Purpose: This study aims to critically examine the effects of COVID-19 social discourses and policy decisions specifically on older adult volunteers in the UK, comparing the responses and their effects in England, Scotland, Wales and Northern Ireland, providing perspectives on effects of policy changes designed to reduce risk of infection as a result of COVID-19, specifically on volunteer involvement of and for older adults, and understand, from the perspectives of volunteer managers, how COVID-19 restrictions had impacted older people's volunteering and situating this within statutory public health policies. Design/methodology/approach: The study uses a critical discourse approach to explore, compare and contrast accounts of volunteering of and for older people in policy, and then compare the discourses within policy documents with the discourses in personal accounts of volunteering in health and social care settings in the four nations of the UK. This paper is co-produced in collaboration with co-authors who have direct experience with volunteer involvement responses and their impact on older people. Findings: The prevailing overall policy approach during the pandemic was that risk of morbidity and mortality to older people was too high to permit them to participate in volunteering activities. Disenfranchising of older people, as exemplified in volunteer involvement, was remarkably uniform across the four nations of the UK. However, the authors find that despite, rather than because of policy changes, older volunteers, as part of, or with the help of, volunteer involving organisations, are taking time to think and to reconsider their involvement and are renewing their volunteer involvement with associated health benefits. Research limitations/implications: Working with participants as co-authors helps to ensure the credibility of results in that there was agreement in the themes identified and the conclusions. A limitation of this study lies in the sampling method, as a convenience sample was used and there is only representation from one organisation in each of the four nations. South Asians constitute Britain's largest ethnic minorities, yet very little is known about their migration history in the prewar era. Recent literature has begun to challenge the widely held view that South Asian migration to Britain was a post-war phenomenon in response to Britain's need for labour as it emerged from two costly wars. The literature has demonstrated that the South Asian presence in Britain can be traced back to the seventeenth century. This paper adds to that literature by analysing the occupation distribution and mobility of South Asian-born migrants during the early twentieth century. Regression results from the 1901 and 1911 Censuses of England and Wales suggest that relative to other foreign-born and British-born, South Asian-born immigrants were primarily employed in white-collar jobs, a position they largely retained until at least 1911. These results suggest that the South Asian diaspora was economically well assimilated into the British economy. [ABSTRACT FROM AUTHOR]

Background: The 2022/23 influenza season in the United Kingdom saw the return of influenza to prepandemic levels following two seasons with low influenza activity. The early season was dominated by A(H3N2), with cocirculation of A(H1N1), reaching a peak late December 2022, while influenza B circulated at low levels during the latter part of the season. From September to March 2022/23, influenza vaccines were offered, free of charge, to all aged 2–13 (and 14–15 in Scotland and Wales), adults up to 49 years of age with clinical risk conditions and adults aged 50 and above across the mainland United Kingdom. Methods: End‐of‐season adjusted vaccine effectiveness (VE) estimates against sentinel primary‐care attendance for influenza‐like illness, where influenza infection was laboratory confirmed, were calculated using the test negative design, adjusting for potential confounders. Methods: Results In the mainland United Kingdom, end‐of‐season VE against all laboratory‐confirmed influenza for all those > 65 years of age, most of whom received adjuvanted quadrivalent vaccines, was 30% (95% CI: −6% to 54%). VE for those aged 18–64, who largely received cell‐based vaccines, was 47% (95% CI: 37%–56%). Overall VE for 2–17 year olds, predominantly receiving live attenuated vaccines, was 66% (95% CI: 53%–76%). Conclusion: The paper provides evidence of moderate influenza VE in 2022/23. Purpose: This paper explores how policy-level stakeholders tackle digital inclusion in the context of UK rural communities. Design/methodology/approach: Semi-structured interviews were conducted with stakeholders that operate nationally in government departments, government funded organisations and third sector organisations that provided a policy-level perspective on digital inclusion initiative provision across England, Scotland and Wales. Activity theory (AT) was utilised as a theoretical framework, where a variety of factors–tools, rules, community, division of labour and contradictions–were found to have an influence on digital inclusion initiative provision. Findings: Digital inclusion initiative provision in UK rural communities is organised through the multi-stakeholder involvement of national organisations, and collaboration with intermediary organisations to provide digital skills training and support. The process is fraught with difficulties and contradictions, limited knowledge sharing; reduced or poor-quality connectivity; lack of funding; lack of local resources; assumptions that organisations will indeed collaborate and assumptions that intermediary organisations have staff with the necessary skills and confidence to provide digital skills training and support within the rural context. Research limitations/implications: This study highlights the benefit of using AT as a lens to develop a nuanced understanding of how policy-level stakeholders tackle digital inclusion. Practical implications: This study can inform policy decisions on digital inclusion initiative provision suitable for rural communities. Originality/value: The contribution of this paper provides new insights into the understanding of how policy-level stakeholders tackle digital inclusion and the provision of digital inclusion initiatives; it builds on the use of AT to help unpick the complexity of digital inclusion initiative provision as a phenomenon; it reveals contradictions in relation to trust, and the need for knowledge sharing mechanisms to span and align different interpretations of digital inclusion across the policy-level; and reveals an extension of AT demonstrated through the "granularity of the subject" which enables the multi-actor involvement of the stakeholders involved in digital inclusion at policy-level to emerge. Thinking about organised crime as an ecosystem is not only novel but also offers much potential to add to the theoretical and policy-based literature in this area. While organised crime is often analysed as relationships between criminal groups and consumers of illicit goods/services, little work has analysed specific forms of organised crime activities as ecosystems where different constituent parts are dependent upon each other for the crime activity to occur. This paper analyses the organisation of sexual exploitation by entrepreneurial organised crime groups selling sex in the United Kingdom. Based upon 30 interviews with 23 law enforcement forces in England and Wales and five interviews with Adult Service Website Operators; it identifies how an ecosystem is built that ensures (a) profit generation and (b) avoidance of legal prosecution. Our analysis not only illustrates how a market 'supply' and 'demand' ecosystem is generated through mutually convenient 'organiser', 'marketer' and 'buyer' relationships, but also how the roles of these actors ensure sexual exploitation continues. Simple Summary: Existing cross-cultural research on animal welfare often overlooks the way that policy and law are not the exclusive domain of central government. This can result in an over-simplification or misrepresentation of the true situation. The political culture and institutional arrangements for governing the modern state are more complex than a "one-size-fits-all" approach. It is argued that cross-cultural research needs to give greater attention to differences within as well as between unitary states. Specifically, it needs to examine developments in constituent nations and territories. Here we illustrate this by drawing on new research in the United Kingdom, and examine how 'devolution'—or decentralized government for Wales and Scotland—is providing contrasting opportunities for NGOs, campaigners, and the public to lobby to improve animal welfare policy based on local practices, beliefs, and demands (collectively known as the "political culture"). Our findings show how this is important because it results in contrasting animal welfare policies and laws in the constituent nations of the UK. It is argued that extant cross-cultural research on animal welfare often overlooks or gives insufficient attention to new governance theory, civil society, politics, and the realities of devolved or (quasi-)federal, multi-level governance in the modern state. This paper synthesizes relevant social theory and draws on new empirical findings of civil society accounts of campaigning on animal welfare