Abstract In December 2014, the Federal Aviation Administration (FAA) completed a major revision of the rules and regulations governing flight and duty time in commercial aviation (Federal Aviation Regulation (FAR) Part 117). Scientists were included in the revision process and provided insights into sleep, sleep loss, the circadian rhythm, and their effects on performance that were incorporated into the new rule. If a planned flight was non-compliant with the regulation, for example if it exceeded flight and duty time limits, it could only be flown under an FAA-approved Fatigue Risk Management System (FRMS) as meeting an Alternative Method of Compliance (AMOC). One method that a flight could qualify as an AMOC is if it could be demonstrated empirically that it was as safe as or safer than a similar flight, designated the Safety Standard Operation (SSO), that was compliant with the regulation. In the present paper, we demonstrate the FRMS process using a comparison between a non-compliant AMOC flight from the US west coast to Australia and a compliant SSO flight from the US west coast to Taiwan. The AMOC was non-compliant because it exceeded the flight time limits in the prescriptive rule. Once a data collection exemption was granted by the FAA, both the outbound and inbound AMOC and SSO routes were studied on four Safety Performance Indicators (SPIs). The SPIs studied were inflight sleep, cognitive performance, self-reported fatigue, and self-reported sleepiness. These measures were made at top of descent (TOD), a critical phase of flight. The study was designed as a paired comparison. Forty volunteer pilots studied flew both the AMOC and the SSO flights for a total of 80 studied flights. Using statistical non-inferiority applied to the AMOC and SSO SPIs, we demonstrated, as required by the new rule, that the US-Australia AMOC flight was "as safe as, or safer than" the US-Taiwan SSO flight. In the context of FRMS, statistical non-inferiority is a concept and technique of great utility, straightforward in application, producing clear visual representations of the findings, and providing a direct answer to the question posed by the regulation - is the AMOC flight "as safe as, or safer than" the SSO. A significant proportion of worker fatalities within Australia result from truck-related incidents. Truck drivers face a number of health and safety concerns. Safety culture, viewed here as the beliefs, attitudes and values shared by an organisation’s workers, which interact with their surrounding context to influence behaviour, may provide a valuable lens for exploring safety-related behaviours in heavy vehicle operations. To date no major research has examined safety culture within heavy vehicle industries. As safety culture provides a means to interpret experiences and generate behaviour, safety culture research should be conducted with an awareness of the context surrounding safety. The current research sought to examine previous health and safety research regarding heavy vehicle operations to profile contextual factors which influence health and safety. A review of 104 peer-reviewed papers was conducted. Findings of these papers were then thematically analysed. A number of behaviours and scenarios linked with crashes and non-crash injuries were identified, along with a selection of health outcomes. Contextual factors which were found to influence these outcomes were explored. These factors were found to originate from government departments, transport organisations, customers and the road and work environment. Despite similarities to the US in terms of transportation, land use, and culture, Australia kills 5.3 people per 100,000 population on the roads each year, as compared to the US rate of 12.4. Similar trends hold when accounting for distance driven and the number of registered cars. This paper seeks to understand what is behind the road safety disparities between these two countries. The results suggest that a number of inter-related factors seem to play a role in the better road safety outcomes of Australia as compared to the US. This includes Australia’s strategies related to seat belt usage and impaired driving as well as their efforts to help curb vehicle speeds and reduce exposure. Design-related differences include a much greater reliance on roundabouts and narrower street cross-sections as well as guidelines that encourage self-enforcing roads. Policy-related differences include stronger and more extensive enforcement programs, restrictive licensing programs, and higher driving costs. Combined with a more urban population and multimodal infrastructure, Australia tends to discourage driving mileage and exposure while encouraging safer modes of transportation such as transit, at least more so than in most of the US. Australia also enacted their version of Vision Zero – called the Safe System Approach – more than a decade before similar policies began cropping up in US cities. While it is difficult to attribute recent road safety successes to any specific policy, Australia continues to expand their lead on the US in terms of safety outcomes and is a road safety example worthy of consideration. Young drivers remain overrepresented in road crashes around the world, with road injury the leading cause of death among adolescents. In addition, the majority of road traffic crashes, fatalities and injuries occur in low- and middle-income countries. All young drivers are at risk due to a