Background The acceptance of a paper in a top-ranked journal depends on the importance of the study, and should not depend on its country of origin. If the papers' citation rate is a proxy for their importance, and the threshold for acceptance is unrelated to the country of origin, papers from different countries published in the same journal should have a similar number of citations. Conversely, if the threshold is lowered for some countries, their papers will have a lower mean citation rate. Methods The number of citations and the corresponding author's country were obtained for 4724 papers published between 1998 and 2002 in the , the , and . Countries were grouped according to the World Bank classification and geographical location: low–middle income countries; high-income European countries; high-income non-European countries; UK and USA. The probability of papers being poorly cited by country of origin was estimated, using domestic papers (British papers published in British journals and US papers published in US journals) as the reference. Results Compared with domestic papers, the OR of being poorly cited was 0.67 (95% CI 0.55 to 0.81) for papers from high-income European countries, 0.97 (0.76 to 1.24) for papers from high-income non-European countries and 1.93 (1.28 to 2.89) for papers from low–middle income countries. Conclusions Papers from different countries published in the same journal have different citation rates. Background Diaries are used in sexual behaviour research to reduce recall bias. Diary collection via mobile phone text messaging (SMS) has not been trialled previously in sexual behaviour research. This randomized controlled trial compared SMS, paper and online diaries on response rate, timeliness, completeness of data and acceptability. The correlation between behaviour reported in all three types of diaries and data collected in a retrospective questionnaire was also determined. Methods Participants were recruited by telephone and randomised into one of three groups. They completed weekly sexual behaviour diaries for 3 months by SMS, online or paper (by post). An online survey was conducted at the end of 3 months to compare retrospective reports to the diaries and assess opinions on the diary collection method. Results 72 participants were enrolled in the study, 24 in each group. Online diaries were more likely to be submitted late than SMS diaries (p<0.001). 3.9% of SMS diaries, 3.1% of paper diaries and 0.5% of online diaries were incomplete (p=0.001). Online data collection was the preferred mode for 51%. 65 participants completed the end point retrospective questionnaire. The correlation between the diary and questionnaire on sexual risk classification was substantial (κ=0.74) regardless of diary mode. Conclusions SMS is a convenient and timely method of collecting brief behavioural data, but online data collection was preferable to most participants and more likely to be complete. Atopic eczema is a chronic skin condition affecting between 5% and 20% of children aged up to 11 years at one time or other. Research suggests that prevalence is increasing and various environmental factors have been implicated in the aetiology. While often seen to be a minor problem, research suggests that it can cause considerable disruption to the lives of children and their carers and involves significant cost for the family and health care systems. The current consensus is that the majority of cases of atopic eczema are most appropriately managed within primary care. However, management of the condition is problematic because diagnosis is often difficult. Consultations tend to focus on the physical aspects of the problems neglecting the psychosocial, while treatment remains mainly palliative and can be as diverse as the condition itself. More appropriate and effective primary care management and support for children with atopic eczema and their carers will only be developed if health professionals become more aware of the social context of the disease and the impact on the lives of those affected. Public health responses focusing on primary prevention are also needed. This paper reviews a diverse literature on the epidemiology of the condition, the way in which it affects the lives of children and their carers and the factors that shape their help seeking decisions. Objective: The effects of binge-drinking during pregnancy on the fetus and child have been an increasing concern for clinicians and policy-makers. This study reviews the available evidence from human observational studies. Design: Systematic review of observational studies. Population: Pregnant women or women who are trying to become pregnant. Methods: A computerised search strategy was run in Medline, Embase, Cinahl and PsychInfo for the years 1970-2005. Titles and abstracts were read by two researchers for eligibility. Eligible papers were then obtained and read in full by two researchers to decide on inclusion. The papers were assessed for quality using the Newcastle-Ottawa Quality Assessment Scales and data were extracted. Main outcome measures: Adverse outcomes considered in this study included miscarriage; stillbirth; intrauterine growth restriction; prematurity; birth-weight; small for gestational age at birth; and birth defects, including fetal alcohol syndrome and neurodevelopmental effects. Results: The search resulted in 3630 titles and abstracts, which were narrowed down to 14 relevant papers. There were no consistently significant effects of alcohol on any of the outcomes considered. There was a possible effect on neurodevelopment. Many of the reported studies had methodological weaknesses despite being assessed as having reasonable quality. Conclusions: This systematic review found no convincing evidence of adverse effects of prenatal binge-drinking, except possibly on neurodevelopmental outcomes. [ABSTRACT FROM AUTHOR]