policies and law in the United Kingdom. It is presented as a corrective to arguably reductive, earlier unitary state-based analyses. Our core, evidence-based argument is that cognizance of civil society activism and the contrasting institutional governance structures and political cultures of constituent nations in unitary states—such as the UK—are providing opportunities for the territorialization of legally grounded animal welfare regimes, and culturally distinctive practices. Using administrative data linkage, this paper sought to quantify the impacts of family homelessness on pupil absence from school. It addresses a gap in United Kingdom (UK) homelessness research, which draws predominantly on qualitative methods and where there is a greater focus on people who fall outside of the statutory system, i.e., single people living on the streets, rather than families. Education records for the academic years 2012/13 to 2015/16 relating to pupils aged 5 to 15 years old living in a coastal city in Wales, UK, were linked to data on households assessed by the statutory housing service operating across the same region. Analysis of mean half-day sessions absent from school, and Poisson panel regression were used to explore associations between absenteeism (authorised, unauthorised, and total), and whether pupils were living in a household making a statutory homelessness application, i.e., experiencing family homelessness. On average, in any given academic year, pupils experiencing family homelessness (PEFH) missed 5 days more of school than pupils not experiencing family homelessness (PnEFH). Adjusted regression analysis found that the rate of total absence was 7% higher amongst PEFH compared to PnEFH, whilst for unauthorised absence it was 13% higher. When a student experienced family homelessness, this led to an increase in their rate of total absences by 5%--adjusting for other factors--compared to when they were not homeless. Findings have implications for statutory education and housing provision, specifically the need for greater cross-disciplinary working to prevent and alleviate the harms caused when families experience homelessness. Technology has become an integral part of our educational systems, and its importance in our schools cannot be overstated. However, digital skills, unlike other literacy skills, such as reading, writing, and numeracy, still have many discontinuities between how children use them at home versus in school. Therefore, in Wales (UK), digital skills are being promoted as part of the Digital Competence Framework (DCF) and feature highly in the new Curriculum for Wales (2022). Moreover, the new Digital Technology General Certificate of Secondary Education (GCSE) in Wales has been introduced to provide learners with the opportunity to gain a qualification that builds digital skills, knowledge, and understanding. However, this also brings many challenges for teachers, such as a lack of confidence, knowledge, and training, as well as a lack of resources and fear of change, to name a few. These challenges, in turn, have an impact on pupils' motivation and performance, as well as parents' ability to support their children. This paper presents a qualitative case study on the development of a new digital technology learning community for primary and secondary school pupils, their teachers, and parents in Blaenau Gwent, Wales (UK). Firstly, the paper will provide insight into what was required to establish an effective learning community, including ensuring engagement and buy-in from all stakeholders. Secondly, through the description, analysis, and interpretation of findings from two studies, the paper will highlight the impact of the DTLSN learning community on teachers and pupils in Blaenau Gwent, especially in terms of their learning and teaching. This article explores research findings regarding the possibilities offered by remote attendance at council meetings as implemented during the coronavirus (Covid-19) pandemic, and reflects upon how this may improve women local councillors' experiences, as well as women's political participation and the accessibility of Welsh local government going forward. Influenced by feminist institutionalist theory, this paper examines how councils' formal organisational norms and practices pre-pandemic privileged presenteeism, and explores participants' perceptions and experiences of the accessibility of local councils, especially for younger women with families and/or in other forms of employment. Presenting data from 19 semi-structured interviews with women local councillors in Wales (UK), both in-person and subsequently online during the pandemic, the paper discusses how remote attendance in local council meetings was considered an enabling shift in formal organisational practices, especially for rural councils. Despite some dissenting opinions and voiced dubiousness (mostly concerning future hybrid implementation), through easing the time costs of being a local councillor, particularly for women balancing a gendered 'triple duty' of the political, personal, and professional, remote meeting attendance is an organisational solution, albeit somewhat forced in implementation, which presents clear means of improving women's political participation and representation in local government. Mortality at older ages varies by season, increasing the uncertainty associated with modelling and projecting mortality at older ages and ultimately contributing to pension providers' overall risk. As the population ages, it becomes more important to understand variations in seasonal mortality between pensioners and to identify those most vulnerable to seasonal mortality differences. Using data from the Self-Administered Pension Schemes mortality investigation of the Continuous Mortality Investigation of the Institute and Faculty of Actuaries, UK, this paper investigates variations in seasonal mortality amongst members of UK occupational pension schemes over the period 2000--2016. Results are also compared with the corresponding population of England and Wales. For the oldest age groups (80+), which are most affected by seasonality, females are more vulnerable to seasonal differences in mortality for each pensioner group relative to males. Following a long-term decline in the winter-summer mortality gap the gap increased over the period, particularly for female pensioners and dependants. Seasonality remains a feature of UK mortality at older ages and risk management for pension schemes should consider seasonality when analysing overall mortality experience. [ABSTRACT FROM AUTHOR]

This paper focuses on the Colleges of Advanced Technology (CATs), specialist providers of advance science and technology which existed in England and Wales for ten years after the 1956 White Paper Technical Education. Its central argument is that recasting the CATs as broader-based universities following the 1963 Robbins Report was a significant error which attenuated the progress of science and technology, and prevented the Colleges' development as viable providers of higher education (HE) outside the university sector. This decision, it is argued, was shaped by typically English views about the relative value of different forms of learning, the nature and purpose of HE, and particular beliefs about the primacy of the university. It also conflated the general desire to increase participation in higher education with the wish to promote science and technology in particular. A bolder option, it is proposed, would have been to build the CATs up as prestigious institutes of technology, along the lines of those found in the USA and continental Europe – although this, it is recognised, would have entailed a substantial shift in the role of the state and reduced the individual and collective autonomy of HE institutions in England and Wales. This paper investigates the effectiveness of liberalization policy on the intercity coach market in England and Wales and evaluates its impact in promoting competition and enhancing welfare. The paper adds to the current literature by assessing this policy focusing on natural monopolies, deriving a structural model of the industry and using web-scraped key market-level data in the study. Regression analysis and descriptive statistics suggest peripheral routes with a small market size are natural monopolies, where passengers pay higher prices. We estimate a structural model, currently absent from the literature, which shows that these routes are characterized by lower welfare levels. The model allows us to simulate a policy promoting competition on such routes showing that a regulator could generate net welfare gains by implementing a more competitive equilibrium on these routes. Family group conferences (FGCs) in child welfare share decision-making with family members by bringing the immediate and wider family together to make a plan to meet a child's needs. This paper reports survey findings on FGC provision in the UK in 2022 and explores whether in England the presence of an FGC service and the rate of FGC provision is associated with the rate of children in care, entering care, in kinship foster care and leaving care. Seventy-nine per cent (n  = 167) of local authorities in the UK provided FGCs to families, and 14 per cent (n  = 29) did not. Services that were more established offered a more diverse range of FGCs. The introduction of FGCs in English local authorities was associated with a higher rate of children in care, but also higher rates of kinship foster care, a key goal of FGCs where it is not possible for children to stay with their parents. Higher rates of FGCs were associated with more children leaving care, possibly due to reunification with birth families. To understand in more detail, the circumstances of children in and leaving care in local authorities with FGCs, individual data linkage studies are needed. [ABSTRACT FROM AUTHOR]