breadth of age- and inexperience-related factors; however it is well recognised that young drivers may also intentionally engage in risky driving behaviours which increase their crash risk. The aim of this paper is to examine the self-reported risky driving behaviour of young drivers in Australia, New Zealand (high-income countries), and Colombia (middle-income country), and to explore the utility of a crash risk assessment model in these three countries. Young drivers aged 16–25 years completed the Behaviour of Young Novice Drivers Scale (BYNDS), in addition to self-reporting crash involvement and driving offences. A hierarchical segmentation analysis via decision trees was used to study the relationship between self-reported crashes and risky driving. Young drivers in Colombia reported more risky driving than young drivers in New Zealand, and considerably more risky driving than young drivers in Australia. Significant differences among and across countries in individual BYNDS items were found, and 23.5% of all participants reported they had been involved in a road crash. Handheld mobile phone usage was the strongest predictor of crashes, followed by driving after drinking alcohol, and carrying friends as passengers. Country of origin predicted mobile phone usage, with New Zealand and Colombia grouped in the same decision tree branch which implies no significant differences in the behaviour between these countries. Despite cultural differences in licensing programs and enforcement, young drivers reported engaging in a similar breadth of risky behaviours. Road crashes were explained by mobile phone usage, drink driving and driving with passengers, suggesting interventions should target these three risk factors. Whilst New Zealand and Australia have implemented graduated driver licensing programs, are geographical neighbours, and are high-income countries, the finding that behaviours of young drivers in New Zealand and Colombia were more similar than those of young drivers in New Zealand and Australia merits further investigation. Abstract: The management of occupational health and safety (OHS) including safety culture interventions is comprised of complex problems that are often hard to scope and define. Due to the dynamic nature and complexity of OHS management, the concept of system dynamics (SD) is used to analyze accident prevention. In this paper, a system dynamics group model building (GMB) approach is used to create a causal loop diagram of the underlying factors influencing the OHS performance of a major drilling and mining contractor in Australia. While the organization has invested considerable resources into OHS their disabling injury frequency rate (DIFR) has not been decreasing. With this in mind, rich individualistic knowledge about the dynamics influencing the DIFR was acquired from experienced employees with operations, health and safety and training background using a GMB workshop. Findings derived from the workshop were used to develop a series of causal loop diagrams that includes a wide range of dynamics that can assist in better understanding the causal influences OHS performance. The causal loop diagram provides a tool for organizations to hypothesize the dynamics influencing effectiveness of OHS management, particularly the impact on DIFR. In addition the paper demonstrates that the SD GMB approach has significant potential in understanding and improving OHS management. [Copyright &y& Elsevier]

This paper examines the determinants of road traffic crash fatalities in Queensland for the period 1958–2007 using a state-space time-series model. In particular, we investigate the effects of policies that aimed to reduce drink-driving on traffic fatalities, as well as indicators of the economic environment that may affect exposure to traffic, and hence affect the number of accidents and fatalities. The results show that the introduction of a random breath testing program in 1988 was associated with a 11.3% reduction in traffic fatalities; its expansion in 1998 was associated with a 26.2% reduction in traffic fatalities; and the effect of the “Safe4life” program, which was introduced in 2004, was a 14.3% reduction in traffic fatalities. Reductions in economic activity are also associated with reductions in road fatalities: we estimate that a one percent increase in the unemployment rate is associated with a 0.2% reduction in traffic fatalities. Abstract: Various systems for rating secondary safety of particular makes and models of vehicles have been developed internationally. These measures generally evaluate crashworthiness (the ability of the vehicle to protect its own occupants in the event of a crash) separately from aggressivity (the harm a vehicle is liable to impose on other road users into which it crashes). This paper describes an approach using Australian and New Zealand data that combines these two facets of secondary safety into one ‘Total Secondary Safety Index’ estimated from real world crash outcomes. The Index estimates the risk of death or serious injury to all key road users in crashes involving light passenger vehicles across the full range of crash types. This paper describes the rationale and method for producing this Index, together with some estimates for common Australian and New Zealand makes and models of light passenger vehicles. Hit-and-run crashes not only degrade the morality, but also result in delays of medical services provided to victims. However, class