-to-customer co-creation experiences of savings/credit groups in the African context and how savings/credit groups influence financial capability and enhance financial well-being. Design/methodology/approach: Using purposive sampling, a study of a total of 18 focus groups was conducted in sub-Saharan Africa. Nine urban-based savings/credit groups were drawn from across South Africa and additional nine, rural-based savings/credit groups were studied in the Monduli district of Tanzania. Findings: Findings demonstrate that the African philosophy of Ubuntu, which promotes customer-to-customer interaction, is the cornerstone of the customer-to-customer co-creation experience. Ubuntu philosophical principles were found to influence the dialogue, access, risk and transparency model of co-creation and customer-dominant logic. The results show further that customer-to-customer co-creation experience positively influences the cognitive, financial, personal and social experiences of members. Specifically, it was found that cognitive and financial experiences positively influence financial satisfaction, financial self-esteem, financial self-efficacy and financial capability, all of which enhance financial well-being. In addition, personal and social experiences positively influence equality, self-confidence, entrepreneurial skills and motivation that in turn enhance social well-being. Research limitations/implications: This study has implications for many different stakeholders concerned with the financial inclusion of low-income consumers, particularly in the southern part of Africa. Originality/value: To the authors' knowledge, this is the first study to explore the effects of customer-to-customer co-creation experiences in traditional financial services settings in order to understand how these indigenous financial services influence the financial capability and financial well-being of co-creation members. This article seeks to ascertain whether refugees who are victims of sexual violence in contracting states enjoy access to courts per Article 16 of the United Nations (UN) Convention Relating to the Status of Refugees (1951 Refugee Convention). It does so by comparing the situation of urban refugees in South Africa with that of refugees in camps in Tanzania and settlements in Uganda, beginning with a description of what "accessing courts" entails in the respective domestic criminal justice systems and of what mechanisms are in place for addressing sexual offences. It further uses the qualitative analysis of documented prosecuted cases of sexual violence in South African, Tanzanian and Ugandan courts between 2013-2017, 2009-2016 and 2013-2017 respectively to establish if these countries prosecute cases of sexual violence suffered by their citizens and whether claims of such violations affecting refugees also enjoy the same treatment. The enquiry found that of 328 documented prosecuted cases of sexual offences in South Africa, victims who were citizens were a majority in number. In Tanzania there appeared to be few prosecuted cases of sexual violence against refugees, but given that limited documentation is available, it is difficult to assess the actual figures. In Uganda the 187 recorded prosecuted cases of sexual offences in the years of investigation all related to citizens, despite the introduction of a mobile court to refugee settlements. Overall, this paper recommends that the countries under review adopt measures to ensure the prompt prosecution of cases of sexual violence against refugees and thereby enable them to access courts and testify against their assailants. Medical education in Africa is in desperate need of reforms, evident in widespread diseases, and an inability to mobilise and train the required medical workforce to deal with these health issues. However, the exponential rise in the use of mobile technologies due to the spread of the Internet and increased telecommunication networks offer an opportunity for the transformation of medical education and practice through the deployment of mobile devices as a medium for learning and conveying health care services to the remote and resource-constrained locations of Sub-Saharan Africa (SSA). This paper reviewed articles on the affordances of m-learning for distributed medical education in SSA published between 2010-2018. Results from 18 articles identified in the review revealed a slow-paced ascendancy of practice and research in the field; it further exposed competing priorities, infrastructural deficit, and chronic workforce shortages as the bane of m-learning implementation in the subregion. This paper makes recommendations that will enhance the growth of mobile-based distance medical education and practice in SSA.