Despite predominantly lower social class origins, the second generation of established immigrant groups in the UK are now attaining high levels of education. However, they continue to experience poorer labour market outcomes than the majority population. These worse outcomes are often attributed in part to their disadvantaged origins, which do not, by contrast, appear to constrain their educational success. This paper engages with this paradox. We discuss potential mechanisms for second‐generation educational success and how far we might expect these to be replicated in labour market outcomes. We substantiate our discussion with new empirical analysis. Drawing on a unique longitudinal study of England and Wales spanning 40 years and encompassing one per cent of the population, we present evidence on the educational and labour market outcomes of the second generation of four groups of immigrants and the white British majority, controlling for multiple measures of social origins. We demonstrate that second‐generation men and women's educational advantage is only partially reflected in the labour market. We reflect on the implications of our findings for future research. In this paper, we use a UK case study to explore how the COVID-19 pandemic affected the mental health (emotional, psychological, social wellbeing) of farmers. We outline the drivers of poor farming mental health, the manifold impacts of the pandemic at a time of policy and environmental change, and identify lessons that can be learned to develop resilience in farming communities against future shocks. We undertook a survey answered by 207 farmers across the UK, focusing on drivers of poor mental health and the effect of the COVID-19 pandemic. We also conducted 22 in-depth interviews with individuals in England, Scotland and Wales who provide mental health support to farmers. These explored how and why the COVID-19 pandemic affected the mental health of farmers. These interviews were supplemented by 93 survey responses from a similar group of support providers (UK-wide). We found that the pandemic exacerbated underlying drivers of poor mental health and wellbeing in farming communities. 67% of farmers surveyed reported feeling more stressed, 63% felt more anxious, 38% felt more depressed, and 12% felt more suicidal. The primary drivers of poor mental health identified by farmers during the pandemic included decreased social contact and loneliness, issues with the general public on private land, and moving online for social events. Support providers also highlighted relationship and financial issues, illness, and government inspections as drivers of poor mental health. Some farmers, conversely, outlined positive impacts of the pandemic. The COVID-19 pandemic is just one of many potential stressors associated with poor farming mental health and its impacts are likely to be long-lasting and delayed. Multiple stressors affecting farmers at the same time can create a tipping point. Therefore, there is a need for long-term support and ongoing evaluation of the drivers of poor mental health in farming families. [ABSTRACT FROM AUTHOR]

In this paper, we propose a mixed methods quantitative and qualitative approach to capture fully the measurable and less tangible social impacts of transport projects on local people and communities. The approach was used to assess the potential social impacts of a strategic road by-pass project case study in a deprived region of Wales in the UK. The project specifically aimed to stimulate local economic growth and regeneration in the local areas it serves. In a 'before and after' case study, we combined fine-grained, GIS-based spatial analysis of secondary datasets with qualitative participative exercises with the local residents of the five communities living adjacent to the road, and interviews with professional local stakeholders. This mixed methods approach significantly enhanced understanding of both the social benefits and disbenefits of the road project. It helped to reveal local concerns that would not otherwise have been apparent from secondary dataset analysis alone. The qualitative studies were also successful in bringing to the table new 'hard to reach' voices that had not been heard through the formal consultation and public engagement process. The study revealed that the social benefits accruing to local people from the project could have been significantly enhanced, whilst a number of its locally occurring negative social impacts could have been avoided had social assessment been employed earlier in the decision processes concerning its routing and design. Recommendations to improve the practice and uptake of social assessments at the option appraisal, project design mitigation and post evaluation stages of transport projects are included in the paper. In order to combat the COVID-19 pandemic, policymakers around the globe have increasingly invested in digital health technologies to support the 'test, track and trace' approach of containing the spread of the novel coronavirus. These technologies include mobile 'contact tracing' applications (apps), which can trace individuals likely to have come into contact with those who have reported symptoms or tested positive for the virus and request that they self-isolate. This paper takes a critical public health perspective that advocates for 'genuine participation' in public health interventions and emphasises the need to take citizen's knowledge into account during public health decision-making. In doing so, it presents and discusses the findings of a UK interview study that explored public views on the possibility of using a COVID-19 contact-tracing app public health intervention at the time the United Kingdom (UK) Government announced their decision to develop such a technology. Findings illustrated interviewees' range and degree of understandings, misconceptions, and concerns about the possibility of using an app. In particular, concerns about privacy and surveillance predominated. Interviewees associated these concerns much more broadly than health by identifying with pre-existent British national narratives associated with individual liberty and autonomy. In extending and contributing to ongoing sociological research with public health, we argue that understanding and responding to these matters is vital, and that our findings demonstrate the need for a forward-looking, anticipatory strategy for public engagement as part of the responsible innovation of the COVID-19 contact-tracing app in the UK. Police recruitment across the UK is under intense political and social pressure to increase representation and legitimacy. This layered in to a quest to raise the number of police officers in England and Wales by 20,000. However, despite decades of reform initiatives, police recruitment continues to be a challenging and potentially exclusionary process for candidates from ethnic minority backgrounds. Research in this area mainly comprises macro-level process evaluation and subsequent extrapolation of the results into positive action initiatives. There is a paucity of research at the micro-interaction level among social actors. In this paper, we present a case study of police recruitment in a large English Police Constabulary over four recruitment cohorts in 2018. We conducted 26 long-form, in-depth interviews with new police recruits. Utilising the embeddedness theoretical framework based on sociological studies of the labour market, we attempt to understand police recruitment at the micro-social interaction level. We demonstrate that police recruitment has a high level of social embeddedness within the Constabulary. Candidates utilise social connections throughout the recruitment process to develop competence in the recruitment stages, while also building their respective police social identities. Although positive action recipients also receive significant organisational and instrumental support, they build a relationship with the abstract organisation and not existing police officers. This creates an imbalance in social identity development and may have implications for how positive action initiative recipients may struggle within the policing environment. This paper examines the relationship between public houses or pubs, and crime rates in England and Wales. The impact of pubs on local communities is generally studied and investigated within the context of third places, thus physical places that facilitate the accumulation of social capital within communities. We estimate Poisson Fixed-Effects (PFE) and a frontier Spatial Autoregressive (SAR) model on a unique panel dataset for 375 local authorities in England and Wales between 2003 and 2018. Results from the analysis indicate that the presence of pubs progressively relates to a higher incidence of major crimes when transitioning from rural to urban areas, mainly due to weaker level of community cohesion and a lack of resources to support formal policing