imbalance problem exists as the number of hit-and-run crashes is much smaller than that of non-hit-and-run crashes. The missing label problem also exists in the crash analysis due to reasons like data barrier such that the information hidden in the unlabelled samples has not been effectively utilised. In this paper, a cost-sensitive semi-supervised logistic regression (CS3LR) model is proposed for hit-and-run analysis, in order to tackle class-imbalanced data distribution and missing label problem, based on the crash dataset of Victorian, Australia (2013–2019). By performing label estimation with logistic regression jointly utilising both labelled and unlabelled data with pseudo labels in a well-designed cost-sensitive semi-supervised maximum likelihood framework, the proposed model can obtain an unbiased likelihood parameter for hit-and-run prediction and analysis. Comparing the experimental results of CS3LR model with two logistic regression models and seven machine learning methods, better performance of CS3LR model is demonstrated. The most significant contributing factors to hit-and-run crashes extracted by CS3LR with only 10% labelled data show a high degree of consistency with the true contributing factors obtained by the supervised cost-sensitive logistic regression with complete hit-and-run labels. The effects of class-weighted ratio and hyper-parameter λ on the performance of hit-and-run crash prediction model have also been analysed. The results can further provide recommendations and implications on the policies and counter-measures for preventing hit-and-run collisions and crimes. • Women cycling patterns were observed in an innovative naturalistic study approach. • Proportions of women riders were similar (22 %) in road and off-road bicycle paths. • Greater likelihood of observing a woman rider on roads with cycling-friendly features. • Lower likelihood observed among road bike riders. • Greater likelihood observed during day hours, on weekends, and in group riding. Women are less likely to ride than men in low cycling countries such as Australia. In Australia, self-reported cycling participation appears to be declining, particularly for women. This paper examines the rider and road environment correlates of women's cycling. While most earlier studies relied on self-report data to understand gender differences in cycling, this study video-recorded 24,868 riders (22 % female) at 17 sites across Queensland, Australia. The probabilities of an observed rider being female under different circumstances (e.g., speed limit, riding location, time of riding, group riding) at these sites were modelled in a binomial logistic regression framework. The likelihood of a rider being a woman was greater during the day (9am-8pm) than the early morning (5–9 a.m.); on weekends than on weekdays; in groups of two or more riders than among single riders; in lower speed zones than speed zones of 60 km/h or over; on roads with bike lanes or multiple traffic lanes or raised medians than on roads without these, and in urban areas than suburban areas. The likelihood of the rider being a woman was lower among those riding road bikes than other types of bicycles. The use of a naturalistic study design marks the key strength of this paper. Findings of this study should help better understand women's cycling patterns and preferred cycling locations, which cycling communities and organisations can use to advocate for better roads and paths that make female riders feel safe. • Various measures used to operationalise drink driving were examined. • Perceptions of both legal and non-legal sanctions for drink driving were high. • Non-legal sanctions were a larger deterrent for drink driving than legal sanctions. • Past drink-driving conviction was a predictor of re-offending. • Age, gender and risky drinking behaviours were drink driving predictors. The threat of application of legal sanctions remains the prominent approach to reduce the prevalence of drink driving in a vast array of motoring jurisdictions. However, ongoing questions remain regarding: (a) the extent that such mechanisms impact upon offending behaviours, (b) the deleterious effect alcohol consumption has on decisions to drink and drive and (c) how best to operationalise (and measure) the concept of drink driving to enhance the accurate measurement of the dependent variable. This paper reports on an examination of 773 Queensland motorists' (across nine local government areas) perceptions of both legal and non-legal drink driving sanctions (as well as alcohol consumption) in order to gauge the deterrent impact upon a range of measures of drink driving: the driver thinking they are over the limit, the driver knowing they are over the limit, attempts to evade random breath testing, and intentions to re-offend. The sample completed an online or paper version of the questionnaire. The majority of participants reported "never" engaging in "possible" (74.5 %) or "acknowledged" (83.4 %) drink driving events, although a considerable proportion of the sample reported engaging in "possible" (25.5 %) or "acknowledged" (16.6 %) drink driving and attempting to evade RBT (18 %) events, as well as possible intentions to drink and drive in the future (22 %). Males were more likely to report such events. Perceptions of both legal sanctions (certainty, severity and swiftness) as well as non-legal sanctions (fear of social, internal or physical harm) were relatively