Background: Peer reviewers are usually unpaid and their efforts not formally acknowledged. Some journals have difficulty finding appropriate reviewers able to complete timely reviews, resulting in publication delay. Objectives and methods: A survey of peer reviewers from five biomedical journals was conducted to determine why reviewers decline to review and their opinions on reviewer incentives. Items were scored on 5-point Likert scales, with low scores indicating low importance or low agreement. Results: 551/890 (62%) reviewers responded. Factors rated most highly in importance for the decision to accept to review a paper included contribution of the paper to subject area (mean 3.67 (standard deviation (SD) 86)), relevance of topic to own work (mean 3.46 (SD 0.99)) and opportunity to learn something new (mean 3.41 (SD 0.96)). The most highly rated factor important in the decision to decline to review was conflict with other workload (mean 4.06 (SD 1 .31)). Most respondents agreed that financial incentives would not be effective when time constraints are prohibitive (mean 3.59 (SD 1 .01)). However, reviewers agreed that non-financial incentives might encourage reviewers to accept requests to review: free subscription to journal content (mean 3.72 (SD 1.04)), annual acknowledgement on the journal's website (mean 3.64 (SD 0.90)), more feedback about the outcome of the submission (mean 3.62 (SD 0.88)) and quality of the review (mean 3.60 (SD 0.89), and appointment of reviewers to the journal's editorial board (mean 3.57 (SD 0.99)). Conclusion: Reviewers are more likely to accept to review a manuscript when it is relevant to their area of interest. Lack of time is the principal factor in the decision to decline. Background: The objective of this study was to identify current patterns of international collaboration based on authorship of epidemiological articles. Methods: All articles published in nine high-impact public health journals in 2006 were read, and information about the study country and the authors' countries of affiliation was extracted. All countries were assigned to an income level using World Bank classifications. This paper presents results for the 1686 articles that focused on a single study country. Results: International collaboration is common, but the dominant partnerships vary by the income level of the study country. 74.2% of articles reporting on research conducted in low- and middle-income study countries involved co-authors from two or more countries, and nearly all of these international collaborations included co-authors from both low/middle-income and high-income countries. Only 13.0% of studies based in high-income countries involved co-authors from two or more countries, and the majority of these studies involved co-authors with affiliations solely from high-income countries. More than 90% of articles from both low/middle- and high-income study countries included at least one co-author from the study country. Conclusion: There is a high rate of local co-authorship in both low/middle- and high-income study countries. Most articles that focus on high-income countries include only authors from high-income countries. Most articles that focus on low- and middle-income study countries are "north-to-south" international collaborations that include co-authors from both low/middle- and high-income countries. A conflict of interest arises by having two conflicting goals in one's research. The primary goal of research relevant to public health is to produce impartial evidence on health hazards for humans. Several entities -including industry - may have public health as a goal among others, but this is not their primary goal. Primary goals are in those cases profit or career, that conflict with the goal of health. It is a role of the State to foster research whose primary goal is impartial evidence on factors affecting population health. Disclosure of conflicts of interest is not enough: the view that disclosure solves all problems amounts to say that a declaration of having produced unbiased evidence is a self-fulfilling guarantee that the evidence will not be affected by conflicts of interest. This concept is seriously misleading. The aim of this paper is to synthesise findings on the salutogenic concept, sense of coherence (SOC), and its correlation with quality of life (QoL). This study is descriptive and analytic, with a systematic integration of the contemporary knowledge base on the salutogenic research published in 1992-2003. This review includes 458 scientific publications and 13 doctoral theses on salutogenesis. In all, 32 papers had the main objective of investigating the relationship between SOC and QoL. This study is based on scientific publications in eight authorised databases, doctoral theses and available books. The SOC seems to have an impact on the QoL; the stronger the SOC, the better the QoL. Furthermore, longitudinal studies confirm the predictive validity of the SOC for a good QoL. The findings correspond to the core of the Ottawa Charter-that is, the process of enabling people to live a good life. Therefore, a certain possibility to modify and extend the health construct is becoming discernible, implicating a construct including salutogenesis and QoL. Community participation and leadership is a