in more urbanized centres. These findings highlight the importance of place-based strategies in tackling rising incidences of crimes, indicating that recent pub closures may have contributed to severing community ties that act as a deterrent to crime in certain areas. This paper examines the intersection between job quality and innovation by exploring policy-led innovation aimed at delivering high-quality adult social care jobs that attract and retain much-needed skilled workers. Through qualitative enquiry, we examine workforce policy's emphasis on training, development and career progression, key elements of job quality, to create higher-skilled, higher-status roles. Drawing on focus groups and interviews with care providers and care workers in Wales (UK), we question the likely success of policy-led innovation given first its ineffective delivery and second its failure to address broader aspects of job quality. Policy thus fails both to institutionalise skilled roles in care work and to raise the status of the care worker occupation. We contribute to emerging HRM and interdisciplinary theory and policy debates about the complex nature of innovation activities and their outcomes for job quality in care work within a sector typified by low-quality jobs. This paper assesses to what extent British councils have addressed onshore wind energy, and the attitudes of councillors and staff about wind farms. Websites of 400 British councils were reviewed regarding assessment of landscapes and policies towards wind energy. An Internet survey of 800 councillors and staff from 275 councils quantified their attitudes towards wind farms, their visual impacts and the acceptability of existing wind farms in Britain. Wind farms reduced perceived scenic quality. Staff opposed wind energy but councillors were more accommodating. Most participants in England found wind farms acceptable, but less than half in Wales and one third in Scotland. Scotland may have reached the limit for acceptability of wind farms. Wales may be approaching that point. Overall, the survey indicated that it is unlikely that there will be widespread installation of additional onshore wind farms across Britain. Expansion of renewable energy may be sought through re-powering of existing sites with larger but fewer turbines. [ABSTRACT FROM AUTHOR]

Intimate partner homicides are often situated within the context of domestic abuse, and although less prevalent than domestic abuse, there have been several multi‐agency approaches to understanding the risk for these fatal crimes. Domestic Homicide Reviews (DHRs) were introduced in 2011 to provide information to help with assessing such risk. This paper aims to analyse DHRs in England and Wales to investigate/determine risk factors for domestic homicide following intimate partner abuse. All publicly available DHRs published between July 2011 and November 2020 where the victim and perpetrator were or had been intimate partners (N = 263) were retrieved from Community Safety Partnership websites in England and Wales. A quantitative design was used to extract data from DHRs, and descriptive and inferential statistics were generated by SPSS 26. Findings identified risk factors relating to domestic abuse, including stalking, separation, and the victim being in a new relationship. Sociodemographic risk factors included higher levels of deprivation, lower income and higher barriers to housing and services. This highlights the role of both individual and sociodemographic factors in domestic homicides, and particularly the need for greater socioeconomic security for victims of domestic abuse. In conclusion, though much of the data is in line with previous research, our analysis highlights the pivotal role of regional poverty, with comfortable socioeconomic conditions offering protection against intimate partner homicides. This research suggests important directions for future research and makes a valuable contribution to a more in‐depth understanding of the relationship between domestic abuse and intimate partner homicide. Flooding in 2021 has highlighted the increased risk to national resilience. This is against a backdrop of the UK climate projected to become more extreme over the next few decades. This paper considers the notable river and surface water flood events within England and Wales during 2021 and examines the performance of the Flood Forecasting Centre in highlighting the flood risk to our customers. We reflect on team debriefs as well as feedback and surveys from our customers. We distil our learnings and make connections with improvements in our underpinning science, forecasting tools, products and services. This paper investigates whether high borrowing costs deterred investment in sanitation infrastructure in late nineteenth-century Britain. Town Councils had to borrow to fund investment, with considerable variation in interest rates across towns and over time. Panel regressions, using annual data from more than 800 town councils, indicate that higher interest rates were associated with lower levels of infrastructure investment between 1887 and 1903. Instrumental variable regressions show that falling interest rates after 1887 stimulated investment and led to lower infant mortality. These findings suggest that Parliament could have expedited mortality decline by subsidizing loans or facilitating private borrowing. [ABSTRACT FROM AUTHOR]

British supermarket-panel data suggest no increases in overall sales and purchases of alcohol following COVID-19 lockdowns, yet survey and mortality data suggest otherwise. This paper attempts to unravel the paradox. Based on purchase data of 79,417 British households from Kantar Worldpanel, we undertake controlled interrupted time series analysis of the impact of COVID-19 confinement introduced on 23rd March 2020, and variably applied during 2020, compared to purchases during 2015 to 2019 as controls. We also undertook Poisson regression analyses to estimate if changes in purchases differed by household socio-demographic and economic factors. Excess off-trade household alcohol purchases (expressed as grams of ethanol) following the introduction of confinement, were 29.2% higher (95% CI = 25.8% to 32.5%) for the post-confinement months of 2020, being larger until mid-July 2020 (37.5%, 95%CI = 33.9 to 41.26%) when pubs re-opened with restrictions, and smaller (24.6%, 95%CI = 21.6 to 27.7) thereafter. During the time of complete pub closures, and fully adjusting for no on-trade purchases, household purchases of alcohol did not change when compared with the same time period during 2015–2019 (coefficient = -0.9%, 95%CI = -5.6 to 3.8). Excess purchases from 23rd March to 31st December 2020 varied by region of Great Britain, being higher in the north of England, and lower in Scotland and Wales. Excess purchases were greater in the most deprived households, compared with the least deprived households. Excess purchases increased substantially as the amount of alcohol normally purchased by a household increased, with the top one fifth of households that normally bought the most alcohol increasing their purchases more than 17 times than the bottom one fifth of households that bought the least alcohol. That the heaviest buyers of alcohol increased their purchases the most, with some independent impact of socio-economic disadvantage, might explain why reported alcohol problems and recent alcohol-related death rates might have increased. A conclusion of this is that alcohol policy to reduce high consumption of alcohol, and the availability of help and treatment to reduce alcohol consumption become more important during extraordinary times, such as COVID lockdowns. Background: Coroners, who hold inquests to determine the causes of unnatural deaths in England and Wales, having recognised factors that could cause other deaths, are legally obliged to signal concerns by sending 'Reports to Prevent Future Deaths' (PFDs) to interested persons. We aimed to establish whether Coroners' concerns about medications are widely recognised. Methods: We searched MEDLINE, Embase and Web of Science up to 30 November, 2022 for publications linking PFDs and medications using a combination of search terms "coroner*", "inquest*", "medicine*", "medication*" and "prevent*". We also searched the BMJ, a UK journal that carries news items; and the databases Nexis Advance and News On the Web for reports in national newspapers between 2013 and 2022, using the search terms ("regulation 28" OR "prevent future deaths" OR "prevention of future deaths") AND "coroner". We recorded the number of publications, as well as their citations in Google Scholar at 23 May, 2023. Results: Only 11 published papers on medicines referenced UK PFDs, nine of which were from our group. The BMJ carried 23 articles mentioning PFDs, five related to medicines. Of 139 PFDs (out of over 4000) mentioned in national newspapers, only nine related to medicines. Conclusions: The PFDs related to medicines are not widely referred to in medical journals or UK national newspapers. By contrast, the Australian and New Zealand National Coronial Information System has contributed cases to 206 publications cited in PubMed, of which 139 are related to medicines. Our search suggests that information from English and Welsh Coroners' PFDs is under-recognised, even though it should inform public health. The results of inquiries by Coroners and medical examiners worldwide