The main goal of this paper is to review the strategies developed across European health care systems during the 1990s to improve coordination among health care providers. A second goal is to provide some analytical insights in two fields. On the one hand, we attempt to clarify the relationships between pro-coordination strategies and organizational change in health care. Our main conclusion is that the specific features of health care impede the operation of either market or hierarchical coordination mechanisms. These can, however, be selectively successful if applied as levers to promote the role and impact of the pro-cooperative coordination strategies which are ultimately required to foster adequate inter-professional and inter-organizational coordination. On the other hand, we try to cast some light on the ongoing debate on convergence versus path dependency within the broader field of welfare state reform. Evidence on pro-coordination reforms in health care apparently supports some insights from previous work on the centrality of the socio-political structure to account for varying patterns of selective path dependency across countries. In particular, the informal power resources of specialist physicians vis-à-vis primary care professionals and the state are critical to explain the different rhythm and fate of pro-coordination reforms across Europe. Against received wisdom, the evidence examined suggests that selective path dependency might apparently be compatible with a general trend towards convergence understood as hybridization. Resource allocation has been a main policy issue in cash‐for‐care schemes (CfCs) for older people in Europe since their inception. It regards how publicly funded care benefits and services are distributed among older people. The raising pressures of an ageing population and the tensions on the financial sustainability of welfare regimes in place have further exacerbated the relevance of this topic over the recent years. Nevertheless, comparative research so far has overlooked changes in resource allocation in CfCs over time. This article contributes to fill this gap, exploring changes in resource allocation of CfCs for older people in a sample of European countries—Austria, England, France, Germany, Italy, and The Netherlands—since the early '90s (or since the introduction of the scheme). It examines three analytical dimensions: (a) The mix of public services and benefits provided to older people (CfCs, community services in kind, residential care); (b) the level of CfCs coverage; and (c) its generosity. A combined view of these dimensions leads to the discussion of two dilemmas: How to allocate the resources devoted to CfCs in the light of the trade‐off between its coverage and intensity? Although sport promotion may play an important role in achieving the recommended levels of physical activity for health, until now, there has been no comprehensive assessment of how sport policies in Europe address health. This article aimed at filling this gap by reviewing and analyzing recent national policy developments in the area of sport promotion, with a focus on synergies and discrepancies with the promotion of health-enhancing physical activity. By applying various search methods, 130 documents focusing on sport and physical activity were identified in the World Health Organization ( WHO) European Region. An analysis grid covering key indicators was developed for a systematic content analysis of 25 documents, selected according to established criteria. Analysis showed that general recommendations for good policy making were followed, such as establishing general goals, specifying time frame and responsible body for implementation and addressing different target groups. Furthermore, all sport strategies addressed health on an overall level and recognized the importance of Sport for All. However, in several strategies, there was a lack of measurable targets, specified budgets, and evaluation plans. The analysis showed that there is a great opportunity for health and sport sectors to work more closely together in the future, and identified areas where this could take place. Objective To compare the extent to which women in three European countries were able to exert influence over the organization and delivery of maternity policy and the factors likely to determine their success. Design Semi-structured interviews used to collect data, which was analysed in a framework that emphasized the importance of contextual environment. Setting and participants Representatives of 19 lay maternity user organizations in England, the Netherlands and Germany interviewed during 1996 and 1997. Variables studied Each interviewee was asked to provide details of their aims and objectives, activities and networks and perception of success. Four areas of contextual environment were used to account for variations. Main outcome measure Self-reported accounts of success in influencing policy agenda, credibility with opinion formers, campaigning activities and political networking were compared between and across countries. Results Marked differences between both the aspirations and the achievements of groups in the three countries. Conclusions Understanding the differences between countries in relation to user involvement entails locating research within the social, political and cultural context of health-care, consumerism and citizen participation. Hepatitis B and C infections are responsible for significant burden of disease accounting for 1.3 million deaths globally. There is a lack of quality data on the burden of disease due to these infections. One approach to informing policy makers on trends in hepatitis B and C is through case reporting of diagnosed cases. Data on these cases can identify outbreaks of hepatitis and monitor trends in acute and chronic infection. The European Centers for Disease Control (ECDC) has developed standardized case definitions and a harmonized reporting framework. Two articles in this issue summarize the trends in hepatitis B and C infection in Europe. The results show considerable variability in reported cases across countries, reflecting in part differences in testing practices. Risk factor information highlights the continued importance of injecting drug use as a risk factor for hepatitis C infection. Hepatitis case reporting provides valuable information, and more complete reporting will improve the utility of the data. For a comprehensive epidemiologic assessment of the burden of hepatitis, case reporting should be complemented by other sources of data, such as serologic and behavioural surveys.