central tenet of public health policy and practice. Community engagement approaches are used in a variety of ways to facilitate participation, ranging from the more utilitarian, involving lay delivery of established health programmes, to more empowerment-oriented approaches. Evaluation methods within public health, adapted from clinical medicine, are most suited to evaluating community engagement as an ‘intervention’, in the utilitarian sense, focusing on the health impacts of professionally determined programmes. However, as communities are empowered and professional control is relinquished, it is likely to be harder to capture the full effects of an intervention and so the current evidence base is skewed away from knowledge about the utility of these approaches. The aim of this paper is to stimulate debate on the evaluation of community engagement. Building on current understandings of evaluation within complex systems, the paper argues that what is needed is a paradigm shift from viewing the involvement of communities as an errant form of public health action, to seeing communities as an essential part of the public health system. This means moving from evaluation being exclusively focused on the linear causal chain between the intervention and the target population, to seeking to build understanding of whether and how the lay contribution has impacted on the social determinants of health, including the system through which the intervention is delivered. Background Recent theorisation has pushed stigma research in new directions, arguing for a need to challenge the unequal power relations that impact groups most at risk for HIV-related stigma rather than locate stigma in the individual. Such a conceptualisation resonates with the growing emphasis on structural interventions for HIV prevention that attempt to alter the social context of risk. Methods The paper predominantly relies on longitudinal interviews conducted three times over a 2-year period with sex workers with varying degrees of involvement with the non-governmental organisation (NGO) and community-based organisation. Results Recognising that stigma is socially constructed and structurally reproduced, the NGO helped mobilise marginalised and hitherto scattered female sex workers to form community-based organisations to challenge their disadvantaged status in society. The authors show how stigma alleviation strategies presented a contradiction: emboldening one group of female sex workers to self-identify as sex workers while making others reluctant to access the intervention-run clinic. Conclusion The paper builds on a growing body of research that acknowledges the struggles in implementing structural interventions, particularly for NGOs working in regions with a diverse population of sex workers with varying needs. The authors argue that intervention goals of reducing stigma and increasing the use of sexually transmitted infection services do not have to conflict and, in fact, must go hand-in-hand for an implementation to be considered a structural intervention. This supplement presents some of the methodological issues that arose during the early phases of protocol development for the National Children's Study (NCS), a probability sample of 100 000 children that will be followed prospectively from pregnancy through 21 years of age, and to share some of the challenges and solutions that were discussed. These papers on motor, social! emotional, psychiatric and neurocognitive/behavioural development do not define the protocol of the NCS, but reflect methodology related to the design of research and assessment of developmental trajectories in children that may be useful to other epidemiologists planning similar longitudinal studies. Background: Less than half of studies presented at conferences remain unpublished two years later, and these studies differ systematically from those that are published. In particular, the unpublished studies are less likely to report statistically significant findings, and this introduces publication bias. This has been well documented for quantitative studies, but has never been explored in relation to qualitative research. Methods: We reviewed the abstracts of qualitative research presented at the 1998 (n = 110) and 1999 (n = 114) British Sociological Association (BSA) Medical Sociology meetings, and attempted to locate those studies in databases or by contacting authors. We also appraised the quality of reporting in each abstract. Results: We found an overall publication rate for these qualitative studies of 44.2%. This is nearly identical to the publication rate for quantitative research. The quality of reporting of study methods and findings in the abstract was positively related to the likelihood of publication. Conclusion: Qualitative research is as likely to remain unpublished as quantitative research. Moreover, non-publication appears to be related to the quality of reporting of methodological information in the original abstract, perhaps because this is a proxy for a study with clear objectives and clear findings. This suggests a mechanism by which "qualitative publication bias" might work: qualitative studies that do not show clear, or striking, or easily described findings may simply disappear from view. Background Amid the calls for 'more public health evidencé, we also need simple understandable methods of determining when more research really is needed. This paper describes a simple decision aid to help policymakers, researchers and other decision makers assess the potential 'information value' of a new public health