The HIV response is hampered by many obstacles to progression along the HIV care cascade, with men, in particular, experiencing different forms of disruption. One group of men, whose stories remain untold, are those who have succumbed to HIV-related illness. In this paper, we explore how next-of-kin account for the death of a male relative. We conducted 26 qualitative after-death interviews with family members of male PLHIV who had recently died from HIV in health and demographic surveillance sites in Malawi, Tanzania, Kenya, Uganda, Zimbabwe and South Africa. The next-of-kin expressed frustration about the defiance of their male relative to disclose his HIV status and ask for support, and attributed this to shame, fear and a lack of self-acceptance of HIV diagnosis. Next-of-kin painted a picture of their male relative as rebellious. Some claimed that their deceased relative deliberately ignored instructions received by the health worker. Others described their male relatives as unable to maintain caring relationships that would avail day-to-day treatment partners, and give purpose to their lives. Through these accounts, next-of-kin vocalised the perceived rebellious behaviour of these men, and in the process of doing so neutralised their responsibility for the premature death of their relative. Background: The importance of tackling economic, social and health-related inequities is increasingly accepted as a core concern for the post-Millennium Development Goal framework. However, there is a global dearth of high-quality, policy-relevant and actionable data on inequities within populations, which means that development solutions seldom focus on the people who need them most. INTREC (INDEPTH Training and Research Centres of Excellence) was established with this concern in mind. It aims to provide training for researchers from the INDEPTH network on associations between health inequities, the social determinants of health (SDH), and health outcomes, and on presenting their findings in a usable form to policy makers. Objective: As part of a baseline situation analysis for INTREC, this paper assesses the current status of SDH training in three of the African INTREC countries - Ghana, Tanzania, and South Africa - as well as the gaps, barriers, and opportunities for training. Methods: SDH-related courses from the three countries were identified through personal knowledge of the researchers, supplemented by snowballing and online searches. Interviews were also conducted with, among others, academics engaged in SDH and public health training in order to provide context and complementary material. Information regarding access to the Internet, as a possible INTREC teaching medium, was gathered in each country through online searches. Results: SDH-relevant training is available, but 1) the number of places available for students is limited; 2) the training tends to be public-health-oriented rather than inclusive of the broader, multi-sectoral issues associated with SDH; and 3) insufficient funding places limitations on both students and on the training institutions themselves, thereby affecting participation and quality. We also identified rapidly expanding Internet connectivity in all three countries, which opens up opportunities for e-learning on SDH, though the current quality of the Internet services remains mixed. Conclusions: SDH training is currently in short supply, and there is a clear role for INTREC to contribute to the training of a critical mass of African researchers on the topic. Purpose - There has been no shortage of rosy visions for the future of Africa or its regions. Almost without exception, these visions have been dashed by reality. The question therefore arises: "To what extent or under what conditions is visioning an ideal future a worthy exercise?" or "Under what conditions is it useful and when is it limiting?" In anticipating the future in general, and seeking a better future for Africa in particular, might foresight tools built on another (non-visionary) basis provide better fruit? This paper aims to address this issue. Design/methodology/approach - The paper considers this question as it applies to scenario planning in particular, by investigating recent, contrasting case studies of scenario-building activities in Africa (Tanzania and South Africa). Findings - The paper determines the appropriate uses and limits of "visionary" scenario planning, and suggests a contrasting "adaptive" basis for scenario work. Originality/value - It is argued that maintaining the purpose-platform distinction between these two modes is fundamental to getting full value out of scenario work in general, and in the African context in particular. The contrasting case studies explain how and why the adaptive-normative distinction is important; how different purposes each demand different use of the scenario process tools that currently exist, and show why failure to fit scenario approaches to purpose greatly diminishes the efficacy of the method for either purpose. This paper explores the thesis of de-globalisation in relation to international education. Through interrogating accounts of international school leadership during the COVID-19 crisis, the tension between international expectations and localised realities is charted, with four central tenets of internationalism undermined by the pandemic experience. It is argued that the COVID-19 crisis, ostensibly a single global event, resulted in the fractalisation of international education; the conceptualisation of unified internationalism was undermined by the inherently localised material effects of the pandemic. In place of an internationalism that is unified, transcendent, inclusive and connected, international school leaders' accounts of leading through the pandemic focused on their sense that their schools were fractured, rooted, privileged and isolated. It is suggested that this international crisis demonstrates the precarious nature of the respatialising of the global that is intrinsic to international schooling.

In sub-Saharan Africa over 52 million children are living with the death of one or both parents. Drivers of this parental mortality include afflictions at levels endemic to the region, including: HIV; malaria and other parasites; lower respiratory infections; diarrhoeal illnesses; and road accidents, among others. This paper examines the impact of orphanhood on learning outcomes among girls and boys in sub-Saharan Africa, conditional on school enrolment. By analysing test scores for approximately 60,000 pupils in 12 countries, we estimate the effect on student test scores by comparing paternal, maternal, and double orphans to non-orphans in the sample, specifically for the subjects of reading, mathematics, and HIV-AIDS knowledge. No previous study has analysed how orphanhood might influence learning by using student test score data, making this paper's approach unique in the literature. This study employs two estimation techniques: Coarsened exact matching calculates the sample average treatment effect on the treated, while matching on students' family structure, household wealth, school resources, and geographic location; and double lasso (DL) regression applies applying machine-learning for variable selection with high-dimensional controls for regional and school identifiers, school location, and student age. Our results show both CEM and DL consistently report a significant negative impact of orphanhood on test scores among specific countries, especially those which faltered in addressing the HIV-AIDS crisis.

