Results obtained from investigations of pedagogical relationships between academic teaching staff and students with sensory-related and physical disabilities (the blind and the deaf, persons with mobility disability: persons who use wheelchairs, persons with absence of upper limbs) are presented in the study. It has been proved that current problems directly affect adaptive processes and successfulness of receiving a higher education by the students of these disability categories. The authors of the paper focus on the social stigmatization of disabled young people. Background: Variation in prescription of antibiotics in primary care can indicate poor clinical practice that contributes to the increase of resistant strains. General Practitioners (GPs), as a professional group, are expected to have a fairly homogeneous prescribing style. In this paper, we describe variation in prescribing style within and across groups of GPs from six countries. Methods: Cross-sectional study with the inclusion of 457 GPs and 6394 sore throat patients. We describe variation in prescribing antibiotics for sore throat patients across six countries and assess whether variation in “prescribing style” - understood as a subjective tendency to prescribe - has an important effect on variation in prescription of antibiotics by using the concept of prescribing style as a latent variable in a multivariable model. We report variation as a Median Odds Ratio (MOR) which is the transformation of the random effect variance onto an odds ratio; Thus, MOR = 1 means similar odds or strict homogeneity between GPs' prescribing style, while a MOR higher than 1 denotes heterogeneity in prescribing style. Results: In all countries some GPs always prescribed antibiotics to all their patients, while other GPs never did. After adjusting for patient and GP characteristics, prescribing style in the group of GPs from Russia was about three times more heterogeneous than the prescribing style in the group of GPs from Denmark - Median Odds Ratio (6.8, 95% CI 3.1;8.8) and (2.6, 95% CI 2.2;4.4) respectively. Conclusion: Prescribing style is an important source of variation in prescription of antibiotics within and across countries, even after adjusting for patient and GP characteristics. Interventions aimed at influencing the prescribing style of GPs must encompass context-specific actions at the policy-making level alongside GP-targeted interventions to enable GPs to react more objectively to the external demands that are in place when making the decision of prescribing antibiotics or not. Background: The countries of Central and Eastern Europe have experienced a lot of changes at the end of the 20th century, including changes in the health care systems and especially in primary care. The aim of this paper is to systematically assess the position of family medicine in these countries, using the same methodology within all the countries. Methods: A key informants survey in 11 Central and Eastern European countries and Russia using a questionnaire developed on the basis of systematic literature review. Results: Formally, family medicine is accepted as a specialty in all the countries, although the levels of its implementation vary across the countries and the differences are important. In most countries, solo practice is the most predominant organisational form of family medicine. Family medicine is just one of many medical specialties (e.g. paediatrics and gynaecology) in primary health care. Full introduction of family medicine was successful only in Estonia. Conclusions: Some of the unification of the systems may have been the result of the EU request for adequate training that has pushed the policies towards higher standards of training for family medicine. The initial enthusiasm of implementing family medicine has decreased because there was no initiative that would support this movement. Objectives Alcohol use in Russia has a number of relatively distinct features which have been shown to be strongly associated with mortality. The aim of this paper is to investigate how specific aspects of Russian drinking behaviour and its consequences are related to scores obtained from the internationally recognised AUDIT instrument for detecting alcohol problems. Design Population-based age-stratified cross sectional survey. Setting Izhevsk, Russia. Participants 925 men aged 25–59 years. Main outcome measure Men and proxy respondents completed an interview including questions on alcohol consumption and socio-economic and demographic variables. These included questions on six drinking behaviours and their consequences. These were: going on zapoi (a period of continuous drunkenness of two or more days); drinking non- beverage alcohol; frequency of excessive drunkenness; frequency of hangover; drinking before noon and frequency of sleeping in clothes at night because of drunkenness. The men also completed a Self Completed Questionnaire containing questions constituting the alcohol use disorders identification test (AUDIT). Results There was a strong association with AUDIT score and hazardous drinking (one or more episodes of zapoi in the past year or drinking non beverage alcohol or twice a week or more occurrence of excessive drunkenness, hangover or going to sleep at night clothed because of drunkenness) using both proxy and self report of drinking behaviours (self-report OR 5.25 95% CI 2.86 to to 9.65). The association was stronger when proxy report was used (proxy-report OR 6.66 95% CI 4.00 to to 11.07). The same pattern was seen for all the drinking behaviours individually. Confirmatory factor analysis supported a two factor structure for the AUDIT measuring alcohol consumption and alcohol related problems. Hazardous drinking behaviour specific to Russia was more strongly associated with AUDIT questions relating to alcohol related problems than to questions measuring consumption. Conclusions The drinking behaviours used in this paper are unconventional measures of hazardous drinking. The strong association with AUDIT shows that both self and in particular proxy report of distinctive directly observable drinking behaviours provide useful data on alcohol consumption and abuse in Russian men.