high and consistent with previous research. Interestingly, non-legal sanctions were reported as stronger deterrents than legal sanctions. However, multivariate analysis revealed that legal deterrents had limited utility predicting offending behaviours, but rather, demographic characteristics (e.g., younger motorists, males) as well as risky drinking behaviour were better predictors. In regards to intentions to offend, a past conviction for drink driving was also a predictor of re-offending. These results highlight the ongoing challenges of addressing the problem of drink driving and that some motorists: (a) have entrenched behaviour and/or (b) make the decision to drink and drive before they are under the influence of alcohol. Abstract: Average speed enforcement is a relatively new approach gaining popularity throughout Europe and Australia. This paper reviews the evidence regarding the impact of this approach on vehicle speeds, crash rates and a number of additional road safety and public health outcomes. The economic and practical viability of the approach as a road safety countermeasure is also explored. A literature review, with an international scope, of both published and grey literature was conducted. There is a growing body of evidence to suggest a number of road safety benefits associated with average speed enforcement, including high rates of compliance with speed limits, reductions in average and 85th percentile speeds and reduced speed variability between vehicles. Moreover, the approach has been demonstrated to be particularly effective in reducing excessive speeding behaviour. Reductions in crash rates have also been reported in association with average speed enforcement, particularly in relation to fatal and serious injury crashes. In addition, the approach has been shown to improve traffic flow, reduce vehicle emissions and has also been associated with high levels of public acceptance. Average speed enforcement offers a greater network-wide approach to managing speeds that reduces the impact of time and distance halo effects associated with other automated speed enforcement approaches. Although comparatively expensive it represents a highly reliable approach to speed enforcement that produces considerable returns on investment through reduced social and economic costs associated with crashes. Abstract: Motorcyclists contribute significantly to road trauma around the world through the high incidence of serious injuries and fatalities. The role of roadside safety barriers in such trauma is an area of growing concern amongst motorcyclists, road authorities and road safety researchers and advocates. This paper presents a case series analysis of motorcyclists that were fatally injured following a collision with a roadside barrier during the period 2001–2006 in Australia and New Zealand. Injury profiles and severities are detailed, and associations with crash characteristics are investigated. It is shown that the thorax region had the highest incidence of injury and the highest incidence of maximum injury in fatal motorcycle-barrier crashes, followed by the head region. This is in contrast to fatal motorcycle crashes in all single- and multi-vehicle crash modes, where head injury predominates. The injury profiles of motorcyclists that slid into barriers and those that collided with barriers in the upright posture were similar. However, those that slid in were more likely to receive thorax and pelvis injuries. [Copyright &y& Elsevier]

Abstract: Understanding the underlying structure of single vehicle crashes (SVCs) is essential for improving safety on the roads. Past research has found that SVCs tend to cluster both spatially and temporally. However, limited research has been conducted to investigate the interaction between the location of SVCs and the time they occur, especially at different levels of scales or spatial extents. This paper applied spatial, temporal and spatio-temporal techniques to investigate patterns of SVCs in Western Australia between 1999 and 2008, at different levels of scale. Spider graphs were adapted to identify temporal patterns of vehicle crashes at two different levels of scales: daily and weekly with respect to their causes. The spatial structures of vehicle crashes were analysed using Kernel Density Estimation analysis at three different scales: West Australia, Metropolitan area, and Perth Local Government Area (LGA). These are illustrated using spatial zooming theory. Comap was then used to demonstrate the spatio-temporal interaction effect on vehicle crashes. The results show significant differences in spatio-temporal patterns of SVCs for various crash causes. The techniques used here have the potential to help decision makers in developing effective road safety strategies. [Copyright &y& Elsevier]