into potentially preventable deaths involving medicines should be used to strengthen the safety of medicines. Due to its progressive nature, dementia is now the leading cause of death in England and Wales, so a palliative approach to care is beneficial because of its emphasis on supportive care. Many people with dementia also have other comorbid health conditions common in older age, that in themselves may induce pain. However, people with dementia may have difficulty in understanding what they are feeling and in describing and locating the pain due to having issues with communicating their needs as their dementia progresses. This paper considers the recognition, assessment, treatment and management of pain in older people with dementia. It introduces the learner to what is known about the subject and why it is important to know the person's past history and previous ways of self-managing pain and discomfort using information gained from their family, carers and friends. Circumferential deep burns on the limb lead to a constrictive, tourniquet-like effect causing critical limb ischaemia. The treatment, escharotomy, is a time-critical procedure that sometimes is required before the patient arrives at a burn centre. At present, no practical method of teaching this procedure is incorporated into formal educational courses. The feasibility of a comprehensive education package to teach upper limb escharotomy was assessed in a group of plastic and general surgery trainees in Wales. Small group workshops focused on the clinical presentation of patients requiring escharotomy. Participants then executed this on a custom-made high-fidelity simulation upper limb model. The articulated limb has subcutaneous silicone fat which bulges upon decompression and a finger-tip which turns pink indicating satisfactory reperfusion. A before and after five-point Likert scale was used to evaluate changes in participants' self-assessed confidence in the surgical management of escharotomy. Statistical significance between scores was assessed using the Wilcoxon signed-rank test. A total of 34 participants took part. Following completion of the course, general surgery trainees' confidence in executing the procedure increased from a median score of 1.00 "not confident at all" (IQR 1.00–2.00) to 4.00 "fairly confident" (IQR 4.00–5.00, p < 0.01). Plastic surgery trainees' confidence increased from a median score of was 3.00 "somewhat confident" (IQR 1.75–4.00) to 4.00 "fairly confident" (IQR 3.00–4.25, p < 0.01). We developed a comprehensive simulator course that has been demonstrated to improve candidate's confidence in performing escharotomy. The next stage in the course development is to confirm the results in a larger cohort. The research goals were to obtain an understanding of who the users of e‐books in the NHS are, what they are using e‐books for, and when and how they use them. This article presents the methodology used and the findings from the research. It also explores the outputs and next steps from the research, both for the individual countries and collectively. The Five Nations group, (library leads in England, Northern Ireland, Ireland, Scotland and Wales) commissioned research into healthcare staff use and non‐use of e‐books to understand the behaviours, needs and expectations of healthcare staff and to identify shared challenges around e‐books to inform policy and practice. Simple Summary: In Great Britain, private animal keepers such as pet owners and farmers require a licence from their local government authority to keep wild animals of species formally categorised as "dangerous". No central record of these licences is maintained by the national Governments in Britain, and the last formal examination of the situation was published more than 20 years ago. We examined the scale and scope of private keeping of dangerous wild animals in England, Scotland, and Wales in 2020. We found a total of 3950 individual dangerous wild animals kept by 210 licensees across more than one-third of British local authorities. While overall numbers of licensed dangerous wild animals have declined over the preceding 20 years, there have been notable increases in the keeping of some taxa such as wild cats, venomous snakes, and crocodilians. There is evidence that the average relative cost to obtain a licence to keep a dangerous wild animal has decreased since the earlier study, and that local authorities with licensed animals have lower licensing fees than those without. We discuss the current system of licensing with a view to making recommendations for improvement. We analysed the licences issued by British local government authorities under the Dangerous Wild Animals Act 1976, which regulates the private keeping of wild animals categorised as "dangerous", to assess the scope and scale of private keeping of dangerous wild animals in Great Britain. Results are compared with historical data from England and Wales, showing that there has been an overall decrease both in the total population of dangerous wild animals privately kept under licence and the number of licences, resulting primarily from a decrease in the farming of wild boar and ostrich, and from certain other species no longer requiring a licence to be kept. Nonetheless, the private keeping of dangerous wild animals remains prevalent, with a total population of 3950 animals kept under licence, and at least one-third of local authorities in Britain licensing keepers of one or more such animals. The population of non-farmed dangerous taxa has increased by 59% in 20 years, with notable increases in crocodilians (198%), venomous snakes (94%), and wild cats (57%). We present evidence that the average cost of a licence to keep dangerous wild animals has fallen over time, and that there is a negative association between cost and licensing. The current schedule of species categorised as dangerous is compared to a formally recognised list of species kept in zoos assessed by risk to the public. This article argues that as a consequence of austerity, police in England and Wales have taken over important roles in welfare and social policy institutions. This renders those institutions more coercive, punitive and exclusionary, and normalises a police worldview in those institutions. This process of what I call austerity-driven policification can be observed specifically well in the increasing numbers of police officers integrated into schools most affected by austerity. Such 'transinstitutional policing' in Britain is triggered by contradictory post-global financial crisis austerity measures, but reliant upon a long, racialised history of authoritarian neoliberalisation. Cuts to public spending in the 2010s reduced state institutions' capacities to provide for vulnerable people, who were further criminalised and whose rights to support and solidarity were further delegitimised by a radicalisation of the framing of welfare recipients as undeserving, social housing estates as drug-infested gang territories, and schools in deprived areas, and Black pupils in particular, as dangerous. Police, while subjected to austerity measures also, functioned as an institution of last resort, supplementing and replacing incapacitated state institutions, while also being presented as an appropriate institution to address problems increasingly understood to be of a criminal rather than educational nature. This article suggests that austerity-driven policification is an intensification of longer-term trends toward a larger role for police in the neoliberal era. The concept 'performance expectancy' is used to theorize about older people's day-to-day use of technology mediated communication (telephone, text/emails and video calls) in family relationships and the impact on social isolation and loneliness. Using theoretical and methodological triangulation six mediation models are conceptualized, followed by empirical validation using a nationally representative dataset of older people (≥ 65 years) living in Wales, UK (N = 2,099). The results reveal that frequency of telephone and video calls mediate the relationship between proximity of family and social isolation, and telephone calls mediate the relationship with loneliness. Text/emails have no significant mediating effect. Qualitative data analysis provides a deeper understanding of conditions that facilitate technology mediated communication. Demonstrating the impact of lifecourse habits, social context and environment on technology mediated communication shifts the focus from individual deficits to other reasons for non-adoption. Understanding the link between the individual, the social-cultural and social structural environment, social interaction, and loneliness requires an understanding of what is important to older people. For example, knowledge concerning the role of embodied presence in alleviating loneliness can be used to better understand the likelihood of implementing successful interventions. The COVID-19 pandemic and the lockdown measures imposed as a result affected the lives of people in all parts of society across the world. In 2020, during the first UK national lockdown, older adults (aged 70 years and over) were told to 'shield' within their homes, as they were regarded as being at higher risk of serious COVID-19 infection compared to other age groups. This paper explores older adults' experiences of COVID-19 lockdown measures whilst living in housing with