randomised controlled trial. Methods The authors developed a flow chart to help make explicit (1) the user's information needs, (2) the intended use of the new information that the study will produce, (3) the added value of the evidence to be derived from the new study and (4) the levels of precision, bias and generalisability required by the user Results The flow chart is briefly illustrated, first in generic form and then in a worked example, showing how it may be used in deciding whether a new study should be commissioned to evaluate the health impact of allowing motorcycles to use bus lanes in London Conclusions In this paper, the authors have presented a flow chart for enacting an informal 'Value-of- Information'-like approach to deciding when a new public health evaluation is needed. The authors do not suggest that the flow chart approach is technically the equivalent of Value-of-Information methods. Nonetheless, it represents a valuable perspective and process to adopt, and this structured approach will be more revealing than an unstructured thought experiment as the basis for decisions about a new study. Background Systematic reviews of the effectiveness of interventions are increasingly used to informrecommendations for public health policy and practice, but outcome reporting bias is rarely assessed.Methods Studies excluded at full-text stage screening for a systematic review of a public health intervention were assessed for evidence of study exclusion resulting from non-reporting of relevant outcomes. Studies included in the review were assessed for evidence of outcome reporting bias and the impact that this had on the evidence synthesised using a formal tool (Outcome Reporting Bias in Trials (ORBIT)). Results None of the reports excluded at full-text stage were excluded because of non-reporting of relevant outcomes. Of the 26 included papers, six were identified as having evidence of missing or incompletely reported outcomes, with 64% of unreported or incompletely reported outcomes identified as to leading to a high risk of bias according to the ORBIT tool. Where there was evidence of the effectiveness of interventions before an assessment of outcome reporting bias was undertaken, identifying possible instances of outcome reporting bias generally led to a reduction in the strength of evidence for the effectiveness of the interventions. Conclusion The findings from this single evaluation provide empirical data to support the call for a prospective public health interventions study registry to aid the identification of unreported or incompletely reported outcomes. Critical appraisal tools can also be used to identify incompletely reported outcomes, but a tool such as ORBIT requires development to be suitable for public health intervention evaluations. Background The adverse health and environmental effects of poor housing quality are well established. A central requirement for evidence-based policies and programmes to improve housing standards is a valid, reliable and practical way of measuring housing quality that is supported by policy agencies, the housing sector, researchers and the public. Methods This paper provides guidance on the development of housing quality-assessment tools that link practical measures of housing conditions to their effects on health, safety and sustainability, with particular reference to tools developed in New Zealand and England. Results The authors describe how information on housing quality can support individuals, agencies and the private sector to make worthwhile improvements to the health, safety and sustainability of housing. The information gathered and the resultant tools developed should be guided by the multiple purposes and end users of this information. Other important issues outlined include deciding on the scope, detailed content, practical administration issues and how the information will be analysed and summarised for its intended end users. There are likely to be considerable benefits from increased international collaboration and standardization of approaches to measuring housing hazards. At the same time, these assessment approaches need to consider local factors such as climate, geography, culture, predominating building practices, important housing-related health issues and existing building codes. Conclusions An effective housing quality-assessment tool has a central role in supporting improvements to housing. Study objective: To examine the Canadian origins of the Lalonde Report and its impact on British and American health promotion activities. Design: A brief history of the development of key Canadian documents and their use by politicians and public health activists in the United Kingdom and United States. Setting: This paper focuses on the impact of the Canadian model on Canada, the United Kingdom and United States. Main results: This paper argues that internal political and economic forces are as important as international trends in determining healthcare policy initiatives. Conclusions: In the 1970s all the English-speaking developed nations were facing deficits as curative costs rose. Adopting health promotion policies permitted them to shift responsibility back to local governments and individuals while limiting their expenditures. Health and community activists, however, used this concept to broaden their focus to include the social, economic and political determinants of health and thus reinvented public health discourse and practice for the 21st century. The ‘Framework for the Delivery of Public Health’ presented in this paper provides a new structure