Abstract: The effectiveness of safety inspections of ships has been analysed from various angles, but until now, relatively little attention has been given to translate risk reduction into incident cost savings. This paper provides a monetary quantification of the cost savings that can be attributed to port state control inspections and industry vetting inspections. The dataset consists of more than half a million ship arrivals between 2002 and 2007 and contains inspections of port state authorities in the USA and Australia and of three industry vetting regimes. The effect of inspections in reducing the risk of total loss accidents is estimated by means of duration models, in terms of the gained probability of survival. The monetary benefit of port state control inspections is estimated to range, on average, from about 70 to 190 thousand dollars, with median values ranging from about 20 to 45 thousand dollars. Industry inspections have even higher benefits, especially for tankers. The savings are in general higher for older and larger vessels, and also for vessels with undefined flag and unknown classification society. As inspection costs are relatively low in comparison to potential cost savings, the results underline the importance of determining ships with relatively high risk of total loss. Abstract: This paper presents findings from the rural and remote road safety study, conducted in Queensland, Australia, from March 2004 till June 2007, and compares fatal crashes and non-fatal but serious crashes in respect of their environmental, vehicle and operator factors. During the study period there were 613 non-fatal crashes resulting in 684 hospitalised casualties and 119 fatal crashes resulting in 130 fatalities. Additional information from police sources was available on 103 fatal and 309 non-fatal serious crashes. Over three quarters of both fatal and hospitalised casualties were male and the median age in both groups was 34 years. Fatal crashes were more likely to involve speed, alcohol and violations of road rules and fatal crash victims were 2½ times more likely to be unrestrained inside the vehicle than non-fatal casualties, consistent with current international evidence. After controlling for human factors, vehicle and road conditions made a minimal contribution to the seriousness of the crash outcome. Targeted interventions to prevent fatalities on rural and remote roads should focus on reducing speed and drink driving and promoting seatbelt wearing. [Copyright &y& Elsevier]

Abstract: An analysis of 2000–2007 single vehicle rollover fatalities in three Australian states was carried out using data from the Australian National Coroners Information System. In this paper, successive selection criteria were applied to the initial dataset to analyse: [-] Occupant fatalities in single passenger car crashes (1743 cases), [-] Occupant fatalities in single passenger car rollovers (474 cases), overall, rollovers accounted for 35% of all occupant fatalities in a single vehicle transport injury event. For these fatalities, the occupant was ejected or stayed contained in equal proportions. However, results showed strong disparities between the more urban and densely populated states of New South Wales and Victoria, compared to the Northern Territory in terms of crash type distribution and containment of the occupant. Differences were also found in rollover initiation, speed at initiation and number of turns. Overall, the strongest association of fatal neck/thoracic spine injuries with head injuries was found for the contained, restrained occupant. This analysis of single vehicle rollover fatalities is consistent with previous findings. It also shows that in Australia, strategies for rollover injury risk mitigation will need to take into account a broad range of characteristics to be effective. [Copyright &y& Elsevier]

Abstract: This paper reports on the findings of a retrospective case series study of fatal motorcyclist–roadside barrier collisions. Cases were retrieved from the National Coroners Information System (NCIS), the coronial case files of Australian jurisdictions, and the Crash Analysis System (CAS) of the New Zealand Transport Agency. Seventy seven (77) motorcycle fatalities involving a roadside barrier in Australia and New Zealand were examined. The fatalities usually involved a single vehicle crash and young men. The roadside barriers predominantly involved were steel W-beams, typically on a bend in the horizontal alignment of the road. A majority of fatalities occurred on a weekend, during daylight hours, on clear days with dry road surface conditions indicating predominantly recreational riding. Speeding and driving with a blood alcohol level higher than the legal limit contributed to a significant number of these fatalities. [Copyright &y& Elsevier]

Abstract: This paper describes an investigation of safety, mobility and travel patterns in a sample of older women drivers and former drivers aged 60 years and over. Participants provided information on general health and functional abilities, travel and driving patterns, driving experiences and confidence, difficulty with and avoidance of driving situations, self-assessment of driving ability, crash and infringement history, the process and experiences leading up to stopping driving, and satisfaction with current mobility. The sample was a fairly active group, travelling frequently and substantial distances, and generally satisfied with their level of mobility. Current drivers were strongly interested in keeping driving for as long as possible, expressed strong concerns about the prospect of stopping driving and reported little evidence of self-regulation. In contrast, former drivers were less negative about driving cessation and mostly reported successful retirement from driving with few negative mobility consequences. Further, a number of relationships between crash involvement and driving experience, confidence of being a safe driver, and problems in driving situations were found. These findings have added to our understanding of the issues concerning the safety and