care schemes for older people. The purpose is to examine the impact of the lockdown measures on scheme life including social connections amongst residents and their general everyday wellbeing during this time. We present qualitative findings based on interviews with 72 residents who took part in longitudinal and cross-sectional interviews across 26 housing with care schemes. Data were analysed using a thematic framework approach to examine specifically their experiences of living in housing with care schemes during the 2020 UK lockdown. The paper highlights that COVID-19 restrictions had a detrimental impact on the social connections and interactions of older residents living in housing with care schemes, as well as on their feelings of autonomy and independence. Despite this, residents adapted and coped with self-isolation restrictions and sought out positive ways to maintain social contact with others inside and outside to the scheme. We further highlight the tensions that providers of housing for older adults faced in promoting residents' autonomy and connectedness whilst also trying to provide a safe living environment and protect residents from risk of COVID-19 infection. Our findings apply not only to a pandemic situation but to the broader understanding of how housing with care for older adults must navigate between autonomy and support. • COVID-19 lockdowns challenged the independence and autonomy of older residents of housing with care in the UK. • Older adults in housing with care exercised autonomy through employing practices to maintain social connections. • Housing staff bolstered residents' autonomy, facilitated social interactions, and supported residents' wellbeing. • Housing staff faced challenges in maintaining the independent ethos of housing schemes whilst also protecting residents. The 1999 Immigration and Asylum Act marked a watershed moment for the politics of asylum in the UK, setting the discursive groundwork for the now infamous 'hostile environment'. This article is the first to compare the discursive framings of this formative act by the Home Office and UK government with those of the then newly devolved Welsh political institutions. While exploring the dominant visions of asylum as fear, unease and hostility marking the act, this paper highlights contrasting national narratives and imaginaries of welcome and hospitality in Welsh institutions. Drawing on a discourse analysis of archived policy documents, newspapers and interview material, the paper argues that these emerging hospitable imaginaries constituted a form of Welsh national identity formation against a less hospitable 'Other'- the UK sovereign state. This article contributes to the critical migration literature by questioning if the notion of hospitality involves more than the ambivalent framing of non-citizens as guests and others, or if instead it is more intended to differentiate from sovereign responses to asylum. Background: The FROM-16 is a generic family quality of life (QoL) instrument that measures the QoL impact of patients' disease on their family members/partners. The study aimed to assess the responsiveness of FROM-16 to change and determine Minimal Important Change (MIC). Methods: Responsiveness and MIC for FROM-16 were assessed prospectively with patients and their family members recruited from outpatient departments of the University Hospital Wales and University Hospital Llandough, Cardiff, United Kingdom. Patients completed the EQ-5D-3L and a global severity question (GSQ) online at baseline and at 3-month follow-up. Family members completed FROM-16 at baseline and a Global Rating of Change (GRC) in addition to FROM-16 at follow-up. Responsiveness was assessed using the distribution-based (effect size-ES, standardized response mean -SRM) and anchor-based (area under the receiver operating characteristics curve ROC-AUC) approaches and by testing hypotheses on expected correlation strength between FROM-16 change score and patient assessment tools (GSQ and EQ-5D). Cohen's criteria were used for assessing ES. The AUC ≥ 0.7 was considered a good measure of responsiveness. MIC was calculated using anchor-based (ROC analysis and adjusted predictive modelling) and distribution methods based on standard deviation (SD) and standard error of the measurement (SEM). Results: Eighty-three patients with 15 different health conditions and their relatives completed baseline and follow-up questionnaires and were included in the responsiveness analysis. The mean FROM-16 change over 3 months = 1.43 (SD = 4.98). The mean patient EQ-5D change over 3 months = −0.059 (SD = 0.14). The responsiveness analysis showed that the FROM-16 was responsive to change (ES = 0.2, SRM = 0.3; p < 0.01). The ES and SRM of FROM-16 change score ranged from small (ES = 0.2; SRM = 0.3) for the distribution-based method to large (ES = 0.8, SRM = 0.85) for anchor-based methods. The AUC value was above 0.7, indicating good responsiveness. There was a significant positive correlation between the FROM-16 change scores and the patient's disease severity change scores (p < 0.001). The MIC analysis was based on data from 100 family members of 100 patients. The MIC value of 4 was suggested for FROM-16. Conclusions: The results of this study confirm the longitudinal validity of FROM-16 which refers to the degree to which an instrument is able to measure change in the construct to be measured. The results yield a MIC value of 4 for FROM-16. These psychometric attributes of the FROM-16 instrument are useful in both clinical research as well as clinical practice. [ABSTRACT FROM AUTHOR]

Introduction: Lamb production systems are under increasing pressure to reduce their environmental footprint, particularly emissions of greenhouse gases (GHGs) such as methane. However, the metrics used to express the carbon footprint of lamb seldom consider its nutritional density and contribution to balanced diets in humans. Lamb production systems vary considerably, from low-input pastoral systems to higher-input systems feeding concentrates for the latter 'finishing' period. To date, no studies have explored the effect of finishing diet on the carbon footprint of lamb meat on a nutritional basis. Methods: Data from 444 carcasses were collected from four abattoirs across Wales, United Kingdom. Lambs were derived from 33 farms with one of four distinct finishing diets: forage crops (n = 5), grass (n = 11), concentrates (n = 7), and grass and concentrates (n = 15). Carcass data were analysed using mixed effects models. Significant differences were found in fatty acid composition of two large commercial cuts of meat from different finishing diets. To illustrate the effect of different measures of footprint, mass (kg dwt) and omega-3 polyunsaturated fatty acid content (g omega-3) were selected as functional units. GHG emission estimates were calculated using Agrecalc. Results: The concentrates diet had the lowest average mass-based product emissions [25.0 kg CO2e/kg deadweight (dwt)] while the grass systems had the highest (28.1 kg CO2e/kg dwt; p < 0.001). The semimembranosus muscle cut from the forage crops diet had the lowest average nutrition-based product emissions (19.2 kg CO2e/g omega-3); whereas the same muscle cut from lambs finished on the grass and concentrates diet had the highest nutrition-based product emissions (29.4 kg CO2e/g omega-3; p < 0.001). Discussion: While mass-based functional units can be useful for comparing efficiencies of different farming systems, they do not reflect how farming systems impact the nutritional differences of the final product. This study demonstrates the importance of considering nutrition when expressing and comparing the carbon footprints of nutrient-dense foods such as lamb. This approach could also help inform discussions around the optimal diets for lamb production systems from both a human nutrition and environmental sustainability perspective. [ABSTRACT FROM AUTHOR]

Background: The use or non-use of mobile technologies and social media in residential outdoor adventurous education (OAE) remains contested and generates an often-cited for-and-against argument in both theory and practice. Purpose: This qualitative study explored instructor perceptions of mobile technologies and social media in their practice as members of instructional staff at the United Kingdom's Outward Bound Trust. Methodology/Approach: Grounded within a multiple-case study design, 20 members of instructional staff were interviewed online from three Outward Bound Trust centers across Wales, England, and Scotland. Findings/Conclusions: Two major themes are shared as statements: (1) phone-free outdoor experiences are more impactful for young people in contemporary society ; (2) social media is fake, outdoor adventurous education necessitates in-person and in-place interaction. These findings are presented within a postdigital conceptual lens which questions whether any residential outdoor experience can be truly "technology free." Implications: The study consolidates perspectives on mobile technologies and social media in the practices of residential OAE instructors. Violence is recognised as a cause of health harm, but it is not consistently or adequately captured in healthcare data systems. While administrative health records could be valuable sources of information for measuring violence, they remain underutilised in violence-related research. The present research aims