that brings together the multiple components of public health, relates them to key influencing factors and provides greater clarity, balance and direction for the effective delivery of public health. The paper outlines the development of the framework and describes the significance behind the key constructs. The symbolisms within the picture and interactions between the varied components are compared with the dynamics of an ecosystem; this aims to emphasise that improving health occurs within a constantly changing and interactive environment. Finally, the applications of the framework are explored, including embedding public health within an organisation or setting, assisting in strategy development for specific topics and as a training and educational aid. Main results: Soc is strongly related to perceived health, especially mental health. The stronger the soc the better the perceived health in general, at least for those with an initial high SOC. This relation is manifested in study populations regardless of age, sex, ethnicity, nationality, and study design. SOC seems to have a main, moderating or mediating role in the explanation of health. Furthermore, the soc seems to be able to predict health. SOC is an important contributor for the development and maintenance of people's health but does not alone explain the overall health. Conclusion: SOC seems to be a health promoting resource, which strengthens resilience and develops a positive subjective state of health. Broad terms such as Black, African, or Black African are entrenched in scientific writings although there is considerable diversity within African descent populations and such terms may be both offensive and inaccurate. This paper outlines the heterogeneity within African populations, and discusses the strengths and limitations of the term Black and related labels from epidemiological and public health perspectives in Europe and the USA. This paper calls for debate on appropriate terminologies for African descent populations and concludes with the proposals that (1) describing the population under consideration is of paramount importance (2) the word African origin or simply African is an appropriate and necessary prefix for an ethnic label, for example, African Caribbean or African Kenyan or African Surinamese (3) documents should define the ethnic labels (4) the label Black should be phased out except when used in political contexts. Study objective: The concept of social capital has influenced mental health policies of nations and international organisations despite its limited evidence base. This papers aims to systematically review quantitative studies examining the association between social capital and mental illness. Design and setting: Twenty electronic databases and the reference sections of papers were searched to identify published studies. Authors of papers were contacted for unpublished work. Anonymised papers were reviewed by the authors of this paper. Papers with a validated mental illness outcome and an exposure variable agreed as measuring social capital were included. No limitations were put on date or language of publication. Main results: Twenty one studies met the inclusion criteria for the review. Fourteen measured social capital at the individual level and seven at an ecological level. The former offered evidence for an inverse relation between cognitive social capital and common mental disorders. There was moderate evidence for an inverse relation between cognitive social capital and child mental illness, and combined measures of social capital and common mental disorders. The seven ecological studies were diverse in methodology, populations investigated, and mental illness outcomes, making them difficult to summarise. Conclusions: Individual and ecological social capital may measure different aspects of the social environment. Current evidence is inadequate to inform the development of specific social capital interventions to combat mental illness. [ABSTRACT FROM AUTHOR]

Study objective: The aim of this paper is to systematically review and analyse the validity and reliability of Antonovsky's life orientation questionnaire/sense of coherence scale (SOC). Design: The study is descriptive and analytical with a systematic integration of the contemporary knowledge base on the salutogenic research published 1992-2003. The review includes 458 scientific publications and 13 doctoral theses. Setting: Worldwide, based on postgraduate scientific publications in eight authorised databases, doctoral theses, and available books. Main results: The Soc questionnaire has been used in at least 33 languages in 32 countries with at least 15 different versions of the questionnaire. In 124 studies using SOC-29 the Cronbach's ez ranges from 0.70 to 0.95. The cx values in 127 studies using SOC-13 range from 0.70 to 0.92, and in 60 studies using a modified soc scale range from 0.35 to 0.91. Test-retest correlation show stability and range from 0.69 to 0.78 (1 year), 0.64 (3 years), 0.42 to 0.45 (4 years), 0.59 to 0.67 (5 years) to 0.54 (10 years). The means of SOC-29 range 100.50 (SD 28.50) to 164.50 (SD 17.10) points and SOC-13 from 35.39 (SD 0.10) to 77.60 (SD 13.80) points. After 10 years SOC seems to be comparatively stable, but not as stable as Antonovsky initially assumed. soc tends to increase with age. The factorial structure of Soc seems rather to be multidimensional than unidimensional. soc predicts a positive outcome in a long term perspective, although there are divergent findings reported. The soc scale seems to be a reliable, valid, and cross culturally applicable instrument measuring how people manage stressful situations and stay well. [ABSTRACT FROM AUTHOR]