mobility of older women. Implications for the promotion of safe driving practices are discussed. Abstract: Falls and their injury outcomes have count distributions that are highly skewed toward the right with clumping at zero, posing analytical challenges. Different modelling approaches have been used in the published literature to describe falls count distributions, often without consideration of the underlying statistical and modelling assumptions. This paper compares the use of modified Poisson and negative binomial (NB) models as alternatives to Poisson (P) regression, for the analysis of fall outcome counts. Four different count-based regression models (P, NB, zero-inflated Poisson (ZIP), zero-inflated negative binomial (ZINB)) were each individually fitted to four separate fall count datasets from Australia, New Zealand and United States. The finite mixtures of P and NB regression models were also compared to the standard NB model. Both analytical (F, Vuong and bootstrap tests) and graphical approaches were used to select and compare models. Simulation studies assessed the size and power of each model fit. This study confirms that falls count distributions are over-dispersed, but not dispersed due to excess zero counts or heterogeneous population. Accordingly, the P model generally provided the poorest fit to all datasets. The fit improved significantly with NB and both zero-inflated models. The fit was also improved with the NB model, compared to finite mixtures of both P and NB regression models. Although there was little difference in fit between NB and ZINB models, in the interests of parsimony it is recommended that future studies involving modelling of falls count data routinely use the NB models in preference to the P or ZINB or finite mixture distribution. The fact that these conclusions apply across four separate datasets from four different samples of older people participating in studies of different methodology, adds strength to this general guiding principle. Abstract: Each year in Australia many thousands of collisions occur between motor vehicles and animals, resulting in considerable vehicle repair costs, injury to persons, and loss of animal life. This paper reviews animal-related road crashes in Australia and presents data from the in-depth Rural and Remote Road Safety Study in North Queensland for serious casualties (n =33) resulting from direct impact with an animal or swerving to avoid an animal on public roads. These crash types accounted for 5.5% of all eligible on-road serious casualties in the study and, hence, are considered to be an important issue that requires particular attention within rural and remote areas. Kangaroos and wallabies were the predominant species involved in these crashes (44.8%). Consistent with international studies, night-time travel was found to be a significant risk factor when comparing animal-related crashes to other serious injury crashes in the study. There were also a significantly higher proportion of motorcyclists (51.7%) than other vehicle occupants involved in animal-related serious crashes compared to all other serious injury crashes. Data matching to official Government records found underreporting of animal-related crashes to be an issue of concern. These findings are discussed in terms of countermeasures suitable for the Australian context and the need for consistent crash reporting across jurisdictions. [Copyright &y& Elsevier]

Abstract: This paper reports the results of an exposure survey of 794 registered motorcycle riders, with an average of 18.1 years of riding experience, in the State of New South Wales, Australia. Respondents completed two postal surveys, separated by about 6 months, that included items relating to their crash history, riding patterns, characteristics of their motorcycle, and its odometer reading. Odometer readings indicated that respondents rode a mean of 5208km each year, and that annual exposure was related to gender, motorcycle type, and dominant riding location, time of week, and purpose. The amount of riding reported for different purposes changed with age, with older riders more likely than younger riders to ride for recreational reasons and on weekends. The mean crash rate (based on self-reported crash involvement) was 0.96 crashes/100,000km. The crash rate declined with age, was highest in the Sydney metropolitan area, was lowest for motorcycles with large engines, and was highest for trail and dual-use motorcycles. There was a relationship between annual exposure and crash risk such that riders who rode relatively little had higher crash risks (per 100,000km travelled) than riders who rode more often. A cluster analysis identified three groups of riders with higher-than-average risks of crash involvement. [Copyright &y& Elsevier]

This paper presents the results of an investigation of the driving experience of a small sample of learner drivers in Victoria, Australia. Participants (n=110) kept a continuous logbook of their driving experience over the 2 years of the learner-driver period, including information about the distance and time of each driving trip, their level of confidence, and monthly data concerning the number of crashes, near misses, and unpleasant emotional interactions with their supervising driver. The analysis of these data suggests that learner drivers accrue relatively little driving experience and that they tend to obtain this experience in daytime, fair-weather driving.