to examine the suitability of violence indicators in emergency care, primary care, and linked healthcare datasets. Descriptive analyses were conducted with the 2015/16 Hospital Episode Statistics Accident and Emergency (HES A&E) and the 2021/22 Emergency Care Data Set (ECDS). The potential of the Clinical Practice Research Datalink (CPRD) and the South Wales Violence Surveillance dataset (a police and emergency department (ED) dataset linked by Public Health Wales) were shown using available evidence. Among the discussed datasets, the South Wales Violence Surveillance dataset has the most detail about violent acts and their contexts, while the CPRD includes a more extensive range of socioeconomic factors about patients and extensive linkage with other datasets. Currently, detailed safeguarding information is routinely removed from the ECDS extracts provided to researchers, limiting its utility for violence research. In the HES A&E, only physical violence was consistently recorded. Addressing these issues has the potential to improve the use of health administrative data in research on violence. Non-masticatory labial striations on human anterior teeth are a form of cultural dental wear well recorded throughout the Pleistocene, which has been interpreted as resulting from the use of the mouth as a 'third hand' when processing different materials during daily activities, such as cutting meat or working hides with stone tools. Non-masticatory scratches have also been reported on the buccal surface of molars and premolars, although at a far lower frequency compared to the anterior dentition. Previous studies observed an apparent decrease through time in the occurrence of non-masticatory scratches on human teeth, with labial striations appearing to be rare for the Neolithic compared to earlier periods. This study further tests this previously observed pattern through the analysis of over 900 human teeth from 20 sites across England and Wales dating from the Upper Palaeolithic, Mesolithic, and Neolithic, to discuss the distribution and aetiology of non-masticatory striations in the British archaeological record. To record and assess the micro-morphometric characteristics of these dental alterations, macroscopic and microscopic analytical techniques were used. Results show that non-masticatory labial striations are still found on Neolithic teeth, although at a decreased frequency when compared to hunter-gatherer (Upper Palaeolithic and Mesolithic) samples. This may be partly due to changes in diets and food processing methods, as well as types of processed materials and changes in manual handling arising from the inception of the Neolithic in Britain. The sample also includes Upper Palaeolithic, Mesolithic and Neolithic teeth with non-masticatory striations likely associated with funerary practices or cannibalistic treatment of cadavers. Analyses of these marks suggest that striations inflicted during the post-mortem cutting of cadavers from cannibalism or funerary practices differ in their location and micro-morphology, compared with non-masticatory striations produced during the life of an individual using the mouth as a 'third hand'. A new species of bubble-headed trilobite, Staurocephalus oliveae sp. nov., is described from 250 specimens collected from a recently discovered locality in the Sholeshook Limestone Formation (upper Katian, Upper Ordovician; Ashgill, Cautleyan Zone 1 equivalent) in the village of Llanddowror, Carmarthenshire. Although no fully articulated specimens were found, the material is the most abundant and well-preserved of any Ordovician Staurocephalus species known from Britain and Ireland, and enables the complicated 3D structure of its cephalon to be reconstructed. Staurocephalus oliveae sp. nov. is the earliest representative of the genus recorded in Avalonia, following its presumed migration into the area from Laurentia or Baltica. Recognition of this Cautleyan species resurrects the potential usefulness of the long established, widely distributed, Staurocephalus clavifrons as a latest Katian (= Rawtheyan) indicator. Screening for vulnerability factors associated with historic suicidality and self-harm on entry to prison is critical to help prisons understand how to allocate extremely limited mental health resources. It has been established that having previous suicide attempts increases odds of future suicidality and self-harm in prison. We utilised administrative screening data from 665 adult male prisoners on entry to a category B prison in Wales, UK, collected using the Do-IT Profiler. This sample represents 16% of all prisoners who entered that prison during a 26-month period. 12% of prisoners reported a history of attempted suicide, 11% reported historic self-harm, and 8% reported a history of both. Historic traumatic brain injury and substance use problems were associated with a 3.3- and 1.9- times increased odds of a historic suicide attempt, respectively, but no significant increased risk of historic self-harm (95% CI: 1.51–6.60 and 1.02–3.50). However, those who were bullied at school had 2.7 times increased odds of reporting a history of self-harm (95% CI: 1.63–6.09). The most salient risk factors associated with both historic suicide and self-harm were higher levels of functional neurodisability (odds ratio 0.6 for a 1 standard deviation change in score, 95% CI: 0.35–0.75), and mood disturbance (odds ratio 2.1 for a 1 standard deviation change in score, 95% CI: 1.26–3.56). Therefore, it could be beneficial for prisons to screen for broader profiles of needs, to better understand how to provide appropriate services to prisoners vulnerable to suicide and self-harm. Multidisciplinary care pathways for prisoner mental health interventions are important, to account for complex multimorbidity. Adaptations may be needed for mental health interventions to be appropriate for, for example, a prisoner with a brain injury. Understanding this broad profile of vulnerability could also contribute to more compassionate responses to suicide and self-harm from prison staff. [ABSTRACT FROM AUTHOR]

Within the context of reopening society in the summer of 2021, as the UK moved away from 'lockdowns', the Government of Wales piloted the return of organized 'mass gatherings' of people at a number of test events. The current study reports behavioral observations that were made at two of the test events to inform this process. The researchers were particularly interested in four key factors: how (1) context within a venue, (2) environmental design, (3) staffing and social norms, and (4) time across an event, affected the personal protective behaviors of social distancing and face-covering use. Data collection was undertaken by trained observers. Adherence to protective behaviors was generally high, but there is clear evidence that these behaviors were shaped in a systematic way by the environment, situational cues, and the passage of time during the events. Some instances of large-scale non-adherence to personal protective behaviors were documented. An analysis within a dual-process framework suggests ways to understand and respond to supporting target health behaviors in groups of people where intervention is deemed valuable, such as in complex or ambiguous contexts. This is one of the first studies to include a 'true' behavioral measure in understanding human responses to COVID-19. It demonstrates that behavioral observations can add precision and granularity to understanding human behavior in complex real-world contexts. Given the significant physical and mental health burden created acutely and chronically by COVID-19, this work has implications for how governments and organizations support target populations in other complex challenges facing us today, such as in sustainability, and healthy lifestyle behaviors. An individual's intentions are not always matched by their actions, and so the findings support a balanced liberal paternalistic approach where system-level changes support appropriate individual-level decisions to engender collective responsibility and action. Background There is an urgent need to address high levels of work-related violence (WRV) in Britain to improve the workplace experience of both employers and employees. Aims This study specifically explores prevalence rates of work-related violence assaults (WRVAs) for staff at high risk that have supervisor responsibilities and work full-time. Methods Five years of data were used for analysis from the Crime Survey of England and Wales (CSEW). Data were filtered for victims of crime from a working population in Britain. Results Forty-four per cent of respondents who are supervisors and have full-time jobs report that they had been victims of WRVAs. This figure is higher than other previous studies of WRV. Conclusions The current study provides further alternative evidence to support high prevalence rates of WRV by selecting for analysis specific work characteristics data that are strongly associated with WRV (supervisor responsibilities and full-time work). It is therefore recommended that future work explores different means of intervention for organizations to implement within the workplace to reduce WRV and protect workers from harm. Rates of Multimorbidity (also called Multiple Long Term Conditions, MLTC) are increasing in many developed nations. People with multimorbidity experience poorer outcomes and require more healthcare