The drives to internationalise the UK curriculum and psychology students' desires to work in communities are brought together in this paper. International community-based learning (ICBL) links with many psychology students' motivations to make contributions to others; with the potential to enhance students' learning and cultural sensitivities. The recently-developed literature on international service learning highlights multiple benefits for students (and sometimes community hosts), as well as the potential tensions that need to be negotiated. With the intentions of creating global citizens interested in social justice, community-based engagement can sometimes reinforce preconceived notions. In addition, benefits to the host communities may only be short-term or questionable, with less research on community partners' perspectives than on those of the students. This paper describes work with UK psychology students who undertook CBL in four different international settings (2008--2015); illustrating the complexities of setting up, facilitating and supporting students' CBL. It will then focus on two African settings (South Africa and Tanzania), with data drawn from group discussions with students, post-experience reflections, observations by the accompanying tutor and feedback from community partners. The findings show the richness of students' learning through CBL in both settings, including deeper insight into applications of psychological theory, the skills they enhanced and the emotional impacts of the work. Students reported changed perspectives, including greater awareness of social issues that impacted on their attitudes. The findings are explored and compared, using tools from Activity Theory, to illustrate points of confluence and tensions in such initiatives, when students, community partners and academic staff members interact. The discussion will reflect on what might guide the integration and optimising of ICBL to benefit both students and community partners.

National heritage is an important part of a country's identity. When a nation appreciates its heritage, it becomes more likely that this heritage will be sustained. In Africa, a lack of appreciation for national heritage is not uncommon (UNESCO 2006), and therefore various efforts are directed towards changing this trend. This paper argues in the affirmative that archives are described as evidence of history and these records, therefore, have the potential to contribute to promoting national heritage. Archives serve as documentary evidence of the individuals or governments that created them. As such, citizens have a right to review such records to learn more about their national heritage. This article seeks to examine the role of archives in promoting national heritage. The perception and experience of national archival institutions on this matter is explored in three Eastern and Southern Africa Regional Branch of the International Council on Archives (ESARBICA) member states, namely Tanzania, South Africa and Botswana. Documentary review and questionnaires were employed to identify, among other things, the types of archival materials preserved in the national archives of each country with regard to national heritage, the number and description of individuals or groups that normally visit such repositories and the categories of information that visitors frequently request. The findings indicate that the three archives, RAMD, NARS, and BNARS, have a good number of archives of national heritage value. However, the number of professionals and strategies to manage, preserve and promote them is inadequate. There is a need to think out of the box to determine different ways to reach out to people and to promote access to information on our national heritage. For instance, social networking is an approach that none of the archival institutions in this study use to promote their collections, despite more and more people in these countries having access to social networks through mobile technologies. The study suggests that, while digitising records has proved to be one solution to preserving archival heritage, it is important that archival institutions use such technological advancement by digitising archival documents and then uploading them to their websites for easy public access. Abstract: At the seventh Waternet/WARFSA/GWP-SA Symposium, Lewis Jonker described the “perceived failure of implementing IWRM in South Africa.” This paper starts from Jonker’s observation – which can certainly be defended – and argues that attempts to implement full Integrated Water Resources Management (IWRM) are doomed to failure and disappointment. The paper therefore offers a more practical ‘expedient’ solution. The paper is based on research done in Tanzania (Ruaha Basin) and South Africa (Olifants Basin) that is further informed by a growing literature critical of the IWRM paradigm as currently understood and practiced. As a guide to actual policies and their implementation in developing countries, IWRM has led to mis-guided priorities and paralysis of development programmes. An alternative approach is one in which basin managers identify priority problem areas, and focus specifically on finding solutions to these problems within an integrated framework, rather than starting with a broad set of principles and trying to implement these. The paper proposes that a shift away from IWRM as a normative concept is now overdue, and argues for realism and action by focusing attention on and prioritizing the critical needs of poor people in Africa. In the field of development, Information & Communication Technology (ICT) is often hailed and cursed at the same time. ICT offers great promise to enhance development activities' efficiency and effectiveness yet the literature is littered with examples of failure. A particular challenge concerns the application of ICT to support collaboration in development contexts. In this paper, we report on field experiences with one particular type of collaboration technology, Group Support Systems (GSS), and its role in supporting groups engaged in development activities. Being an North-American invention, research into GSS is predominantly focused on Western Euro-American settings. GSS field studies in other cultural environments are scarce. The objective of our study is to explore and compare the applicability of GSS in two particular environments: Tanzania and South Africa. Our data suggest that the use of GSS is evaluated positively in both countries, although Tanzanian groups perceived more benefits. In South Africa, top management displayed very open and non-conservative behavior towards the technology, while in Tanzania hesitance from top management can be expected to be the greatest hindrance for GSS acceptance and application. The data further indicate that GSS do not replace existing meeting customs, but rather introduce new ones that co-exist next to the traditional ones. A key difference between application of GSS in western and African environments is a stronger focus on the electronic part of discussions in Africa. Anonymity is perceived as the key feature. The challenges to teacher educators in sub-Saharan Africa are acute. This paper describes how the Teacher Education in Sub-Saharan Africa (TESSA) consortium is working within institutional and national policy systems to support school-based teacher professional development. The TESSA consortium (13 African institutions and 5 international organisations delivering teacher education across 9 countries) designed and produced a bank of open educational resources (OERs) to guide teachers' classroom practices in school-based teacher education. Drawing on examples from the TESSA consortium and from the University of Fort Hare, South Africa, the authors categorize the forms of TESSA OER integration as highly structured, loosely structured, or guided use. The paper concludes by outlining success factors for the integration of OERs: accessibility, adequate resources, support for teachers, accommodation of local cultural and institutional practices, and sustainable funding. Undetected and unmonitored hypertension carries substantial mortality and morbidity, especially during pregnancy. We assessed the accuracy of OptiBPTM, a smartphone application for estimating blood pressure (BP), across diverse settings. The study was conducted in community settings: Gaibandha, Bangladesh and Ifakara, Tanzania for general populations, and Kalafong Provincial Tertiary Hospital, South Africa for pregnant populations. Based on guidance from the International Organization for Standardization (ISO) 81,060–2:2018 for non-invasive BP devices and global consensus statement, we compared BP measurements taken by two independent trained nurses on a standard auscultatory cuff to the BP measurements taken by a research version of OptiBPTM called CamBP. For ISO criterion 1, the mean error was 0.5 ± 5.8 mm Hg for the systolic blood pressure (SBP) and 0.1 ± 3.9 mmHg for the diastolic blood pressure (DBP) in South Africa; 0.8 ± 7.0 mmHg for the SBP and −0.4 ± 4.0 mmHg for the DBP in Tanzania; 3.3 ± 7.4 mmHg for the SBP and −0.4 ± 4.3 mmHg for the DBP in Bangladesh. For ISO criterion 2, the average standard deviation of the mean error per subject was 4.9 mmHg for the SBP and 3.4 mmHg for the DBP in South Africa; 6.3 mmHg for the SBP and 3.6 mmHg for the DBP in Tanzania; 6.4 mmHg for the SBP and 3.8 mmHg for the DBP in Bangladesh. OptiBPTM demonstrated accuracy against ISO standards in study populations, including pregnant populations, except in Bangladesh for SBP (criterion 2). Small pelagic fish, including anchovies, sardines and sardinellas, mackerels, capelin, hilsa, sprats and herrings, are distributed widely, from the tropics to the far north Atlantic Ocean and to the southern oceans off Chile and South Africa. They are most abundant in the highly productive major eastern boundary upwelling systems and are characterised by significant natural variations in biomass. Overall, small pelagic fisheries represent about one third of global fish landings although a large proportion of the catch is processed into animal feeds. Nonetheless, in some developing countries in addition to their economic value, small pelagic fisheries also make an important contribution to human diets and the food security of many low-income households. Such is the case for many communities in the Zanzibar Archipelago and on mainland Tanzania in the Western Indian Ocean. Of great concern in this region, as elsewhere, is the potential impact of climate change on marine and coastal ecosystems in general, and on small pelagic fisheries in particular. This paper describes data and information available on Tanzania's small pelagic fisheries, including catch and effort, management protocols and socio-economic significance. Then, incorporating the rapidly improving understanding of the region's oceanography resulting from the application of remote sensing and oceanographic modelling, the paper undertakes the most complete assessment to date of the potential impacts of climate change on the small pelagic fishery of the Pemba Channel. Pathways of climate change impact are explored and crucial knowledge gaps, both in terms of the fishery itself and the wider ecosystem, are identified in order to guide future research activities. Although we analyse small pelagics in the specific context of the Pemba Channel, the key challenges identified in the analysis are likely to be relevant to many small pelagic fisheries in coastal nations heavily dependent on living marine resources. The importance of ill-health in perpetuating poverty is well recognized. In order to prevent the damaging downward spiral of poverty and illness, there is a need for a greater level of social protection, with greater cross-subsidization between the poor and wealthy, and the healthy and those with ill-health. The aim of this paper is to examine individual preferences for willingness to pre-pay for health care and willingness to cross-subsidize the sick and the poor in Ghana, South Africa and Tanzania. Household surveys in the three countries elicited views on cross-subsidization within health care financing. The paper examines how these preferences varied by socio-economic status, other respondent characteristics, and the extent and type of experience of health insurance in the light of country context. In South Africa and Ghana, 62% and 55% of total respondents, respectively, were in favour of a progressive financing system in which richer groups would pay a higher proportion of income than poorer groups, rather than a system where individuals pay the same proportion of income irrespective of their wealth (proportional). In Tanzania, 45% of the total sample were willing to pay for the health care of the poor. However, in all three countries, a progressive system was favoured by a smaller proportion of the most well off than of less well off groups. Solidarity has been considered to be a collective property of a specific socio-political culture, based on shared expectations and developed as part of a communal, historical learning process. The three countries had different experiences of health insurance and this may have contributed to the above differences in expressed willingness to pay between countries. Building and ‘living with’ institutions that provide affordable universal coverage is likely to be an essential part of the learning process which supports the development of social solidarity. This paper reports a phenomenographic study exploring diverse understandings and experiences of teacher leadership among 12 members of the International Study of Teacher Leadership research team comprised of 20 academics located in 10 countries. Mind mapping and semi-structured, online interviews were used to explore the ways that the participants related with the phenomenon of interest: 'teacher leadership'. Phenomenographic analysis of interview artefacts revealed nine qualitatively different conceptions of teacher leadership in the study's outcome space across three broad domains: A: The school, school community and formal education system; B: The teacher leader's professional self; C: The broader historical, socio-political and global contexts of teacher leadership. In addition to providing a 'touchstone' for the team's ongoing research, these findings serve as an experiential framework for thinking about teacher leadership, potentially encouraging more inclusive, more complete and richer understandings of the phenomenon.