STUDY OBJECTIVE: It is increasingly recognised that different dimensions of social inequality may be linked to health by different pathways. Furthermore, factors operating at the individual level such as employment conditions may affect health in a different way from household level factors. The paper examines the associations between self rated health and four measures of social position— occupational class, household social advantage, personal and household income. DESIGN: Multilevel logistic regression models were used to predict self rated health using longitudinal data from the British Household panel survey (BHPS) with respondents nested within households. Separate analyses were carried out for economically active and inactive respondents. SETTING: Interview based surveys of adults living within households that are representative of British households. PARTICIPANTS: Adult respondents from the BHPS. Main results: Occupational class has relatively strong effects on the self rated health of the economically active, although household level factors also seem to influence their health. Household social advantage has relatively strong effects on the self rated health of the economically inactive. CONCLUSIONS: The paper found evidence in support of the view that different dimensions of social inequality have different pathways to self rated health. In two 1959 papers, one coauthored, Jerome Cornfield asserts that ‘relative’ measures are more useful for causal inference while ‘absolute’ measures are more useful for public health purposes. In one of these papers (the single-authored one), he asks how epidemiology should respond to the fact that its domain is not a highly ‘articulated’ one—it is not susceptible to being subsumed under general laws. What is the connection between these issues? There has recently been some backlash against ‘risk relativism’, and Charles Poole has recently dismantled the mathematical argument for the first claim. However the problem with ‘Cornfield’s Principle’ seems to go much deeper. The whole attempt to partition measures into absolute and relative is fundamentally mistaken. Why, then, has it seemed so appealing? Perhaps one reason is the influence that early education in the physical sciences continues to exert on the way epidemiologists think, and their response to the low articulation of their domain of study. Background Previous research showed that younger adult males in the USA have, since the 1950s, died at a faster rate than females of the same age. In this paper, we quantify this difference, and explore possible explanations for the differences at different ages and in different years. Methods Using data from the Human Mortality Database (HMD), the number of additional male deaths per 10 000 female deaths was calculated for each year from 1933 to 2010, and for each year of age from 0 to 60 years, for the USA, and a number of other countries for comparison. The data were explored visually using shaded contour plots. Results Gender differences in excess mortality have increased. Coming of age (between the ages of 15 and 25 years of age) is especially perilous for men relative to women now compared with the past in the USA; the visualisations highlight this change as important. Conclusions Sex differences in mortality risks at various ages are not static. While women may today have an advantage when it comes to life expectancy, in the USA, this has greatly increased since the 1930s. Just as young adulthood for women has been made safer through safer antenatal and childbirth practices, changes in public policy can make the social environment safer for men. [ABSTRACT FROM AUTHOR]

Background Replication is a vital part of the research process and has recently received considerable attention. Analyses using publicly available data should, if adequately described, be reproducible without assistance from the original investigators. Using data from the US National Health and Nutrition Examination Survey (NHANES), a recent study reported a statistically significant difference in cardiovascular disease risk comparing subgroups of sexual minority men. We attempted to reproduce these findings and assessed whether the results were robust to alternative analytic strategies and assumptions. Methods We used the exclusion criteria and coding strategy described in the original paper to construct our analytical data set. Sampling weights were constructed in accordance with NHANES analytical guidelines. We estimated crude and covariate-adjusted associations between sexual orientation and vascular age using the regression models specified in the original report. We also conducted a series of sensitivity analyses to improve on the original findings. Results Our replication attempt was partially successful: we replicated the general trends reported in the original analysis, but not identical effect estimates. Importantly, we identified a potential misapplication of the Framingham Risk Score; correcting for this increased the probability that the reported null hypothesis test was a type I error. Conclusions This paper supports the recent calls for greater transparency and improved reporting in research. Even with a publicly available and well-documented data source, we were unable to exactly replicate another study's original findings. Our sensitivity analyses revealed key issues in the original analysis and demonstrate the scientific importance of research replication. [ABSTRACT FROM AUTHOR]