intervention. Grouping of conditions by health service utilisation is poorly researched. The study population consisted of a cohort of people living in Wales, UK aged 20 years or older in 2000 who were followed up until the end of 2017. Multimorbidity clusters by prevalence and healthcare resource use (HRU) were modelled using hypergraphs, mathematical objects relating diseases via links which can connect any number of diseases, thus capturing information about sets of diseases of any size. The cohort included 2,178,938 people. The most prevalent diseases were hypertension (13.3%), diabetes (6.9%), depression (6.7%) and chronic obstructive pulmonary disease (5.9%). The most important sets of diseases when considering prevalence generally contained a small number of diseases, while the most important sets of diseases when considering HRU were sets containing many diseases. The most important set of diseases taking prevalence and HRU into account was diabetes & hypertension and this combined measure of importance featured hypertension most often in the most important sets of diseases. We have used a single approach to find the most important sets of diseases based on co-occurrence and HRU measures, demonstrating the flexibility of the hypergraph approach. Hypertension, the most important single disease, is silent, underdiagnosed and increases the risk of life threatening co-morbidities. Co-occurrence of endocrine and cardiovascular diseases was common in the most important sets. Background: Addressing violence related harm is a global public health priority. While violence is primarily managed in the criminal justice system, healthcare supports and manages those injured by violence. Emergency Departments (EDs), the primary destination for those seriously injured, have emerged as a candidate location for violence prevention initiatives. There is limited evaluation of ED-based violence prevention, and a lack of guidance for the implementation and delivery of them. Nurse-led Violence Prevention Teams (VPTs) have been developed and implemented in two EDs in Wales, UK. This protocol describes methods used in the process evaluation of these VPTs. Aim: To understand how VPTs function, how they were implemented, and mechanisms of impact, as well as the exploration of wider contextual factors influencing their function. Methods: Adopting a critical realist approach and informed by the Medical Research Council (MRC) guidance for process evaluations, the process evaluation will employ qualitative methods to collect and analyse data: a scoping review of evidence of effectiveness that considers the causal mechanisms underpinning violence; a documentary analysis to determine operational considerations concerning the development, implementation and delivery of the VPTs; a descriptive analysis of routine ED data to characterise the prevalence of violence-related attendances in each ED; interviews with professional stakeholders (N = 60) from the violence prevention ecologies in which the VPTs are embedded. Discussion: This protocol outlines a process evaluation of a novel, nurse led violence prevention intervention. Findings will be used to inform policy makers' decision making on whether and how VPTs should be used in practice in other EDs across the UK, and the extent that a single operational model should be adjusted to address the local characteristic of violence. Echiurans (spoonworms) are a very distinctive group of polychaete annelids that had long been considered to constitute a separate phylum. Their fossil record is extremely limited, although trace fossils that have been suggested to be attributable to them date back as far as the Cambrian Period. The oldest body fossils are from the Carboniferous Mazon Creek Biota, and preserve only limited morphological detail. New material from the Middle Ordovician (Darrivilian, Didymograptus murchisoni Biozone) Castle Bank Biota of Wales shows fine detail of the morphology of a new taxon, Llwygarua suzannae gen. et sp. nov., including several details that indicate an assignment to the derived family Thalassematidae, allied to the speciose genus Ochetostoma. These details include proboscis morphology, anterior setae, and muscle organisation within the trunk. An additional specimen is described in open nomenclature, as it may be either a distinct species, or a juvenile of Llwygarua suzannae gen. et sp. nov. with a relatively elongated proboscis. These worms demonstrate a very early and previously unrecognised diversification of the echiuran grown group, further supporting an early diversification of Annelida as a whole. • The Hostile Environment and austerity create challenges for migrant civil society. • Women in migrant civil society are active providers of care, not only recipients. • Migrant civil society makes use of the scant resources available to provide care. • These actions are tenacious, continuing despite the effects to their own wellbeing. In this paper, we examine the role of migrant women in civil society in Wales in a triply-hostile environment created by UK government policy since 2010. Drawing on interviews carried out with EU migrants between 2016 and 2017, we outline the active support and care work provided by these women to migrants and others and the way in which they navigated austere and hostile conditions (contrasting the popular construction of migrants passively requiring support and care). Contributing to the literature on resourcefulness, we introduce the notion of tenacity to highlight the exhausting care-full labour of these migrants, who continue despite challenging circumstances and impact on their own wellbeing. We conclude that the care work provided by these women plays an important civil society role in tackling ongoing austerity and hostility but that precarious conditions bring a lack of sustainability which can heighten the socio-spatial inequalities seen across the UK. Background: The importance of good oral hygiene for patients in Intensive Care Units (ICUs) is well recognized, however, the most effective way to achieve good oral care in the ICU is unclear. Aim: This study aimed to provide a national picture of oral care practices in adult ICUs in the United Kingdom (UK) to identify areas for improvement. Study Design: A national one‐day point prevalence study was undertaken in adult ICUs in the UK in the period from 30th September to 14th October 2021. Data were collected on all patients in the ICU on the date of data collection. Using a validated electronic data collection form, anonymised data were collected on methods and frequency of oral care provided, and the use of oral care protocols within the ICU. Data were analysed using descriptive analysis. Results: Data from 195 patients in 15 ICUs in England, Wales and Northern Ireland were collected. Written oral care protocols were available for use in the care of 65% (n = 127) of patients. 73% (n = 142) of patients received oral care within the 24‐h period. Oral care methods included toothbrushing 41% (n = 79), foam sticks 3% (n = 5), moisturizing the oral cavity 10% (n = 19) and mouth rinse with chlorhexidine 3% (n = 5) and other oral care methods not specified 12% (n = 23). 44% (n = 85) of patients had an oral assessment within the 24‐h period and variable assessment methods were used. Conclusion: There is large variability in oral care provision and methods for intubated ICU patients and a lack of consensus was revealed in the study. Oral assessment is conducted less frequently using multiple tools. Optimal oral care standards and further research into oral care provision is pivotal to address this important patient‐relevant practice. Relevance to Clinical Practice: Oral care is a fundamental part of care for ICU patients, however, there is a large degree of variability, and oral care is often not based upon oral assessment. The use of an oral care protocol and oral assessments would help to improve patient care, ease of use for staff and provide a tailored oral care plan for patients, improving efficiency and preventing wasted resources. Following the finding of the World Breastfeeding Trends Initiative 2016 assessment that infant feeding is not mentioned in United Kingdom emergency planning guidance, in 2017 information was sought about Local Resilience Forum (LRF) planning. This research covered both guidance for parents on LRF websites and local planning. Considerable variation between areas and a dependence on the existing knowledge and experience of frontline staff was found. In 2020−2022 LRF websites in England and Wales were revisited and information requested about rest centre facilities, procurement of items needed by evacuated parents, provision of skilled support and staff training. Twenty‐nine of the 42 LRFs replied, partially or fully, directly or through their local authorities. The variation between areas found in 2017 was confirmed. Areas endeavour to provide for the needs of families but specific training in infant feeding was lacking, although several requested relevant resources or responded that the lack of training would now be reviewed. A short leaflet was produced and circulated to LRFs that had responded to aid training. Using established international guidance to include infant feeding in United Kingdom emergency planning guidance, implemented by national coordinators, would improve consistency between LRFs and better protect infant, child and maternal health and well‐being.