The Burgersdorp Formation of South Africa is a richly fossiliferous rock sequence at the top of the Permian-Triassic Beaufort Group and is known for its abundance of Early-Middle Triassic vertebrate remains, particularly cynodonts. Fossils from the Burgersdorp Formation are referred biostratigraphically to the Cynognathus Assemblage Zone (CAZ), which is further divided into three subzones: Langbergia-Garjainia, Trirachodon-Kannemeyeria, and Cricodon-Ufudocyclops. Each subzone is characterised by the presence of a distinct species of trirachodontid, a group of gomphodont cynodonts found relatively abundantly throughout the CAZ, with the lower two subzones characterised by the medium-sized trirachodontids Langbergia and Trirachodon. The uppermost part of the formation, the Cricodon-Ufudocyclops subzone, yields trirachodontids of larger size. The majority of these trirachodontid specimens have previously been referred to Cricodon metabolus, a taxon also known from the Manda Beds of Tanzania and the Ntawere Formation of Zambia. Here we identify one of the specimens (BP/1/5538) previously referred to Cricodon as a new taxon, Guttigomphus avilionis. Guttigomphus can be distinguished from other gomphodont cynodonts by features of the upper postcanine teeth, such as an asymmetric crown in occlusal view (crown narrower along the lingual margin than the labial). Our phylogenetic analysis recovers Guttigomphus as a basal member of Trirachodontidae, outside of the clade including Cricodon, Langbergia and Trirachodon. [ABSTRACT FROM AUTHOR]

Background Whilst cleaning agents are commonly used in workplaces and homes, health workers (HWs) are at increased risk of exposure to significantly higher concentrations used to prevent healthcare-associated infections. Exposure assessment has been challenging partly because many are used simultaneously resulting in complex airborne exposures with various chemicals requiring different sampling techniques. The main objective of this study was to characterize exposures of HWs to various cleaning agents in two tertiary academic hospitals in Southern Africa. Methods A cross-sectional study of HWs was conducted in two tertiary hospitals in South Africa (SAH) and Tanzania (TAH). Exposure assessment involved systematic workplace observations, interviews with key personnel, passive personal environmental sampling for aldehydes (ortho -phthalaldehyde—OPA, glutaraldehyde and formaldehyde), and biomonitoring for chlorhexidine. Results Overall, 269 samples were collected from SAH, with 62 (23%) collected from HWs that used OPA on the day of monitoring. OPA was detectable in 6 (2%) of all samples analysed, all of which were collected in the gastrointestinal unit of the SAH. Overall, department, job title, individual HW use of OPA and duration of OPA use were the important predictors of OPA exposure. Formaldehyde was detectable in 103 (38%) samples (GM = 0.0025 ppm; range: <0.0030 to 0.0270). Formaldehyde levels were below the ACGIH TLV-TWA (0.1 ppm). While individual HW use and duration of formaldehyde use were not associated with formaldehyde exposure, working in an ear, nose, and throat ward was positively associated with detectable exposures (P -value = 0.002). Glutaraldehyde was not detected in samples from the SAH. In the preliminary sampling conducted in the TAH, glutaraldehyde was detectable in 8 (73%) of the 11 samples collected (GM = 0.003 ppm; range: <0.002 to 0.028). Glutaraldehyde levels were lower than the ACGIH's TLV-Ceiling Limit of 0.05 ppm. p -chloroaniline was detectable in 13 (4%) of the 336 urine samples (GM = 0.02 ng/ml range: <1.00 to 25.80). Conclusion The study concluded that detectable exposures to OPA were isolated to certain departments and were dependent on the dedicated use of OPA by the HW being monitored. In contrast, low-level formaldehyde exposures were present throughout the hospital. Summary: The tribe Diosmeae (Rutaceae) encompasses circa 278 species classified in 11 genera: ten of them are shrubs endemic to the Cape Floristic Region (South Africa), while the only arborescent genus (two species) extends from South Africa to Tanzania. We examined the wood structure of 21 species representing nine genera of Diosmeae and analysed it with respect to their life form and climatic niches. Studied taxa share a suite of traits typical for the whole family: radial vessel multiples, simple perforation plates, minute alternate intervessel pits, distinctly bordered vessel-ray pits, non-septate libriform fibres, and — except for Empleurum — marginal axial parenchyma. However, unlike most Rutaceae, they also occasionally feature vessel-ray pits with reduced borders. Most of the studied species are very similar to each other in their wood structure and habit. The two exceptions are Calodendrum capense (the only canopy tree under study) and Empleurum unicapsulare (a large riparian shrub). The first differs from other species in having wider vessels, longer libriform fibres, prismatic crystals in ray cells and winged-aliform axial parenchyma (which may be plesiomorphic for the tribe); the latter is distinctive because it lacks growth rings and banded axial parenchyma. Analysing anatomy–climate relationship, we found that wood anatomy among shrubby Diosmeae relates to their size and environmental factors, rather than taxonomy. Particularly, the vessel grouping weakly increases with stricter water limitation along the gradient from a semi-arid winter-dry climate to a Mediterranean winter-wet climate. [ABSTRACT FROM AUTHOR]

As scholarship on episodic volunteering expands, researchers question if episodic volunteering is similar to, and/or different from, long-term, membership-based volunteering. This article examines the motivations of Ghanaians, South Africans, and Tanzanians to engage in event-based, episodic volunteering. Based on surveys collected from over 1000 participants in 2018, we use logistic regression models to distinguish differences in motivations between novice, occasional, and regular episodic volunteers. The results show that age and student status are influential in distinguishing novice volunteers from regular volunteers, but more importantly that novices are motivated for primarily social reasons, while regular volunteers are motivated by more altruistic reasons. Our study reinforces established knowledge that people are motivated to volunteer for many reasons that may overlap or occur simultaneously, and that these motivations differ by stage of life. Background: Health workers (HWs) are exposed to diverse cleaning agents in large hospitals. This study determined the prevalence of work‐related symptoms, allergic sensitization, and lung function abnormalities in HWs of two tertiary hospitals in Southern Africa. Methods: A cross‐sectional study of 699 HWs (South Africa: SAH, n = 346; Tanzania: TAH, n = 353) was conducted. Health outcomes were assessed using a standardized ECRHS questionnaire, immunological tests (specific IgE antibody to common aero‐allergens and to occupational allergens: natural rubber latex [NRL] Hev b5 and Hev b6.02, chlorhexidine, and ortho‐phthalaldehyde [OPA]), spirometry [pre‐and post‐ bronchodilator], methacholine challenge, and fractional exhaled nitric oxide (FeNO). Results: A large proportion of participants (78%) were women. Median age was 42 years, with 76% nurses, 12% cleaners, and 5% administrative workers. Current smoking was more common in SAHWs (12%) than TAHWs (1%). The overall prevalence of doctor‐diagnosed asthma was 7%. Atopy was present in 43% of HWs, while 4% were sensitized to OPA, 2% to NRL, and 1% to chlorhexidine. Prevalence of work‐related ocular‐nasal symptoms (16%) was higher than skin (12%) and chest (7%) symptoms. TAHWs had significantly lower mean lung volumes, higher degrees of significant airflow obstruction and impaired lung function. The prevalence of bronchial hyperresponsiveness in SAHWs (14%) was high. Overall, 23% of HWs had abnormal FeNO; 6% having high (>50 ppb) levels. FeNO was positively associated with sensitization to occupational allergens, primarily OPA and NRL. Conclusions: HWs from both hospitals had similar prevalence of work‐related respiratory symptoms. Sensitization to OPA and NRL appears to be contributing to allergic airway inflammation in these HWs. Purpose: Considerable debate since the global financial crisis has been evident concerning the role of business schools. This article aims to outline the authors' research on their role. Design/methodology/approach: The paper begins with an overview of the significant literature highlighting the current debates impacting on business schools and their leaders. From this the authors argue that the most significant challenge is the role business schools should play in the resolution of urgent global issues. The paper then details and analyses the research and provides recommendations. Findings: The main finding from the research was that the leaders of participating business schools did not convey the impression that steering business toward helping humanity achieve a sustainable future and tackling urgent global issues was seen by them as one of their main current or future roles. Research limitations/implications: Funding constraints and the desire to make a timely contribution to the debate mean that this research is limited in scale. It is therefore viewed as a scoping study, paving the way for more detailed investigation and analysis, and recommendations for further research are provided. Originality/value: The authors argue that, rather than make the same mistakes as the West, BRIC economies and related business school leaders should be looking for an alternative strategy that would lead to a just, sustainable and fulfilling human presence on the planet. The authors also suggest that Type 1 European business schools are well advised to focus their limited resources locally but should be aware of global issues and bear them in mind in their actions. The paper sets out to show that teaching is among the five undeniably oldest and historically character-shaping professions in the world, the others being engineering-architecture, medicine, law, and accounting and each of these professions has a unique story in connection with its genesis and its influence on social organisation. The paper indicates, however, that while the other four have had a comparatively better advantage in terms of occupational status, social esteem, popular veneration or respect, teaching has not enjoyed a similar experience and treatment. An analysis is undertaken into the historical and contemporary factors of similarity and contrast among the professions in the varying levels of self-image building, status and public adulation that have made the teaching profession "an unequal among equals". Recommendations and propositions are then offered towards correcting the situation and making the teaching profession attractive to the younger generation of men and women of tomorrow and the future, particularly within the eastern and southern Africa region.

Modelling the likely financial resource requirements and potential sources of revenue for health system reform options is of great potential value to policy-makers. Models provide an indication of the financial feasibility and sustainability of such reforms and highlight the implications of alternative reform paths. There has been increasing use of financial models of health sector reform in recent years, particularly since the development of user-friendly software such as SimIns, which was developed by the World Health Organization (WHO) and Deutsche Gesellschaft für Technische Zusammenarbeit (GTZ). This paper outlines the process of developing country-specific spreadsheet-based models to explore the financial resource requirements of health system reform options in South Africa and Tanzania. Building one's own model, although time consuming, allows for greater flexibility and forces the analysts to give careful consideration to the assumptions underlying the model. The core variables in our models are: population, health service utilization rates and unit costs. The paper outlines the types of disaggregation of these variables, the range of possible data sources, key challenges with securing accurate data for each variable, and relevant evidence on which to base key assumptions, and how we went about addressing these challenges. We also briefly review how to model the revenue-generating potential of alternative sources of health care financing. The intention of the paper is to provide guidance for analysts who wish to develop their own models, and to illustrate, with reference to the South African and Tanzanian modelling experience, how one has to adapt to data constraints and context-specific modelling requirements. Stakeholder analysis is widely recommended as a tool for gathering insights on policy actor interests in, positions on, and power to influence, health policy issues. Such information is recognized to be critical in developing viable health policy proposals, and is particularly important for new health care financing proposals that aim to secure universal coverage (UC).However, there remain surprisingly few published accounts of the use of stakeholder analysis in health policy development generally, and health financing specifically, and even fewer that draw lessons from experience about how to do and how to use such analysis. This paper, therefore, aims to support those developing or researching UC reforms to think both about how to conduct stakeholder analysis, and how to use it to support evidence-informed pro-poor health policy development. It presents practical lessons and ideas drawn from experience of doing stakeholder analysis around UC reforms in South Africa and Tanzania, combined with insights from other relevant material. The paper has two parts. The first presents lessons of experience for conducting a stakeholder analysis, and the second, ideas about how to use the analysis to support policy design and the development of actor and broader political management strategies.Comparison of experience across South Africa and Tanzania shows that there are some commonalities concerning which stakeholders have general interests in UC reform. However, differences in context and in reform proposals generate differences in the particular interests of stakeholders and their likely positioning on reform proposals, as well as in their relative balance of power. It is, therefore, difficult to draw cross-national policy comparisons around these specific issues. Nonetheless, the paper shows that cross-national policy learning is possible around the approach to analysis, the factors influencing judgements and the implications for, and possible approaches to, management of policy processes. Such learning does not entail generalization about which UC reform package offers most gain in any setting, but rather about how to manage the reform process within a particular context. A desire to enhance protection against health care costs and improve equity of access to health care lies at the core of many health sector financing initiatives. Until recently, international debates about financing and health equity have focused primarily on mechanisms to promote equity in relation to very specific elements of health systems. However, in recent years there has been growing interest in considering these equity challenges from a more systemic perspective. In this context, universal health coverage is becoming a rallying call, with a focus on how best universal coverage can be financed. This paper is the first in a special issue which presents a body of research whose overall aim was to critically evaluate existing inequities in health care financing and provision in Ghana, South Africa and Tanzania, and the extent to which health insurance mechanisms (broadly defined) could address financial protection and equity of access challenges. In this first paper we introduce the countries’ health systems, with a special emphasis on existing mechanisms for financial protection. Abstract: Coastal social ecological systems in eastern Africa are subject to a range of environmental, social and economic changes. They are already vulnerable to these multiple stressors, and the impacts of climate change are likely to further exacerbate their vulnerabilities. Some of these impacts may be observed and experienced already. The analysis presented in this paper is based on mixed methods empirical research exploring local perceptions of recent changes at four sites in coastal Tanzania and Mozambique. People recognise and rank a number of climate and non-climate stressors which have contributed towards more risky and less diverse livelihoods. Importantly, regional and international policy initiatives – in the form of river basin management in Mozambique and South Africa, and development of a Marine Protected Area in Tanzania – are perceived to further erode resilience and exacerbate vulnerabilities. We suggest this is a form of policy misfit, where policies developed to address a specific issue do not take account of cross-scale dynamics of change, the interactions between multiple stressors, nor longer term climate change. This policy misfit may be remedied by a move towards adaptive forms of governance, and necessitates an explicit focus on building the adaptive capacity of the poor and most vulnerable in society. This paper presents cytotaxonomic details of five populations of the Simulium damnosum complex from South Africa, Swaziland and Ethiopia. The 'Nkusi SW' and 'Pienaars' forms are newly designated members of the complex from South Africa, but the taxonomic rank of an isolate indistinguishable chromosomally from the 'Nkusi' cytoform remains unclear. From Ethiopia two cytoforms were identified, one of which shares two diagnostic chromosome inversions with the cytoform 'Kisiwani' from Tanzania. The second form belongs to S. kaffaense, and is the suspected local vector of Onchocerca volvulus. In addition, a re-analysis of the cytoform 'Kibwezi' from north–eastern Tanzania provided further insights into its population subdivision, and its genetic and morphological characteristics. Cytotaxonomic similarities between 'Kibwezi', S. mengense and S. pandanophilum, along with their biogeography, indicate a relict status of each of these taxa. [ABSTRACT FROM AUTHOR]

This paper examines three interrelated factors outside of formal provision of technical and vocational education and training (TVET) in sub-Saharan Africa that have undermined TVET systems. The first is the process, pace, and levels of industrialisation, which has had a direct effect on TVET provision: low numbers of well-paying jobs requiring technical expertise. This has an indirect effect, which is the second crucial factor: lack of economic development and change in labour markets. There are very few jobs that would be considered 'middle class' or 'mid-level' in wealthy countries. Most people are in survivalist work. An international consensus since the 2000s on palliative approaches to development which address the effects but not the causes of the lack of economic development in Africa has resulted in mass poor quality provision of education--the third factor. Education systems are rapidly expanding and achievement levels rising, in the context of very little possibility of labour market rewards for most people, and substantial labour market rewards confined mainly to graduates. This reinforces deeply embedded cultural preferences for general education, which originate in the type of education systems established by colonial powers, as well as the relationships between educational credentials and elite jobs.

Sub-Saharan Africa secondary school science curricula recognise language as the most important communication tool for learners' biology learning outcomes. Despite this prominent goal, learners' scientific literacy at secondary school appears to be insufficient to cope with classroom teaching and learning of biology. This paper presents a review of literature regarding the way English as the language of instruction and inquiry-based learning strategies affect biology learning in Sub-Saharan Africa secondary schools. The scope of the review included journal articles, books and book chapters published in the period from 2005 to 2020 on the English language of instruction in biology education in Sub-Saharan Africa. We established criteria for first article searches indexed in Scopus, African Journals Online, the Education Resources Information Center, Web of Science and Psych Info as recognised index systems for science education. We considered studies related to the understanding of learners' language backgrounds, language-in-education policy and pedagogical language strategies (practice) that mediate learning. In this case, 53 studies were identified after removing redundancies; 27 studies satisfied the inclusion criteria when screened by key words and abstract and reviewed for eligibility; and only nine were selected after evaluating the full texts. The review analysis suggests that there is a gap in the corpus of literature on what and how pedagogical language strategies should be utilised to support meaningful learning of biology in multilingual settings. Poor learners' academic achievement and conceptual engagement in biology at secondary school level were the main effects reported by the reviewed literature. Less is still known about the effect of the use of inquiry-based learning strategies on learners' conceptual engagement in multilingual biology classrooms. We recommend formulating a clear policy articulation on the use of bilingual instructions in teaching and learning of biology at the secondary school level; assessing teacher and learner performance over pedagogical language strategies; and evaluating the contribution of IBL strategies in addressing language barriers in biology learning for higher order thinking skills in Sub-Saharan Africa secondary schools.

This paper discusses the Solomon Mahlangu Freedom College, SOMAFCO, the African National Congress liberation school in Tanzania between 1978 and 1992. The authors reflect on the process of writing about recent and possibly controversial episodes in the history of South, and Southern, Africa. They also outline a number of areas that they argue reveal points of tension within the ANC's educational enterprise. Hinging on the question of what a 'political' or 'revolutionary' school might be, they probe areas such as gender, 'education with production', ethnicity and nationality in the exile context, and the curriculum. They conclude that while it is difficult to draw direct lessons for contemporary South Africa from the educational experience at SOMAFCO, there are many intriguing resonances that should be explored. This paper summarises experiences of efforts made by twenty individuals when implementing small-scale ICT development projects in their organizations located in seven developing countries. The main focus of these projects was the use of ICT in educational settings. Challenges encountered and the contributing factors for implementation success of the projects are systematically investigated using interviews and follow up surveys. Results show that the typical limitations of technology and infrastructure were the key obstacles. The commitment of individual project managers in the role of "change agents" and organizational support in the were the strengths behind the success of the projects. Based on the outcome of this study, professional development of the change agents is a key factor for the success of projects. IT and infrastructure limitations contributed to the failure of the majority of the ICT related projects.
